abnormalities of mineral and bone metabolism develop, which result in altered serum levels of minerals
such as calcium and phosphorus, as well as abnormalities in parathyroid hormone (PTH) or vitamin D
metabolism. Chronic Kidney Disease-Mineral Bone Disease (CKD-MBD) is a serious burden because of
increased cardiovascular mortality thus making therapeutic improvements essential in CKD-MBD. The
present study was aimed at evaluation of pharmacological management of CKD-MBD.
Methods:A retrospective study including 180 patients divided into two groups of 90 each (diabetes
mellitus and non-Diabetes) was performed in the Department of Nephrology, SVIMS, Tirupati. Patients
who were on follow up for at least 3 years (2015-2017) were considered, serum parameters were measured at every six months with a total of 6 visits. First visit was taken as baseline and sixth visit as
conclusion.
Results:The disease incidence of CKD-MBD is more common in male patients i.e. 67.8%. Serum calcium
levels were significantly increased and eGFR was significantly decreased in all patients with CKD at
conclusion compared to baseline.Further, Serum calcium levels were significantly increased at conclusion
in CKD patients without DM and eGFR was significantly decreased at conclusion compared to baseline
in CKD patients with DM. The proportion of untreated patients is high for all the drugs except vitamin D
analogues in both subgroups of CKD patients.
Conclusion:Pharmacological intervention in CKD patients helps in the effective management of mineral
bone disease by maintaining serum calcium, phosphate and calcium phosphorous product status.

Blood glucose (BG) prediction system can help gestational diabetes mellitus (GDM) patient to improve the BG control with managing their dietary intake based on healthy food. Many techniques have been developed to deal with blood glucose prediction, especially those for recommender system. In this study, we conduct a systematic mapping study to investigate recent research about BG prediction in recommender systems. This study describes an overview of research (2014-2018) about BG prediction techniques that has been used for BG recommender system. As results, 25 studies concerning BG prediction in recommender system were selected. We observed that although there is numerous studies published, only a few studies took serious discussion about techniques used to incorporate the BG algorithms. Our result highlighted that only one study discusses hybrid filtering technique in BG recommender system for GDM even though it has an ability to learn from experience and to improve prediction performance. We hope that this study will encourage researchers to consider not only machine learning and artificial intelligent techniques but also hybrid filtering technique for BG recommender system in the future research.

Albuminuria has been recognized as a marker for prognosis of renal and cardiovascular risk in diabetic patients. Role of microalbuminuria in cardiac disease and nephropathy has not been surveyed in Pakistani population and its foretelling importance in diabetic individuals is undetermined. In this study we examined the relation between microalbuminuria, HbA1c and serum albumin levels in association with diabetes in population of Pakistan based on equal number of male and female subjects with and without prevalent baseline diabetes. We found that increased levels of micro albuminuria are associated with cardiovascular disease, HbA1c with nephropathy and serum albumin with cardiovascular disease, nephropathy and hypertension in the diabetic patient.

Diabetic is a well known public health problem of today. There are many risk factors of it, which can be identified in pre-diabetic state. So the present study was conducted with the aim to know the status of anthropometric and haematological parameters in pre-diabetic states. For this hospital based study pre-diabetic subjects were identified from first degree relatives of type 2 DM Patients, enrolled in diabetic research centre P.B.M. hospital Bikaner. Relevant investigations were done. Data thus collected on semi-structured questionnaire and analysed using content analysis. Data analysis revealed that although mean Body Mass Index (BMI) was within normal range but Waist circumference (WC), West Hip (W/H) Ratio, Systolic blood pressure were higher than the normal range accepted for that parameter. But mean value of all the studied haematological parameter were within the normal range accepted for that parameter. So it can be conclude that anthropology of an individual may be associated with the pre-diabetic state. Hypertension was found in 25.35% of pre-diabetics. Further researches are necessary to find out this possible association of anthropologic parameter and pre-diabetic state.
Background: One of the commonest complications of poorly controlled Type 2 diabetes mellitus (T2DM) is Diabetic nephropathy (DN), which occurs in 30-40% of DM cases. It is important to identify the high-risk group who are likely to develop DN with the modifiable and non-modifiable risk factors. This study had the objectives to estimate and correlate the levels of the urine albumin creatinine ratio (UACR) with age, anthropometric measures, glycaemic control markers, lipids, and renal function. To estimate each variable as independent and multivariate risk factors.
Materials and Methods: It was an observational and cross-sectional study conducted in a tertiary care center in Eastern India. Totally, 221 consecutive ambulatory T2DM subjects were recruited after obtaining their written consent.
Results: The diabetics were classified as having diabetic nephropathy by the urine albumin creatinine ratio (ACR) of >30 mg/gm. 53.4% of our study group had DN. There was a significant risk associated with PPBS with p=0.043 (<0.05), serum creatinine with p=0.032 (<0.05), and urine albumin with p=0.0001 (<0.001). In the multivariate regression analysis of all these variables, there was a highly significant likelihood ratio for predicting DN with p=0.0001 (<0.001) with a predictive value of 74.5% in females and 75% in males.
Conclusion: The additive factors contributed by the risk factors in the prediction of DN will benefit the DM in the prevention of DN.

