This study examined the relationship between serum Omentin-1 levels and insulin resistance in type 2 diabetes mellitus patients. It found that type 2 diabetes patients had significantly lower serum Omentin-1 levels compared to healthy controls. Serum Omentin-1 levels were negatively correlated with fasting blood sugar, fasting insulin levels, and insulin resistance (as measured by HOMA-IR) in diabetes patients. This suggests that higher insulin resistance is associated with lower serum Omentin-1 levels. The study concludes that Omentin-1 may play a role in insulin resistance and have potential as a future therapeutic target for diabetes.

Diabetes is a very serious, chronic, and complex metabolic disorder that is based on multiples aetiologies with both acute and chronic. There are two types of diabetes, one types 1 DM and the other is type two DM. Both T1DM and T2DM are the most serious type of chronic conditions that are typical cannot be cured. Insulin stimulates adipocytes, myocytes, and hepatocytes, which uptake glucose from the circulatory system. Antidiabetic drugs show useful effects through decreasing glucose absorption in the intestines increasing insulin levels in the body or increasing the bodys sensitivity or decreasing its resistance to insulin. Most of these plants contain bioactive compounds. Such compounds are alkaloids, flavonoids, glycosides, terpenoids, carotenoids, etc., which are frequently implicated as having an Antidiabetic effect. The different types of phytochemicals include flavonoids, saponins, phenolic acids, alkaloids, tannin, stillness, and polysaccharides. They have two types of drug design structure based drug desingand ligand based drug design. The protein is a structure based ligand, which is based on drug design for the role of significantly quantitative structure activity relationship QSAR and pharmacophore analysis. Ligand based pharmacophore model generation depends on the information regarding the known biological activity without any structural information depends on the macromolecular target. The ligand based VS is more expensive due to structure based methods. A comparative study of clinical and laboratory parameters was conducted when prescribing complex therapy to postmenopausal women with a history of PCOS. Depending on the type of prescribed therapy, groups of patients with hyperglycemia and impaired glucose tolerance were formed, they underwent M T in combination with drugs for the correction of insulin resistance. It was revealed that MGT in the composition with drospirenone, prescribed to women with menopausal complications with PCOS in the anamnesis, relieves menopausal disorders, has a favorable effect on the lipid profile, prevents the development of fatal complications of menopause. However, the addition of insulin sensitizers to complex therapy demonstrated significant significant differences in the levels of the studied parameters, which made it possible to achieve a significant increase in the effectiveness of the therapy. In patients with chronic kidney disease (CKD), along with progression of CKD,
abnormalities of mineral and bone metabolism develop, which result in altered serum levels of minerals
such as calcium and phosphorus, as well as abnormalities in parathyroid hormone (PTH) or vitamin D
metabolism. Chronic Kidney Disease-Mineral Bone Disease (CKD-MBD) is a serious burden because of
increased cardiovascular mortality thus making therapeutic improvements essential in CKD-MBD. The
present study was aimed at evaluation of pharmacological management of CKD-MBD.
Methods:A retrospective study including 180 patients divided into two groups of 90 each (diabetes
mellitus and non-Diabetes) was performed in the Department of Nephrology, SVIMS, Tirupati. Patients
who were on follow up for at least 3 years (2015-2017) were considered, serum parameters were measured at every six months with a total of 6 visits. First visit was taken as baseline and sixth visit as
conclusion.
Results:The disease incidence of CKD-MBD is more common in male patients i.e. 67.8%. Serum calcium
levels were significantly increased and eGFR was significantly decreased in all patients with CKD at
conclusion compared to baseline.Further, Serum calcium levels were significantly increased at conclusion
in CKD patients without DM and eGFR was significantly decreased at conclusion compared to baseline
in CKD patients with DM. The proportion of untreated patients is high for all the drugs except vitamin D
analogues in both subgroups of CKD patients.
Conclusion:Pharmacological intervention in CKD patients helps in the effective management of mineral
bone disease by maintaining serum calcium, phosphate and calcium phosphorous product status.

Blood glucose (BG) prediction system can help gestational diabetes mellitus (GDM) patient to improve the BG control with managing their dietary intake based on healthy food. Many techniques have been developed to deal with blood glucose prediction, especially those for recommender system. In this study, we conduct a systematic mapping study to investigate recent research about BG prediction in recommender systems. This study describes an overview of research (2014-2018) about BG prediction techniques that has been used for BG recommender system. As results, 25 studies concerning BG prediction in recommender system were selected. We observed that although there is numerous studies published, only a few studies took serious discussion about techniques used to incorporate the BG algorithms. Our result highlighted that only one study discusses hybrid filtering technique in BG recommender system for GDM even though it has an ability to learn from experience and to improve prediction performance. We hope that this study will encourage researchers to consider not only machine learning and artificial intelligent techniques but also hybrid filtering technique for BG recommender system in the future research.

