Adjuvant chemoradiotherapy after surgery has shown improved survival outcomes over surgery alone in retrospective studies, with a 30% reduction in mortality seen with the addition of chemoradiotherapy. High-risk patients such as those with node-positive disease or margins positive for tumor have been shown to benefit most from adjuvant chemoradiotherapy. For advanced or unresectable gallbladder cancer, chemotherapy with gemcitabine-based regimens represents the standard of care.

This study evaluated the feasibility of using deep inspiration breath-hold (DIBH) technique with AlignRT InBore for lung stereotactic body radiotherapy (SBRT) treatments on the Halcyon system. Thirteen patients underwent lung SBRT in DIBH, which reduced intrafraction tumor motion to less than 2 mm/° compared to free breathing. DIBH with AlignRT InBore also decreased planning target volumes by an average of 33% and mean lung doses by 35% on average compared to free breathing. Real-time position monitoring with AlignRT InBore showed high correlation and reproducibility between CBCT and breath hold position. Doğum yapmış kadınların %40'ında dereceden bağımsız pelvik organ prolapsusu (POP) görülmektedir. Dokuz kadından biri POP nedeni ile cerrahiye ihtiyaç duymakta, cerrahi geçirenlerin de %25'i maalesef ileriki dönemde tekrar cerrahiye gereksinim duymaktadır. Vestibüler schwannoma varlığında petrous kemik anatomisindeki topografik değişiklikler ve retrosigmoid transmeatal yaklaşıma etkileri
Transmeatal drilling(oymak-delmek) işlemi erken veya geç işitme kaybına yol açabilecek olan iç kulak yapılarına hasar verme riski taşır.
Çalışmanın amacı tümörün petrous kemik anatomisinde yaptığı değişiklikleri tanımlamak ve endolenfatik sistem hasarının hem risk hem de insidansını analiz etmek.
Bu prospektif çalışmaya vestibüler schwannoma ameliyatı olmuş 100 hasta dahil edildi. Ameliyat öncesi ve sonrası bilgisayarlı tomografi uygulandı ve hem patolojik hem sağlıklı alanların topografik ölçüleri değerlendirildi. Postoperatif anatomik ve fonksiyonel değerler ölçüldü.
Sonuçlarda iç kulak yolunun çapı petrous kemiğin etkilenen alanlarında karşı taraftaki sağlıklı alanlara göre daha büyüktü. Vestibüler kanalın hasar oranı artan tümör büyüklüğü ile beraber arttı.

Sigara kullanımının çok sayıda kronik hastalıkla ilişkisi olduğu bilinmektedir.
Aynı zamanda sigara içen hastalarda post-op ilk 30 günde enfeksiyonlar dahil olmak üzere pek çok olumsuz sonuçla karşılaşılmaktadır.Bu durum preoperatif dönemde sigarayı bırakmanın perioperatif ve postoperatif komplikasyonları azaltacağını öngörebiliriz.

4. ÖZET
Amaç
BOS şantları, psödotümör serebri semptomlarını postoperatif
dönemde efektif bir şekilde geriletir fakat, uzun dönem
sonuçları araştırılmamıştır.Lumboperitoneal şantlar (LP)
psödotümör serebrinin BOS şantlarının dayanak noktasıdır
ancak; stereotaksi ve nöroendoskopi kılavuz görüntüleri, bu
vakalarda, ventrikülomegali olmadan ventriküler kateter
yerleştirilmesine izin verir. BOS şantlarının psödotümör
serebride uzun dönem rahatlama sağlayıp sağlamadığı ve
ventriküler şantın LP şanttan daha iyi olup olmadığı
bilinmezliğini sürdürmektedir.
Metodlar
Yazarlar 1973-2003 yılları arasında, bir enstitüde psödotümör
serebrinin yol açtığı inatçı başağrılarında şant yerleştirme
prosedürlerinin bütün kayıtlarını incelediler.
5. 42 hasta 115 şant yerleştirme prosedürüne katıldı; 79unda LP
şant, 36sında VP ya da VAT şant kullanıldı.
Şant yerleştirildikten sonra 40 hastada anlamlı başağrısı gelişti.
Papilödemsiz 17 hasta ve preoperatif semptomları iki yıldan
fazla devam eden 19 kişi rekürren başağrısı yaşadı. Papilödem
ya da uzun dönem semptomları rekürren başağrısı riskini 5 kat
artırır.
VP ya da VAT şantların aksine LP şantlar; şant revizyon
riskinde 2.5 kat, şant obstrüksiyon riskinde 3 kat artışla ilişkilidir.
Fakat iki şant tipi arasında overdrenaj, distal kateter göçü, şant
enfeksiyon riskleri benzerdir.
Çıkarımlar
Bu hastaların tedavisindeki 30 yıllık deneyimlerine dayanarak
yazarlar, BOS şantlarının inatçı başağrısıyla ilişkili psödotümör
serebrinin akut tedavisinde son derece etkili olduğunu,
hastaların çoğunda uzun dönem rahatlama sağladığını buldular.
6. Papilödemin olmaması ve uzun süren semptomlar tedavi
başarısızlığı açısından risk faktörüdür. Ventriküler şantların
kullanımı şant obstrüksiyonu ve revizyon açısından LP
şantlardan daha düşük risklidir.

