Thermals Physiology and Homoeopathy
Gibson Miller defines general thermal modality as ‘the effect of different temperature upon the patient as a whole.’
Regulation of body temperature
Man is a Homoeothermic animal and thermoregulation is managed by the pre-optic anterior hypothalamus.
The mechanism of temperature of body in every individual differs depending upon his cellular and biochemical reactions. The normal
temperature of body is 37 ºC (98.60 F) in human beings
There are two centers in the Hypothalamus, which are concerned with temperature regulation
1. Heat loss center.
2. Heat gain center.
Skin and anterior hypothalamus have more heat sensitive neurons, while internal organs and posterior hypothalamus have more cold
sensitive neurons.
The impulses from skin and anterior hypothalamus are carried to posterior hypothalamus and then the appropriate response is given.
All the temperature control mechanisms continually attempt to bring the body temperature back to the set point level that is 37.1ºC

Behavioral control of temperature
Whenever the internal body temperature becomes too high, signals from the temperature-controlling areas in the brain give the
person a psychic sensation of being overheated; so all the mechanisms to decrease body temperature are stimulated. The behavior of
the person also changes; he wants fanning or air conditioner for comfort.
Whenever the body becomes too cold, signals from the skin and deep body receptors elicit the feeling of cold discomfort. Therefore
the person makes appropriate environmental adjustment to re-establish comfort, such as moving to a heated room or wearing warm
clothing.
Dr. Hahnemann while testing cinchona officinalis (china off.) the first medicine, noted down the thermal sensation (heat sensation) i.e.
fever and chill sensation i.e. rigor. Every remedy that was discovered by him manifests its own thermal sensations. Therefore, Dr.
Hahnemann has comprehended that thermals are important in forming the symptom totality and in selecting exact similimum

Every medicine contains all the three types of thermal sensations but their thermal presentation is different according
to atmospheric changes, weather changes, dietary changes or geographical changes. Different types are given below:
Thermally Hot : These patients are
sensitive to hot and even warm air,
summer , Sun exposure, warm clothing or
covering & also feel uneasy in a closed
room or crowded places.
For bathing, they require cold or lukewarm
water because their body cellular setup
expresses extreme sensitivity towards heat
sensations.
Hence, they feel comfortable only in cool
air/fresh open air. They enjoy fanning or
A.C. and dislike covering.
Thermally Chilly : Some individuals are
sensitive to slight cold air or even open air.
These patients require warm covering,
woolen clothing to protect their body from
cold.
They feel comfortable in warm room. Also
they are better by warm application and for
bathing they require hot or warm water in
every season.
Ambithermal : Some individuals are
equally sensitive to both heat & cold
sensation. For example, they can
neither bear heat, nor Sun exposure nor
feel better by draft of air and cold
application. Whenever they feel cold they
like to move to warm places and when they
feel warm they feel like moving to colder
places. These individuals have a mercurial
character,

Towards CHILLY : These individuals are basically highly
sensitive to draft of cold air or low temperature. But they are also
sensitive to warm weather, hot air, or Sun exposure.
Sometimes they are aggravated by cold wind or air draft, but they
are also better by cold air draft.
Example : KALI BICH (Towards Chilly)
Towards HOT : These individuals are generally sensitive to high
temperature but sometimes they are also sensitive to cold
temperature.
For ex. when riding on a bicycle, the patient needs warm covering
over the exposed parts.
These individuals are aggravated by hot bathing and hot weather
but are also better by warmth.

Many consider some other rubrics like ‘heat vital lack of’ and ‘tendency to take cold easily’ etc. rubrics for chilly patient.
To quote Gibson Miller himself on errors in forming rubrics;
.“Another frequent source of error is the tendency to mistake any undue readiness to perspire as an indication that heat aggravates.
On the other hand, many confuse an undue tendency to catch cold, with aggravation from cold, but when we have eliminated these
errors and find the patient markedly aggravated as a whole by heat or cold, we are greatly aided in our choice of remedy.”

Selection of the similimum:
● The pain in stomach was > hot drinks ; even wine which suits her generally had to be
given up owing to its coldness, and as Phos. patients crave cold drinks (even ices) in
gastric troubles, we are left with Ars.
Follow up results:
● Pain > (which had been present for years) in a few days,
● In a few weeks she was almost well, being much less excitable.
● This case shows the importance of Generals in their order - mental, climatic, desires
and aversions in food ; all of which must be markedly present to be of any value.

Remarks
● Nat-mur. seems to suit the case, not because it is numerically highest alone, but because it corresponds to the type of the
patient.
● Nat-mur. has marked aversion to fats, which this patient desires : but that would not rule out the drug for this reason ; that,
though the desires and aversions are general symptoms, they rank much lower in the scale than the mental symptoms. A
strong mental symptom like the markedly < consolation would rule out many of lesser importance. Puls. again could never
suit this case, no matter how great its numerical value, because of the marked < from consolation. Repertory work is never
mechanical, and is only a guide to the study of Materia Medica.

