This document presents the design of a telemedical system for remote monitoring of cardiac insufficiency. The system includes an electrocardiography (ECG) device that collects and digitizes ECG signals. The ECG signals undergo digital signal processing including autocorrelation analysis. Graphical interfaces allow patients and doctors to view ECG data and attenuation coefficients derived from autocorrelation analysis. Data is transmitted between parties using TCP/IP protocol. The system aims to facilitate remote monitoring of cardiac patients to reduce hospitalizations through early detection of health changes.

This report goes over low power Arduino, mobile platform Panda board and Zigbee technology to couple ECG over mobile board.

Abstract This paper proposes a new design to monitor the Heart Rate from Electrocardiogram (ECG) signal. The proposed design is based on the concept of identifying the voltage level of the R-wave complex component of the ECG signal above a threshold level. A 100 Hertz sample rate is selected to sample the complex ECG signal. A dual-counter based calculation method is used to obtain the mathematical value of Heart Rate. The proposed FPGA based ECG Heart Rate monitoring system can operate with high performance with respect to the low-power and high speed. PERFORMANCE EVALUATION OF ARTIFICIAL NEURAL NETWORKS FOR CARDIAC ARRHYTHMIA C...IAEME Publication
In this paper an effective and most reliable method for appropriate classification of cardiac arrhythmia using automatic Artificial Neural Network (ANN) has been proposed. The results are encouraging and are found to have produced a very confident and efficient arrhythmia classification, which is easily applicable in diagnostic decision support system. The authors have employed 3 neural network classifiers to classify three types of beats of ECG signal, namely Normal (N), and two abnormal beats Right Bundle Branch Block (RBBB) and Premature Ventricular Contraction (PVC). The classifiers used in this paper are K-Nearest Neighbor (KNN), Naive Bayes Classifier (NBC) and Multi-Class Support Vector Machine (MSVM). The performance of the classifiers is evaluated using 5 parametric measures namely Sensitivity (Se), Specificity (Sp), Precision (Pr), Bit Error Rate (BER) and Accuracy (A). Hence MSVM classifier using Crammers method is very effective for proper ECG beat classification.
Evaluation of patient electrocardiogram datasets using signal quality indexingjournalBEEI
Electrocardiogram (ECG) is widely used in the hospital emergency rooms for detecting vital signs, such as heart rate variability and respiratory rate. However, the quality of the ECGs is inconsistent. ECG signals lose information because of noise resulting from motion artifacts. To obtain an accurate information from ECG, signal quality indexing (SQI) is used where acceptable thresholds are set in order to select or eliminate the signals for the subsequent information extraction process. A good evaluation of SQI depends on the R-peak detection quality. Nevertheless, most R-peak detectors in the literature are prone to noise. This paper assessed and compared five peak detectors from different resources. The two best peak detectors were further tested using MIT-BIH arrhythmia database and then used for SQI evaluation. These peak detectors robustly detected the R-peak for signals that include noise. Finally, the overall SQI of three patient datasets, namely, Fantasia, CapnoBase, and MIMIC-II, was conducted by providing the interquartile range (IQR) and median SQI of the signals as the outputs. The evaluation results revealed that the R-peak detectors developed by Clifford and Behar showed accuracies of 98% and 97%, respectively. By introducing SQI and choosing only high-quality ECG signals, more accurate vital sign information will be achieved.

ECG (electrocardiogram) is very essential component for the doctors to diagnose the state of patient’s cardiovascular system. In critical situations doctors may need to examine ECG of patient instantly to take a firm and better decision in their absence near patient. In this paper a better way of instant ECG datatransfer, processing and display is demonstrated. Here ECG is acquired using simple 3 electrode single lead configuration then it is digitized and transmitted to Android smart phone in SMS message format. This SMS data is a bundle of values representing digital ECG. Acquired SMS data is fetched from inbox of the phone and processed for calculation of heart rate and detection of arrhythmia by Android application software. Then ECG is displayed on phone screen along with conclusion of heart rate and arrhythmia (if any).

