Adipose tissues contain lipid-filled adipocytes but are also composed of several other cell types including adipogenic progenitors, endothelial cells, immune cells and neuronal cells. Brown adipose tissue (BAT) contains thermogenic adipocytes which produce heat in response to cold, an energy-consuming process. Today, it is well established that adipose tissue function is highly dependent on the plasticity of the tissue due to the interaction of different cell types present within the tissue. In addition, the function of adipose tissue is controlled by secreted factors, called adipokines or batokines. Some of these cytokines affect nearby cells within the adipose tissue while others are secreted into the circulation for communication at the organ level.

In many western countries, nearly a quarter of us meet the criteria for clinical obesity and more than half of us are overweight. This is a medical concern because obesity is a serious risk factor for many major chronic illnesses, such as heart disease, type 2 diabetes and cancer, and as a result, obesity is associated with reduced lifespan by almost a decade. The rapid escalation in the prevalence of obesity and the paucity of obesity medications underscores the necessity of an understanding of the basic neurobiology underlying body weight.
During this webinar, Professor Heisler will discuss brain circuits that are the main known controllers of body weight, such as those activated by the adipocyte hormone leptin. She will review how our genes impact our waistline and will discuss crucial genes such as those in the melanocortin system. Professor Heisler will discuss how obesity medications capitalize on this basic neurobiology to promote satiety, reduce hunger and decrease body weight.

Despite the publication of a number of studies over recent years looking at energy delivery and outcomes in the critically ill population we remain uncertain how best to determine optimal calorie delivery for our patients. The concept that energy delivery should match energy consumption is plausible and intellectually attractive bu Broadly speaking clinicians can be divided into 3 categories according to their approach on energy delivery to the critically ill. Some believe that optimal clinical outcomes are achieved by closely approximating energy consumption i.e. providing full calorie requirement, usually around 2000kcal/d for the standard sized adult. This position is supported by a number of observational studies, however, patients usually only receive about 60% of what they are prescribed. Some believe that attempting to provide full feeding exposes the patient to the risk of overfeeding and that ‘permissive’ underfeeding is safe and better tolerated in critically ill patients where gastrointestinal function is frequently deranged. Interestingly, recent data suggest that the patient group potentially most at risk of overfeeding are those who are malnourished at presentation. Finally, some believe that the amount of energy delivered during ICU stay has little impact on recovery. Only when the ICU stay becomes unusually prolonged may the amount of energy delivered become important. There is evidence to suggest that some nutrition should be given enterally from early in the ICU stay to provide gastrointestinal mucosal protection and improve subsequent gut function. In recent years there have been several randomised controlled trials addressing energy delivery but they have unfortunately given conflicting results. Furthermore, these studies have had a number of limitations including: being underpowered to show an effect on survival; open to bias because of being open-labelled; most have not delivered full energy requirements so the effect of this on outcomes remains uncertain. It is hoped that many of these issues will be addressed in the currently recruiting TARGET trial which will be completed next year.t, while energy delivery can be measured with indirect calorimetry, this is not a technique that lends itself to routine clinical care. Accurate measurement or calculation of day to day energy expenditure is not currently routinely possible. Delivery of nutrition is an important supportive activity in the ICU. Patients generally receive less than prescribed nutritional needs and there is no robust evidence as yet to suggest that this is deleterious to outcomes.

Continuous Glucose Monitoring and Glycemic Variability in Pregnancy
Join Caroline Wuyts, MSc for a deep dive into her research on pregnancy and monitoring glucose availability throughout the reproductive timeline of a mouse model.
Pregnancy is a physiological challenge for the female body. Metabolic adaptations are necessary to maintain the balance between maternal and fetal growth. To gain insight in these pregnancy-induced adaptations, continuous glucose was monitored in mice via telemetric transmitters.
Caroline’s group investigated the effect of telemetry on pregnancy outcome by comparing mice with an implanted transmitter and mice that underwent a sham surgery. They developed an overview of glucose tolerance before, during and after pregnancy by performing oral glucose tolerance tests. Glycemic variability was monitored continuously using camera surveillance, and it was determined that continuous glucose monitoring is a safe method that provides insight into glycemic control during pregnancy with unprecedented detail.

For nearly 10 years, Dr. Dominic D’Agostino has served as an assistant professor at the University of South Florida (USF), where he actively researches prospective solutions to managing cancer and to treating neuropathologies that prevent and impair muscle control, such as seizure disorders, Alzheimer’s and Lou Gehrig’s disease (ALS). In this effort, Dr. Dominic D’Agostino has focused on the effects of a ketogenic diet, ketone esters and ketone supplementation and recently contributed to a study published in the Nature Medicine journal that demonstrates the anti-inflammatory effects of nutritional ketosis.

Understanding Keto-Adaptation Mechanisms across the Lifespan
An associate professor at the University of South Florida, Dominic D’Agostino focuses his clinical research in areas such as muscle function, ketone supplementation, KetoNutrition, and ketone technologies. A respected voice in his field, Dominic D’Agostino is co-author of the article “Metabolic switching is impaired by aging and facilitated by ketosis independent of glycogen” (Aging, May 2020).
The paper focuses on the body’s ability to transition between glycolysis and ketosis. This process enables fat oxidation metabolism and is a survival mechanism during times of fasting. Also elicited by stress and carbohydrate restriction, metabolic switching is delayed among aged rats.
The study involved young and aged rats being administered time-restricted feeding (TRF) with either ketogenic diets (KD) or standard diets. Glycogen dynamics, as well as glycolysis and ketosis switching, were then measured. TRF alone had an effect in reversing insulin-related metabolic deficit markers and accelerated the metabolic switching process among aged rats. The KD+TRF combination added to these benefits, with abilities to keto-adapt shown as not correlated with glycogen levels.
While the reason for the age-related impairment and delayed Keto-adapting from glycolysis is still unknown, the study provides new insight. In particular, it helps define keto-adaptation mechanisms, and potential pathways across the lifespan, for treating metabolic impairment through dietary interventions.

In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

INTRODUCTION A University of South Florida (USF) associate professor, Dominic D’Agostino, PhD, maintains a research focus on metabolic therapies for epilepsy, seizures, brain cancer, and other conditions. Dr. Dominic D’Agostino’s present focus is on the ketogenic diet and ketone esters, and their neuroprotective and anticonvulsant effects. As principal investigator with USF’s Department of Molecular Pharmacology and Physiology, Dr. D'Agostino looked into the combined effect of a pair of nontoxic adjuvant cancer treatments, hyperbaric oxygen therapy and a ketogenic diet. Using a late-stage, metastatic cancer mouse model, Dr. D'Agostino's team found that this therapy increased survival time.
- 3. KETOGENIC DIET The ketogenic diet caused a substrate utilization shift, with ketone bodies and fatty acids being used by muscles for energy instead of glucose. While normal cells can use ketone bodies for fuel if necessary, cancerous cells cannot and are vulnerable to less availability of glucose. At the same time, solid tumors are aided in their growth and spread by areas of low oxygen. Hyperbaric oxygen therapy addresses this by providing patients with 100 percent oxygen at a barometric pressure that is higher than normal. Breathing in this environment saturates cancerous tumors with oxygen. Dr. D’Agostino described these positive results as providing a pathway toward safe and cost-effective therapies that are not toxic to the body.
