BLC also used participatory approaches and knowledge sharing to strengthen management systems and make the organizations more sustainable. By 2015, several organizations had "graduated" to directly receive and manage US government funding.

A 2008 strategic plan recommended increased access to mental health services for those with autism or other neurodevelopmental disabilities. A subcommittee identified the START model for crisis intervention for those with dual diagnoses. A $50,000 contract was authorized to conduct a gap analysis of a potential Colorado Cross System Crisis Intervention Program. The analysis will include regional meetings, an online survey, analyzing relevant policies, a cost analysis of 40 individuals, and a final report.

The case study describes how CWP has invested in a number of plans to tackle unwarranted variations in health care by helping staff to deliver continuous improvement. The campaign has already started to make a positive difference – CWP achieved the highest score in the country for ‘overall experience of services’ in the CQC survey of users of its mental health community services.

“Decisions of value – how the NHS can balance quality and finance in decision-making”
NHS decision-makers have to balance the priorities of quality improvement and financial sustainability, in other words they have to deliver value. This balancing act is increasingly challenging as the demands on the NHS change and grow, with more expected within an ever tighter budget. Decisions of Value is a project commissioned by the Department of Health and led jointly by the Academy of Medical Royal Colleges and the NHS Confederation. It has spent six months studying what influences how decisions are made and brings together a large amount of research to show how factors such as relationships, behaviours and environment influence the value delivered, extending beyond Whitehall to the front line.
The project’s findings have recently been published and emphasise the importance of the cultural, rather than structural, changes needed to move towards delivering better value and look at how they rely on having the right relationships, behaviours and environments in place. It presents insights into how people interact in the NHS and the crucial factors affecting how they operate within a particular context. In many cases, it indicates a ‘back to basics’ approach that involves a fundamental understanding of how humans interact and operate. As such, it doesn’t look to define good decisions, but rather gives an insight into the principles of good decision-making.

This webinar was based on the June 23, 2017 event, "Critical Pathways to Improved Care for Serious Illness: Concluding Convening." At this event, leading health care executives, experts, policymakers, and other thought leaders gathered to conclude a project to develop a guiding framework for providing improved care for people with serious illness. Participants observed the final working session where distinguished panelists discussed innovations in program design and pathways for delivering high quality care to an aging population with chronic illnesses, especially those with declining function and complex care needs. The panelists engaged audience members in Q&A sessions during each panel, as well as at breakout sessions over lunch.
This project was funded by the Gordon & Betty Moore Foundation, and this convening was part of the Project on Advanced Care and Health Policy, a collaboration between the Coalition to Transform Advanced Care (C-TAC) and the Petrie-Flom Center for Health Law Policy, Biotechnology, and Bioethics at Harvard Law School.

Food ontologies will be used to build schemas for integrating nutritional data from apps and databases into personal health records. This could help users better track food intake and calories to manage their health.

At this event, leading health care executives, experts, policymakers, and other thought leaders gathered to conclude a project to develop a guiding framework for providing improved care for people with serious illness. Participants observed the final working session where distinguished panelists discussed innovations in program design and pathways for delivering high quality care to an aging population with chronic illnesses, especially those with declining function and complex care needs. The panelists engaged audience members in Q&A sessions during each panel, as well as at breakout sessions over lunch.
This project was funded by the Gordon & Betty Moore Foundation, and this convening was part of the Project on Advanced Care and Health Policy, a collaboration between the Coalition to Transform Advanced Care (C-TAC) and the Petrie-Flom Center for Health Law Policy, Biotechnology, and Bioethics at Harvard Law School.

TADAA - Towards Automated Detection of Anaesthetic Activity

Abstract
Chronic kidney disease is considered one of the most significant health issues affecting
morbidity and mortality and contributes heavily to the state of global health. Chronic kidney
disease (CKD) and end-stage renal disease (ESRD) are chronic illnesses that have a dramatic
impact on the cost of health care delivery in the United States. Early detection and intervention
are critical to the long-term prognosis of this patient population; however, a health disparity
exists because not everyone who is at risk for CKD has access to resources for screening and
treatment. One of the goals of community-level and national programs is to create parity of care
by focusing attention on marginalized communities that are at a statistically higher risk for CKD.
The global impact of CKD and ESRD is significant because long-term survival depends on
expensive technology and many regions of the world lack the resources needed to treat this
disease. Health behavior and culture are known contributors to the long-term survivability of the
disease. Since early detection is the key, creating screening programs that target populations at
greatest risk will have the highest impact, and be the most cost-effective solution to combating
this chronic illness.

