This document outlines the terms for an offline HDFC form filling process. Centers would be provided hard copy forms to fill out and upload daily, with a minimum of 2,000 forms per day. The rate per form is Rs. 6. There are 14 fields to fill out. Billing is monthly. A minimum of 10 seats is required. An upfront payment of Rs. 1.2 lakh is due, with a 10% royalty for 3 months. The contract period is 11 months. Interested centers must submit various legal documents and certifications, pay Rs. 1 lakh in business fees and Rs. 20,000 in consultancy charges, and ensure all information provided is true.

1) Center sends LOI, signs MOU.
2) Center submits following legal papers & certifications :
Bank Statements for last 6 months + IT Returns of 1 years)
3) Company Profile with Pictures of Existing Infra.
4) Pan card Snapshot Required.
5) Center has to pay Rs 1 lac business procurement fee & Rs.20,000 Consultancy Charge.

2. 6) Center has to ensure that all that is mentioned in the company profile and other legal papers &
certification is true and correct.
7) Center has to send LOI & sign MOU.
