Understandably so, as it relates to automatic thoughts, much consideration has been given to its
effects resulting in various studies focusing on cognitive approaches that would improve symptoms.

It was during my first week clinical placement in the ward when I came across with the Waterlow
risk assessment tool. My mentor made sure that I got all the risk assessment tools commonly used in
the ward as these will play a key part in my duty as a qualified staff nurse. True enough, this risk
assessment tool became visible in every patient's charts and serial risk assessment was done on a
weekly basis.
When I started shadowing with a qualified staff nurse and began caring for patients, performing this
skin assessment tool became more prevalent. Every time a new patient was admitted either from
another local hospital or ward this tool was never missed out. Furthermore, weekly accomplishment
of this tool was mandatory as part of the prevention of pressure ulcers. On one occasion, I was asked
to do a risk assessment to an acute patient. I filled in every portions of the assessment tool to the
best of my ability and from the data I gathered from the patient. Apparently, after checking the
assessment I was advised to read further about Waterlow risk assessment so that I would have a
clear and better understanding of using it.

understanding of some of the vocabulary. This is seen by the matching section of the assessment.
The student was able to define about half of the vocabulary words, with only two days of review.

"I'm going to kill you!"... We've all said this or even heard it, but was it litteral? In 12 angry men,
written by Reginald Rose, the jurors were discussing if this one statement can lead to the
prosecution of a young man who allegedly killed his father. At the end of scene 2 juror no.3 lunges
at juror no.8 screaming "i am going to kill you", the same statement the kid had used. Now, did Juror
no.3 really mean it? A 19 year old kid who comes from the slums has been prosecuted for killing his
father. He lied about where he was during the time of the killing, stating he was at the movies, but
could not remember what film he was watching. In scene 3, juror no.3 constantly uses "they" to
define who and where this 19 year old comes from which let's us as readers know that this court
case happened when discrimination was at it's peak. There was only one juror that gave this kid his
benefit of the doubt, which was juror no. 8( his description reads," A man who sees all sides and
seeks the truth. He wants justice to be served and will fight to see that it is"). The facts were being
analyzed once more as juror no.8 had felt everyone wrote the case as a textbook murder, open and
close. The jury had viewed the knife that was used to kill the father being unique , that no one has

Introduction Upon analyzing, and synthesizing two scholarly articles to facilitate this writer with a
better understanding of authentic assessment, scenarios and examples were provided as evidence on
the impact and importance of authentic assessment in a natural environment (caregiver, home or
school). Both articles revealed the authors' purpose and perspective that authentic assessment
provides a representation of students' strengths and weakness along with opportunities for them to
associate and link this learning to life experiences. Similarities and Contrast Bergen (1993) and
Dennis, Rueter, & Simpson (2013) both shared similarities that authentic assessment is performance
based, and addresses how it relates to their belief that it provides numerous opportunities for
students to show mastery of content. This manifested when Bergen (1993) shared a story of her
earliest association with authentic assessment: I still remember my first authentic performance

The writer of this case study met with client Maria Sanchez on February 16th, 2015. This client was
referred to this writer after a recent diagnosis of stage 4 Alzheimer's disease by a clinician to assist
with planning for her future. Upon initially meeting with this client, this writer could determine her
demographic information. Maria Sanchez is married; however her husband is currently residing in
Mexico City. This client is 68 years old. Sanchez has been living in the United States for the last 20
years. She is a legal permanent resident (LPR). Client has spent 17 of these 20 years in the work
force, and paid into social security. Currently this client's only income comes from social security
and from her daughter for child and household care (100 dollars per month). Client is currently
receiving a social security check of 356.12 each month. Client has saved up 4500 dollars in the last
couple of years. This client currently has no health insurance. Sanchez has been given a probable
diagnosis of stage 4 Alzheimer's disease. Through the initial assessment signs of depression can be
seen were seen by this writer. Client Sanchez has a very close bond with her family, and uses them
as her support system. She will be going through the assessment process and interventions with the

Abstract The information collected for this research focuses on the re–entry of children into the
child protection system for assessment in relation to being within sight or sound of intimate partner
violence also referred to as domestic violence. The sample of this research is composed of a
collection of 298 assessment reports, consisting of 429 individuals from Olmsted County Child and
Family Services. The data collected was specific to assessments completed by the Domestic
Violence Response Team, of Olmsted County, MN, a program cooperatively ran through Family
Service Rochester and Olmsted County Child and Family Services. The hypothesis of this research
was supported; the early intervention and work of the Domestic Violence Response Team minimizes
the re–entry of children into the child protection system in regards to being within sight or sound of

When they see other children of similar age start teething or walking earlier than their own, the
concerns about child's health grows tremendously. Assurance from paediatrician or GP helps them
understand that their child is growing at his/her pace. As physical developments are easily
noticeable, parents/carer can raise concerns at proper stage. Identifying speech related issues is
critical at young age. It can be mistaken as babbling or as baby talk which is common.
In Sam's case there was possibility that his new parents taking his shy behaviour or lack of
confidence as his way to adjust with new setting and new family. If SLCN was not identified
immediately it would have affected Sam's confidence and his possibility of starting regular nursery
school.
From this case study we understand it is important to act quickly to support children who have
speech and language or communication needs, as language is crucial to learning and enables one to
express thoughts in an organised way. Children with language delay may also find it harder to
communicate and form relationships with others. They may become frustrated, leading to possible