The aim of the study was to evaluate the clinical and
biochemical characteristics of geriatric patients with type 2
diabetes mellitus (DM) attending tertiary care teaching hospital,
SVIMS, Tirupati. OBJECTIVES: To document clinical
features, biochemical parameters and anti-diabetic medications
received as per records. METHODOLOGY: This
retrospective study was performed in Department of
Endocrinology in SVIMS, Tirupati, over a period of 6 months.
Demograghic details, laboratory parameters and
pharmacotherapy details were collected in the pre-designed
annexure form. RESULTS: A total of 100 diabetic elderly
patients were included in the study. Out of which 57 were men,
43 were women. Most of them were under the age group of
60-64 years followed by other age groups. 28 patients were
having the diabetes duration of 11-15 years. Some of them were
having diabetic complications (such as diabetic retinopathy,
diabetic neuropathy, diabetic nephropathy). Among 100
patients, 32 patients were normal weight, 22 patients were
overweight, 28 patients were obese-I, 18 patients were obese-II.
About 85% of patients were having high lipid levels. Some
patients were on Oral Hypoglycemic Agents (OHAS), some
patients were using both Insulin & OHAS.CONCLUSION:
The present study comprised of 100 elderly type 2 diabetic
patients in which males were higher in number when compared
to females, most of the patients were in the age of group of
60-64 years. Hypertension was the most common co-morbidity
associated with DM followed by Coronary Artery Disease
(CAD). Diabetic neuropathy was the most prevalent
complication followed by Diabetic retinopathy and diabetic
nephropathy. In this study 46% of the patients were obese. Most
of the patients were on OHAS alone, some were on OHAS and
Insulin combination therapy while very few were on insulin
therapy alone. Dyslipidemia was present in 85% of the patients
and the most common form of dyslipidemia was low HDL and
high LD

Abstract—Diabetes Mellitus is a lifestyle disease it is increasing with increase of urbanization. It is a side effect of development. Nowadays with the development of community it is also on increase trend. So this study was conducted on 250 patients of diabetes attended at Diabetic Clinic of SMS Hospital Jaipur, with the aim to find out socio-demographic profile of these diabetes cases. General information about the these case was gathered in a pre-designed semi-structured performa. It was found in this study that majority of cases were in age group of 31 to 45 years with slight male dominance. Education wise majority were Graduate followed by secondary educated and others. Likewise occupation wise majority were either unemployed of professional. Majority of cases were from Socio economic Class II and III. So it can be concluded that diabetes is a disease of middle age slight male dominance and of educated middle class individuals. Further studies are required to establish this fact.

Microalbuminuria in Saudi Adults with Type 1 Diabetes Mellitus_Crimson Publis...CrimsonPublishersIOD
Background: Diabetes mellitus is among the most common chronic non-communicable diseases. The development of microalbuminuria in type 1 diabetes increases the risk for renal and cardiovascular disease.
Methods: A cross sectional study was conducted at the Primary Health Care Clinics at King Fahad Armed Forces Hospital, Jeddah, Saudi Arabia. A total of 334 Saudi with type 1 diabetes were randomly selected.
Results: Total of 334 patients with T2DM included in this study; 102 (30.5%) male and 232 (69.5%) female with mean age 25.8±3.4. MA was present in 99 (29.6%). MA was not significantly more prevalent in female (69.4%) with female predominance (sex ratio male: female) 1:2.3. HTN with MA was significantly more prevalent in 51(51.5%) of MA group with odd ratio 1.7 (1.2-2.4), p=0.001 with no siginificant difference between both gender. Patients with MA have significant higher HbA1c than patients with normal buminuria and there was a significant difference between gender (p< 0.0001) and when compared to HbA1c groups (p=0.002).
Conclusion: The frequency of microalbuminuria in patients with type 1 diabetes in this study is high. It is mandatory to have adequate diagnostic, therapeutic and educational resources in addition to competent physicians who can manage microalbuminuria in diabetic patients by using a continuing, comprehensive and coordinated approach.

This study evaluated the biochemical and the hematological parameters in diabetic and non- diabetic patients. The measured biochemical parameters were fasting blood sugar, serum alanine aminotransferase (SGPT/ALT), total cholesterol, urea, creatinine and hematological parameters were hemoglobin, total white blood cell, neutrophil, lymphocyte,monocyte, eosinophil and ESR. There were 403 diabetic and 320 non-diabetic subjects included in this study and the study was carried out in BIRDEM (Bangladesh Institute of Research & Rehabilitation in Diabetes, Endocrine and Metabolic Disorders) General Hospital). It was observed that the mean values of SGPT/ALT (p<0.001),><0.001)><0.001)><0.001),><0.004),><0.001) of hematological parameters were significantly higher in diabetic patients than in the non-diabetic patients. In univariate analysis, all biochemical parameters and only four hematological parameters were found significantly associated with fasting blood sugar after adjusted with age and sex. The fasting blood sugar correlates highly with the other biochemical parameters but less or none with the hematological parameters. Our findings demonstrated that control of increased biochemical parameters and abnormal hematological levels in the early stage of diabetes mellitus may help the patients to raise quality of life.

Over the last decade, diabetes mellitus has emerged as an important clinical and public health
problem throughout the world. The aim of the study is perceive the Potentiality of a newer oral
Antihyperglycemic combination therapy over conventional therapy in type 2 diabetes. The
prospective study was conducted over a period of six months in the department of Medicine,
Guntur City Hospital. The prevalence of type2 diabetes was high in male 65.79 % than female
34.21%. Majority of the patients (23.68 %) belonged to age group of 51–55 years. Majority of
patients (55.26%) having a family history of Diabetes. Majority of patients receiving Combination
of Glibenclamide + Metformin (60.53%), evaluated for effect on FPG for both combinations. The
mean changes in FPG were noted. In the same way effect on HbA1c also noted. Mean changes in
for every month HbA1c will be noted. Our study reveals that Combination therapy with Metformin
plus Glimepiride is more effective than Glibenclamide plus Metformin; in improving glycemic
control in type 2 diabetes, while also allowing a reduction of the dosage of each drug.

