 However, the employer is not obligated to
accept the SAP’s counsel regarding follow-
up requirements or additional support
programs

decisions regarding employee’s follow-up
(including Last Chance Agreement) should

  • 15. SECTION 3:SECTION 3: Psychoactive Substances: Overview and Signs and Symptoms

Range of Substance Use  Recreational Use – casual, non- problematic  Substance Abuse – moderately problematic, may have mild cravings/withdrawal symptoms, may impact on work/relationships  Substance Dependence – very problematic, likely to impact work/relationships, physical/psychological problems/withdrawal symptoms
  • 17. Drug Tolerance  Takes more of that substance to produce the same effect  Due to the body’s healthy desire for balance, attempts to counteract the effects of the drug  Almost all psychoactive substances taken regularly over a period of time lead to tolerance  Significantly increases the risk of drug overdose
  • 18. Recognizing Signs and Symptoms  Absenteeism/lateness  Frequent breaks  Mood swings  Alcohol on breath  Irritability  Memory loss/lapse  Borrowing money
  • 19. Physical Signs of Substance Abuse  Slowed, staggering walk, poor coordination  Red, watery eyes, pupils smaller or larger than usual, blank stare  Puffy face, blushing or paleness  Cold, sweaty palms
  • 20. Physical Signs of Substance Abuse (cont’d)  Tremors or shakes of hands, feet, head  Extreme hyperactivity, unusual talkativeness  Needle marks on arms, legs, feet
  • 21. Emotional Experience of Substance Abuser  Denial of problem  Anger at self and others  Mistrust of others  Controlling  Low self esteem  Avoidance of feelings  Isolation
  • 22. Enabling: Help or Harm?  Rescuing behaviour that allows substance abuser to maintain his/her way of life  Enabler takes responsibility, blame or makes accommodations for substance abuser  Becomes pattern for family, friends, co-workers, management
  • 23. We Need to Deal with Substance Abuse @ Work  Increase of employee suffering which is hidden  Social cost of inactivity are staggering  Increase of cost to organizations – health care, WSIB claims, lost productivity – impact on corporate culture is enormous  It is our job!
  • 24. SECTION 4:SECTION 4: Workplace Responses: SAP vs. EAP Assistance
  • 25. Workplace Responses  Employees may request assistance (i.e. self-disclosure) – Best case scenario – Respond supportively according to A & D Policy
  • 26. Workplace Responses (cont’d)  Employees may be hiding the problem – More difficult situation – Engage in non-accusing conversation – Respond supportively according to A & D Policy in the following situations Positive drug/alcohol test With training, can act using “Reasonable Cause” – Disciplinary action
  • 27. Substance Abuse Professionals (SAPs)  Regulated term under 49 CFR Part 40 of US DOT and specifies the following: – Professional qualifications – Addictions experience – Training and rigorous exam – Monitored continuing education – Subject to DOT audits  SAP is not a regulated term in Canada
  • 28. Substance Abuse Professionals (cont’d)  Use of a DOT SAP ensures: – Properly credentialed – Addictions experience – Understands workplace concerns – Background in the community – Understands appropriate reporting constraints – Makes appropriate relapse prevention recommendations
  • 29. EAP Counselling  Voluntary  Confidential  Designed for employees with self-disclosed issues  No need for diagnostic tools
  • 30. SAP Assessment and Reporting  Mandatory  Not confidential – reports provided to the Employer  Designed for employees who have breached A & D Policy or US DOT Regulation  Requires objective diagnostic tools
  • 31. SAP Assessments and Reporting (cont’d)  Reports and Assessments should: – Balance the organizations legal requirement and right to privacy – Understand current federal and provincial legal and human rights rulings
  • 32. SAP Process 1. Organization determines level of reporting 2. SAP conducts a thorough professional assessment 3. SAP determines type of treatment OR education needed 4. SAP refers employee to the community resource best matching needs 5. SAP provides organization’s representative with initial report
  • 33. SAP Process (cont’d) 6. SAP tracks progress of employee through treatment or education 7. SAP provides company representative with updates and notifies rep if employee fails to comply 8. SAP conducts post-treatment (education) follow-up assessment to determine level of employee compliance 9. SAP provides company representative with follow-up report, which includes relapse-prevention recommendations
  • 34. Company Representative’s Options  IWS strongly suggests that the employer follow the SAP’s recommendations  However, the employer is not obligated to accept the SAP’s counsel regarding follow- up requirements or additional support programs
  • 35. Company Representative’s Options (cont’d)  Organization can use SAP’s recommendations solely to advise its decisions regarding employee’s follow-up (including Last Chance Agreement) should employment continue

  1. Recreational Use – Recreational Use is seen as casual, unproblematic use of a substance. Use is strictly limited to non-work hours Use does not negatively impact personal relationships or work; if use were to impact relationships/work, the individual would be able and willing to immediately stop use Use does not create problems with the law Substance Abuse – Substance Abuse is seen as use of a substance in a somewhat to moderately problematic way. Use is usually limited to non-work hours; however, use may impact work (i.e. absenteeism, presenteeism) – perhaps leading to disciplinary measures Use may negatively impact personal relationships (i.e. my wife says I drink too much) Use may create problems with the law (i.e. driving while intoxicated) Individuals may continue to consume despite the knowledge that continued consumption poses significant social, interpersonal, or work problems Individuals may experience some mild cravings and/or mild negative physical effects (i.e. hangovers) Substance Dependence – Substance Dependence is seen as use of a substance in a very to extremely problematic way Use very likely to impact work (i.e. absenteeism, presenteeism) – often leading to disciplinary measures Use very likely to negatively impact personal relationships Use may create problems with the law (i.e. driving while intoxicated) Individuals continue to consume despite the knowledge that continued consumption poses significant social, interpersonal, or work problems Individuals continue to consume despite the knowledge that continued consumption poses significant psychological or physical problems (i.e. depression, blackouts, liver disease) Use is characterized by 1) withdrawal symptoms; 2) cravings; 3) tolerance; 4) compulsive behaviour

Signs of Substance Abuse can be very similar to signs of mental health or performance difficulties; it is important to keep in mind the possibility of substance abuse issues when signs of distress are noted Borrowing money can be for substances or for gambling addiction. It is the only one that can wipe out your life in a few hours, depending on how big the wager.
  3. Denial –important to pretend person has control, no problem, just one more drink/pill/fix and I’ll be able to turn my life around Anger at self is manifested through anger at others, blame to defend against feelings of helplessness Mistrust – fear that others are conspiring to deprive person of substance Controlling of others because cannot control self and own destiny Low self esteem – disgusted by own behaviour – anyone who has any value would leave, family/friends/coworker still involved so they must have no value-therefore worthy of abuse Feelings – important not to feel Isolation of self and impose isolation on family members
  4. Is it help or harm? Can you tell me some rescuing behaviours? bailing them out of jail, lie to police, lend money, buy substance Often situation is complicated by other family members involved (little children are hungry) Sometimes another person is scapegoated to draw fire away from the abuser, a child could act out
  5. Even if you don’t buy all of the statistics, as Kevin Roxby likes to say, even if we cut them by half--or three quarters EAP’s need to get back to basics, get back to their tradional roots and address these issues for all of these reason and one more-- because it is our job.