Chronic kidney disease (CKD) is considered one of the primary global health issues and
contributes significantly to the social burden of care. CKD, along with cardiovascular disease,
diabetes, chronic respiratory disease, and cancer, is a chronic illness that is classified as a non-
communicable disease (Healthy People 2020, 2013). Non-communicable diseases have a
significant societal impact to domestic growth, productivity, and health care costs and are the
most common cause of morbidity and premature death in the United States (Couser, Remuzzi,
Mendis, & Tonelli, 2011). Chronic illnesses are also characterized by physical and emotional
stressors that can become overwhelming when simultaneously coping with multiple
comorbidities (Moulton, 2008). Although the World Health Assembly has determined that non-
communicable diseases contribute heavily to the state of global health, they concede that public
health policy can dramatically affect patient morbidity and mortality (Couser et al., 2011). CKD
is a public health threat that is on the rise and will likely not slow without deliberate intervention.

Introduction
My study is focused on chronic kidney disease (CKD). The research I conducted elaborated that chronic kidney disease has been established as a cause of renal failure. Kidneys have the responsibility of excess fluids plus blood waste filtration, the building up of wastes results to kidney failure. Two major causes of CKD exist; these are categorized into high blood pressure and the element of diabetes. Acute kidney failure may lead to fatalities why may need intensive treatment immediately; however, this condition can be reversed, by the right health conditions, patients can regain normalcy of kidney functions. However, other patients may need a kidney transplant. Various resources have been used in the mitigation of CKD at both the community and national levels (Thomas, 2019).
According to estimates, more than 240 people who are on dialysis die daily. The ratings establish that 15%, which is approximately 37 million civilians, are living with chronic kidney disease. These estimates determine that 9 out of 10 people who live with chronic kidney disease aren't aware of their health condition. On the other hand, 1 out of two people that have very low kidney functionality but not on dialysis aren't mindful of their CKD condition (Daugirdas, 2012).
Healthy people 2020 on CKD
The main objective of healthy people 2020 on CKD is to help mitigate and enhance a reduction in new cases of infection of CKD and complications that are associated with it, for instance, economic implications and death. In the United States, CKD and ESRD have been established as a significant problem in the public health sector, which is the primary source of suffering and cases of poor life quality for the affected persons. Both are accountable for premature deaths and exacting high economic afflictions in both the public and private health sectors (Bomback & Bakris, 2010).
Achievement of Healthy people 2020 objectives on CKD may lead to a burden decrease in CKD, and increased life expectancy, and better life quality for civilians living with CKD and the disparities elimination, which exist, between patients who have chronic kidney disease. Generally, ERSD and CKD are highly cost intensive; approximately 25% of the established Medicare budget is applied in the treatment of people living with CKD and ESRD (Thomas, 2019).

Approximately over 50 years of telehealth development has seen tackling of numerous medical conditions through the utilization of different types of technologies. Tele healthcare has been associated with a wide variety of outcomes in the healthcare. Despite the fact that it as shown great promise in the modern times, it has also triggered a number of challenges for interpretation and technical issues that are linked to the lack of technological expertise among the healthcare practitioners. For instance, then challenges that exist in defining such terms as telehealth is a reflection of the broader difficulties when it comes to the interpretation of the complex interplay that exists between clinical output, patient involvement, service designs and technology. Despite these significant challenges whose impacts in the healthcare delivery is great, the significance of telehealth practices cannot be overlooked in terms of the patient outcomes and efficiency of healthcare delivery. Tele healthcare has enhanced information sharing between patients and doctors even those who are located in the remote areas. It has made it possible to monitor the progress of patients within and outside the facilities devoid of the need to have physical meetings. The benefits of tele healthcare outweigh its pitfalls hence it is important that is integrated into medical practices to optimize patient outcomes.
Tele healthcare is depicted as personalized healthcare that is delivered over a long distance such that data is transferred from the patients to the professionals and back. It has played a key role in enhancing feedback from the patients to the professionals for the interest of evidence-based decision making and treatment. For instance, the significance of telehealth can be evident among the inpatients with severe long-term conditions such as diabetes and long-term asthma. It has played a key role in reducing hospital admissions without increasing mortality.

There are numerous advantages of tele healthcare. To start with is that it is powered by technology. Technological advancement in the modern times has made it easy for various organizational operations. Technological advancement has been adopted across all spheres of life and healthcare is not an exemption. Breast cancer: Post menopausal endocrine therapy
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.
PGx Analysis in VarSeq: A User’s Perspective
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data.