The price of electrocardiograph (ECG) machine on the market is very high. Currently, the technology used is still very complicated and ineffective, and the ECG machine cannot be connected to other devices. A new development of a low-cost ECG machine with a customized design was needed to integrate the machine with other devices. Therefore, the purpose of this study is to develop a low-cost ECG machine which can be connected to other devices and equipped with sensitivity and paper speed setting. So that portable ECG machines can be produced and used at small clinics in the society. In this study, the main controller of the 12 channels ECG machines was supported by ATMEGA16 microcontroller, that is available on the market at low prices. The main part of the ECG amplifier is built using a high common mode rejection ratio (CMRR) instrumentation amplifier (AD620) and a bandpass filter which the cutoff frequency for highpass filter and lowpass filter are 0.05 Hz and 100 Hz, respectively. In order to complement the previous study, some features were introduced such as selectivity and motor speed option. In this study, 10 participants are involved for data acquisition,and an ECG phantom was used to calibrate the machine. The performance of the ECG machine was evaluated using standard measurement namely relative percentage error (% error) and uncertainty (UA). The result shows that %error from all of the feature is less than 2% and the UA is 0.0 which shows that the ECG machine is feasible for diagnostic purposes.

Abstract The electrocardiogram (ECG) is a well known method that can be used to measure Heart Rate Variability (HRV). This paper describes a procedure for processing electrocardiogram signals (ECG) to detect Heart Rate Variability (HRV). In recent years, there have been wide-ranging studies on Heart rate variability in ECG signals and analysis of Respiratory Sinus Arrhythmia (RSA). Normally the Heart rate variability is studied based on cycle length variability, heart period variability, RR variability and RR interval tachogram. The HRV provides information about the sympathetic-parasympathetic autonomic stability and consequently about the risk of unpredicted cardiac death. The heart beats in ECG signal are detected by detecting R-Peaks in ECG signals and used to determine useful information about the various cardiac abnormalities. The temporal locations of the R-wave are identified as the locations of the QRS complexes. In the presence of poor signal-to-noise ratios or pathological signals and wrong placement of ECG electrodes, the QRS complex may be missed or falsely detected and may lead to poor results in calculating heart beat in turn inter-beat intervals. We have studied the effects of number of common elements of QRS detection methods using MIT/BIH arrhythmia database and devised a simple and effective method. In this method, first the ECG signal is preprocessed using band-pass filter; later the Hilbert Transform is applied on filtered ECG signal to enhance the presence of QRS complexes, to detect R-Peaks by setting a threshold and finally the RR-intervals are calculated to determine Heart Rate. We have implemented our method using MATLAB on ECG signal which is obtained from MIT/BIH arrhythmia database. Our MATLAB implementation results in the detection of QRS complexes in ECG signal, locate the R-Peaks, computes Heart Rate (HR) by calculating RR-internal and plotting of HR signal to show the information about HRV. Due to the growing number of cardiac patients, an automatic detection that detects various heart abnormalities has been developed to relieve and share physicians’ workload. Many of the depolarization of ventricles complex waves (QRS) detection algorithms with multiple properties have recently been presented; nevertheless, real-time implementations in low-cost systems remain a challenge due to limited hardware resources. The proposed algorithm finds a solution for the delay in processing by minimizing the input vector’s dimension and, as a result, the classifier’s complexity. In this paper, the wavelet transform is employed for feature extraction. The optimized neural network is used for classification with 8-classes for the electrocardiogram (ECG) signal this data is taken from two ECG signals (ST-T and MIT-BIH database). The wavelet transform coefficients are used for the artificial neural network’s training process and optimized by using the invasive weed optimization (IWO) algorithm. The suggested system has a sensitivity of over 70%, a specificity of over 94%, a positive predictive of over 65%, a negative predictive of more than 93%, and a classification accuracy of more than 80%. CLASSIFICATION OF ECG ARRHYTHMIAS USING /DISCRETE WAVELET TRANSFORM AND NEURA...IJCSEA Journal
Automatic recognition of cardiac arrhythmias is important for diagnosis of cardiac abnormalies. Several algorithms have been proposed to classify ECG arrhythmias; however, they cannot perform very well. Therefore, in this paper, an expert system for ElectroCardioGram (ECG) arrhythmia classification is proposed. Discrete wavelet transform is used for processing ECG recordings, and extracting some features, and the Multi-Layer Perceptron (MLP) neural network performs the classification task. Two types of arrhythmias can be detected by the proposed system. Some recordings of the MIT-BIH arrhythmias database have been used for training and testing our neural network based classifier. The simulation results show that the classification accuracy of our algorithm is 96.5% using 10 files including normal and two arrhythmias.

