Dr. Kaoru Ishikawa
Kaoru Ishikawa is known as "the 'Father of Quality Circles' and as a founder of the Japanese quality
movement". (Beckford, John. Quality: A Critical Introduction) Ishikawa was also a pioneer in Total
Quality Management. He believed in working for the customer before, during, and after product
and/or services were delivered. Ishikawa was a Chemist, held a doctorate in Engineering, and was
Professor at Tokyo University
Key Ideas
Dr. Ishikawa believed in the customers' needs. One of the many things he's famous for is the
"bottom up view of quality". (Managing for Quality and Performance Excellence, p.110) He ...

6. Take appropriate action.
The above steps assist companies to reach their goals and improve in their overall processes. It also
helped in the area of quality improvement, which he emphasized that quality is expected throughout
a product 's life cycle and not just during production. Ishikawa was honored with the Deming, Nihon
Keizai Press and Industrial Standardization prizes and the Grant Award from the American Society
for Quality Control. (Beckford, John. Quality: A Critical Introduction, p 121.)
Ishikawa 's ideas for the future of quality He created many of these ideas and trainings so that
companies not just in Japan, but in other countries such as the US and others would continue to use,
adapt and improve their mindset on Total Quality Management. Managers can use these processes to
reach their goals, but also to think of additional ways to improve these processes and possibly add to
them as different issues come up and as time changes. Companies should adapt to training models
for their management and employees. This will continue to legacy and possibly improve it as it
grows to something that other companies can use. It will work if the analysis shows growth in the
company and low issue rates. Ishikawa also used and showed the importance of the seven quality
tools: control chart, run chart, histogram, scatter diagram,

Evaluation Of A Program For Incentive Spirometry
1. Education provided must be specific to the patient's needs and appropriate to the care, treatment
and services provided. Initial teaching for Incentive spirometry is failing to be documented in all
cases in the hospital's electronic health record.
2. Patient–specific education is designed to help medical professionals and patients make better
decisions about their health and links to relevant information with extensive articles, videos, and
images for the patient. (Office of the National Coordinator for Health Information Technology,
2014). Properly documented patient education can help organizations meet Meaningful Use
Requirements (Centers for Medicare & Medicaid Services, 2012, 2016; National Learning
Consortium, 2012) for reimbursement under Centers for Medicare & Medicaid Services guidelines.

Deming's 14 Points On Quality Management
Deming
Dr. W. Edward Deming is known for reminding management that most problems are systemic and it
is management's responsibility to enhance the system in order for the workers to do their job in a
more effective way. Deming states that higher quality leads to higher productivity, as a result leads
to long term competitive strength. The theory is that improvements in quality lead to lower costs and
high productivity because they result in less rework, fewer mistakes, fewer delays, and better use of
time and materials. with better quality and lower prices, a firm can achieve a greater market share
and thus stay in business, providing more and more jobs.
Deming is best known for his theory of management as embodied in his 14 points. The 14 points on
quality management, that helps a set of management practices to help companies increase their
quality and productivity:
Create constancy of purpose toward improving of products and services with the aim to become
competitive.
Adopt a new philosophy
Cease dependence on mass inspection to improve quality
End the practice of awarding business on the basis of price tag alone
Improve constantly and forever the system of production and service, to improve quality and
productivity.

Joseph Juran is considered to have had the greatest impact on quality management. Juran took a
more strategic and planning based approach to improvement than Deming. Juran promotes the view
that organisational quality problems are largely the result of insufficient and ineffective planning for
quality. He argues that companies must revise strategic planning process and achieve mastery over
these processes. The means proposed by Juran establish specific goals to be reached and plans for
reaching those goals. In addition, Juran's process assigns clear responsibility for meeting the goals
and bases rewards on results

