A case presentation on infected non unionAtanu Kayal
This document describes a case study of a patient with an infected non-union of the left distal femur fracture treated using the Masquelet technique. The patient underwent debridement, antibiotic cement spacer placement with external fixation, followed by bone grafting and plating. However, the initial plating failed after 8 months, requiring another surgery with fibular grafting and double plating for stability. At the 8-month follow up, the patient had healed with no pain or infection and was walking normally. The conclusion drawn is that Masquelet technique for large distal femur defects should use double plating rather than single plating for sufficient stability.
Limb lengthening with a submuscular locking plate provides an alternative for patients where lengthening over an intramedullary nail would be difficult, such as those with open growth plates or narrow medullary canals. The procedure involves submuscular plating followed by external fixation and corticotomy for lengthening, then locking the distal segment and removing the external fixator. In a study of 10 patients who underwent this procedure, the mean lengthening was 4cm with a healing index of 48.1 days/cm and no major complications. This technique permits earlier removal of the external fixator compared to external fixation alone.
This document summarizes a journal article discussing the use of noncontact locking plates as an internal fixator for open fractures. Some key points:
- Open fractures involve communication between the external environment and fracture site, complicating treatment. Traditional fixation methods like plates and screws risk high infection rates.
- The study evaluated outcomes of using noncontact locking plates in 42 patients with open tibia or femur fractures. All fractures united within 19.7 weeks on average with minimal complications.
- Advantages of this technique include minimizing contact between implant and bone to reduce risks of infection, while still providing stable fixation comparable to traditional plating. The results were satisfactory compared to other studies.
This document describes the steps of a laparoscopic appendectomy procedure. It begins by outlining the patient positioning in the supine and Trendelenburg positions. It then discusses the trocar placement and preparation steps like catheterization and insufflation. The key steps of the procedure are exposing the appendix, controlling the mesentery with ligation or clips, dividing the appendix base with sutures, extracting the specimen, and performing irrigation and closure of the trocar sites. Postoperative care including pain management and typical length of stay are also summarized.

This document provides information about different options for breast reconstruction after mastectomy for breast cancer. It discusses evaluating factors like cancer treatment history and body type. Reconstruction options include tissue expanders/implants or autologous flaps using the patient's own tissue, like latissimus dorsi or DIEP flaps. Each option has pros and cons in terms of operation length, recovery, complications and long term results. The full reconstruction process takes about a year from mastectomy to final areola tattoo.
This document discusses casts and slabs for immobilizing musculoskeletal injuries. Casts fully surround the limb while slabs only partially surround. Casts provide better immobilization but have higher risks of complications like pressure sores. The document outlines indications, advantages, disadvantages and application steps for casts and slabs. It emphasizes the importance of monitoring for complications, starting physiotherapy early, and removing the cast or slab as soon as possible.

Experience with semi occlusive dressing in fingertip injuriesCAMILA AZOCAR
This document describes a case series study that evaluated the use of semi-occlusive dressings for fingertip injuries. The study included 6 male patients between 40-65 years old who sustained distal phalanx fractures from saws, knives, or animal attacks. The patients received antibiotics, debridement, and a semi-occlusive dressing that remained for 6 weeks until complete coverage was achieved. At 3 month follow up, all patients had good or excellent functional results on a satisfaction scale, an average QuickDASH score of 4.5, and satisfactory cosmetic results with full range of motion and no complications. The conclusion is that semi-occlusive dressings provide an alternative treatment to skin flaps for fingertip injuries with excellent
Bracing is an effective conservative treatment for adolescent idiopathic scoliosis, as demonstrated by controlled clinical trials showing bracing slows or arrests curve progression compared to no treatment. Bracing works best when worn for 23 hours per day, and treatment is generally indicated for curves between 25 and 40 degrees or lesser curves showing recent progression. Regular x-rays are needed to monitor the curve during bracing and after weaning from the brace to determine if the spine remains stable.
This study evaluated the use of external locking plates for stabilization of compound tibial fractures in 23 patients. Most fractures united without need for further intervention, with the external plate providing stabilization and load sharing during healing. Outcomes were generally good, with minimal complications and near normal range of motion achieved in most cases. The external locking plate technique provides effective stabilization with less soft tissue disruption compared to other methods, encouraging fracture union.