The electrical activities of the heart are used to monitor cardiovascular diseases. It can be measured using electrocardiogram (ECG), a simple, painless test that can be recorded graphically. The physician, to predict the patient’s heart conditions and recommend suitable treatments, uses electrodes placed on the patient’s skin surface, to record these signals. The P, Q, R, S, T waves in the ECG signal can be used to determine the normality and abnormality of the heart's condition. The time interval differs for each cardiovascular condition of the heart. In this work, the ECG signal is acquired real-time using an intelligent sensor module, and the recorded value is processed to find the peak values. The data is sent to the web server using internet of things technology at a minimal time, where the physician can view it and proper decision can be taken. The real-time ECG data acquisition is also made using the field programmable gate array kit as it is a low cost, high-speed device and the output is viewed in the computer. The developed model is validated through MATLAB software and implemented for real-time applications.

It was designed specially to help patients with heart disease.This includes three common sub units. They are Circuit, Analysis Algorithm, and Bluetooth Communication. The first one is to wear on the wrist of the patient to captures the abnormal heart beat waves from the victim and the alternate methods are installed under the stick. This project is based on the previous project “Wireless Heart Attack Detector with GPS†of Fall 2004 [1]. we consider a big improvement in reducing the complexity of the project greatly, in saving power consumption of the project to run much fewer codes and in making the project to run at a faster time. No wire is attached to the wrists. In our project, the ECG waveform is transmitted wirelessly from the wrists to the watch. This gives the user great flexibility while the program is switched on and running. User can drive safely, can use restroom easily and can function normally like without the project. Previous project had the wire connection. All the hardware on the walking watch would have been strapped to the wrists. This will not make the project functional and marketable. Our project is completely portable. Heart condition is displayed in our project. The previous project did not inform the user about his heart condition. We display the heart condition through two LEDs as low-risk (alert level between 4 and 6) and high risk (alert level between 7 and 9). The user can know their heart condition and take proper action to avoid the fatal moment. Proper action could be slowing down and taking a rest.

In the various field of life person identification is an essential and important task. This helps for the investigation of criminal activities and used in various type of forensic applications like surveillance. For biometric recognition iris, face, voice and fingerprint have a limited fabrication and from there the exact decision regarding liveliness of the subject can be drawn. The aim of the approach is to construct a biometric recognition system based on ECG which processes the raw ECG signal. The entire process is supported by different filters for noise elimination and ECG characteristics waves gone through time domain analysis. Based on the analysis an efficient feature extraction model is developed where several best P-QRS-T signal parts are taken and the positions of the fragmented signals are normalized depends on the priorities of their positions. The calculation of domain features done 72 times. It checks the data sets (train and test) and from feature vector matching to each of the individual signal, separately. The performance and utility of the system are analyzed and feature vectors are examined by different classification algorithms of machine learning. The leading algorithms like K-nearest neighbor, artificial neural network and support vector machine are used to classify different features of ECG, and it is tested using standard cardiac database i.e. the MIT-BIH ECG -ID database.

Electrocardiogram (ECG) signals are normally affected by artifacts that require manual assessment or use of other reference signals. Currently, Cardiographs are used to achieve basic necessary heart rate monitoring in real conditions. This work aims to study and identify main ECG features, QRS complexes, as one of the steps of a comprehensive ECG signal analysis. The proposed algorithm suggested an automatic recognition of QRS complexes in ECG rhythm. This method is designed based on several filter structure composes low pass, difference and summation filters. The filtered signal is fed to an adaptive threshold function to detect QRS complexes. The algorithm was validated and results were checked with experimental data based on sensitivity test.