Oral hypoglycemic agents like sulphonylureas and biguanides are still the major players in the management of the disease but there is growing interest in herbal remedies due to the side effects associated with the oral hypoglycemic agents. Herbal medicines have been the highly esteemed source of medicine throughout human history. Hyperlipidemia has been ranked as one of the greatest risk factors contributing to prevalence and severity of coronary heart diseases. Hyperlipidemia is a condition when abnormally high levels of lipids i.e. the fatty substances are found in the blood. Hypolipidemic drugs are extensively used as prophylactic agents to prevent such atherosclerosis induced disorders. But these hypolipidemic drugs are not free from adverse effects. Many plant derivatives and domestic remedies have been screened for their hypolipidemic action. More than 70 medicinal plants have been documented to have significant hypolipidemic action. During the last decade, an increase in the use of medicinal plants has been observed in metropolitan areas of developed countries. Medicinal plants play a major role in diabetes and hypolipidemic activity. The advantages of herbal medicines reported are effectiveness, safety, affordability and acceptability, this review focus on diabeties and hyperlipidemia and the role of plants used for the treatment of diabeties and hyperlipidemia.

For a long time, several herbal medicines have been used for the treatment of diabetes in the form of compound drugs. Moreover, after the references made by researchers on diabetes mellitus, investigations on the hypoglycemic activity of compound drugs from medicinal plants have been more important. Although, the molecular mechanisms behind this effect is not much explored yet. There are various approaches to reduce the diabetes effect and its secondary complications, and herbal drugs are more preferred due to its less side effects and low cost. One of the major factors in the development of diabetes and its complications is the damage induced by free radicals. Therefore antidiabetic compounds with antioxidant properties would be more beneficial. It is hypothesized that the insulin mimetic effect, hypoglycemic effect and β-cell function of herbal remedies might add to glucose uptake through improvement in the expression of genes of the glucose transporter (GLUT) family in liver and skeletal muscles. Here we selected some plants with the ability to control blood glucose as well as to modulate some of the mechanisms involved in insulin resistance like β-cell function, glucose transport (GLUT) gene and incretin related
pathways. Therefore, plants remedies may be appealing as an alternative and adjunctive treatment for diabetes mellitus.

work aimed to study the variables associated with DME in a cohort of patients with type 2 diabetes mellitus from Basrah (Southern Iraq).

Abstract—Diabetes Mellitus is a lifestyle disease it is increasing with increase of urbanization. It is a side effect of development. Nowadays with the development of community it is also on increase trend. So this study was conducted on 250 patients of diabetes attended at Diabetic Clinic of SMS Hospital Jaipur, with the aim to find out socio-demographic profile of these diabetes cases. General information about the these case was gathered in a pre-designed semi-structured performa. It was found in this study that majority of cases were in age group of 31 to 45 years with slight male dominance. Education wise majority were Graduate followed by secondary educated and others. Likewise occupation wise majority were either unemployed of professional. Majority of cases were from Socio economic Class II and III. So it can be concluded that diabetes is a disease of middle age slight male dominance and of educated middle class individuals. Further studies are required to establish this fact.

Microalbuminuria in Saudi Adults with Type 1 Diabetes Mellitus_Crimson Publis...CrimsonPublishersIOD
Background: Diabetes mellitus is among the most common chronic non-communicable diseases. The development of microalbuminuria in type 1 diabetes increases the risk for renal and cardiovascular disease.
Methods: A cross sectional study was conducted at the Primary Health Care Clinics at King Fahad Armed Forces Hospital, Jeddah, Saudi Arabia. A total of 334 Saudi with type 1 diabetes were randomly selected.
Results: Total of 334 patients with T2DM included in this study; 102 (30.5%) male and 232 (69.5%) female with mean age 25.8±3.4. MA was present in 99 (29.6%). MA was not significantly more prevalent in female (69.4%) with female predominance (sex ratio male: female) 1:2.3. HTN with MA was significantly more prevalent in 51(51.5%) of MA group with odd ratio 1.7 (1.2-2.4), p=0.001 with no siginificant difference between both gender. Patients with MA have significant higher HbA1c than patients with normal buminuria and there was a significant difference between gender (p< 0.0001) and when compared to HbA1c groups (p=0.002).
Conclusion: The frequency of microalbuminuria in patients with type 1 diabetes in this study is high. It is mandatory to have adequate diagnostic, therapeutic and educational resources in addition to competent physicians who can manage microalbuminuria in diabetic patients by using a continuing, comprehensive and coordinated approach.

This study evaluated the biochemical and the hematological parameters in diabetic and non- diabetic patients. The measured biochemical parameters were fasting blood sugar, serum alanine aminotransferase (SGPT/ALT), total cholesterol, urea, creatinine and hematological parameters were hemoglobin, total white blood cell, neutrophil, lymphocyte,monocyte, eosinophil and ESR. There were 403 diabetic and 320 non-diabetic subjects included in this study and the study was carried out in BIRDEM (Bangladesh Institute of Research & Rehabilitation in Diabetes, Endocrine and Metabolic Disorders) General Hospital). It was observed that the mean values of SGPT/ALT (p<0.001),><0.001)><0.001)><0.001),><0.004),><0.001) of hematological parameters were significantly higher in diabetic patients than in the non-diabetic patients. In univariate analysis, all biochemical parameters and only four hematological parameters were found significantly associated with fasting blood sugar after adjusted with age and sex. The fasting blood sugar correlates highly with the other biochemical parameters but less or none with the hematological parameters. Our findings demonstrated that control of increased biochemical parameters and abnormal hematological levels in the early stage of diabetes mellitus may help the patients to raise quality of life.