In Pakistan, the overall prevalence of dyslipidemia in adolescents aged 10–18 years is 21.7~25.2%; prevalence is reported to be two times higher (53.1~56.1%) in obese adolescents. However, few studies have been conducted on the relationship between height and blood lipid concentrations in children and adolescents The recent emphasis on treatment of the dyslipidemia of the metabolic syndrome (hypertriglyceridemia, reduced high-density lipoprotein, and increased small, dense low-density lipoprotein particle number) has compelled practitioners to consider lipid-lowering therapy in a greater number of their patients, as one in two individuals over age 50 has the metabolic syndrome. Individuals with the metabolic syndrome typically have normal low-density lipoprotein cholesterol levels, and current lipid-lowering guidelines may underestimate their cardiovascular risk. Two subgroups of patients with the metabolic syndrome are at particularly high risk for premature CAD. One, individuals with type 2 diabetes, accounts for 20-30% of early cardiovascular disease. The second, familial combined hyperlipidemia, accounts for an additional 10-20% of premature CAD. Familial combined hyperlipidemia is characterized by the metabolic syndrome in addition to a disproportionate elevation of apolipoprotein B levels. The measurement of fasting glucose and apolipoprotein B, in addition to the fasting lipid profile, can help to estimate CAD risk in patients with the metabolic syndrome. In this research we compared allopathic medication and medicinal herb in treating hyperlipidemia.

Objective: This study aimed to evaluate the prevalence and visual impact of Diabetic Retinopathy (DR) among individuals with Type 2 Diabetes (T2D) in Hazara, Pakistan. Methods: A cross-sectional study was conducted from May to August 2023. The sample consisted of 1332 patients who attended the Outpatient Department for eye examination, with 133 (10%) identified as diabetics. Parameters such as glycemic control, HbA1C levels, comorbidities, family history, medication, lifestyle factors, and ocular manifestations were analyzed. Results: The study indicated that 73.01% of diabetic patients had uncontrolled glycemic levels. The prevalence of refractive errors was high (84.12%), and the incidence of DR was significant, with 6.34% having proliferative DR. The findings also emphasized lifestyle factors, including screen usage and spectacle usage patterns. In addition, weight-height proportions and a family history of diabetes were associated with the incidence of DR. Conclusion: The high prevalence of uncontrolled diabetes and significant incidence of DR underscores the urgent need for improved diabetes management and regular screenings for early detection of DR. The results advocate for prioritizing regular health checkups, enhancing public health strategies, and improving accessibility to healthcare facilities, particularly in rural regions.
Diabetes is a rapidly and serious health problem in Pakistan. This chronic condition is associated with serious long-term complications, including higher risk of heart disease and stroke. Aggressive treatment of hypertension and hyperlipideamia can result in a substantial reduction in cardiovascular events in patients with diabetes 1. Consequently pharmacist-led diabetes cardiovascular risk (DCVR) clinics have been established in both primary and secondary care sites in NHS Lothian during the past five years. An audit of the pharmaceutical care delivery at the clinics was conducted in order to evaluate practice and to standardize the pharmacists’ documentation of outcomes. Pharmaceutical care issues (PCI) and patient details were collected both prospectively and retrospectively from three DCVR clinics. The PCI`s were categorized according to a triangularised system consisting of multiple categories. These were ‘checks’, ‘changes’ (‘change in drug therapy process’ and ‘change in drug therapy’), ‘drug therapy problems’ and ‘quality assurance descriptors’ (‘timer perspective’ and ‘degree of change’). A verified medication assessment tool (MAT) for patients with chronic cardiovascular disease was applied to the patients from one of the clinics. The tool was used to quantify PCI`s and pharmacist actions that were centered on implementing or enforcing clinical guideline standards. A database was developed to be used as an assessment tool and to standardize the documentation of achievement of outcomes. Feedback on the audit of the pharmaceutical care delivery and the database was received from the DCVR clinic pharmacist at a focus group meeting.

