Questa presentazione contiene i dati più recenti riguardo l' uso del test all' atropina per la discriminazione di pazienti con malattia aritmica atriale e sospetta disfunzione del nodo senoatriale. A mio avviso ed anche dalla mia esperienza personale può essere utile per escludere i pazienti con risposta cronotropa normale (una specificità abbastanza alta) che peraltro non possono essere sottoposti a test da sforzo o altro per una valutazione appropriata. I candidati ideali sarebbero pazienti di età maggiore a 75 anni con sintomatologia fugace e non specifica ai quali non si può risalire ad un interpretazione univoca dei sintomi e prima di ricorrere all' impianto a un loop recorder ed infine un PM. Tengo a sottolineare che suddetto test fu usato molto quando l` impianto del loop recorder non era diffuso e cmq i costi erano alti (tuttora pero i costi rispetto al test rimangono elevati). In ogni caso è un test semplice, bedside, non costoso e molto ben tollerato dai pazienti. Il resto viene ben descritto nella presentazione.

Furthermore stress induced by acute illness definitely impair metabolic control in known diabetic patients. Insulin is the therapy of choice in acutely ill hospitalised patients. You have to respect some important rules and algorithms, such as the Yale protocol, approaching insulin therapy in these patients, expecially to avoid the rollercoaster glycemic profile which subtends dangerous hypoglycemias and the increased risk of mortality. Use basal-bolus insulin regimens or continuous subcutaneous insulin infusion and tailor insulin regimens to the individual’s treatment.
New rapid-acting and long-acting insulin analogues profiles help to reach better glycemic control.

Comorbidities affect clinical hystory and life expectancy in people with chronic heart failure (CHF). Hospital re-admissions are direct consequence of difficulties in management of CHF. Appropriate pathways are essential following an episode of acute heart failure. Chronic renal failure, diabetes and anemia are significantly connected with mortality and hospital admission in patients with chronic heart failure. Most of the patients (74%) have three or more comorbidieties. Hospital admissions mainly explain the economic burden of CHF. Non-cardiac comorbidities increase preventable hospitalizations (i.e. ambulatory care sensitive conditions). It's mandatory to ensure a seamless transition from inpatient to outpatient care for all patients, and transfer logistic and professional management from hospital to territory to improve quality of life, life expectancy and reduce economic burden of CHF.

This document discusses functional constipation. It provides the Rome IV diagnostic criteria for functional constipation which includes symptoms like straining, hard stools, sensation of incomplete evacuation occurring in over 25% of bowel movements. It notes that loose stools are rarely present without laxative use. Therapeutic options for functional constipation are discussed including fiber, PEG, linaclotide, prucalopride, and lubiprostone. anticoagulante: che fare in fase acuta ora?
Nei pz con emorragia cerebrale in trattamento anticoagulante è indicata una rapida
correzione dell’emostasi che si ottiene a seconda della terapia anticoagulante in
corso con:
• solfato di protamina in pazienti in trattamento con eparina
ev o sc
• plasma fresco congelato o concentrati di complesso
protrombinico associati con vitamina K ev in pazienti in terapia
con anticoagulanti orali (aVK)
GPP: il gruppo SPREAD raccomanda il complesso protrombinico per la
maggiore rapidità d'azione, il minor volume infuso e l’assenza di trasmissione di virus

15. Emorragia cerebrale in pz. in terapia con
nuovi ACO: che fare in fase acuta in futuro?
In caso di emorragia cerebrale in pazienti in trattamento con i nuovi ACO
sarà necessario disporre di:
• test quantitativi di anticoagulazione, specifici e sensibili
• antidoti: complesso protrombinico? Fattore VII?

antitrombotica, il problema immediato, in caso di evoluzione
clinica favorevole, è la sua ripresa. In assenza di dati certi in

non essendo considerata mandatoria la completa scomparsa
del sanguinamento alla TC, che abbisogna di molte settimane”

ed il rischio emorragico (HASBLED)
• Considerato il profilo di maggior sicurezza dei nuovi ACO
rispetto a warfarin relativamente al rischio di emorragie
intracraniche, dopo un emorragia intracranica da nuovi ACO
è ragionevole che il trattamento venga reintrodotto dopo un
intervallo di tempo dall’evento emorragico almeno pari a
quello previsto dalle linee guida SPREAD per il trattamento con
warfarin