Electrocardiogram (ECG) is the cheap and noninvasive method of depicting the heart activity and abnormalities.
It provides information about the functionality of the heart. It is the record of variation of bioelectric potential
with respect to time as the human heart beats. The classification of ECG signals is an important application since
the early detection of heart diseases/abnormalities can prolong life and enhance the quality of living through
appropriate treatment. Since the ECG signals, while recording are contaminated by several noises it is necessary
to preprocess the signals prior to classification. Digital filters are used to remove noise from the signal. Principal
component analysis is applied on the 12 lead signal to extract various features. The present paper shows the
unique feature, point score calculated on the basis of the features extracted from the ECG signal. The point
score calculation is tested for 40 myocardial infarction ECG signals and 25 Normal ECG signals from the PTB
Diagnostic database with 94% sensitivity.
The ECG signals captured from the body of the patient using three electrode model is processed and
conditioned by the analog front end device is finally sent to the data acquisition unit. The data acquisition
unit used is the user pc/ laptop with MATLAB. Using very specific image processing techniques the critical
intelligence from the captured image is extracted. From this processed image any sort of abnormal
conditions is determined which is informed to the corresponding doctor via text message. Simultaneously
the processed image is sent to the doctor mail by using specific TCP/IP protocol.

Abstract

In this paper newly adapted algorithm is used for the filtering of ECG signal that is a Swarm algorithm which is used for the Error signal optimization from the original corrupted ECG signal. This algorithm is implemented with Adaptive filter to removes Power Line Interference noise having Frequency component of 50 Hz. The ECG signal considered may be retrieved from ECG acquisition system or from MIT-BIH database.

A Joint QRS Detection and Data Compression Scheme for Wearable Sensorsecgpapers
Abstract—This paper presents a novel electrocardiogram (ECG)
processing technique for joint data compression and QRS detection
in awireless wearable sensor. The proposed algorithm is aimed
at lowering the average complexity per task by sharing the computational
load among multiple essential signal-processing tasks
needed for wearable devices. The compression algorithm, which is
based on an adaptive linear data prediction scheme, achieves a lossless
bit compression ratio of 2.286x. The QRS detection algorithm
achieves a sensitivity (Se) of 99.64% and positive prediction (+P)
of 99.81% when tested with the MIT/BIH Arrhythmia database.
Lower overall complexity and good performance renders the proposed
technique suitable for wearable/ambulatory ECG devices.

Abstract: The Telemedicine is a new form of improvement of health care quality through new information and
communication technologies, it also creator of a new services facilitating the daily lives of patients with chronic
diseases such as cardiac insufficiency, like professionals, allowing them to be in permanent relation with their
patients receiving vital and pertinent parameters representative of their physiopathological states.
We propose in this work the presentation of a telemedical interface development dedicated to telemonitoring of
cardiac function through a correlative analysis.
This interface comprises a DTE (Data Terminal Equipment Medical Treatment) which is dedicated to recording
of the myocardium electrical activity (electrocardiogram ECG) revealing an eventual deficiency of cardiac
function, using an electrocardiographic amplifier constructed around a microcontroller 16F876A, Charged to
digitizing the signals issued from EDTM and transfer them to a terminal local computer. A Soft interface is
developed on Visual Basic environment, which is charged to control the acquisition, correlative analysis,
archiving and the transfer of a medical data through medical networks via TCP / IP protocol for purposes such
as prevention of sudden death, therapeutic monitoring, medical assistance at home and the urgent intervention
of telemedical actors.

I. INTRODUCTION
Cardiac insufficiency [1] is the incapacity of heart to pump enough blood to respond the needs of the
[2] organism. It is a chronic disease touching approximately 2% of the world population and 10% of patients
older than 80 years [3]. This last is at the origin of repeated hospitalizations and an impaired quality of life of
patients with a very high risk of sudden death touching two on three patients [3]. Indeed, 40% patients die in the
year of diagnosis cardiac Insufficiency.