NPWT, also known as VAC, is a therapeutic technique that uses negative pressure to promote wound healing. It works by applying a vacuum drainage through a sealed dressing to remove excess fluid, increase blood flow, and promote the formation of granulation tissue. The key components of VAC include a polyurethane foam, vacuum pump, tubes, and transparent adhesive dressing. It has several advantages like maintaining a moist wound environment and removing interstitial fluid. NPWT is indicated for almost any open wound where closure is not feasible, such as pressure ulcers, surgical wounds, and traumatic wounds. Contraindications include necrotic tissue, exposed organs or blood vessels. Monitoring includes pressure levels, effluent,

2. For both closed and open fractures, the key steps of treatment include resuscitation, recognition of the injury, reduction of fractures, retention through immobilization, and rehabilitation.
3. Additional details are provided on splinting fractures, different methods of reduction and immobilization including casting, internal and external fixation, and the importance of antibiotic treatment and debridement for open fractures.
The document discusses managing patients after joint replacement surgery in a home setting, including reviewing the patient's history and assessments prior to an initial visit, performing a comprehensive head-to-toe assessment during the visit to evaluate wound status, medications, activity level and needs, and cognitive and home environmental factors that could impact recovery. The goal is to provide skilled nursing care, ensure pain is managed, involve therapy services, and educate the patient on wound and activity guidelines for their recovery at home post-surgery.
Community Minimally Invasive Total Hip Replacement Slideshowscottau
Minimally invasive total hip replacement aims to reduce recovery time. The direct anterior approach is promising but requires experience. It provides good exposure through a small incision without detaching muscles. Early results show less pain and faster recovery compared to traditional approaches, though blood loss and time were initially higher for surgeons adopting the technique. With a multidisciplinary team and careful patient selection, the direct anterior approach shows potential to improve outcomes for total hip replacement.

Joint contractures are a common complication of prolonged immobilization that can be prevented through early passive stretching, splinting, and other nonsurgical interventions. Once contractures develop, surgery may be required to restore range of motion and muscular balance. The article reviews evidence from 41 studies on the etiology, risk factors, prevention, and management of joint contractures resulting from prolonged immobilization.

Osvaldo Bernardo Muchanga-GASTROINTESTINAL INFECTIONS AND GASTRITIS-2024.pdfOsvaldo Bernardo Muchanga
GASTROINTESTINAL INFECTIONS AND GASTRITIS
Osvaldo Bernardo Muchanga
Gastrointestinal Infections
GASTROINTESTINAL INFECTIONS result from the ingestion of pathogens that cause infections at the level of this tract, generally being transmitted by food, water and hands contaminated by microorganisms such as E. coli, Salmonella, Shigella, Vibrio cholerae, Campylobacter, Staphylococcus, Rotavirus among others that are generally contained in feces, thus configuring a FECAL-ORAL type of transmission.
Among the factors that lead to the occurrence of gastrointestinal infections are the hygienic and sanitary deficiencies that characterize our markets and other places where raw or cooked food is sold, poor environmental sanitation in communities, deficiencies in water treatment (or in the process of its plumbing), risky hygienic-sanitary habits (not washing hands after major and/or minor needs), among others.
These are generally consequences (signs and symptoms) resulting from gastrointestinal infections: diarrhea, vomiting, fever and malaise, among others.
The treatment consists of replacing lost liquids and electrolytes (drinking drinking water and other recommended liquids, including consumption of juicy fruits such as papayas, apples, pears, among others that contain water in their composition).
To prevent this, it is necessary to promote health education, improve the hygienic-sanitary conditions of markets and communities in general as a way of promoting, preserving and prolonging PUBLIC HEALTH.
Gastritis and Gastric Health
Gastric Health is one of the most relevant concerns in human health, with gastrointestinal infections being among the main illnesses that affect humans.
Among gastric problems, we have GASTRITIS AND GASTRIC ULCERS as the main public health problems. Gastritis and gastric ulcers normally result from inflammation and corrosion of the walls of the stomach (gastric mucosa) and are generally associated (caused) by the bacterium Helicobacter pylor, which, according to the literature, this bacterium settles on these walls (of the stomach) and starts to release urease that ends up altering the normal pH of the stomach (acid), which leads to inflammation and corrosion of the mucous membranes and consequent gastritis or ulcers, respectively.
In addition to bacterial infections, gastritis and gastric ulcers are associated with several factors, with emphasis on prolonged fasting, chemical substances including drugs, alcohol, foods with strong seasonings including chilli, which ends up causing inflammation of the stomach walls and/or corrosion. of the same, resulting in the appearance of wounds and consequent gastritis or ulcers, respectively.
Among patients with gastritis and/or ulcers, one of the dilemmas is associated with the foods to consume in order to minimize the sensation of pain and discomfort.

Are you looking for a long-lasting solution to your missing tooth?
Dental implants are the most common type of method for replacing the missing tooth. Unlike dentures or bridges, implants are surgically placed in the jawbone. In layman’s terms, a dental implant is similar to the natural root of the tooth. It offers a stable foundation for the artificial tooth giving it the look, feel, and function similar to the natural tooth.
