Approximately 10% to 30% of patients submitted to BCS alone are not satisfied with the aesthetic outcomes like “swan beak/ parrot beak deformities. The main reasons are related this is the tumour resection which can produce asymmetry, retraction, and volume changes in the breast.
Recently, increasing attention has been focused on oncoplastic procedures since the immediate application of plastic breast surgery techniques provide a wider local excision while still achieving the goals of a better breast shape and symmetry to obtain oncologically sound and aesthetically pleasing results. Thus, by means of customized techniques the surgeon ensures that oncologic principles are not jeopardized while meeting the needs of the patient from an aesthetic point of view.

SSM aims to remove all breast tissue while preserving the skin and NAC. NSM further preserves the nipple. Selection criteria for NSM include small tumor size and distance from NAC. Outcomes of SSM and NSM are similar to MRM with acceptable morbidity. Complication rates of NSM include nipple necrosis around 7% and occult nipple involvement around 10%. The chest wall the bones, skin, fat, muscles, and various other tissues that protect the organs between the neck and the abdomen.

The lobules are glands that are responsible for producing milk in the breast. The lobules are in clusters and form lobes.

MILK Ducts/LACTIFEROUS DUCT
The ducts are the tubes that carry milk from the lobules to the nipple.
The lactiferous ducts are responsible for delivering the milk to the surface of the skin and out of the mother through tiny pores in the nipple.
These ducts form a tree-branch-like network that converges at the nipple.
Lactiferous ducts are known by many names, including milk ducts, mammary ducts, and galactophores.
Mammary glands
The mammary gland is a gland located in the breasts of females that is responsible for lactation, or the production of milk.
Both males and females have glandular tissue within the breasts; however, in females the glandular tissue begins to develop after puberty in response to estrogen release.
Mammary glands only produce milk after childbirth. During pregnancy, the hormones progesterone and prolactin stimulates the glandular cells to produce breast milk.
Fatty tissue
Fatty tissue, along with fibrous tissue, makes up the connective tissue that holds all the structures within the breasts in place.
The fatty tissue fills the space between glandular (lobes and ducts) and fibrous (ligaments) tissue.
The fatty tissue determines the size of the breast.
During puberty, increased estrogen levels stimulate the buildup of fatty tissue and glandular tissue which increases the size of the female breast.
Areola
The areola is the darker, pigmented, round area around the nipple. The areola has small bumps on its surface. These are sebaceous glands that release oily secretions that stop the areola and nipple from cracking.
Nipple

- 3. SITUATION AND EXTENSION - • Lies in superficial fascia of pectoral region. • Extend:- Vertically- from 2nd to 6th ribs Horizontally- Lateral border of sternum to maxillary line.
- 4. SITUATION AND EXTENSION - • Breast is devided into four quadrants – -Upper Lateral -Upper Medial -Lower Lateral -Lower Medial - A small extension Of the righ upper lateral quadrant called axillary tail of spence, passing (foramen of Langer) through an opening in deep fascia and lies in the axilla.
- 5. STRUCTURE OF THE BREAST - It can be decided in to 3 components- 1. SKIN WITH NIPPLE AREOLA 2. PARANCHYMA 3. STROMA
- 6. 1. NIPPLE- • A conical projection • Present just below the centre of the breast at the level of Four intercostal space 10 cm from the mid line. • The nipple is pierced by 15 -20 lactiferous ducts. • It has a few modified sweat and sebaceous glands.
- 7. AREOLA - • Pigmented skin surrounding nipple. • Rich in modified sebaceous glands.These become enlarged during pregnancy and lactation to from raised tubercles of Montgomery. • Oily secretion of these glands lubricate the nipple and areola, and prevent them from cracking during lactation.
- 8. AREOLA- • Apart from sebaceous glands,the areola also contains some sweat glands,and accessory mammary gland. • The skin of the areola and nipple Is devoid of hairs, and there is no fat subjacent to it. • Below the areola lie Lactiferous sinus, where stored milk is seen.
- 9. 2. PARANCHYMA- • It is compound tubulo-alveolar glands which secretes milk. • The gland consists with 15 to 20 lobes. • Each lobe is a cluster of alveoli, and it drained by a lactiferous duct. • The lactiferous ducts converge towards the nipple and open on it. • Near it termination each duct has a dilatation called a lactiferous sinus.

STROMA- • stroma forms the support framework of the breast/gland. • It is partly fibrous and partly fatty. • There are fibrous bands that provided structural Support and insert perpendicularly into the dermis terms the Suspensory ligament of cooper. That is the involvement of the Cooper’s ligament ➡️ Skin retraction.
- 11. BLOOD SUPPLY - • The mammary glands is extremely vascular. 1. Internal thoracic artery – A branch of subclavian artery, through its perforating branches. 2. The lateral thoracic, superior thoracic and acromiothoracic branches of the axillary artery. 3. Lateral branch of the posterior intercostal arteries.