The objective of our work is Therefore to realize a telemedical channel able to ensure a telemonitoring of
cardiac function by the acquisition of electrocardiographic signals.
These ECG signals benefit from a digital processing realizing the following functionality:
The autocorrelation, the layout of the envelope of the autocorrelation function, the approximation of the
envelope of the autocorrelation function by determining their respective attenuation coefficients.
A follow-up in telemonitoring of patients with cardiac Insufficiency [4] and the closer clinical monitoring of
patients suffering with this disease limits the number of hospitalizations [5].
II. MATERIALS AND METHODS
II.1. COLLECTION OF ELECTROCARDIOGRAPHIC SIGNAL
The ECG signal is collected on derivation DI [6] by means of a classical amplifier of instrumentation (figure1)
[7].
It was constructed around a 16F876A microcontroller [8] equipped with a 10bits ADC module and a USART
module with Sampling frequency of 2,4KHz .
Local Protocol Communication (RS232): The parameters RS232 [9] which we have used are:
Transmission speed 57600 bauds
8 bits of data
A bit of parity
A bit of stop
Distant Protocol Communication: The distant transmission of the electrocardiographic signals is done according
to protocol TCP/IP [10] in order to connect the different actors of telemedicine.

II.2. CORRELATIVE ANALYSIS
Correlative Analysis permits the calculation and plot of the autocorrelation function of the ECG signal. The
calculating algorithm of the auto correlation discrete functions has been implemented in accordance with the
following relation definition (i.e., (1)) [11]:

III. RESULTS
In this work, we are targeting the problematic of telemonitoring of cardiac Insufficiency by a correlative
analysis showing in particular the attenuation coefficients, which seem to be
relevant indices for pathological cases.
III.1. INTERFACE OF ECG ACQUISITION AND FILES TRANSFER
We present the interface of communication and ECG file transfer between the patient and the doctor
implemented on Visual Basic environment.
The principal window of the patient consists of command buttons to manage connection, transfer and display of
the patient files recorded previously.
The figure 2.a and the figure 2.b show the transfer interface of the file patient towards the televigilance center or
telemedical center.

III.2. AUTOCORRELATION FUNCTION
To extract the information from different signal we have used the autocorrelation function of an ECG signal is
shown in figure 3

III.3. LAYOUT OF THE ENVELOPE OF THE AUTOCORRELATION FUNCTIONS
Establishing relevant indices of informative processes such as physiological signals is fact by means of the
envelope of the autocorrelation function which allows to deduce the attenuation coefficients of this one. It seems
to us interesting to put at the disposal of medical practitioners these indices for the prevention of acute phases of
cardiac Insufficiency.
The determination of the envelope and the calculation of the attenuation coefficients is done by using the low
pass filter.
The transfer function of an ideal low pass filter (i.e., (2)) is:

This filter is use to extract the averages values or the signal envelope.

coefficients using the algorithm that involves a decreasing exponential (figure 5) according to the following
definition equation (i.e., (6)):

IV. CONCLUSION
The assumption of responsibility of the chronic cardiac insufficient patient is a real problem of health
public.
The incidence of this disease increases considerably with age and exposes patients to several hospitalizations.
This paper is for us the occasion to present works which are articulated around the design of a telemedical
support dedicated to telemonitoring of the cardiac function by the means of a cardiac functional exploration
using electrocardiography which permits to evaluate the severity of cardiac deficiency.
The elaboration of a telemonitoring process of the cardiac function by electrocardiography appeared to us the
most interesting because of its mobility, its non invasivity, its reliability as well as the facility of its use by the
patient himself or any else actor of the telemedicine.
The local control parameters established a hardware and software device based on a microcontroller and RS232
protocol. The distant control uses the TCP / IP protocol involving the Winsock component in Visual Basic
programming environment.