BOB XOURAFUS
MOTIVE– The main objective of studying this unit is to understand the control quality process. In
addition to this, we will also study in depth the seven basic quality tools that are data table,
Histogram, Pareto Diagram, scatter diagram, trend analysis and run chart.
REFLECTION– as per EPM 2092 study notes provided by Bob, Control quality is a process
monitoring and recording the results of execution of quality activities to access the performance and
recommending necessary changes. In other words, during quality control process the project team
will analyze project results (which includes both deliverables and the management processes
followed) to meet the specific quality standards and if something is wrong (that means it is not
meeting the quality standards set by the stakeholders and organization as written in project
management plan) then make it under control by taking corrective, preventive or defect repair
action. In addition to this we will to make use of quality tools and techniques which will help us to
analyze if the project results are meeting the quality standards.

Case Study : Samsung Beginnings Of Samsung
Samsung Beginnings
Samsung was founded on March 1, 1938 by Byung–Chull Lee in Taegu, Korea for 30,000 won. In
the beginning the business focused on trade, selling dried fish, vegetables and fruit. Samsung grew
over the next decade to include its own flour mills, manufacturing and sales operation. In 1969
Samsung–Sanyo electronics was established and in 1970 the company started producing its first
black and white TV.
In 1972 Samsung began production of black and white TV's for domestic sale, by 1972 the company
had produced one million black and white televisions. By 1978 the number of black and white TV's
had jumped to 4 million produced. In 1977 Samsung started to export color televisions. In addition

Samsung is a well–known company with a lot of information available on the company. While the
company is very well known, and has been considered a reliable brand it recently had some serious
quality issues with a cell phone that the company produced. The quality issues where well
documented in the press and cause some serious issues with some of Samsung's valued customers.
The intent of this paper is to discuss some of the quality issues that Samsung has faced over the past
year and what steps they are taking to correct the issues moving forward.
Quality Issues
Samsung Note 7 phone launched in August of 2016 the phone started with rave reviews however,
that quickly came to an end when customers started reporting their phones catching fire. Samsung
first recalled the battery of the phone offering a replacement battery to all Note 7 users. The fix for
Samsung was not so easy the new replacement battery was also defective and caught fire. Samsung
decided to recall and discontinue selling the phone. In October 2016 the company estimated that the
issue would cost the company $5.3 billion dollars.
"Due to unexplained fires and overheating problems, more than 2.5 million Galaxy Note 7
smartphones were recalled before the product was discontinued earlier this week just two months
after its launch in early August." (Associated Press, 2016) Luckily Samsung could cover the costs of
the recall because of its

Development Of A Service Innovation
Introduction:
The aim of the report is to discuss the development of a service innovation in a hospital care setting.
Therefore the innovation being introduced in the report will focus on the pain management tool for
assessing chronic conditions in service users with dementia in hospital settings. As pain is a very
subjective experience it is important that the tool is specifically based on the individual's own way
of expressing and communicating their pain in relation to their chronic conditions.
The first section of this report will review and evaluate a body of evidence to support the
development of the pain management tool by looking at the subject of the change management
proposal, the relevant stake holders and the change agent to support the change. Secondly the report
will focus on theories and strategies to management of change by using change management model
and explaining the process of it. Thirdly the issues and experience of being a team member in the
development of the service improvement proposal will be reflected and also explore the dynamics of
the team. Lastly for the transition from student nurse to registered nurse, based on the findings from
the evaluation of personal performance further development will be discussed identifying the
leadership skills needed and design an action plan which includes outcomes and how these
outcomes will be measured.

