Selgin Tablets (Generic Selegiline Hydrochloride Tablets)

Selgin Tablets (Generic Selegiline Hydrochloride Tablets) are
used to treat movement disorders due to Parkinson's
disease. It does not cure Parkinson's disease, but it could
improve shakiness (tremor), muscle stiffness, loss of normal
movement as your dose of other Parkinson's medication
wears off (end-of-dose failure), and sudden switching
between normal movement and stiffness ("on-off" problems).
It may improve the range of motion and ability to walk, dress,
and exercise. Selegiline is used in combination with Levodopa
or Levodopa and Carbidopa combination to treat Parkinson's
disease (sometimes called "shaking palsy" or "paralysis
agitans").

Active Ingredient
The active ingredient present in Selgin
tablets is Selegiline Hydrochloride.
Each uncoated tablet of Selgin contains 5 mg
of Selegiline Hydrochloride.

Selegiline Hydrochloride Tablets – Brand Names
Selegiline Hydrochloride tablets are
available under various drug trade names
that include Elegelin, Selerin, Eldepryl,
Jumax, Jumex, Selgin and Selerin.

Uses of Selgin Tablets
Selgin Tablets (Generic Selegiline Hydrochloride Tablets) is used
for the treatment of Parkinson's disease, or symptomatic
parkinsonism. It may be used alone in early Parkinson's disease for
symptomatic relief to delay the need for levodopa (with or without
decarboxylase inhibitor) or as an adjunct to Levodopa (with or
without decarboxylase inhibitor).
Selegiline in combination with maximal Levodopa therapy is
indicated particularly in patients who experience fluctuations in
their condition such as 'end-dose' type fluctuations, 'on-off'
symptoms or other dyskinesias.

Selgin Tablets Dosage
The dosage of Selgin tablets (Generic Selegiline Hydrochloride
Tablets) is given below:
The recommended dosage is 10 mg per day administered as divided
doses of 5 mg each taken at breakfast and lunch.
There is no evidence that additional benefit will be obtained from the
administration of higher doses. Moreover, higher doses should
ordinarily be avoided because of the increased risk of side effects.
After two to three days of treatment with Selegiline tablets, an attempt
may be made to reduce the dose of Levodopa / Carbidopa. A reduction
of 10% to 30% was achieved with the typical participant in the domestic
placebo controlled trials.

Contraindications
Selgin tablets are contraindicated in patients with a hypersensitivity to
Selegiline Hydrochloride or any other component of this medication.
Generic Selegiline HCl Tablets (Selgin) is contraindicated in patients
using the following medicines: Meperidine, Tramadol, Methadone,
Propoxyphene, Mao Inhibitors Including Selective Mao-B Inhibitors,
Dextromethorphan, St. John's Wort, and Cyclobenzaprine.
Pethidine is contraindicated in combination with MAOIs like Selgin
because of reports of serious, sometimes fatal, reactions in patients
receiving this combination and in patients who have discontinued MAOI
therapy and are then started on Pethidine within two weeks of
discontinuation of therapy.

Contraindications
Selegiline Hydrochloride tablets (Selgin) are contraindicated in
patients using Selective Serotonin Reuptake Inhibitors (SSRIs)
or within five weeks of stopping a SSRI.
There have been reports of serious, sometimes fatal, reactions
in patients that have been administered SSRIs in combination
with Selegiline tablets and in patients who have recently
discontinued SSRIs and are then started on a MAOI. At
least five weeks should be allowed between
discontinuation of, for example, Fluoxetine, Sertraline or
Paroxetine and starting treatment with Selegiline tablets.

Storage Instructions
Selgin tablets (Generic Selegiline
Hydrochloride tablets) have to be stored at
controlled room temperature i.e. from 20°C to
25°C (68°F to 77°F) with excursions
permitted between 15° to 30°C (59° to 86°F).
Keep this as well as all other medicines away
from children and pets.

Warnings
Generic Selegiline Tablets (Selgin) can cause hypertension above 2.5
mg/day. The recommended dose of 10 mg /day should not be
exceeded because of the risks associated with non-selective inhibition
of MAO i.e. 'cheese reactions'.
This medicine may cause Serotonin syndrome when used with
antidepressants ,cause falling asleep during activities of daily living,
cause hypotension/orthostatic hypotension, cause or exacerbate
dyskinesia, cause hallucinations and psychotic-like behavior, cause
irritation of the buccal mucosa, Increased risk of melanoma, Increased
risk for phenylketonuric patients and cause problems with impulse control
and compulsive behaviors.
Abrupt discontinuation of this medicine may cause hyperpyrexia and
confusion.