Over the last decade, diabetes mellitus has emerged as an important clinical and public health
problem throughout the world. The aim of the study is perceive the Potentiality of a newer oral
Antihyperglycemic combination therapy over conventional therapy in type 2 diabetes. The
prospective study was conducted over a period of six months in the department of Medicine,
Guntur City Hospital. The prevalence of type2 diabetes was high in male 65.79 % than female
34.21%. Majority of the patients (23.68 %) belonged to age group of 51–55 years. Majority of
patients (55.26%) having a family history of Diabetes. Majority of patients receiving Combination
of Glibenclamide + Metformin (60.53%), evaluated for effect on FPG for both combinations. The
mean changes in FPG were noted. In the same way effect on HbA1c also noted. Mean changes in
for every month HbA1c will be noted. Our study reveals that Combination therapy with Metformin
plus Glimepiride is more effective than Glibenclamide plus Metformin; in improving glycemic
control in type 2 diabetes, while also allowing a reduction of the dosage of each drug.

Despite the vast amount of work done by many organizations worldwide, particularly in Third World
countries like Zimbabwe, the effectiveness of their partnerships in programming is still marginal. The major
focus of this study was to evaluate the partnership that exists between International Non Governmental
Organizations {INGOs} and their local implementing Partner Non-Governmental Originations in order to
establish areas that need improvement and strengthening. This study revealed that some of the partnerships
that exist between these agencies are marred by challenges such as funding, implementing strategy, capacity
building/training, strategic networking, consultative and joint initiatives. An analysis of interviews established
that most partnerships existed in the area of implementation where local organizations are funded and this
accounted for 30%. This is an area where NGOs obtain financial assistance to implement programmes. The
other areas; capacity building/training shows 25% while strategic networking and consultative show 15%. The
area showing the least was joint initiatives which had only 10% of the existing partnerships. This reflects the
situation in most African countries where joint partnerships with NGOs are new phenomena and that the
founding members of such NGOs embark on the programmes/projects for philanthropic reasons with very little
knowledge of professional expertise needed to carry out these projects jointly. They outsource/consult or apply
for people to help them beef up their expertise or train their staff to do so. The paper concludes by determining
areas that need strengthening and proposes relevant policy recommendations which will contribute to future
research on the effectiveness of partnerships. The need for partnerships to strengthen projects and programmes
that are sustainable and which do not reinforce dependence was observed. The study further observed that
conflict and misunderstandings within the partnering groups as another barrier which led to local organizations
being reluctant to assume equal part in partnerships and to share information about the programme preferring
to maintain confidentiality or protect their sovereignty. The study adds knowledge to our understanding of
partnerships that exist between International Organizations and Local Organizations in Zimbabwe

Diabetes is a rapidly and serious health problem in Pakistan. This chronic condition is associated with serious long-term complications, including higher risk of heart disease and stroke. Aggressive treatment of hypertension and hyperlipideamia can result in a substantial reduction in cardiovascular events in patients with diabetes 1. Consequently pharmacist-led diabetes cardiovascular risk (DCVR) clinics have been established in both primary and secondary care sites in NHS Lothian during the past five years. An audit of the pharmaceutical care delivery at the clinics was conducted in order to evaluate practice and to standardize the pharmacists’ documentation of outcomes. Pharmaceutical care issues (PCI) and patient details were collected both prospectively and retrospectively from three DCVR clinics. The PCI`s were categorized according to a triangularised system consisting of multiple categories. These were ‘checks’, ‘changes’ (‘change in drug therapy process’ and ‘change in drug therapy’), ‘drug therapy problems’ and ‘quality assurance descriptors’ (‘timer perspective’ and ‘degree of change’). A verified medication assessment tool (MAT) for patients with chronic cardiovascular disease was applied to the patients from one of the clinics. The tool was used to quantify PCI`s and pharmacist actions that were centered on implementing or enforcing clinical guideline standards. A database was developed to be used as an assessment tool and to standardize the documentation of achievement of outcomes. Feedback on the audit of the pharmaceutical care delivery and the database was received from the DCVR clinic pharmacist at a focus group meeting.

In Pakistan, the overall prevalence of dyslipidemia in adolescents aged 10–18 years is 21.7~25.2%; prevalence is reported to be two times higher (53.1~56.1%) in obese adolescents. However, few studies have been conducted on the relationship between height and blood lipid concentrations in children and adolescents The recent emphasis on treatment of the dyslipidemia of the metabolic syndrome (hypertriglyceridemia, reduced high-density lipoprotein, and increased small, dense low-density lipoprotein particle number) has compelled practitioners to consider lipid-lowering therapy in a greater number of their patients, as one in two individuals over age 50 has the metabolic syndrome. Individuals with the metabolic syndrome typically have normal low-density lipoprotein cholesterol levels, and current lipid-lowering guidelines may underestimate their cardiovascular risk. Two subgroups of patients with the metabolic syndrome are at particularly high risk for premature CAD. One, individuals with type 2 diabetes, accounts for 20-30% of early cardiovascular disease. The second, familial combined hyperlipidemia, accounts for an additional 10-20% of premature CAD. Familial combined hyperlipidemia is characterized by the metabolic syndrome in addition to a disproportionate elevation of apolipoprotein B levels. The measurement of fasting glucose and apolipoprotein B, in addition to the fasting lipid profile, can help to estimate CAD risk in patients with the metabolic syndrome. In this research we compared allopathic medication and medicinal herb in treating hyperlipidemia.