Quality Management Process BJB manufacturing quality approach will include quality control,
quality improvement, quality assurance, and the implementation of quality management. The quality
management process is important within BJB Company's strategy because it ensures the products
produced, in fact meets the necessities of our clients. The quality management process will ensure
the company improves the quality of the products. The quality process has a set of specific measures
that guarantee the product produced by the company "fit the purpose." The quality process involves
setting goals, for which both companies agree; afterwards the quality assurance team checks and
rechecks for the possibility of failure, and the effects of the product from the engineering to the
experimental design phase for reliability and durability.
The quality control process measures and reports the tangible quality of the CD players. As part of
the quality assurance process, any issues identified must be resolved promptly. The quality
management process is implemented to ensure that the company improves the quality of the
company products. Whether the products are, produce for primary sales or the car manufacturing
aftermarket an efficient quality assurance and quality management process are valuable. By
implementing the quality management process, the management team can make sure that the

The literature search strategy focused on leadership barriers and organizational disconnections
related to the marketing of potentially unsafe medical devices. Many qualitative studies were found
in the NCU ProQuest, EBSCOhost databases, and Dissertation resources on organizational barriers
and disconnection. The starting point of the literature review is the theory of quality management
and the subsequent emergence of quality–related standards and regulations. Information sources for
the proposed study included books on quality management theory obtained from the EBSCOHost
database and peer–reviewed articles from the ProQuest database.
Also, the literature search included research articles, private and government websites, and ... Show

The ProQuest keyword advance search for the key words was narrowed by requesting full text, peer
reviewed and scholarly journal articles in the last three years. Furthermore, the search phrases
included variations of the key problem statement words disconnection, misalignment and process
management. The articles returned and chosen were those that included information about
leadership process disconnections, misalignment, and disconnected processes related to quality and
medical devices
The purpose of this qualitative, multiple–case study is to investigate leadership barriers and
organizational process disconnections that have led to the marketing of potentially unsafe medical
devices. Many qualitative studies (e.g., Iden, 2012, Oakland, 2011, Obeidat et al., 2014, Pater, 2013,
Raeesi et, al., 2013, Rising et al. 2012, Seethamraju, 2012, Suckley et al., 2013; Storbacka, 2012,
Talib et al., 2011; Tucker et al., 2013) have been conducted on organizational barriers and
disconnection. However, no studies were located related to the marketing of potentially unsafe
medical devices. In the proposed study, factors that influence or hinder the manufacture of safe and
effective medical devices will be identified and explored.
Medical devices are used to mitigate health issues and to maintain an acceptable quality of life
(Harrison et al. 2013). Because medical devices are expected to perform without failure,

The Philosophy Of Quality Management
1.0 Introduction
Quality management is defined as management activities and functions involved in determination of
quality policy and its implementation through means such as quality planning and quality assurance
(including quality control), which also called total quality management (TQM) (Business
Dictionary). Total Quality Management (TQM) is a comprehensive and structured approach to
organizational management that seeks to improve the quality of products and services through
ongoing refinements in response to continuous feedback. (Search CIO)
Philosophy of quality management
W Edwards Deming placed great importance and responsibility on management, at both the
individual and company level, believing management to be responsible for 94% of quality problems.
His fourteen–point plan is a complete philosophy of management, that can be applied to small or
large organizations in the public, private or service sectors. (Deming, 2000). Deming's wheel /
PDCA cycle is a problem solving tools adopted by firms engaged in continuous improvements.

Literature Study On Six Sigma
Literature Study
Six Sigma
Six Sigma is a household name in the world of industry. Six Sigma is a union of of methods and
instruments that are created for one sole purpose–process improvement. An engineer named Bill
Smith, who worked at the cellphone company, Motorola, in 1986, first initiated this principle. 9
years later, Jack Welch from General Electric made Six Sigma central to his business strategy.
The name Six Sigma was originated from the Greek letter sigma, which has a function of denoting
variation from a certain standard. In statistics, the word sigma measures how much is the distance of
a certain process with perfection. The main idea of Six Sigma is that if the "defects" or flaws of a
certain process can be defined, then it is certain that the aforementioned "defects" and flaws can be
eliminated in order to achieve perfection or the stage of "zero defects". In order to achieve the
aforementioned goal, a process must be able to produce no more than 3.4 defects per million
opportunities. An "opportunity" in this case is defined as a moment of ... Show more content on