Diabetes mellitus is taken from the Greek word diabetes, meaning siphon - to pass through and the Latin word mellitus meaning sweet. A review of the history shows that the term "diabetes" was first used by Apollonius of Memphis around 250 to 300 BC. Ancient Greek, Indian, and Egyptian civilizations discovered the sweet nature of urine in this condition, and hence the propagation of the word Diabetes Mellitus came into being. Mering and Minkowski, in 1889, discovered the role of the pancreas in the pathogenesis of diabetes. In 1922 Banting, Best, and Collip purified the hormone insulin from the pancreas of cows at the University of Toronto, leading to the availability of an effective treatment for diabetes in 1922. Over the years, exceptional work has taken place, and multiple discoveries, as well as management strategies, have been created to tackle this growing problem. Unfortunately, even today, diabetes is one of the most common chronic diseases in the country and worldwide. In the US, it remains as the seventh leading cause of death.

T1DM presents in children or adolescents, while T2DM is thought to affect middle-aged and older adults who have prolonged hyperglycemia due to poor lifestyle and dietary choices. The pathogenesis for T1DM and T2DM is drastically different, and therefore each type has various etiologies, presentations, and treatments.
Abstract— Diabetes Mellitus (DM) is a chronic and progressive condition with a hereditary predisposition which is further induced by unhealthy lifestyle. It is a silent killer with cardiovascular complications being most common cause of morbidity and mortality in patients with T2DM.
Objective: To find out association of socio demographic and clinical parameters of diabetes type 2 with hypertension and dyslipidemia among Diabetes Mellitus type 2 cases aged 18 to 70 years. Methodology: A cross sectional hospital based study was conducted on 272 type 2 DM patients attending Department of Medicine in a secondary care referral hospital after taking consent.
Results: Hypertension was present in 192 (70.59%) and dyslipidemia was present in 93 (34.19%) of type 2 diabetes patients. Age, family income, presence of family history, duration of illness, type of treatment, consumption of alcohol , BMI, Hba1c level were found to be associated significantly with both hypertension and dyslipidemia in type 2 DM patients.

Abstract:
Back ground: Diabetes mellitus is a metabolic disorder characterized clinically by the presence of features
such as polyuria, polydypsia and polyphagia. Patients with this problem are at increased risk for the
development of specific complications including retinopathy, nephropathy, neuropathy and atherosclerosis .
Microalbuminuria and serum creatinine has emerged as a important indicators to diagnose the high renal risk
.The scope of this study is to observe levels of microalbuminuria and Serum Creatinine in the Diabetic and Non
diabetic group.
Materials and Methods:
The comparative study was conducted between diabetes and non diabetic group with 30 participants in each
group at Maharajah’s Institute of Medical Sciences , Nellimarla during the period from April 2012 to March
2013. Estimation of serum creatinine was done by Jaffe’s method and estimation of micro protein by Pyrogallol
Red method.
Results
Diabetic group has more mean serum creatinine values i.e.1.20 ±0.78 mg/dL when compared to non diabetic
group 0.94±0.15 mg/dL which is not statistically significant. The mean urine micro albumin values in Diabetics
was higher i.e.76.26±96.31 when compared to the Non Diabetic group 22.26±4.40 and this difference is
statistically significant.

I. Introduction:
Diabetes mellitus is a metabolic disorder involving carbohydrate, lipid and protein metabolism
resulting from defects in insulin secretion, insulin action or both1
in which glucose is underutilized, producing
hyperglycemia and is characterized clinically by the presence of features such as polyuria, polydypsia and
polyphagia2
. As the disease progresses, patients are at increased risk for the development of specific
complications including retinopathy, nephropathy, neuropathy and atherosclerosis3
. Albumin is a protein, which
is present in blood. The kidneys act as a filter for waste products in the blood but protein is not allowed to spill
over into the urine unless the filter system is leaky4
.The term microalbuminuria refers to urinary excretion of
albumin, a major component of serum proteins, in micro quantities within the range of 30 –300 mg/24 hrs.
Serum creatinine is another important indicator of renal function .Evidence from large prospective studies
suggest that, diabetes mellitus is currently a leading cause of end-stage renal disease5
. In the quest for new
clinical tools that enable the identification of patients at high renal risk, Microalbuminuria and serum creatinine
has emerged as a important indicators . The scope of this study is to observe levels of microalbuminuria and