Diabetes is fast gaining the status of a potential epidemic in India with more than 62 million diabetic
individuals currently diagnosed with the disease. In 2000, India (31.7 million) topped the world with the highest
number of people with diabetes mellitus followed by China (20.8 million) with the United States (17.7 million)
in second and third place respectively. The prevalence of diabetes is predicted to double globally from 171
million in 2000 to 366 million in 2030 with a maximum increase in India..In addition to this, the complications
associated with diabetes is also in the higher rate .

The Management of diabetes is most critical part, Lifestyle modifications like dietary modifications, physical
activity like aerobic exercise, some major drugs of diabetes management, insulin therapy, foot care of diabetes
and annual screening. Most of the patients are economically poor. So the researcher found that aerobic exercise
is economical and have lot of health benefits on diabetic patients.
Aim: The aim of study is to check the effectiveness of video assisted teaching regarding aerobic exercise and
practice to maintain blood glucose level among diabetes.
Methodology: The research approach adopted for the present study is an Quantitative research approach and
the design adopted was quasi experimental design. The setting for the study was Crescent Hospital,
Alathur .Palakkad, Kerala. The sample size of the present study was 50, simple random sampling technique was
used to select samples.
Finding: The pre test and post mean value of blood glucose among experimental and control group was
173.2,149.96 &153.16 ,148.92 .The pre test and post mean score of knowledge on practice among
experimental and control group was 11.4,15.08 and 11.04,11.68 .In experimental group the obtained ‘t’value
for the blood glucose was 2.347 and obtained ‘t’value knowledge on practice was 8.742 .The demographic
variables had no association with the post blood glucose value . Conclusion: The present study result shows
the effectiveness of video assisted teaching regarding aerobic exercise in reduction of blood glucose level
among diabetes.

Abstract:
Aim and Objective: Omentin-1 is a novel adipokine expressed in visceral adipose tissue and has been reported
to be negatively associated with insulin resistance (IR). The present study aimed at assessing the correlation of
serum Omentin-1 with IR in Type II Diabetes mellitus patients as measured by Homeostasis Model Assessment
(HOMA-IR). There has been very few reports (if any) from Indian subcontinent on the role of Omentin in IR.
Materials and Methods: It is a case control study, fifty cases of Type II DM patients and equal number of age
and sex matched controls were included in the study. Fasting serum Omentin, fasting serum insulin were
estimated by ELISA using commercially available ELISA kit and fasting blood sugar by glucose oxidase method
by using EM360 autoanalyzer. HOMA-IR score was calculated as HOMA-IR = [Fasting Glucose (mg/dl) ×
Fasting Insulin (µIU/ml)] / 405. Serum Omentin-1 was correlated with FBS, serum insulin and HOMA-IR
using SPSS version-20.
Results: Significant difference was observed in serum Omentin levels between patient and control group( 27.57
± 6.67 vs 49.78 ±4.34ng/ml:p<0.001). Significantly high insulin levels( 33.11 ± 13.75 vs 10.96 ±2.4µIU/ml) and
high HOMA-IR levels (18.58±11.96 vs 2.85 ± 0.85) observed in patients as compared to controls. Serum
Omentin correlated negatively with FBS(p<0.001,r=-.766), insulin(p<0.001,r=-.883) and HOMA-IR(p<0.001,r
= -.810) in patient group.
Conclusion: Omentin-1 level was observed to be significantly low in diabetic patients. The negative correlation
signifies that when IR worsens the level of serum Omentin-1 decreases. Thus this adipokine holds promise as a
therapeutic target in future.

1. Introduction
Type II diabetes mellitus is a quickly growing global metabolic disease characterized by impaired
insulin secretion from pancreatic cells and insulin resistance in liver, muscle and adipose tissue, in the presence
of appropriate environmental factors, particularly those leading to obesity.[1]
The prevalence of diabetes is rising
all over the world due to population growth, aging, urbanisation and an increase in obesity and physical
inactivity. Unlike in the west, where older persons are worst affected, diabetes in Asian countries is
disproportionately high in young to middle-aged adults[2,3]
. This would have long lasting adverse effects on a
nation’s health and economy, especially for developing countries. According to recent estimates by International
Diabetes Federation 2014, approximately 387 million people worldwide ( prevalence rate of 8.3%) in the 20-79
year age group have diabetes in 2014 and by 2035, 592 million people of the adult population is expected to
have diabetes, a jump by 55 percent[4]
. The largest increase will take place in the regions dominated by
developing economies[4]
.The number of people with type II DM is increasing in every country. 77% of people
with diabetes live in middle and low-income countries. The greatest number of people with diabetes are between
40 to 59 years of age[4]
. Estimated global health care expenditure to treat and prevent diabetes and its
complications are expected to a total of at least 548.5 billion US $ in 2013[4]
. Among the top ten countries in the
world by number of people with diabetes in 2013, ages 20 to 79 years, the three countries with the largest
number of people with diabetes are China, India and the U.S [4]
. India is the second largest country in the world
with 65.1 million people affected with diabetes[4]
. Roughly 80% of people with diabetes are in developing
countries, of which India and China share the largest contribution. It is evident from the above figures that DM
is becoming a major public health concern and hence research is important for prevention and treatment of the
disease. Recently, a new protein called Omentin has been identified as a novel depot-specific adipokine in
human adipose tissue, which could have a possible role in modulating insulin action [5]
. The study on Omentin-1
and its relation with type II DM, degree of IR is of recent interest and holds therapeutic promises.