The meaning behind the 80/20 rule is that 20% of the problems in a certain process hold the key to
80% of the impact. The manager and the team of a certain company use Pareto Analysis in order to
achieve the perfect, "no defect" production process. Pareto Analysis is efficient to use because
Pareto Analysis quickly directs the focus of the analysts on the key causes of certain problem.
Therefore, Pareto Analysis is the right tool to use when the company has limited resources, because
the resources will not be allocated to means with less significant meaning. However, Pareto Analysis
requires trained personnel and data, because without those, Pareto Analysis will not be able to be
conducted. In a nutshell, Pareto Analysis is an analytical tool that allows analysts to focus on the

focus is to provide person centred care and holistic assessment for the service user. The chart will
also incorporate the service user's family and carers to assist nurses in the assessment process.
The relevant stakeholders in the change are:

The Change agent:
The change agent chosen for the innovation is the ward sister who will advocate, lead and

Dementia is an umbrella term used to describe a decline in mental ability that has an impact on a
person's daily living. By reviewing literature on dementia it was evident that there are currently
800,000 people with dementia in the UK. One in three people over 65 will develop dementia and
there will be over a million people with dementia by 2021 (Alzheimer's Society, 2013).
According to the Alzheimer's Society (2013) pain is still poorly detected and undertreated in people
with dementia admitted to acute hospital sectors. This is because those with dementia or with more
than mild cognitive impairment can often find it difficult to express that they are in pain or are

What is a 'SWOT Analysis'?
SWOT Analysis is a simple yet extremely useful decision–making tool. SWOT is an acronym for
Strengths, Weaknesses, Opportunities and Threats. It can be used to assess the current and future
situation of the product, brand, company, proposal or decision. A SWOT Analysis considers both
internal factors (the strengths and weaknesses) and external factors (the opportunities and threats)
that are relevant to the issue or problem under investigation 5. Outline three advantages and two
disadvantages of using a SWOT analysis.
Advantages of SWOT: * Completing a SWOT Analysis can be quite simple and quick * It can be
used for a wide range of decisions, such as how to react to the threat of a competitor entering the
market * SWOT Analysis helps to determine the organisation's position in the marketplace and
therefore aids the formulation of corporate strategy for its long–term survival * The SWOT
framework encourages foresight and proactive thinking, rather than relying on emotional or intuitive
feelings * It can help to reduce the risks of decision–making by demanding objective and logical
thinking
Limitations of SWOT: * Critics of SWOT Analysis argue that it is too simplistic and does not

What Was W. Edwards Deming 's Quality Philosophy? Essay
1. What was W. Edwards Deming's quality philosophy?
The key ideas of Deming 's on quality lies in his understanding the importance of change. In Out of
the crisis he states: "The key problem in both management and leadership is failing to understand
the information in variation".
Deming was engaged with, why things do not behave as predicted? Every system will have
variation, but he disagrees that it is crucial for managers to be able to differentiate between common
and special causes of variation. Deming then developed a theory of variation – in which special
causes of modifications are mostly easily available to quickly noticeable factors such as change of
shift or operator, changes of procedure etc., but that most common causes will stand when special
causes have been eliminated. These common causes are often identified by workers, but only
managers will have the power or authority to change them to evade repeated occurrence of the same
problem. He also estimated that management will be answerable for more than 85% of the causes of
variation.
The Deming Cycle which is also known as the Shewhart Cycle, often called the PDCA, which was a
result of the need to link the assemble of products with the needs of the customer along with
departmental resources to meet those needs.
Plan: Construct a user research technique that will inform business process components.

