Diseases such as obesity, diabetes, and high blood pressure have had devastating effects on the health and well-being of the public and have led to the creation of hundreds of new dialysis treatment facilities. In order to provide comprehensive and accessible care, administrators have started recruiting thousands of new technicians to perform routine patient care activities. This has resulted in many new employment opportunities for unlicensed allied health personnel who are looking for a career path that has relatively few barriers to entry. Those who choose to pursue this profession can complete their education and training through either an in-house program offered by the employer or through an independent educational institution such as a community college or vocational school that administers a certificate option specific to the specialty. After training has been completed, the technician will typically be required to pass a national certification examination.

What are the economics associated with a program intended to influence large scale organizational change in a healthcare setting? This work reports on the exploration of the economic linkages among the resources used and the benefits achieved from a training intervention. The training program is intended to develop quality improvement capability among training participants in a medical center. This economic evaluation involves the application of utility analysis to value the costs of the program and to estimate the benefit as the value of trained individual. Utility analysis was further enhanced by integrating the analysis within a dynamic system’s model. This extension provided a more precise understanding of the economics over time as training participants flow through a training intervention and then back into the workplace. Finally we explore the potential to quantify the linkage between interventions with learners and the impact of large scale change as a means for considering the value of the intervention.

I. Health Problem.
I have chosen to complete my final project discussing Human Immunodeficiency Virus (HIV). There are quite a few people that I know that are near and dear to my heart living with this virus. There are many misconceptions about HIV that the public has simply due to unawareness. With most common public health issues there are many questions that need to be answered to solve the myths. Mental health and substance use disorders can also make it difficult for people to take their medications as prescribed. When HIV is not treated or maintained, it can lead to the fatal diseases Acquired Immune Deficiency Syndrome (AIDS). Of note, you cannot have AIDS without being infected with HIV. According to the Center for Disease Control, HIV can affect anyone regardless of sexual orientation, race, ethnicity, gender, or age (CDC, 2022). However, certain groups are at a higher risk for HIV and weigh special considerations due to the specific risk factors. Gay and Bisexual men, pregnant women, infants, children, Transgender people, people who exchange sex for money, people who inject drugs, etc. These are all considerations of the causes for higher risk for being affected by HIV. However, with the modern medicines and treatments for HIV, many have been known to live long and healthy lives.
II. Organization.
The organization that I have chosen is the World Health Organization (WHO). WHO was founded in 1948 and is the United Nations Agency that connects nations, partners, and people to promote health, keep the world safe and serve the vulnerable – so everyone, everywhere can attain the highest level of health (WHO, 2022). My role in this organization will be a Public Health Physician. WHO works closely with United Nations Children’s Fund (UNICEF) on Elimination of Mother-to Child Transmission (EMTCT) of HIV and pediatric AIDS and works with United Nations Population Fund (UNFPA) on the integration of HIV and Sexual and Reproductive Health and Rights (SRHR). WHO considers the epidemiological, technological, and contextual trends of previous years, promotes learning with each disease area, and generates opportunities to leverage innovations and knowledge for efficient responses to HIV and sexually transmitted infections (STI). Thanks to awareness and past research, WHO is able to provide a in depth analysis of the past and current trends of HIV and other STI through multiple platforms.

Global Ophthalmology Training Opportunities in U.S. Residency Programs – A Survey of Program Directors
54% have int’l (global) program, 90% have some global health curriculum
In Jan 2005, out of 43, 20 had residents go abroad over prior 2 years, but only 6 reported supporting int’l programs; & only 9 residencies had global health curriculum
The most common length of time permitted for resident travel abroad was 2 weeks (11 programs), with a range of 7 to 35 days. Eight programs allowed resident travel for 3 weeks or more. Of programs offering GOTOs, most (19/28, 68%) allowed residents to use at least some academic time for global travel. Some programs (10/28, 36%) allowed residents to use 100% academic time, while others (13/28, 46%) required residents to use 100% vacation time. Most departments (18/28, 64%) provided full or partial funding for resident GOTOs, while a minority provided no departmental funding. The vast majority of GOTOs (24/28, 86%) included resident surgical experience. Most programs (16/24, 67%) reported that resident surgery performed abroad was staffed by attendings from their home institution. Fourteen programs also permitted residents to be staffed by attendings from trusted partner institutions.
The ideal time to recruit ophthalmologists to this cause is during residency. While still in training, residents begin making the decisions that will shape their future careers. They have both the time and the creativity to consider how international ophthalmology could fit into their life goals as they are not yet burdened with the financial obligations and stress of the first years of practice. Residents are a captive audience and view the prospect of international work as exciting, invigorating and rewarding.

The Wills International Resident Experience (WIRE) is a chance for residents to participate in ophthalmology in diverse global settings. We examined whether WIRE augmented residents’ surgical experience, especially in small-incision extracapsular cataract surgery (SICS), and fulfilled American College of Graduate Medical Education (ACGME) core competencies.
Methods
Residents participating in WIRE completed post-program surveys to determine their surgical experience in SICS and subspecialty surgery. Survey questions also addressed the relevance of this experience specifically to each of the six ACGME core competencies: patient care; medical knowledge; practice-based learning and improvement; interpersonal and communication skills; professionalism; and systems-based practice. Descriptive statistics were calculated and a binomial test was used to determine the significance of survey response data.
Results
Twelve residents completed surveys (100% response rate). Residents performed an average of 22.1 surgeries as primary surgeon, including 10.6 SICS. Key benefits of WIRE that residents cited include: surgical and clinical variety, including experience treating diseases rarely encountered in the United States; experience performing SICS; exposure to distinct healthcare systems; and providing high-quality care with limited resources. All six ACGME core competencies were covered by WIRE (binomial test, p<0.01).
Conclusion
Global ophthalmology rotations like WIRE can provide an opportunity for residents to gain exposure to medical and surgical conditions and interventions rarely encountered in the United States. Additionally, as part of a residency curriculum, experiences like WIRE can fulfill ACGME core competencies while fostering an interest in global eye care.