Insulin is a key regulator of glucose homeostasis. Insulin resistance is established by genetic and
environmental factors. Insulin resistance (IR) leads to impaired glucose tolerance, and plays an important
pathophysiological role in the development of diabetes[6]
. The insulin resistance (IR) is defined as a diminished
physiological response of tissues to insulin action, particularly at the level of muscles and adipose tissues with a
consequent compensatory hyperinsulinemia, which initially maintains plasma glucose levels in normal ranges. It
is a hallmark of type II diabetes and cardiovascular diseases, and leads to many of the abnormalities associated
with metabolic syndrome [7],
determination of which has become important at the population level.
The hyperinsulinemic euglycemic clamp is considered to be the gold standard of laboratory methods to
confirm the diagnosis of IR, but it is a complex and invasive technique, not suitable for implementation at the
population level[8]
. Homeostasis model assessment (HOMA-IR) developed by Matthews in 1985 estimated IR
by the homeostasis basal fasting concentrations of glucose and insulin [9]
and has been proven to be a good
equivalent of IR measurements as evidence against clamp euglycemic, the clamp or minimal model
hyperglycaemic in different age groups and even in diabetic patients[10]
. It is therefore, a non-invasive, fast,
inexpensive alternative and reliable way to estimate IR, allowing its applications in large scale epidemiological
studies.
Like an endocrine organ, adipose tissues secretes a variety of adipokines, including leptin,
adiponectin, visfatin, TNF-α and IL-6[11,12]
. These adipokines have widespread effects on carbohydrate and lipid
metabolism and appear to play an important role in the pathogenesis of insulin resistance, diabetes,
atherosclerosis, vascular endothelial dysfunction and inflammation[13-17]
. Recently Omentin, a novel fat depot-
specific adipokine that was identified from a cDNA library from visceral omental adipose tissue by Yang et.al.
in 2003[18]
.The Omentin gene is located in the 1q22-q23 chromosomal region, which has been linked to type II
diabetes in different populations[19-22]
. Omentin m-RNA is predominantly expressed in the stromal vascular
fraction of visceral adipose tissue and is barely detectable in subcutaneous fat depots and mature adipocytes.
There are two highly homologous isoforms of Omentin, Omentin-1 and Omentin-2; Omentin-1 is the major
circulating form in human plasma[23]
.The biological activity of Omentin is not well understood. Recombinant
Omentin enhances insulin-stimulated glucose uptake and Akt phosphorylation in human subcutaneous and
visceral adipocytes in vitro, but has no effect on basal glucose uptake[5]
. Recent studies had shown that
Omentin-I plasma levels and gene expression correlated negatively with obesity and insulin resistance and
positively with adiponectin and HDL (High Density Lipoprotein) levels.[23,24]
Research on Omentin has generated interest in its biological roles in DM with respect to generation of
insulin resistance. As it has been seen that Omentin increases insulin sensitivity, it is supposed to have potential
theraputic role. The research on Omentin is very limited from this part of the world. The present work is taken
up to identify the role of Omentin in insulin resistance which could add to its role in therapeutic importance.
2. Study Design and Methods
2.1 Subjects
A total of 50 male newly diagnosed type II diabetes from 33-68 yrs of age were recruited from the
outpatients attending Endocrinology OPD M.K.C.G Medical College Hospital, Berhampur, Odisha from 1st
January 2013 to 1st may 2014. 50 members of age and sex matched healthy subjects formed the control group.
Diagnoses were based on the diagnostic criteria of World Health Organisation (WHO) 1999[25]
. No subjects had
acute or chronic infectious diseases, hypertension, heart failure, stroke, peripheral vascular diseases, hepatic or
renal disease or cancer; none of the subjects had taken any medicine affecting blood glucose levels. This study
was approved by the institutional ethical committee. Informed consent was obtained from each participant.
2.2 Biochemical measurements
After an 8-12 hr overnight fast, fasting venous blood samples were obtained. Plasma glucose was
measured by glucose oxidase method by using EM 360 autoanalyzer from ERBA Transasia using commercial
kits from Erba diagnostics. Fasting serum Omentin was measured by using commercially available enzyme-
linked immunosorbent assay (ELISA) from EMD Millipore corporation, USA according to manufacture’
s
protocol ( HUMAN OMENTIN-1, Cat # EZHOMNTIN1-29K). Intra-assay coefficient of variation was 1.10%,
inter-assay coefficient of variation was 2.9%. Insulin levels were assessed by using commercially available
ELISA, made by Diagnostic Automation /Cortez Diagnostics, Inc. (Insulin ELISA,Cat #1606Z). A homeostasis
model assessment of insulin resistance was calculated as HOMA-IR= [Fasting Insulin (µIU/ml) X FPG
(mg/dl)] /405.

