This document discusses better care for chronic kidney disease patients. It notes that 10% of the population has CKD, but DHBs only know about 1/3 of patients with moderate to severe disease. The challenges are identifying CKD patients, risk stratifying them, managing resources, and improving integration between primary and secondary care. Progress so far includes collaborating with the Ministry of Health to develop consensus guidelines and an IT system, as well as establishing a working group. Future goals are endorsing the consensus statement, further developing IT tools, and improving management of CKD patients across primary and secondary care.
