Experience:
Project Manager for Riverside Health System Program of All Inclusive Care for the Elderly (PACE), Executive
Administrative Team, promotion, February 2015 to present
• Work under the joint supervision of the PACE Vice President and PACE Executive Director to lead, support, facilitate,
and implement projects that support the strategic, operational and program needs of PACE throughout entire company.
Partner with PACE leadership to identify and achieve priorities, goals, and objectives. Assist with the development and
overall strategy for projects and appropriate work plans/timelines for completion. Ensure communication to appropriate
parties. Plan, manage, and maintain process improvement initiatives among the organization. Support streamlining of
processes within all centers and ensure compliance. Design and manage the implementation of needed aspects and or
features for all facilities. Ensure all environments are conducive to evolving development within the organization, and
facilitate positive and effective change among all sites. Develop Process Improvement initiatives that improve work flows
among various departments, and meet organizational goals.
• Participated in and managed major re-alignment of the company’s goals, budget, and operational process. Maintain
tracking of task dates to ensure completion and efficient follow through in order to meet set goals of the company.
Coordinate with Medicare and Medicaid liaisons concerning site activities and insure compliance.
• Served as interim home care management support for the PACE Home Care Department, with continual support as
needed. Responsibilities included supervision of home care staff, efficient scheduling, care planning, designing more
efficient processes, identification and implementation of practices for successful outcomes, realignment of home care
utilization hours, tracking metrics per month to report and meet goals, communication with families and participants,
establishing initiatives for team members to meet goals, regulatory compliance, identifying best assessment processes,
financial management of supply utilization. Continue to support and improve department operations. Also incorporated
in this role was maintaining employee files, and ensuring compliance with Human Resource documents.
• Researched, trialed, and implemented Home care scheduling software. Worked with one site to trial software and design
best practices around the software’s functions and what was needed operationally for the department. Outlined training
and directions for using the system and educated staff on the new tool. Continue to provide support around using the
system and improving efficiency. After successful trial with one center, continued implementation and training of
software to the other three sites.
• Active role in the implementation of the Epic Electronic Medical Record System for all sites of the PACE division within
the Health System. Work collaboratively with the IT Manager and Administrative team to design, create, and reorganize
operational flows and processes to ensure best practice of all departments to be developed into the software. Provide
efficient and clear communication between PACE Sites and builders of the Epic software. Development of training
materials and communication tools to ensure all parties are sufficiently updated through steps of the project. Continual
support to both the PACE sites and the software builders through implementation, roll out, and once all sites are live with
the new system.
Project Coordinator for Riverside Health System PACE Program, Executive Administrative Team, August 2014
to February 2015
• Worked under the supervision of the Executive Director of Riverside PACE. Supported, facilitated, and implemented
projects that support the strategic, operational and program needs of PACE throughout all 6 centers. Partnered with
PACE leadership to identify and achieve priorities, goals, and objectives. Assisted with the development of overall
strategy for projects and appropriate work plans/time lines for completion, and ensured communication to appropriate
parties. Major tasks included project planning and implementation within all facets of the company, home care
utilization and scheduling, participant demographic management and tracking, transportation efficiency, policy
implantation and control, state regulation project implementation, process mapping, task management, creation of new
tools to regulate processes and procedures, creation and training of new materials and communication tools, maintenance
of document control, and administrative support.
Internship with Riverside Health System PACE program, May and June 2014
• During the tenure of this internship, I shadowed the Executive Director of PACE programs and was exposed to all facets
of the program. My objectives were to experience the role and function of care management in PACE, and observe and
contrast the role of acute-care hospital care management with that of community-based care management. I participated
in Interdisciplinary team meetings, observed and researched effective day room use within the centers, traveled to all site
locations and observed and shadowed a variety of disciplines, and engaged in activities with participants on a daily basis.
I also participated in interviewing and hiring processes.
Adult Day Service of Virginia Tech, Fall 2012 to Summer 2014
• Planned and participated in daily activities with participants; Helped with serving and supervising meals. Assessed and
performed daily assessment of health and quality of life. Recorded daily data and transcribed into outcomes dataset
computer system. Interaction with facility nursing staff, onsite therapists, family members, and participants daily.
Development with Early Childhood center to plan interaction and activities between adolescents and elderly participants
of the program. Gained experience in pediatric development.
Caretaker for elderly South African couple in the community, Fall 2013 to Summer 2014
• Planned and communicated with couple and their family to coordinate outings, activities, and trips weekly. Assisted with
strengthening skills for independence with driving, language, money transactions, and social confidence.