2.3 Statistical analysis
Data was presented as mean ± SD. The data was analysed by student’s t’ test for unpaired data.
Correlation was derived by Pearson’s correlation analysis. A p-value of < 0.05 was considered significant.
Statistical analysis was done using SPSS version 20 software.
3. Results
3.1 The Clinical and biochemical parameters of cases and controls :
The study was performed with 50 newly diagnosed type II diabetic males and similar number of age
and sex matched control subjects. Table-1 shows the age distribution of controls and cases. The age distribution
of the two groups did not show significant difference. In the present study, 68% of cases (34 out of 50 cases)
belonged to the age-group of 40-54 yrs. While that in the control group was 56% (28 out of 50 controls). The
difference of mean age was insignificant among the two groups (p>0.05).
The biochemical parameters of the cases and control group were shown in Table 2. Fig-1 compares
fasting insulin, serum Omentin, insulin resistance (as measured by HOMA-IR) between control and patient
group. The mean ± SD of FBS level was 207.12 ±55.56 (mg/dl) in cases while that in the control group was
102.20 ± 10.79(mg/dl). The difference of fasting blood sugar between the two groups was significant (t=13.10, p
< 0.001). The mean ± SD of fasting insulin was 33.11 ± 13.75 (µIU/ml) in cases while that in the control group
was 10.96 ±2.48 (µIU/ml). The difference of fasting insulin level between the two groups was significant
(t=11.20, p < 0.001). The mean ± SD of serum Omentin was 27.57 ± 6.67 (ng/ml) in cases while that in the
control group was 49.78 ±4.34 (ng/ml). The difference of fasting Omentin level between the two groups was
significant (t=-19.71, p < 0.001). The mean ± SD of HOMA-IR was 18.58 ± 11.96 in cases while that in the
control group was 2.85 ± 0.85. The difference of insulin resistance as measured by HOMA-IR between the two
groups was significant (t= 9.27, p< 0.001). Thus It was observed that FBS, Fasting Plasma insulin and HOMA-
IR levels were significantly higher (p<0.001) in case group as compared to control group and serum Omentin
was found to be significantly lower in cases as compared to control group with p-value <0.001.
3.2 Correlation of serum Omentin-1 with fasting blood sugar, fasting insulin, insulin resistance as
measured by HOMA- IR in cases and control group :
Table 3 and Fig. 2 show the correlation between serum Omentin with FBS in the cases. Plasma
Omentin correlated negatively with Fasting Blood Sugar in cases (r = -0.766, p <0.001), which was statistically
significant. Thus, it was observed that as the FBS level rises there is significant fall in serum Omentin level.
Table 4 and Fig.3 show the correlation between serum Omentin with FBS in the controls. Serum
Omentin correlated negatively with Fasting Blood Sugar in control group (r = - 0.698, p <0.001) which was
statistically significant. Thus it was observed that serum Omentin level decreased with rise of FBS both in cases
and control group.
Table 5 and Fig.4 show the correlation between plasma Omentin with Fasting insulin in cases. Serum
Omentin correlated negatively with Fasting insulin in cases (r=-0.883, p <0.001) which was statistically
significant. Thus it was observed that serum Omentin level decreased with rise of Fasting insulin in cases.
Table 6 and Fig.5 shows the correlation between serum Omentin with Fasting insulin in controls.
Serum Omentin correlated negatively with Fasting insulin in control group (r=-0.067, p < 0.001) which was
statistically significant. Thus it was observed that serum Omentin level decreased with rise of Fasting insulin in
control group.
Table 7 and Fig. 6 show the correlation between serum Omentin with insulin resistance as measured by
HOMA-IR in patient group. Serum Omentin correlated negatively with HOMA-IR in cases (r=-0.810, p <
0.001) which was statistically significant. Thus it was observed that serum Omentin level decreased with rise of
insulin resistance in diabetic patients.
Table 8 and Fig. 7 show the correlation between plasma Omentin with insulin resistance as measured
by HOMA-IR in control group. Plasma Omentin correlated negatively with HOMA-IR in control (r=-0.544, p
<0.001) which was statistically significant. Thus it was observed that plasma Omentin level decreased with rise
of insulin resistance in control group.

4. Discussion
The current study was conducted in the Department of Biochemistry in collaboration with the
department of Endocrinology, MKCG Medical College and Hospital, Berhampur, Odisha, India to evaluate the
level of plasma Omentin and correlate it with the levels of fasting blood sugar, fasting insulin and insulin
resistance as measured by HOMA-IR in newly detected diabetic patients in south Odisha.
In recent studies, role of various adipokines have been highlighted in the aetiopathogenesis of diabetes
mellitus. Recently, Omentin a novel depot-specific adipokine in human adipose tissue has been identified in last
one and half decade only, which could have a possible role in modulating insulin action[5]
. Omentin was
identified as a 35-kDa protein with 313 amino acids, named after its preferred expression in omental (visceral)
rather than subcutaneous fat tissue[18]
. Omentin, the fat depot-specific protein is synthesized by visceral stromal
cells, but not by adipocytes[18]
. It has been proposed to increase insulin sensitivity in human adipocytes [5]
.There
are two other homologs of Omentin which has been identified, but it is the Omentin -1 which is the major
circulating form of Omentin[ 23]
. In vitro studies have shown that Omentin has an insulin sensitizing effect on
adipocytes in both visceral and subcutaneous adipose depot through increased insulin signal transduction by
activation of Akt/ Protin Kinase B (Akt/pkb)[5]
. It also enhances insulin–stimulated glucose uptake in human
adipocytes[5]
. It has been shown that plasma Omentin levels were inversely correlated with BMI, Waist
circumference, leptin, resistin, insulin and insulin resistance as measured by homeotstatis model assessment and
positively correlated with adiponectin and high-density lipoprotein (HDL-cholesterol) [23,24]
. In vitro studies
have shown that administration of glucose and insulin to human omental adipose tissue resulted in a dose-
dependent reduction of Omentin-1 expression[5]
. Furthermore, prolonged insulin glucose infusion in healthy
individuals significantly decreased the serum Omentin-levels. These findings suggest that Omentin has a
paracrine or endocrine role in modulating insulin sensitivity. There have been very few reports from Indian
subcontinent on the role of Omentin in IR. So the present study may help in assessing the correlation of plasma
Omentin-1 with insulin resistance in Type II DM patients.

