This document outlines the process and requirements for signing up centers to complete data entry work for Life Insurance Corporation of India. Centers would fill data into online forms, with a minimum of 3000 forms required per seat per month. Payment is made monthly at a rate of Rs. 5 per page with 20 fields per form. To sign up, centers must pay a Rs. 1,50,000 business procurement fee per 10 seat slot and submit legal documents including bank statements, tax returns, and a company profile with infrastructure photos.

No. of Forms: Unlimited (Minimum 3000 forms have to completed per seat.)
Payment Cycle: Monthly
Rate: Rs 5 per page
No of Entries in one form 20 fields
Sign up: Directly with LIC Local Operator.
1 slot: 10 seats minimum.
Business Procurement Fees: Rs.1,50,000 per slot
Agreement (After this only Center would be confirmed for Sign up)
Sign up Procedure:
1.Send your Company profile.
2.After giving the amount the sign up will happen at the LIC office.
3.Training
4. Advance & Go live.

2. Requirements :
1) Center sends LOI, signs MOU.
2) Center submits following legal papers & certifications :
Bank Statements for last 6 months + IT Returns of 1 years)
3) Company Profile with Pictures of Existing Infra.
4) Pan card Snapshot Required.
5) Center has to pay Rs. 1.50 lac business procurement fee & Rs.25,000 Consultancy Charge.
6) Center has to ensure that all that is mentioned in the company profile and other legal papers &
certification is true and correct.
7) Center has to send LOI & sign MOU.