Warnings
Some patients on adjunctive therapy may experience exacerbation
of Levodopa-associated side effects, presumably due to the increased
amounts of dopamine reacting with super-sensitive postsynaptic
receptors. These effects may often be mitigated by reducing the dose of
Levodopa by approximately 10% to 30%.
Selegiline should be used cautiously in patients with severe liver or
kidney dysfunction, as there is no pharmacokinetic information
available on selegiline or it's metabolites in these patients.
Selegiline should be given cautiously to patients having peptic or
duodenal ulcer, labile hypertension, cardiac arrhythmias, severe
angina pectoris, or psychosis as Selegiline may exacerbate these
conditions.

Warnings
Transient increases in liver transaminases have been reported during
selegiline treatment.
Parkinson's disease patients treated with dopamine agonists and other
dopaminergic treatments such as Selegiline have been reported as
exhibiting impulse control disorders and compulsions like pathalogical
gambling, increased libido, hypersexuality, binge eating, shopping and
different kinds of compulsive/repetitive activities (punding).
Treatment with heterogenous serotonergic drugs in patients primarily with
psychiatric illness taken alone or in combination with other drugs such as
MAOIs has been uncommonly associated with symptoms of, myoclonus,
tremor, confusion, restlessness, ataxia and hyperreflexia. While usually short-
lived, it can lead to intensive care admissions and is potentially fatal.

Warnings
Severe CNS toxicity has been reported in patients when a
combination of tricyclic antidepressants and Selgin tablets (Generic
Selegiline Hydrochloride tablets) have been used.
Concomitant use of oral contraceptives (tablets containing the
combination of Gestodene / Ethinyl Estradiol or Levonorgestrel /
Ethinyl Estradiol) and Selegiline may cause an increase in the oral
bioavailability of Selegiline.
Foods containing various exogenous amines such as tyramine have not
been reported to induce hypertensive reactions during Selegiline
treatment at doses used in the treatment of Parkinson's disease.

Selgin Tablets Side Effects
Side effects of Selegiline Hydrochloride Tablets (Selgin) with an
incidence at least 3% greater than on placebo are constipation, skin
disorders, vomiting, dizziness, dyskinesia, insomnia, dyspnea, myalgia,
and rash.
Due to the fact that Selegiline tablets potentiates the clinical efficacy
of Levodopa, the adverse effects of Levodopa, e.g. abnormal
movements such as dyskinesias, nausea, agitation, confusion,
hallucinations, headache, postural hypotension, cardiac arrhythmias
and vertigo, may worsen when Selegiline is added to the Maximum
tolerated Levodopa dose. Micturition difficulties and skin reactions
have also been observed during therapy with Selegiline HCl tablets.

Selgin Tablets Overdose
In cases of overdose with Selegiline, dietary tyramine restriction must be
observed for several weeks to avoid the risk of a hypertensive reaction.
Signs and symptoms of overdosage of of non-selective MAO inhibitor
could include, alone or in combination, any of the following symptoms:
drowsiness, dizziness, faintness, irritability, hyperactivity, agitation,
severe headache, hallucinations, trismus, opisthotonos, convulsions, and
coma; rapid and irregular pulse, hypertension, hypotension and vascular
collapse; precordial pain, respiratory depression and failure,
hyperpyrexia, diaphoresis, and cool, clammy skin.

Treatment of Selgin Tablets Overdose
There is no specific antidote to treat overdose with non-selective
MAO inhibitors like Selegiline, and the treatment is symptomatic and
supportive. Induction of emesis or gastric lavage with instillation of
charcoal slurry may be helpful in early poisoning, provided the airway
has been protected against aspiration.

Selgin Tablets During Pregnancy
Selegiline Hydrochloride (Selgin tablets) has been classified by the US FDA
as Pregnancy Category C.
There are no adequate and well-controlled studies conducted with this
medicine in pregnant women. During animal studies, administration of
Selegiline during pregnancy was associated with developmental toxicity
which included decreased embryo-fetal and postnatal offspring growth and
survival at doses greater than those used clinically.
There are no adequate and well-controlled studies in pregnant women.
Selgin tablets can be used during pregnancy only if the potential benefit
justifies the potential risk to the fetus.