Follow up results
● Dec. 9th, 1910 - Natrum-mur. 200, 3 doses.
● Dec. 15th, 1910 - A marked aggravation of the headache three days after taking the powders.
● February 14th, 1911 - Not had a bad headache since ; no heaviness or caged-in feeling. Bowels began to act naturally soon
after the medicine. Hardly any flushes of heat. Much better generally ; got up after influenza and not felt shaky, used to take her
weeks to recover. No medicine.
● February 20th, 1911 - Got bad colic due to chill which caused return of headache for one day only. Natrum-mur. 200, 3 doses.
● April 7th, 1911 - No real headache since last note, slight attempts ; no caged-in feeling. Bowels quite regular. Bearing down with
burning and distension entirely gone. No flushes of heat. No medicine.
● May 16th, 1911 - A little vertigo on first lying down : goes off immediately. Headache absent despite severe thunderstorm which
formerly caused her to go to bed ; not once the cased-in sensation. Bowels still good.
● Generally a different person in every way ; brighter ; more cheerful ; husband declares her to be a different person to live
with. Nat-m. 200, 3 doses.
● December 27th, 1911 - May have had a slight attempt at headache, but never came to anything ; feels a new creature ; able to
do much more work ; very energetic ; general health excellent. Had no medicine since above.

SLIDESMANIA Lesson 1.2 Lesson 1.3 Materials Videos Activities Lesson 1.1 Thermals Physiology and Homoeopathy Gibson Miller defines general thermal modality as ‘the effect of different temperature upon the patient as a whole.’ Regulation of body temperature Man is a Homoeothermic animal and thermoregulation is managed by the pre-optic anterior hypothalamus. The mechanism of temperature of body in every individual differs depending upon his cellular and biochemical reactions. The normal temperature of body is 37 ºC (98.60 F) in human beings There are two centers in the Hypothalamus, which are concerned with temperature regulation 1. Heat loss center. 2. Heat gain center. Skin and anterior hypothalamus have more heat sensitive neurons, while internal organs and posterior hypothalamus have more cold sensitive neurons. The impulses from skin and anterior hypothalamus are carried to posterior hypothalamus and then the appropriate response is given. All the temperature control mechanisms continually attempt to bring the body temperature back to the set point level that is 37.1ºC Next

The behavior of the person also changes; he wants fanning or air conditioner for comfort. Whenever the body becomes too cold, signals from the skin and deep body receptors elicit the feeling of cold discomfort. Therefore the person makes appropriate environmental adjustment to re-establish comfort, such as moving to a heated room or wearing warm clothing. Dr. Hahnemann while testing cinchona officinalis (china off.) the first medicine, noted down the thermal sensation (heat sensation) i.e. fever and chill sensation i.e. rigor. Different types are given below: Thermally Hot : These patients are sensitive to hot and even warm air, summer , Sun exposure, warm clothing or covering & also feel uneasy in a closed room or crowded places. For bathing, they require cold or lukewarm water because their body cellular setup expresses extreme sensitivity towards heat sensations. Hence, they feel comfortable only in cool air/fresh open air. They enjoy fanning or A.C. and dislike covering. Thermally Chilly : Some individuals are sensitive to slight cold air or even open air. These patients require warm covering, woolen clothing to protect their body from cold. They feel comfortable in warm room. Also they are better by warm application and for bathing they require hot or warm water in every season. Ambithermal : Some individuals are equally sensitive to both heat & cold sensation. For example, they can neither bear heat, nor Sun exposure nor feel better by draft of air and cold application. But they are also sensitive to warm weather, hot air, or Sun exposure. Sometimes they are aggravated by cold wind or air draft, but they are also better by cold air draft. Example : KALI BICH (Towards Chilly) Towards HOT : These individuals are generally sensitive to high temperature but sometimes they are also sensitive to cold temperature. For ex. when riding on a bicycle, the patient needs warm covering over the exposed parts. These individuals are aggravated by hot bathing and hot weather but are also better by warmth. To quote Gibson Miller himself on errors in forming rubrics; .“Another frequent source of error is the tendency to mistake any undue readiness to perspire as an indication that heat aggravates. A strong mental symptom like the markedly < consolation would rule out many of lesser importance. Puls. again could never suit this case, no matter how great its numerical value, because of the marked < from consolation. Bowels began to act naturally soon after the medicine. Hardly any flushes of heat. Much better generally ; got up after influenza and not felt shaky, used to take her weeks to recover. No medicine. ● February 20th, 1911 - Got bad colic due to chill which caused return of headache for one day only. Natrum-mur. 200, 3 doses. ● April 7th, 1911 - No real headache since last note, slight attempts ; no caged-in feeling. Bowels quite regular. Bearing down with burning and distension entirely gone. No flushes of heat. No medicine. ● May 16th, 1911 - A little vertigo on first lying down : goes off immediately. Headache absent despite severe thunderstorm which formerly caused her to go to bed ; not once the cased-in sensation. Bowels still good. ● Generally a different person in every way ; brighter ; more cheerful ; husband declares her to be a different person to live with. Nat-m. 200, 3 doses. ● December 27th, 1911 - May have had a slight attempt at headache, but never came to anything ; feels a new creature ; able to do much more work ; very energetic ; general health excellent.