GİRİŞ
Psödotümör serebride (benign intrakranial hipertansiyon olarak
da bilinir) net bir patofizyoloji ortaya konamadığında, bu
kompleks hastalıkta uygun ve en etkili tedavi stratejisi
bilinmeyen olmaya devam etmektedir.
Halen tercih edilen tedavi medikaldir; BOS şantları genellikle
medikal tedavi başarısızlığında ya da ciddi ve akut görme kaybı
geliştiğinde kullanılır.
Son günlerde, literatürün büyük bir kısmı, psödotümör
serebrinin altında yatan nedenin artmış BOS volümü olduğu
fikrini desteklemektedir. Bundan dolayı BOS şantı yerleştirme,
değişen BOS volümünde en etkili ve en rasyonel tedavi şeklidir.
Çalışmalar şant yerleştirmenin semptomlarda hızlı gerilemeyi
sağladığını göstermiştir; fakat bu etkinin sürekliliği postoperatif
dönemde henüz araştırılmamıştır. Çoğu yazar, şant
başarısızlığı ve revizyon oranlarının şant yerleştirmeden aylaryıllar sonra ortaya çıktığını göstermiştir.
8. BOS şantlarının medikal tedaviye refrakter olgularda, uzun
dönem semptomlarda rahatlama sağlayıp sağlamadığı hala
bilinmemektedir.
45 yıldır psödotümör serebri için BOS şantlarının dayanak
noktası LP (Lumboperitoneal) şanttır. Fakat bu şant tipi, erişkin
ve pediatrik hidrosefali vakalarında yüksek revizyon oranlarıyla
ilişkilidir.
Ventriküler şantlar peritona ya da atriuma yerleştirilir; bu
prosedür, ventriküler kateteri yerleştirmenin zorluğundan dolayı,
daha az sıklıkla kullanılır.
The Johns Hopkins Hospital’da 30 yıllık bir periyodu kapsayan
yapılan çalışma bugüne kadar yapılan en kapsamlı çalışmadır.
Yapılan çalışmalarda, psödotümör serebrinin yol açtığı inatçı
başağrılarında (medikal tedaviye dirençli vakalarda) BOS
şantlarının uzun dönem klinik etkisi gösterilmiştir ve lumbar ve
ventriküler şantların revizyon ve komplikasyon oranları
karşılaştırılmıştır.
9. KLİNİK MATERYAL VE METODLAR
Klinik Tanı ve Tedavi Algoritması
1973-2003 yılları arasında, psödotümör serebri tanılı 42 hasta
The Johns Hopkins Hospital’a, Friedman ve arkadaşları
tarafından tanımlanan ciddi zayıflatıcı başağrısının yönetimi için
başvurdu:
1) Generalize intrakranial hipertansiyonla tutarlı semptomatoloji
ya da belirlenmiş papilödem
2) Lateral dekübit pozisyonda ölçülen intrakranial basınçta 25
cmH2O üzerinde yükselme
3) Normal BOS kompozisyonu
4) Kanıtlanamamış hidrosefali ya da kitle, MR görüntülemede
yapısal ya da vasküler lezyon
5) İntrakranial hipertansiyonun diğer sistemik nedenlerinin
tanımlanamaması
10. Psödotümör serebri kriterlerini karşılayan hastalara şant
yerleştirme prosedürleri, son 6 ayda uygulanan medikal tedavi
(BOS inhibitörleri, steroidler, ağrı kesiciler-narkotikler,
nortriptilin, neurontin gibi-) başarısız olduğunda uygulandı.
Çerçevesiz stereotaksik görüntüleme sisteminin kullanımı; bir
küçük-kalibreli çubuk bir peel-away sheath(?) aracılığıyla lateral
ventrikülün ipsilateral frontal hornuna yerleştirildi. Çubuk,
ventrikülün içinde peel-away sheathten ayrılarak uzaklaştırıldı.
Ventriküler kateter peel-away sheath aracılığıyla yerleştirildi ve
peel-away sheath uzaklaştırıldı. Ventriküler kateterden serbest
BOS akımı, kateterin doğru yerleştirildiğini konfirme etti.
11. Standart takip 1., 3., 6. ve 12. aylarda ve klinik semptom
göstermeyen hastalarda değişen aralıklarla klinik muayeneleri
içerir.
Semptom gösteren hastalarda düz grafi ya da radyonüklid
çalışmalar, şant fonksiyon bozukluğunu (obstrüksiyon,
overdrenaj, enfeksiyon) BOS basınç monitörizasyonu kadar iyi
ekarte ettirir.
Data Toplanması ve İstatiksel Analiz
Bütün şant yerleştirme prosedürleri kayıtları retrospektif olarak
incelendi.
Hasta yaşı, ırkı, cinsiyeti, mevcut semptom ve bulguları,
semptom süreci, oftalmik muayene ya da beyin MR
görüntüleme bulguları, ilaçlar, operatif notlar, şant
başarısızlığının nedeni, başağrısının niteliği kaydedildi.
Tedavi başarısızlığı, fonksiyonel bir şanta rağmen preoperatif
başağrısının geri dönüşü ya da devamı olarak tanımlandı.
12. SONUÇLAR
42 hastaya 115 şant yerleştirme prosedürü uygulandı (79 LP,
36 VP/VAT şant).
Hastaların yaş ortalaması 37+/- 10, 32 hasta bayan, 31 hasta
obez, 25 hasta papilödemli, 15 hastanın başağrısı sırasında
vizüel defisiti mevcut.
40 hastada (bir ay içinde) anlamlı başağrısı, 17 hastada
papilödem olmadan rekürren başağrısı, 19 hastada iki yıldan
uzun süren semptomlar mevcut, postoperatif 10 hastada yenibaşlangıçlı hafif diffüz başağrısı gelişti.
Stereotaksik ventriküler şant yerleştirme girişimi yapılan bütün
vakalarda ventriküler kateter başarıyla yerleştirildi.
Bütün şantların %75i başarılıydı (24 ay içinde).
Lumbar şantlar, ventriküler şantlarla karşılaştırıldığında;
şant revizyonunda 2,5 kat
şant obstrüksiyonunda 3 kat artışla ilişkilidir.

