Accountable Care Workgroup: Draft Recommendations

This presentation was given to the UK's Department for International Development on 30th July 2019.
Comprehensive reviews of health system strengthening interventions are rare, partly because of lack of clarity on definitions of the term but also the potentially huge scale of the evidence. In our talk, we will reflect on the process of undertaking such an evidence review for DFID recently (attached again), drawing out suggestions on definitions of HSS and approaches to assessment, as well as summarising some key conclusions from the current evidence base. Most HSS interventions have theories of change relating to specific system blocks, but more work is needed on capturing their spill-over effects and their contribution to meeting over-arching health system process goals. We will make some initial suggestions about such goals, to reflect the features that characterise a ‘strong health system’. We will highlight current findings on ‘what works’ but also that these are just indicative, given the limitations and biases in what has been studied and how, and argue that there is need to re-think evaluation methods for HSS beyond finite interventions and narrow outcomes. Enabling Telehealth - Lessons for the Gulf Cooperation Council
The technologies of telehealth are advancing quickly as part of the ‘connected care’ revolution. Patients and health providers are ever more closely linked through real-time electronic tools. From digital imaging to allow remote viewing of CT scans, through to patient diagnosis, videoconferencing and monitoring, these tools could touch all aspects of the patient-provider relationship.
Much of the promise of telehealth is predicated on its ‘access’ benefits: the improved access of the patient to medical expertise regardless of location, and improved access of health providers to their patients, for the purpose of diagnosis, consultation and monitoring. Yet access to telehealth depends not only on telehealth technology. Policy frameworks must be modernised, communications infrastructures such as broadband and mobile network coverage must be improved, and skillsets – both of clinicians and patients – need to be strengthened. This briefing paper outlines three factors shaping the telehealth access environment: government readiness (e.g. legal and regulatory clarity and harmonisation, especially across states and borders), communications infrastructure, and skills.

Health Information Exchange (HIE)
Interoperability is one of the most critical issues facing the health care industry today. A universal exchange language is needed to assist health care providers in sharing health information in order to coordinate diagnosis and treatment, while maintaining privacy and security of personal data. Health Information Exchanges (HIE) allow for the movement of clinical data between disparate systems; they enable providers to electronically share health records through a network. This presentation provides an overview of HIE and the Meaningful Use requirement related to the exchange of clinical information as well as information about standards of exchange and the recommended "next steps" for providers.

Enabling Telehealth - Lessons for the Gulf Cooperation Council
The technologies of telehealth are advancing quickly as part of the ‘connected care’ revolution. Patients and health providers are ever more closely linked through real-time electronic tools. From digital imaging to allow remote viewing of CT scans, through to patient diagnosis, videoconferencing and monitoring, these tools could touch all aspects of the patient-provider relationship.
Much of the promise of telehealth is predicated on its ‘access’ benefits: the improved access of the patient to medical expertise regardless of location, and improved access of health providers to their patients, for the purpose of diagnosis, consultation and monitoring. Yet access to telehealth depends not only on telehealth technology. Policy frameworks must be modernised, communications infrastructures such as broadband and mobile network coverage must be improved, and skillsets – both of clinicians and patients – need to be strengthened. This briefing paper outlines three factors shaping the telehealth access environment: government readiness (e.g. legal and regulatory clarity and harmonisation, especially across states and borders), communications infrastructure, and skills.

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
