This document provides information on wound management and wound care. It discusses the different types of wounds including acute and chronic wounds. The goals of wound care are to identify any wounds or complications, prescribe preventative measures to promote skin integrity, and treat any wounds. There are four phases of wound healing: hemostasis, inflammation, proliferation and remodeling. The three types of wound healing and closure are primary, secondary and tertiary. Various wound dressings are also described like hydrocolloid, hydrogel and alginate. The wound care market is large and growing, dominated by major players. Pricing for common wound dressings is also listed.
2. Wounds:
A wound is an injury to living tissue caused by a cut, blow, or
other impact,
typically one in which the skin is cut or broken.
3. Types of
Wounds:Acute
Follows the normal stages of wound healing to achieve
complete repair by definition within 12 weeks. Such as:
• Incisions and excisions
• Abrasions
• Puncture wounds
• Penetration wounds
Chronic
The process of repair continues on for greater than 12 weeks
4. Wound care goals:
Identify patient’s wound and wound complications; prescribe
preventive measures to promote skin integrity; and to treat wound
problems once they develop.
Wound care is the key for achieving the best result, patient
satisfaction and healing downtime.
5. Wound healing physiology:
Wound healing is a physiologic process in which the skin or organ or tissue
repairs itself after injury.
- Skin healing goes through 4 phases in order as follow:
8. Types of Wound Healing & closure :
There are 3 different types of wound healing:
Primary healing :
occurs when the tissue is cleanly incised and realigned anatomically
minimizing the volume of repair and the wound healing process
usually proceeds without complication.
9. Types of Wound Healing & closure :
There are 3 different types of wound healing:
Secondary healing:
occurs when a wound is not closed with sutures and
is instead left to heal by granulation, contraction and epithelialization.
Wounds at risks of infection as well as burns heal in this manner.
10. Types of Wound Healing & closure:
There are 3 different types of wound healing closure:
Tertiary healing :
Uses both principles of primary and secondary healing and is the method most
often used for dirty and/or infected wounds.
By delaying the closure of the wound and keeping the wound environment
moist the rate of bacterial replication is reduced.
The wound is packed to prevent natural closure and is watched closely, usually
for at least 3 to 5 days.
11. Wound Covers:
Hydrocolloid:
They are used on burns, light to moderately draining wounds, necrotic wounds, under compression
wraps, pressure ulcers and venous ulcers.
Hydrogel:
They are for wounds with little to no excess fluid, painful wounds, necrotic wounds, pressure ulcers, donor
sites, second degree or higher burns and infected wounds.
Alginate:
Alginate dressings are used for moderate to high amounts of wound drainage, venous ulcers, packing wounds
and pressure ulcers in stage III or IV.
Collagen:
A collagen dressing can be used for chronic or stalled wounds, ulcers, bed sores, transplant sites, surgical
wounds, second degree or higher burns and wounds with large surface areas.
In addition to the wound product categories listed above, there are other wound dressings available, such as
foams and compression in addition to secondary and cover dressings like wraps, gauze and tape.
12. Wound Assessment:
1- General Health Assessment:
- Age – medical history - obesity or poor nutrition - Impaired blood supply - Lifestyle (smoking).
2- Local Wound Assessment:
- location, size, base/depth - presence of pain - condition of the wound bed.
3- Healing Local Barriers:
- wound contains dead tissue or debris - Infection or inflammation - Wound is too moist or too dry –
wound failing to close, edges worn away or damaged.
13. Wound Assessment:
Appearance of Wound:
Wound with clean granular base
Wound with crater
Wound with necrotic/non-viable tissue
Wound with exudate
Wound with sinus, tunnel, undermining
Wound with infection
14. Wound appearance :
Treatment Dressing
Hydrocolloid Hydrogel Alginate Foam (VAC) device
Wound with clean granular base
Wound with crater
Wound with necrotic/non-viable tissue
Wound with exudate
Wound with sinus, tunnel, undermining
Wound with infection
Slow healing wounds
