STARSurg ASME Conference Presentation
This document describes a study evaluating the educational impact of medical students participating in STARSurg, a collaborative multi-center national cohort study. The study involved 273 student collaborators from 32 of the 33 UK medical schools. Hospital Concentration and Patients Outcomes in the NHS: New Research
Health care reforms in England during the last decade have been influenced by the idea that encouraging competition between hospitals, with nationally fixed prices, will increase the quality of care for patients. Some research has found a positive connection between competition and outcomes. A principal criticism of such studies has been the measures of quality of care that were used. This analysis uses NHS PROMs data, collected both before and after treatment, indicating the extent to which the surgery produces improvement in patients’ self-reported health status.
Following an approach common in the literature on competition, hospital market concentration is used as an indicator of competition. Data were collected for 2011–12 for all English NHS hospitals and all elective primary hip replacements.

Opioid Substitution Therapy (OST) and TB/MDR integration
This document discusses Ukraine's experience integrating tuberculosis (TB) screening and treatment into opioid substitution therapy (OST) sites. It finds that:
1) Over 9,000 people in Ukraine were diagnosed with multi-drug resistant TB in 2013, and OST sites are screening their over 8,000 patients for TB.

Health care reforms in England during the last decade have been influenced by the idea that encouraging competition between hospitals, with nationally fixed prices, will increase the quality of care for patients. Some research has found a positive connection between competition and outcomes. A principal criticism of such studies has been the measures of quality of care that were used. This analysis uses NHS PROMs data, collected both before and after treatment, indicating the extent to which the surgery produces improvement in patients’ self-reported health status.
Following an approach common in the literature on competition, hospital market concentration is used as an indicator of competition. Data were collected for 2011–12 for all English NHS hospitals and all elective primary hip replacements.

Opioid Substitution Therapy (OST) and TB/MDR integration
This document discusses Ukraine's experience integrating tuberculosis (TB) screening and treatment into opioid substitution therapy (OST) sites. It finds that:
1) Over 9,000 people in Ukraine were diagnosed with multi-drug resistant TB in 2013, and OST sites are screening their over 8,000 patients for TB.
2) OST sites located in TB clinics and integrated care centers provide TB screening and treatment to their 301 patients, while basic OST sites serve 68 patients.
3) Studies found that OST patients had good access to TB diagnosis and treatment through collaboration between OST sites and on-site or nearby TB clinics and doctors.

Allied health professionals (AHPs) are a group of 12 distinct professions that form a critical part of health care. By 2013 AHPs had made up around 6% of the NHS workforce and accounted for an estimated £2bn of the NHS salary bill.
The role that AHPs play in delivering care is likely to increase further into the future as the population ages and people live with more complex needs. Yet despite the size of the workforce and the broad scope of care, there is very little systematic information available to describe the quality of care delivered by AHPs.

The Northern Territory POCT Program in Australia implemented point-of-care testing (POCT) in remote Aboriginal communities using Abbott i-STAT devices. Quality control testing showed analytical quality met goals. Case studies demonstrated the clinical effectiveness of POCT for acute care like cardiac troponin testing and chronic care like INR monitoring for warfarin therapy. For example, effective capacity planning in imaging services should enable improved patient access balanced against the need to avoid over-use of interventions that have the potential to cause harm, such as ionising radiation.

Dr Peter Watkinson, Associate Professor of Intensive Care Medicine, is joint clinical lead for the Critical Care Research Group based at the Kadoorie Centre for Critical Care Research & Education at the John Radcliffe Hospital, Oxford.
He is an NHS consultant in intensive care and acute medicine and is part of the senior clinical team at the Oxford University Hospitals NHS Foundation Trust. His research interests focus on the identification of the deteriorating patient in hospital and he has designed and run a number of studies in the field of wearable monitoring devices. The research group is now exploring the opportunities offered through non-contact monitoring and standard electronically-recorded descriptors of a patient’s condition.
The research group has a strong link with the University of Oxford Institute of Biomedical Engineering. Using data collected from thousands of patients’ vital signs in Oxford and elsewhere the multi-disciplinary team investigates ways to locate patterns which precede and predict clinical deterioration in hospitalised patients.
Other areas of interest for the research group include development of electronic monitoring systems, use of human factors techniques to introduce new technology into the healthcare environment, and assessing the longer-term effects of critical illnesses on patients’ quality of life.

Stroke Event 13 Sep - First morning presentations
This document summarizes research on early supported discharge (ESD) services for stroke patients. The research aimed to evaluate whether the benefits of ESD seen in clinical trials are still evident in practice. The study compared outcomes for 135 patients receiving ESD services to 158 patients receiving usual care without ESD following a stroke. Results showed that the ESD group had significantly shorter hospital lengths of stay and were more knowledgeable about community support services. Statistical modeling also found that ESD patients were more likely to score higher on measures of independence at 6 weeks, 6 months and 12 months post-stroke. The research provides real-world evidence that ESD services continue to provide benefits for stroke rehabilitation compared to usual care without ESD support.

Sam Foster: Nursing and quality
Sam Foster is the Chief Nurse at Heart of England NHS Foundation Trust, which includes 3 acute sites and community services with over 10,500 staff and an annual turnover of £650 million. Some key challenges for nursing include rebuilding public trust in the profession, providing robust assurance to the Board with meaningful data rather than just numbers, and addressing professional concerns about nursing practice while also improving efficiency given nursing's large workforce and paybill. Emma Rourke: The quality dataset roadmap

Martin Bardsley: Introducing QualityWatch

1. The CQC has developed a new approach to inspecting hospitals that involves a 3 phase process of preparation, site visits, and reporting.
2. Site visits will involve both announced and unannounced components, including interviews, focus groups, observations, and reviewing data. Inspections will evaluate hospitals on whether they are safe, effective, caring, responsive, and well-led.
3. Hospitals will receive ratings of outstanding, good, requires improvement, or inadequate. The goal is to inspect all acute NHS hospitals by December 2015 and provide ratings to help improve quality of care.

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
