SBIRT in Schools
• With grant support from the Conrad N. Hilton
Foundation, C4 and YPR are developing and
evaluating new approaches to support youth who
screen as low to moderate risk for substance use.
• C4 will partner with six communities to implement
Screening, Brief Intervention, and Referral to
Treatment (SBIRT) and link adolescents to a brief
mentoring intervention, with the goal of enhancing
positive social supports and other protective
factors.
Benefits of Being a Project
Amp Pilot Site
• Increase capacity for youth SBIRT, including training
and technical assistance resources
• Serve as a demonstration site for a cutting-edge
youth peer prevention program
• Connect with a national community of pilot site
providers and peers
• Contribute to the emerging evidence base on
youth SBIRT and peer supports
• Test out approaches that align with new integration
standards under the Affordable Care Act
• Receive a sub-grant to support implementation
costs

Brief Intervention
Brief BI provided to all, setting staff
will inform adolescent that they may
be contacted to participate in new
program, then contact parent for
permission to contact/ informed
consent for Project Amp.
No Referral
Permission to Contact
Site supervisor or designee contact parent for permission to contact/ informed
consent for Project Amp in person or via email/phone (screen results remain
confidential).
Adolescent Enrollment
Mentor to contact adolescent (via brief in-person or electronic) within 24 hours
of permission to contact to coordinate first session and request:

1. Informed Consent
2. Baseline Data
3. Interest/Goal Generation
Hand-off to Project Amp Coordinator
Site Supervisor or designee provides contact information (follow-up within 24
hours of screen). Coordinator identifies available mentor and provides contact
information.
Brief Peer Mentorship (4 one hour sessions over 2 months)
Peer Mentor meets face-to-face with adolescent to achieve the following* :
· Session 1: Get to Know You and Goal Setting
o Describe parameters; answer questions
o Develop goal action plan; discuss how activities align with goals
o Make referral connections as appropriate (use Asset Map tool)
· Session 2: Wellness
o Discuss five components of wellness: centering, compassion,
community, confidence, commitment, and link to goals
o Follow-up on referrals
· Session 3: Social Supports
o Conversational discussion on social influences
o Discuss older influences and interest in including parent or adult
natural mentor in final meeting
o Follow-up on referrals
· Session 4: Wrap-up and Long-term Plan
o Discuss strategies for finding resources that meet needs/interests
o Discuss follow-up obligations for study
Group Activity Connection
Refer to group activity that exists in community and is aligned with interests
that can serve as positive social support beyond Project Amp. May occur at
anytime in 4 weeks. Mentor to encourage adolescent to attend and discuss
what they thought (mentor to attend with adolescent as appropriate).

Looking ahead
• While the pilot phase of this study will engage
approximately 20 adolescents at each site, our goal
is to establish an SBIRT-based, peer-delivered brief
intervention approach that can be widely
replicated.
• The intervention design calls for the engagement of
a broad range of prevention, treatment, and
recovery stakeholders, which strengthens a
community’s capacity to prevent problem
substance use.
• Mentors:
o Kennesaw State University Collegiate
Recovery Program
o Trained in Motivational Interviewing,
ethical practices and prevention science
o Use their lived experience
o Focus on wellness

SBIRT in Schools

  • 2. • With grant support from the Conrad N. Hilton Foundation, C4 and YPR are developing and evaluating new approaches to support youth who screen as low to moderate risk for substance use. • C4 will partner with six communities to implement Screening, Brief Intervention, and Referral to Treatment (SBIRT) and link adolescents to a brief mentoring intervention, with the goal of enhancing positive social supports and other protective factors.
  • 3. Benefits of Being a Project Amp Pilot Site • Increase capacity for youth SBIRT, including training and technical assistance resources • Serve as a demonstration site for a cutting-edge youth peer prevention program • Connect with a national community of pilot site providers and peers • Contribute to the emerging evidence base on youth SBIRT and peer supports • Test out approaches that align with new integration standards under the Affordable Care Act • Receive a sub-grant to support implementation costs
  • 4. Screening (SBIRT)* * Screening, Brief Intervention, Referral to Treatment (SBIRT) * * CRAFFT, a six-question questionnaire, is the recommended tool. We will work with sites already using a different screening to identify the appropriate population for the Brief Intervention/ Referral to Treatment (SBIRT) Project Amp (Referral to Extended BI)* * The sequence of handoffs, sessions, and other components are flexible based on Adolescent Screened Low Risk (CRAFFT* * negative) Moderate Risk (CRAFFT positive; score 0-1) High Risk (CRAFFT positive; score >1) Referral to Treatment Follow SBIRT referral procedures of setting; assist in identifying and implementing referral practices for setting new to referral, guided by SBIRT best practices Brief Intervention Brief BI provided to all, setting staff will inform adolescent that they may be contacted to participate in new program, then contact parent for permission to contact/ informed consent for Project Amp. No Referral Permission to Contact Site supervisor or designee contact parent for permission to contact/ informed consent for Project Amp in person or via email/phone (screen results remain confidential). Adolescent Enrollment Mentor to contact adolescent (via brief in-person or electronic) within 24 hours of permission to contact to coordinate first session and request: . Request the following via online portal: 1. Informed Consent 2. Baseline Data 3. Interest/Goal Generation Hand-off to Project Amp Coordinator Site Supervisor or designee provides contact information (follow-up within 24 hours of screen). Coordinator identifies available mentor and provides contact information. Brief Peer Mentorship (4 one hour sessions over 2 months) Peer Mentor meets face-to-face with adolescent to achieve the following* : · Session 1: Get to Know You and Goal Setting o Describe parameters; answer questions o Develop goal action plan; discuss how activities align with goals o Make referral connections as appropriate (use Asset Map tool) · Session 2: Wellness o Discuss five components of wellness: centering, compassion, community, confidence, commitment, and link to goals o Follow-up on referrals · Session 3: Social Supports o Conversational discussion on social influences o Discuss older influences and interest in including parent or adult natural mentor in final meeting o Follow-up on referrals · Session 4: Wrap-up and Long-term Plan o Discuss strategies for finding resources that meet needs/interests o Discuss follow-up obligations for study Group Activity Connection Refer to group activity that exists in community and is aligned with interests that can serve as positive social support beyond Project Amp. May occur at anytime in 4 weeks. Mentor to encourage adolescent to attend and discuss what they thought (mentor to attend with adolescent as appropriate). Project Amp: Amplifying our Futures Screening and Program Design
  • 5. Looking ahead • While the pilot phase of this study will engage approximately 20 adolescents at each site, our goal is to establish an SBIRT-based, peer-delivered brief intervention approach that can be widely replicated. • The intervention design calls for the engagement of a broad range of prevention, treatment, and recovery stakeholders, which strengthens a community’s capacity to prevent problem substance use.
  • 6. • Mentors: o Kennesaw State University Collegiate Recovery Program o Trained in Motivational Interviewing, ethical practices and prevention science o Use their lived experience o Focus on wellness
