complaint department telling them that this is very frustrating us a lot and that it is making wish
that I have never signed up for such promotion in the first place and was to the point of canceling

Questionnaire (SIQ) is a self report assessment designed to determine an adolescent s present
level of suicidal ideation. Adolescents completing the SIQ are asked to report how often they
have suicidal thoughts. There are two forms of the SIQ, the SIQ and the SIQ JR. The SIQ, a 30
item scale, is developed for senior high schoolstudents in grades 10 12, and the SIQ JR, a 15 item
scale, is developed for junior high school students in grades 7 9. The SIQ is reported to require 5
10 minutes to complete and can be used in both large scale screening as well as individual
assessment. The SIQ seeks to assess thoughts of suicide, or suicidal ideation and not suicidal
behavior. As such, test administers are instructed to refer to individuals with high scores as scoring
high in suicidal ideation and not as suicidal.
DEVELOPMENT. Suicidal ideation as a construct is defined as the domain of thoughts and ideas
about: death, suicide, and serious self injurious behaviors, including thoughts related to the