Selection of groups
Two groups of subjects were included in the study. In both the two groups, ages of the subjects were
from 45 years to 65 years. 60cases were taken up for the study and divided into two groups, each comprising of
30cases. First group included the subjects who were healthy and non-diabetic and the second group included the
subjects of Type 2 diabetes mellitus.
1. Non Diabetics
This group included completely healthy subjects (n=30) of either sex, who were not suffering from
Type 2 diabetes mellitus and came as attendants of the patients admitted to Department of General Medicine . A
detailed clinical examination was performed upon them with reference to age and gender. The individuals with
conditions, likely to alter the albumin level in urine such as inflammatory conditions (sepsis, trauma, acute
pancreatitis), pregnancy, and hypertension were excluded from the study.
2.Patients (type 2 Diabeted Milletes)
This group of subjects (n=30) was selected from persons of either sex of Type 2 DM. They were
diagnosed as type 2 DM on the basis of history, clinical features and laboratory investigations (FBS & PPBS).
Patients who have inflammatory conditions (sepsis, trauma, acute pancreatitis), pregnancy and hypertension are
excluded.
Estimation of serum creatinine by Jaffe’s method6
.
Estimation of micro protein by Pyrogallol Red method, End point7,8
.
III. Results:
In the present study the mean age of Diabetic was 52 ± 5 years and the mean age of Non Diabetics was
53± 5 years.

The mean values Fasting Blood Sugar of diabetic group is 152±19 and non diabetic group is 81±12 and
the difference is found to be statistically significant with Z value 17.13 and p <0.001.
The mean values PPBS of diabetic group is 226.33±18.46 and non diabetic group is 110.43±15.21 and
the difference is found to be statistically significant with Z value 26.53 and p <0.001.

The proportion of people showing higher serum creatinine values are more in diabetic group than non
diabetic group .

Table 2, shows that the mean serum creatinine value in diabetic group was higher than non diabetic
group but the difference was statistically not significant.

The higher Urinary micro albumin values are found in Diabetics than Non Diabetics. The mean urine
microalbumin value registered in Diabetics was higher when compared to the Non Diabetic group and this
difference is statistically significant. (Table 3).

In Non Diabetic groups all are normoalbuminuria, whereas in Diabetic microalbuminuria accounts for
27% and in macroalbuminuria accounted for 6%.(Table 4).
IV. Discussion:
Many studies showed that people with poor glycemic control have higher levels of microalbuminuria.
Nelson RG in his study on 2728 Pima Indians found that excessive albumin excretion was present in
8% of subjects with normal glucose tolerance, 15% of those with impaired glucose tolerance, and 47% of
subjects with diabetes. The intermediate prevalence of abnormal albuminuria in those with impaired glucose
tolerance suggests that hyperglycemia even at levels below those diagnostic of diabetes is associated with renal
abnormalities in some subjects and that these abnormalities may precede the onset of diabetes. Abnormal
albuminuria at levels not reliably detected by the usual dipstick methods was commonly observed in Pima
Indians with diabetes, even those with diabetes of recent onset. Associations were found with age, duration of
diabetes, level of glycemia, blood pressure, and treatment with insulin. 9
Haffner SM measured lipids, lipoproteins, Lp(a), blood pressure, and albumin excretion in 234 subjects
with NIDDM from the San Antonio Heart Study found that Seventy-two subjects had microalbuminuria (> or =
30 mg/dl). 10
Wagle T.J in his comparative study of serum sugar and creatinine levels in male and female type 2
diabetic patients observed that blood glucose and serum creatinine concentrations are elevated in type 2 diabetic
patients compared with non-diabetic male and female. Male diabetic patients were found to present significantly
higher serum creatinine level than females. 11
Blessing O. Idonije showed that elevated blood sugar level in type 2 diabetes mellitus, plasma
creatinine and urea concentration are significantly increased in diabetics compared with their levels in
apparently healthy non-diabetics 12.
V. Conclusion:
The findings from our study showed that serum sugar ,creatinine ,albumin levels were higher in the
diabetic patients compared with non-diabetic people. Albumin levels are significantly higher in diabetic group
which shows that it can be used as a good tool for screening in preventing renal complications.