The present study included 50 newly diagnosed diabetic male patients and 50 healthy age and sex
matched controls. Various parameters studied among these groups were Fasting blood sugar, Fasting serum
insulin, plasma Omentin and insulin resistance as measured by HOMA-IR.
In the present study the mean ± SD of plasma Omentin level in the patient group was 27.57±6.67 ng/ml
while that in the control group was 49.78±4.34 ng/ml. The difference was significant (p<0.001). There was
significant decrease in plasma Omentin-1 level in cases as compared to control group. Plasma Omentin level
correlated negatively with FBS among patients with a ‘r’ value of -0.766. However the correlation was less
observed among the control group with a ‘r’ value of -0.698.
In a study by Jose maria et al. [26]
, 2010, the level of serum Omentin (ng/ml) among lean, overweight,
obese were 53.7 ± 16.9, 45.2 ± 16.8, 40.1 ± 15.5 respectively. They also found plasma Omentin-1 levels among
normal glucose tolerance group and impaired glucose tolerance group were 47.5 ± 18.1(ng/ml) and 44.1 ±
15(ng/ml) respectively. The serum Omentin-1 levels were found to be significantly low in obese and impaired
glucose tolerance group.
Hong-yan Pan et al. [27]
in the year 2010 had similar serum Omentin-1, among normal glucose tolerance
and impaired glucose tolerance groups with values of 18.85±3.23 ng / ml in the former group and 16.22±4.08 in
the later group. In their study serum Omentin-1 levels correlated negatively with plasma glucose levels. Our
study results had a similar pattern of relation between serum Omentin-1 and fasting Plasma glucose levels.
Similarly Tan et al., 2008a[28]
; Tan et al., 2008b [29]
; Souza –Batista et al., 2007 [23]
; Cai et al., 2009
[30]
;Pan et al., 2010 [31]
; also found in their studies that plasma Omentin-1 levels decreased in diabetic patients
and negatively correlated with plasma glucose levels.
In another study of M. Urbanova, T Dostalova et al. [32]
, it was detected that serum concentration of
Omentin significantly decreased in both obese and type II DM patients relative to control subjects. However,
this group failed to find any significant relationship of baseline serum Omentin-1 to fasting glucose levels.
These results suggest that Omentin is important for glucose metabolism. It has been observed that in
vitro, Omentin increases insulin signal transduction by activating protein kinase B and enhances insulin–
mediated glucose transport in adipocytes. It is possible that the decreased serum Omentin-1 levels observed in
patients with impaired glucose regulation may cause a reduction of insulin stimulated glucose uptake in visceral
and subcutaneous adipocytes or other insulin sensitive tissues; this way contributes to insulin resistance and
development of diabetes.
In the present study the fasting serum insulin level (µU/ml) was found to be high in patient group
(31.11±7.75) as compared to control group (10.96±2.48) and the difference was significant. Thus there was
some degree of insulin resistance in the patient group. Also, the serum Omentin level decreased with the rise in
fasting insulin level both among the controls and patients.
Our data are in concordance with that of previous studies by M. Urbanova et al.[32]
, G. Gursoy et al.[33]
,
Celia M. De Souza Batista et al. [23]
, Bee K. Tan et al. [28]
, Arassh Hossein Nezhad et al.[34]
, where it has been
shown that fasting plasma insulin level correlated negatively with plasma Omentin-1 levels. This finding
suggests that plasma Omentin-1 could play an important role in the pathogenesis of insulin resistance in type II
DM.
In the present study, the insulin resistance as measured by Homostasis model assessment score
(HOMA-IR) was 18.58 ±11.96 in diabetic patients, while in the control group it was 2.85±0.85. And the
different was significant (p<0.001). Plasma Omentin-1 level correlated negatively with HOMA-IR both in
patients and control group, which is more marked in patient group with a ‘r’ value of (-.810) as compared to
control group (‘r’ value of -.544).
M Urbanova, I Dostalova et al. [32]
have found that fasting insulin (µIU/ml) among Type II DM group
was 37.8±4.43 and HOMA-IR index among Type II DM was 11.6±2.19. Our study also corroborates with their
findings with fasting insulin (µIU/ml) and HOMA-IR among patient groups being 33.11±13.75 and 18.58
±11.96 respectively.
It has been reported by Maria Luisa et al. [35]
that the HOMA-IR score as a measure of IR was found to
be 2.04 to 2.33 among healthy adults. G. Gursoy, N. G. Kirnap et al. [33]
reported that the HOMA-IR among
healthy adults was 2.2±0.8. Bee K Tan et al. [28]
reported that it was 1.8 ±0.6. In our study, the HOMA-IR in the
control group was 2.85 ±0.85 which is similar to the findings by the above groups. Serum Omentin was
negatively correlated with fasting insulin level and HOMA-IR both in cases and in patient group. This suggests
that Omentin has a role in insulin resistance. DM is characterised by a state of insulin resistance which was also

found in the present study where the level of fasting insulin was significantly higher among the patient group as
compared to controls.
Thus the present study highlighted the role of Omentin in glucose homeostasis, serum insulin level and
indirect measure of IR as calculated by HOMA-IR. Several adipokines hold promise as therapeutic candidates to
improve IR in type II DM. Omentin discovered only about one and half a decade ago joins the league of
potential therapeutic molecules in the treatment of DM.
5. Summary and Conclusion
The fasting insulin level was found be high among the cases. HOMA-IR score as a measure of insulin
resistance was observed to be high in the cases. Serum Omentin level decreased in TYPE II DM cases. Serum
Omentin correlated negatively with fasting blood sugar, fasting insulin and HOMA-IR in cases. However large
multicentric study regarding the role of Omentin in improving insulin resistance in type II DM is required to
assess its potential role as a therapeutic molecule.
