Goal of the Care Connect a service linecreated to reduce Emergency Department visits thatarenon-
emergent careand connect these patients to a medical home for any long-term illness or minor illness.
The Care Connect started servingpatients in the Garland and Baylor Dallaslocationsin 2012. The team's
progress duringFY 12 and FY 13 included enrolling2,091 patients into the program and providingservices:
784 confirmed connections to a medical home and 379 referrals to community services. The increased
medical home access has resulted in emergency department reduction and improved cost savings atboth
hospitals.