Undergraduate Research with Virginia Tech Adult Day Services Delphi Panel Study, Fall 2013 to Summer 2014
• Researched effective ways of recording data and served on the development team for improving current methods as well
as sharing our methods with similar sites. Edited and drafted documents for research director, and improved life
satisfaction measures within Adult Day Service system. Experience in acute attention to detail as well as organization
skills, teamwork, independent work, communication skills, and design work.
• Worked directly on research teams and with service learning in Adult Day Service setting. Communicated with
gerontology experts and practitioners in determining what cognitive and quality of life measurements are most effective
and what types of measurements were needed.
• Participated in a Delphi Panel study and studied effectiveness of ADL (activities of daily living) measurements including
the Geriatrics Depression Scale (GDS), the Mini Mental Status Exam (MMSE), the Life Satisfaction Scale (LSS), and
the Measureable Assessment in recreation for resident- centered care (MARRCC), the Quality of Life in Alzheimer’s
disease (Qol-AD), and the Montreal Cognitive Assessment (MoCA). Also studied were participant and caregiver intake
and satisfaction surveys as well as demographics and descriptives data Flowsheets.
Undergraduate Research with Warm Hearth Village Continuing Care Facility, Spring 2014 to Summer 2014
• Worked directly with activity director of Warm Hearth Facility to research and engage with the participants in a variety
of different activities. This included researching and designing effective environments and settings for the participants to
promote wellbeing and comfortable environments.
• Created “Companion Visiting Kits” to aid in visitations with participants who are suffering from dementia illnesses and
to ease daily interaction with staff in the memory care unit. Experience in program development and advancement, with
knowledge of healthcare design and practice.

Assisted with the development of overall strategy for projects and appropriate work plans/time lines for completion, and ensured communication to appropriate parties. Internship with Riverside Health System PACE program, May and June 2014 • During the tenure of this internship, I shadowed the Executive Director of PACE programs and was exposed to all facets of the program. My objectives were to experience the role and function of care management in PACE, and observe and contrast the role of acute-care hospital care management with that of community-based care management. I participated in Interdisciplinary team meetings, observed and researched effective day room use within the centers, traveled to all site locations and observed and shadowed a variety of disciplines, and engaged in activities with participants on a daily basis. I also participated in interviewing and hiring processes. Adult Day Service of Virginia Tech, Fall 2012 to Summer 2014 • Planned and participated in daily activities with participants; Helped with serving and supervising meals. Assessed and performed daily assessment of health and quality of life. Recorded daily data and transcribed into outcomes dataset computer system. Interaction with facility nursing staff, onsite therapists, family members, and participants daily. Development with Early Childhood center to plan interaction and activities between adolescents and elderly participants of the program. Gained experience in pediatric development. Caretaker for elderly South African couple in the community, Fall 2013 to Summer 2014 • Planned and communicated with couple and their family to coordinate outings, activities, and trips weekly. Assisted with strengthening skills for independence with driving, language, money transactions, and social confidence. Undergraduate Research with Virginia Tech Adult Day Services Delphi Panel Study, Fall 2013 to Summer 2014 • Researched effective ways of recording data and served on the development team for improving current methods as well as sharing our methods with similar sites. Edited and drafted documents for research director, and improved life satisfaction measures within Adult Day Service system. Experience in acute attention to detail as well as organization skills, teamwork, independent work, communication skills, and design work. • Worked directly on research teams and with service learning in Adult Day Service setting. Communicated with gerontology experts and practitioners in determining what cognitive and quality of life measurements are most effective and what types of measurements were needed. • Participated in a Delphi Panel study and studied effectiveness of ADL (activities of daily living) measurements including the Geriatrics Depression Scale (GDS), the Mini Mental Status Exam (MMSE), the Life Satisfaction Scale (LSS), and the Measureable Assessment in recreation for resident- centered care (MARRCC), the Quality of Life in Alzheimer’s disease (Qol-AD), and the Montreal Cognitive Assessment (MoCA). Also studied were participant and caregiver intake and satisfaction surveys as well as demographics and descriptives data Flowsheets. Undergraduate Research with Warm Hearth Village Continuing Care Facility, Spring 2014 to Summer 2014 • Worked directly with activity director of Warm Hearth Facility to research and engage with the participants in a variety of different activities. This included researching and designing effective environments and settings for the participants to promote wellbeing and comfortable environments. • Created “Companion Visiting Kits” to aid in visitations with participants who are suffering from dementia illnesses and to ease daily interaction with staff in the memory care unit. Experience in program development and advancement, with knowledge of healthcare design and practice. • Applied for and rewarded a grant through Virginia Tech Undergraduate Research Institute, January 2014, to go towards the development of “Companion Visiting Kits.”