Oakland's Soft factors of TQM and Organizational Performance:
Examining the Impact and Relationship
1.Introduction
Total Quality Management (TQM) is a long–term process the involves commitment, at all employee
levels, and continuous improvement as to how activities are managed and controlled by
organizations to present quality goods or services that satisfy internal and external customers' need
and expectation(Miller, 1996). TQM is a bundle of tools, techniques, processes, principles and
experts practices that over the years has been evolved and improved(Shiba,Graham and Walden,

An Introduction to Total Quality Management
Introduction
The name 'Federal Express' is synonymous with quality. It is recognized that the Federal Express
Company a world leader in transportation area and information field. Federal Express can deliver
packages overnight to almost anywhere in the world. Simultaneously, people can track packages at
any time so that they can know where their goods are. (Neff & Citrin, 2001) As the good quality and
services Federal Express provided, people like it, trust it and make it universally. Thus, a distinctive
quality is vital to every organization at present if you want to have bigger market share in the world.
However, how could people achieve it?

There are a large number of people who contributed to the development of the extensive concepts
that are wised up as total quality. This part will mainly introduce three contributors who are W.
Edwards Deming, Joseph M. Juran, and Kaoru Ishikawa.
 W. Edwards Deming
The first contributor is Deming. His method strives to 'delight customers', which is an approach that
is briefly and wisely depicted by 'the Joiner Triangle'. (Watson & Howarth, 2011) As the following
points, there are three main concepts and contributions of Deming:
a) The Plan, Do, Check, Act (PDCA) Cycle–an approach for resolving problems;
b) Deming Fourteen Points–key ideas for business to survive
c) Seven Deadly Diseases
 Joseph M. Juran
Juran quality focuses on 'fitness for use or purpose'. (Watson & Howarth, 2011) On a basis of this,
Juran published On Planning for Quality. 'Quality does not happen by accident, it must be planned'
is a major idea of Juran – planning which is just one component of his 'Quality Trilogy' that consists
of three main parts: Quality Planning, Quality Control and Quality Improvement.
 Kaoru Ishikawa
Professor Kaoru Ishikawa is famous as 'Father of Quality Circles', (Bank, 2000) which is because

TQM is composed of three paradigms:
* Total: Organization wide * Quality: With its usual Definitions, with all its complexities (External
Definition) * Management: The system of managing with steps like Plan, Organize, Control, Lead,

One popular test is a "life test" in which the sample product is operated until a part fails. Another
popular test is called "shake and bake", in which the product is mounted on a vibrator in an
environmental oven, and operated at progressively more extreme vibration and temperatures until
something fails. The failure is then isolated and engineers design an improvement.
A commonly–discovered failure is for the product to disintegrate. If fasteners fail, the improvements
might be to use measured–tension nutdrivers to ensure that screws don 't come off, or improved
adhesives to ensure that parts remain glued.
If a gearbox wears out first, a typical engineering design improvement might be to substitute a
brushless stepper motor for a DC motor with a gearbox. The improvement is that a stepper motor
has no brushes or gears to wear out, so it lasts ten or more times as long. The stepper motor is more
expensive than a DC motor, but cheaper than a DC motor combined with a gearbox. The electronics
are radically different, but equally expensive. One disadvantage might be that a stepper motor can
hum or whine, and usually needs noise–isolating mounts.
Often, a "TQMed" product is cheaper to produce because of efficiency/performance improvements
and because there 's no need to repair dead–on–arrival products, which represents an immensely
more desirable product.

The steps required to carry out the process are defined, and performance measures are continuously
monitored in order to detect unexpected variation.
4. Integrated system
Although an organization may consist of many different functional specialties often organized into
vertically structured departments, it is the horizontal processes interconnecting these functions that
are the focus of TQM.
Micro–processes add up to larger processes, and all processes aggregate into the business processes
required for defining and implementing strategy. Everyone must understand the vision, mission, and
guiding principles as well as the quality policies, objectives, and critical processes of the
organization. Business performance must be monitored and communicated continuously.
An integrated business system may be modeled after the Baldrige National Quality Program criteria
and/or incorporate the ISO 9000 standards. Every organization has a unique work culture, and it is
virtually impossible to achieve excellence in its products and services unless a good quality culture
has been fostered. Thus, an integrated system connects business improvement elements in an
attempt to continually improve and exceed the expectations of customers, employees, and other
stakeholders.

