Ovarian preservation in Children
This document summarizes the experience of treating various ovarian pathologies in children through laparoscopic surgery. 46 patients presented with conditions such as simple cysts, dermoid cysts, functional cysts, and paraovarian cysts. 16 cases involved ovarian torsion. This study evaluated the efficacy of laparoscopically assisted high ligation of patent processus vaginalis in 40 children aged 6 months to 7 years. The laparoscopic procedure detected unsuspected contralateral hernias in 28.1% of cases, without increasing operative time. All procedures were completed without complications. The mean operative time was 25 minutes for unilateral hernias and 35 minutes for bilateral cases. The mean post-operative hospital stay was 4 hours. One recurrence occurred among 57 hernias repaired over an average 18-month follow-up. Laparoscopic repair was found to be a safe and effective procedure for detecting and repairing hernias in children.

Laparoscopy and Laparoscopic Surgery
Today, Laparoscopy is an alternative technique for carrying out many operations that have traditionally required an open approach. The benefits of minimal access surgery have been well recorded, including lower post-operative morbidity, shorter duration of hospital stay and a shorter return to work.
Laparoscopy and fertility
Laparoscopy still remains an important diagnostic and therapeutic tool in the management of subfertile women. Optimal and prudent use of this minimally invasive technique may avert costly treatment like IVF but overzealous and unindicated use may compromise future fertility. The evidence indicates that laparoscopic surgery improves fertility outcomes for conditions like mild to moderate endometriosis, myomectomy for submucosal fibroids, and tubal surgery. However, more high-quality randomized controlled trials are needed to provide stronger evidence and guidelines for clinical practice in reproductive surgery.

This study assessed the diagnostic value of obtaining an additional upright kidney-ureter-bladder (KUB) radiograph during routine intravenous urography (IVU). Of 164 patients who underwent IVU with an additional upright KUB, the upright view provided diagnostic benefit in 72 patients (43.9%), identifying issues like nephroptosis, better filling of the collecting system, differentiation of phleboliths from stones, and evaluation of emptying. While newer modalities have advantages, upright positioning remains a simple way for IVU to provide unique diagnostic information by leveraging gravitational effects.
Laparoscopic Ovarian Surgery
Laparoscopic ovarian surgery can be used to manage most ovarian abnormalities. Key steps in laparoscopic ovarian cystectomy include aspirating cyst contents, stripping the cyst capsule from the ovarian cortex, and extracting the capsule. It is important to avoid injury to nearby structures like the ureter and completely remove the cyst to evaluate for early carcinoma. This study evaluated the efficacy of laparoscopically assisted high ligation of patent processus vaginalis in 40 children aged 6 months to 7 years. The mean operative time was 25 minutes for unilateral hernias and 35 minutes for bilateral cases. The mean post-operative hospital stay was 4 hours. Optimal and prudent use of this minimally invasive technique may avert costly treatment like IVF but overzealous and unindicated use may compromise future fertility. The evidence indicates that laparoscopic surgery improves fertility outcomes for conditions like mild to moderate endometriosis, myomectomy for submucosal fibroids, and tubal surgery. However, more high-quality randomized controlled trials are needed to provide stronger evidence and guidelines for clinical practice in reproductive surgery.

Ultrasound-guided forceps biopsy had a sensitivity of 85% for malignant lesions and 88% for tuberculosis, with 100% specificity for both. It provided multiple pleural biopsy samples safely and was better than needle biopsy alone, with a diagnostic yield nearly similar to thoracoscopy but less invasive.

ABSTRACT:Infertility is a great psychological burden to the infertile couple. Scrotal ultrasonography and colour Doppler imaging of the scrotum are useful adjuncts to clinical examination in assessing intratesticular and extratesticular abnormalities. Methodology:All men who presented with infertility were evaluated. These included comprehensive history, physical examination and investigation, in this case seminal fluid analysis and scrotal ultrasonography. Results:This was prospective study carried out at the Jos University Teaching Hospital and a fertility centre in Jos from 2012 to 2017. A total of 67 men were involved in this study. The mean age was 39.39yrs. Age range was 28 to 59yrs. Sixty three (N=63) of the men had abnormal semen parameters representing 94.03% while four men (N=4) had normal semen parameters. Thirty eight patients representing 56.72% had azoospermia while 5.97% had normozoospermia following seminal fluid analysis. The mean volume of the right testis was 11.93ml. The range was 2.9ml to 25ml. The mean volume of the left testis was 11.76ml. The range was 2.9ml to 22ml. Overall mean testicular volume was 11.85ml. Forty two men (N=42) had abnormalities on scrotal ultrasound representing 62.69%. Abnormalities on ultrasonographyinclude varicocele33%, cryptorchidism31%, hydrocele 17%, testicularmicrolithiasis7%, multiple complex testicular cyst5%, epididymal cyst5% and echogenic testis2%. Conclusion:Scrotal ultrasonography is important in the assessment of testicular volume and abnormalities such as varicocele, cryptorchidism and hydrocele which affects male fertility.

This document summarizes a study that investigated the efficacy of hysteroscopic endometrial resection (HER) in treating abnormal uterine bleeding (AUB) among Libyan women. The study found that HER was an effective treatment, achieving a 92.8% success rate in reducing symptoms after 2 years of follow up among the 70 women who completed the study. Younger age (<40 years), thicker endometrium (>7mm), larger uterine size, and the presence of fibroids were found to be factors associated with treatment failure. The study concludes that HER is an effective procedure for AUB, especially in women over age 40 without an enlarged uterus or fibroids.

These drugs competitively antagonize actions of
histamine at the H1 receptors.
Pharmacological actions-
Antagonism of histamine-The H1 antagonists effectively block histamine induced bronchoconstriction, contraction of intestinal and other smooth muscle and triple response especially wheal, flare and itch. Constriction of larger blood vessel by histamine is also antagonized.
2) Antiallergic actions-Many manifestations of immediate hypersensitivity (type I reactions)are suppressed. Urticaria, itching and angioedema are well controlled.3) CNS action-The older antihistamines produce variable degree of CNS depression.But in case of 2nd gen antihistaminics there is less CNS depressant property as these cross BBB to significantly lesser extent.
4) Anticholinergic action- many H1 blockers
in addition antagonize muscarinic actions of ACh. BUT IN 2ND gen histaminics there is Higher H1 selectivitiy : no anticholinergic side effects

Gene therapy can be broadly defined as the transfer of genetic material to cure a disease or at least to improve the clinical status of a patient.
One of the basic concepts of gene therapy is to transform viruses into genetic shuttles, which will deliver the gene of interest into the target cells.

Choice of viral vector is dependent on gene transfer efficiency, capacity to carry foreign genes, toxicity, stability, immune responses towards viral antigens and potential viral recombination.
There are a wide variety of vectors used to deliver DNA or oligo nucleotides into mammalian cells, either in vitro or in vivo.
The most common vector system based on retroviruses, adenoviruses, herpes simplex viruses, adeno associated viruses.
