The temporalis muscle flap is used to repair defects after oncologic resections or craniofacial reconstruction. It involves elevating the temporalis muscle while protecting branches of the facial nerve within the temporoparietal fascia. An incision is made above the zygomatic arch and the muscle is elevated off the bone to preserve its blood supply. It can then be rotated through a tunnel to reach the maxilla or oral cavity. Complications include potential facial nerve injury, flap loss, infection, or cosmetic deformities.