When Have I Learned?
1. When have I learned? In what circumstances? Under what conditions?
During these two weeks my learning activities have been focused on the creation of our power point
presentation, as well as the cause and effect diagram. We started with the Cause and Effect Analysis.
With this analysis, we want to find and verify the critical causes that potential impact to the PACU
workflow process. Consequently, a set of recommendation will be presented to the CCF OR
management team.
2. How have I learned or not, and do I know what kind of learner I am?
The cause and effect diagram help us to identify the possible obstacles (variation) that have
negatively impacted the OR 's workflow, and have caused an increase of the patient length of stay in
the PACU. Furthermore it also allows us to brainstorm all possible explanations for current
performance in a more organized way, by grouping all possible causes as there are six major factors
affecting the performance of the process: people, material, environment, method, machine, and
measurement.
3. What difference has the learning made in my intellectual, personal, and ethical development?
What I have learned during these two weeks make differences in my intellectual, personal, and
ethical development. For example, I realized, that even if there are probably more accurate statistic
methods, for data analysis, an analytical tool as simple as the Fishbone diagram, may help the OR

After World War II Japanese manufacturing companies where struggling to compete in the
worldwide markets. This was mainly due to the fact that most consumers believed that products
produced in Japan cheaply made and of poor quality. Business leaders in Japan knew that changes in
quality control needed to happen in manufacturing to change the consumers' minds about their
products. They turned to Dr. Kaoru Ishikawa to rally the Japanese government and people toward
the specific goal of improving quality control in their manufacturing companies, thus making Dr.
Ishikawa an early pioneer in Japan's quality revolution.
Dr. Ishikawa spend many years as a professor of engineering at Tokyo University and became a
Japanese icon as an influential quality management innovator. He helped develop the bottom–up
view of quality management which the Japanese used to approach quality management. Dr.
Ishikawa not only help the Japanese workers see the need for quality in their product he was able to
persuade top management to get involved and make total quality control a success in Japanese
companies. Dr. Ishikawa, with the help of the Japanese Union of Scientist and Engineers, was able
to implement changes in the way companies in Japan thought about total quality control through in
the introduction of quality circles.

During the identification stage for process improvement in the previous assessment, we were able to

production in order to fix identified problems and getting quality right the first time. Furthermore,
leading to Toyota's biggest crisis "sudden unintended acceleration" in their top selling models
causing a number of losses across the board. It is clear that Toyota must follow the TPS, precisely
the Total Quality Management process (TQM), in order to properly address and resolve any issues
associated with the safety and quality of its products. It is imperative to have the complete buy–in
and support of Upper Management to encourage accountability and ensure the sustainability of this
process. Thereby, enabling Toyota to re–build its brand, re–gain consumer trust and confidence in
their products and reposition themselves as the leading automotive manufacturer internationally.
During this assessment we will create a current state and new process map for the recommended
process improvement; create a cause–and–effect diagram; and identify one main cause as a result of
the examination of the cause–and –effect diagram and current state process map.

Business Activities Of The Operation Function
There are different activities involved in the operation function. Activities are a series of process that
assist in transforming a product or service. The transformation process can be physical, exchange,
locational, physiological, or informational. In each line of business the activities may vary. Example
for automobile companies' activities in the operation function may include "Transforming a sheet of
steel into different shapes to be painted, finished and assembled with thousands of other component
parts to produce a working automobile". An operation is the vital process of any business or
company. Operation interacts with other functional areas to produce goods and provide services for
customers. Operations bring together many disciples and provide integrated views of business
organizations. Competitiveness is the way companies and people or organizations to meet the test of
markets while at the same time maintaining or expanding the real income of its people.
Competitiveness is measured by productivity; productivity is measured by dividing units of output
by units of input. A great sign of productivity is when the economy expands its capacity to supply