Lumbar ve ventriküler şantlar overdrenaj, distal kateter
migrasyonu ve şant enfeksiyonu riski açısından benzerdir.
4 lumbar şant bel ağrısına yol açtı ve 3’ü sonraki tonsiller
herniasyonla ilişkiliydi.
4 VP şant, şant yerleştirildikten sonra gelişen intraabdominal
ağrıyla ilişkiliydi.
14. TARTIŞMA
30 yıldır izlenen, 115 şant yerleştirme prosedürü uygulanan 42
psödotümör serebri tanılı hastada, BOS şantlarının ciddi inatçı
başağrılarının takibinde son derece önemli olduğu bulunmuştur.
Hastaların çoğu postoperatif dönemde başağrılarında
rahatlama göstermiştir.
Burgett ve arkadaşları, postoperatif başağrısı iyileşme oranını
%82 olarak tanımlamışlardır.
Johnston ve arkadaşları, psödotümör serebri semptom
iyileşmesinde LP şantlarla %100 başarı gözlemlemişlerdir.
Çalışılan serilerde başağrısının yeterince iyileşmesi yıllarca
sürmüştür (bazı hastalarda 20 yıla kadar).
Şant yerleştirilmesinden iki yıl sonra revizyon oranları %86 LP
şantlarla ilişkilidir, ventriküler şantlarla ilişkili oranlar daha
düşüktür.
15. Şant başarısızlığı nedenlerinin analizi, ventriküler şant
uygulanan hastalarda şant obstrüksiyonlarında azalma
olduğunu göstermiştir.
Overdrenaj ventriküler ve LP şantlarda benzerdir.
Enfeksiyon riski, ventriküler şant operasyonu yapılan hastalarda
ilave donanıma (stereotaktik çubuk) rağmen, ventriküler şantta
LP şanta göre daha fazla değildir.
Bu çalışmada önceki çalışmalara (%60-85) göre daha yüksek
(%86) LP şant başarısızlık oranı gözlemlenmiştir.
Hastaların çoğunda şant yerleştirildikten sonra yıllar içinde
zayıflatıcı başağrıları gerilemesine rağmen, %24 ünde şantın
neden olduğu hafif diffüz bir başağrısı gelişmiştir.
VP şantlı hastalarda LP şantlı hastalara göre, intraabdominal
ağrı insidansının daha fazla olduğu (%24) görülmüştür.
Buna rağmen ventriküler şantlar, şant başarısızlığında azalmış
riskle ilişkili değildir; psödotümör serebrili hastalara BOS şantı
yerleştirmede ilk seçenek olabilir.
16. ÇIKARIMLAR
Özetle, 30 yıllık çalışmalara bakılarak şu varsayılabilir ki; BOS
şantları psödotümör serebriyle ilişkili inatçı başağrılarının
tedavisinde son derece etkilidir ve hastaların yarısında şantın
yerleştirilmesinden yıllar sonra rahatlama devam eder.
Papilödemin olmaması ve şant yerleştirilmeden önce 2 yıldan
fazla bir süre var olan semptomlar tedavi başarısızlığı açısından
risk faktörüdür.
Ventriküler şantlar, şant obstrüksiyonu ve revizyon açısından
LP şantlara göre düşük risklidir. Ventriküler şantların papilödemi
olan ya da semptomları iki yıldan az süren hastalarda kullanımı,
psödotümör serebriyle ilişkili inatçı başağrılarında optimal tedavi
sağlar.
