Matricing
Matricing involves creating a temporary wall opposite prepared tooth surfaces using a matrix band and retainer. The Tofflemire matrix is a commonly used universal matrix that can be placed buccally or lingually. It has a head, sliding body, rotating spindle and set screw to secure the matrix band in place and contour it to the tooth. Other matrix types include Black's matrices, compound supported matrices, and the AutoMatrix retainerless system. Matrices are selected based on tooth location and cavity classification to restore anatomy and contacts while preventing excess material.
Microtomography of Deciduous Teeth Enamel
Purpose: To develop an in vitro model of de- and re-mineralization with an etching gel (i-Gel) and with a
mineralization varnish - Clinpro ™ White Varnish with TCP (3M).
Methods: The material used is from 20 temporary intact teeth. They are first applied throughe preparation of the
smooth temporary enamel surfaces with a 30 second demineralization with 37% phosphoric acid (i - gel -
etching gel). Then the samples are washed and dried with water and airflow. Demineralized surfaces of the
temporary teeth go through a remineralization with application of varnish - Clinpro ™ White Varnish with TCP.
The measurement was done with a desktop X-ray microtomography scanner SkyScan 1272 produced by the
company Bruker.
Results: Representative images of the observed tooth sample are shown in Figure 3. Figure 1 shows the outer
surface of the tooth on which there are visible specific roughness. Clearly differed are two areas; a lighter one,
i. e. denser, which is on the outside – the tooth enamel, and a darker one – from the inner side of the tooth –
dentin. These two areas are visible on all transverse (software) sections. Such a section is shown in Figure 1.
The enamel thickness of a test sample varies between 300 and 500 μm.
There are noticeable defects on the tooth enamel, which represent dark areas, i.e. areas of reduced density. Such
areas in the depth of the enamel are presented in Figure 3 and 5 and Figure 4, clearly shows their volumetric
nature.Monitoring of the area with "damage" of dental caries, i.e. darker areas of enamel in Figure 4 show that
the larger of the two, which is located closer to the tooth surface in fact does not go out of it. It is covered by a
layer with a thickness between 10 and 14 μm, whose density is very close, and perhaps a bit higher than that of
tooth enamel.
Conclusions:
1. CT has observations that result in such a direction that the tooth surface is uneven as there are areas with
varying degrees of unevenness. In the uneven areas, their height, potentially that of the crystals from
remineralizing coverage, is between 6 and 14 μm, with a horizontal size 14-40 μm.
2. In the tooth enamel monitor an area is observed that is "sealed" with a layer of a density close to that of
healthy dental enamel and a thickness of 10-14 μm. In cutting of a sample such layer has also emerged on its
surface. This leads to the hypothesis of the presence of the enamel coating layer whose density is very similar to
that of the enamel.

Fractured MOL composite restoration by Dr Simon McDonald

SEALING ABILITY OF MTA FOR REPAIR OF LATERAL ROOT PERFORATION
This study compared the sealing ability of mineral trioxide aggregate (MTA), amalgam, and IRM for repairing lateral root perforations using methylene blue dye. MTA showed significantly less dye penetration at an average of 0.28mm compared to amalgam at 1.52mm and IRM at 1.30mm. While amalgam and IRM showed considerable dye leakage, MTA provided the best seal of the perforations. The results suggest that MTA is superior to amalgam and IRM for repairing lateral root perforations due to its ability to better seal the perforation site.

SEALING ABILITY OF MTA FOR REPAIR OF LATERAL ROOT PERFORATION
This study compared the sealing ability of mineral trioxide aggregate (MTA), amalgam, and IRM for repairing lateral root perforations using methylene blue dye. MTA showed significantly less dye penetration at an average of 0.28mm compared to amalgam at 1.52mm and IRM at 1.30mm. While amalgam and IRM showed considerable dye leakage, MTA provided the best seal of the perforations. The results suggest that MTA is superior to amalgam and IRM for repairing lateral root perforations due to its ability to better seal the perforation site.

History:
Was introduced by Ballard in 1944, it is used to improve areas of interdental gingival recession (Turverson1980)
Indications:
1- Esthetic contouring of teeth malformed
2- When space requirement to correct the malocclusion below 2.5mm
3- If Bolton analysis shows mild tooth material discrepancy in either of arches
4- It can undertake in the lower anterior segment as an aid of retention
5- It can be undertaking as an adjunctive procedure in adult orthodontic
6- In the case of 1st premolar extraction to balance the Bolton ratio
Contraindications:
1- In young patients as they possess large pulp chamber which increased risk of pulp exposure
2- Patients with high caries index and are susceptible to caries
3- Patient with bad oral hygiene
4- Poorly formed enamel because increased risk for exposure
Advantages:
1- Avoid in border line cases where spaces required in minimal
2- Correct molars and incisors relationship by eliminate excess tooth material
3- More stable contact can be established
Disadvantages:
1- Stripping procedure creates rough surface which attract plaque formation and is caries prone
2- Increase sensitivity of the tooth may occur
3- Alteration of tooth morphology giving un-natural appearance
4- Improper contact between the teeth may invite food impaction and hence periodontal breakdown
Interproximal Procedure
The procedure of enamel stripping involves the following steps:
1- Assessing space requirements:
a- Model analysis:
- Arch parameter in upper arch and Carey’s analysis in lower
- Bolton analysis
- Peck and Peck ratio
= if arch discrepancy between 0 --- 2.5mm select stripping
2.5 ---5mm extraction of 2nd premolars
5mm and more, extraction of 1st premolars
= it has been reported that 11----13.5% of populations have overall Bolton ratio problem and
22 ---30% of orthodontic populations have anterior Bolton ratio problem (tooth size discrepancy TSD)
= estimate the mesiodistal dimension to the labiolingual thickness of lower incisors at cingulum that is 88% --- 92% -- for lower central, 90% ---95% ---- for lower lateral, long ratio need proximal re-contouring

There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