Ten years later, he finds a home at a failing dojo, only to be caught up in a string of murders related
to an opium ring. His commitment to never taking a life is put to the test as the people he 's come to
care about are threatened by figures from his past. Written by Tyler Herbolsheimer
Plot Summary: Himura Kenshin is a vagabond with a dark past and sunny disposition. Not a ronin
but a rurouni, he was never a samurai, but an assassin of utmost skill in the Meiji restoration, who in
the turning point of the war simply walked away. His travels lead him to Tokyo in the 11th year of
the Meiji era, where he befriends a female Kendo master, a former thief, a brawler and a doctor all
with their own secrets. Together they fight off the enemies surfacing from the dark past that Kenshin
cannot escape.

sheets verifying that they were sanitized and restocked. Furthermore, changing tables, hand
sanitizers and baby wipes should be available in all restrooms. Even the music in the restrooms was
an added bonus. Third, cloth napkins and finer utensils

Therefore the innovation being introduced in the report will focus on the pain management tool for assessing chronic conditions in service users with dementia in hospital settings. As pain is a very subjective experience it is important that the tool is specifically based on the individual's own way of expressing and communicating their pain in relation to their chronic conditions. The first section of this report will review and evaluate a body of evidence to support the development of the pain management tool by looking at the subject of the change management proposal, the relevant stake holders and the change agent to support the change. Six Sigma is a union of of methods and instruments that are created for one sole purpose–process improvement. An engineer named Bill Smith, who worked at the cellphone company, Motorola, in 1986, first initiated this principle. 9 years later, Jack Welch from General Electric made Six Sigma central to his business strategy. The name Six Sigma was originated from the Greek letter sigma, which has a function of denoting variation from a certain standard. In statistics, the word sigma measures how much is the distance of a certain process with perfection. The main idea of Six Sigma is that if the "defects" or flaws of a certain process can be defined, then it is certain that the aforementioned "defects" and flaws can be eliminated in order to achieve perfection or the stage of "zero defects". In order to achieve the aforementioned goal, a process must be able to produce no more than 3.4 defects per million opportunities. An "opportunity" in this case is defined as a moment of ... • 37. What Was W. Edwards Deming 's Quality Philosophy? Essay 1. What was W. Edwards Deming's quality philosophy? The key ideas of Deming 's on quality lies in his understanding the importance of change. In Out of the crisis he states: "The key problem in both management and leadership is failing to understand the information in variation". Deming was engaged with, why things do not behave as predicted? Every system will have variation, but he disagrees that it is crucial for managers to be able to differentiate between common and special causes of variation. Deming then developed a theory of variation – in which special causes of modifications are mostly easily available to quickly noticeable factors such as change of shift or operator, changes of procedure etc., but that most common causes will stand when special causes have been eliminated. These common causes are often identified by workers, but only managers will have the power or authority to change them to evade repeated occurrence of the same problem. He also estimated that management will be answerable for more than 85% of the causes of variation. The Deming Cycle which is also known as the Shewhart Cycle, often called the PDCA, which was a result of the need to link the assemble of products with the needs of the customer along with departmental resources to meet those needs. Another popular test is called "shake and bake", in which the product is mounted on a vibrator in an environmental oven, and operated at progressively more extreme vibration and temperatures until something fails. The failure is then isolated and engineers design an improvement. A commonly–discovered failure is for the product to disintegrate. If fasteners fail, the improvements might be to use measured–tension nutdrivers to ensure that screws don 't come off, or improved adhesives to ensure that parts remain glued. If a gearbox wears out first, a typical engineering design improvement might be to substitute a brushless stepper motor for a DC motor with a gearbox. The improvement is that a stepper motor has no brushes or gears to wear out, so it lasts ten or more times as long. The stepper motor is more expensive than a DC motor, but cheaper than a DC motor combined with a gearbox. The electronics are radically different, but equally expensive. One disadvantage might be that a stepper motor can hum or whine, and usually needs noise–isolating mounts. Often, a "TQMed" product is cheaper to produce because of efficiency/performance improvements and because there 's no need to repair dead–on–arrival products, which represents an immensely more desirable product.
