The National Commission on Macroeconomics and Health (NCMH) was established in March 2004 to strengthen disease control and primary healthcare in India. Its overall objective was to assess how increased investments in the health sector impact poverty and economic development.
In this report, the Commission discusses the economic basis for investing in health and how public financing can be most effectively utilised. It discusses the critical issues plaguing the health sector, such as inequitable access to basic services, inefficiencies in the system, and an absence of patients’ rights.
The report states that liberalisation of the economy increased employment opportunities and incomes, thus reducing poverty levels. These developments also introduced changes in lifestyles, increased urbanisation and connectivity, and enhanced access to information. Together, this has had a profound impact on the epidemiologic and health-seeking behaviour of people.
The rising demand for health services has revealed the inadequacies of the current healthcare system, both in the public and private domains. It is the responsibility of the government to provide an efficient healthcare system, along with health education, preventive programmes, curative services, and affordable health services for the poor. This report reviews the public and private healthcare systems, and provides policy makers with a framework to improve the funding of public health.
Delivering micro health insurance through national rural health missionCIRM
The Indian health scenario is fairly complex and challenging with successful reductions in fertility and mortality offset by a significant and growing communicable as well noncommunicable disease burden, persistently high levels of child under-nutrition, increasing polarisation in the health status of the rich and the poor and inadequate primary health care coexisting with burgeoning medical tourism. This situation is further complicated by the presence and practice of multiple systems of medicine and medical practitioners (several of whom are not formally certified and recognised) and very limited regulation.
In this context, this paper highlights the challenges in financing health in India and examines the role of health insurance in addressing these. It proposes a workable model for developing sustainable health insurance models under the National Rural Health Mission, responding to the contextual needs of different states.
A regulatory review assessing JKN implementation versus designHFG Project
The purpose of the regulatory review undertaken in late 2015 was to understand the effectiveness of existing regulations in implementing JKN, potential shortcomings and opportunities for revisions or clarifications, as well as how implementation deviated or aligned to the original design in the regulations. The regulatory review findings would in turn feed into IR for UHC national and subnational stakeholder engagement process and support the identification of the research questions for cycle 1 of implementation research.
Provision of care at the primary level is the backbone of JKN system and requires effective
regulation. Primary health care is provided in puskesmas (public health centers) and private
practices. The regulatory review focuses on five major features of JKN implementation in
primary health care (PHC) including: provider payment and incentives, service package,
utilization of capitation payment, referral within a multi-tiered system, and enrollment of the
poor and vulnerable. Such features were identified by stakeholders early in the IR for UHC process as important themes to untangle the major problems in the implementation of JKN.
Hence, the study team completed the regulatory review to understand the existing regulations
surrounding the five features and their potential shortcomings and opportunities. Each of the
five topics is addressed below. We first describe the background for each topic, what the
regulations say and then what happened during implementation.

This study clarifies the current pharmaceutical drug promotion and prescribing practices in Pakistan. The majority of prescribers and national pharmaceutical firms and to some extent the multinational pharmaceuticals are involved in unethical practices in drug promotion and prescribing. Alarming policies governing the drug promotion and prescribing are required to be implemented by the concerned regulatory authorities to avoid unnecessary harm to the patient’s life and pocket through the unethical drug promotion. The prescribers should not accept any incentives, gifts of financial value from any pharmaceutical companies in return for an increase in prescribing selected brand. On the other hand, pharmaceutical companies must compete in the market on the basis of the drug quality and do not offer any valuable gift and incentives to the prescribers. The interaction between doctors and phar- maceutical firms should be restricted within acceptable boundaries and the authorities must be prepared to play an active role. Strengthening the regulatory machinery and formulating policies in this regard in neces- sary. It is essential that a health care professional such as a pharmacist can play an important role in this process since he/she is an expert in the pharmaceutical field as well as more aware of the outcomes of unethical drug prescribing practices such as polypharmacy and adverse drug reactions.

and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,

AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).

T
PREFACE
he High Level Expert Group (HLEG) on Universal Health Coverage (UHC) was
constituted by the Planning Commission of India in October 2010, with the mandate
of developing a framework for providing easily accessible and affordable health care
to all Indians. While financial protection was the principal objective of this initiative, it was
recognised that the delivery of UHC also requires the availability of adequate healthcare
infrastructure, skilled health workforce and access to affordable drugs and technologies
to ensure the entitled level and quality of care given to every citizen. Further, the design
and delivery of health programmes and services call for efficient management systems as
well as active engagement of empowered communities. The original terms of reference
directed the HLEG to address all of these needs of UHC. Since the social determinants of
health have a profound influence not only on the health of populations but also on the
ability of individuals to access healthcare, the HLEG decided to include a clear reference
to them, though such determinants are conventionally regarded as falling in the domain
of non-health sectors.
The HLEG undertook a situational analysis of each of the key elements of the existing
health system and has developed recommendations for reconfiguring and strengthening
the health system to align it with the objectives of UHC, bridging the presently identified
gaps and meeting the projected healthneeds of the people of India over the next decade. In
this exercise, it was greatly enabled by the expert advice provided by a number of Indian
and international organizations and individuals who shared the varied perspectives of
policymakers, health professionals, health system analysts and managers, civil society,
private sector, development partners and academia. It drew upon the work and wisdom
of several past expert committees and study groups which had provided valuable
recommendations on strengthening different elements of the health system in India.
The HLEG was provided valuable assistance by the energetic group of researchers who
constituted its technical secretariat at the Public Health Foundation of India (PHFI). It also
benefited immensely from the intermittent consultations with members of the Planning
Commission while its work was in progress.
The HLEG is submitting its report at a time of historically unprecedented opportunity for
advancing people’s health through the introduction and effective implementation of UHC.
The Prime Minister has declared, in his Independence Day Address on August 15, 2011,
that health would be accorded the highest priority in the 12th Five Year Plan which would
become operational in 2012. There is a clearly articulated governmental intent to increase

the public financing of health to 2.5% of India’s GDP, during the course of the 12th Plan. The
growth of India’s economy permits this long overdue increase in public financing of health.
The recognition of investment in health as both a developmental imperative and a pathway
for winning popular political support has been evident in many recent initiatives ranging from
the National Rural Health Mission (NRHM), the Rashtriya Swasthya Bima Yojana (RSBY) and
a multitude of state sponsored health insurance schemes. The social objectives of all of these
schemes would need to be merged and their scope considerably expanded to create a valued
and viable model of UHC in India.
The adoption of programmes for promoting UHC, by many other countries, provides a stimulus
not only to act in conformity with a globally progressive commitment to health equity but also
to become a leader of the movement by creating the best contemporary model of UHC. The
HLEG has studied the experience of other countries, especially of those in the low and middle
income categories, while developing its recommendations for India.
The HLEG’s vision of UHC transcends the narrow, inadequate and often inequitable view of UHC
as merely a system of health insurance. UHC, in its understanding, moves beyond ‘insurance’
by providing an ‘assurance’ of health care for multiple needs and includes health beyond
health care, going beyond a mere illness response. UHC should address health in all of its
dimensions and emphasize prevention and primary health care, which are ignored, neglected
or even undermined by the usual systems of health insurance. Such an assurance has to be
provided by the government, which has to act as the guarantor of UHC and ensure its success
and sustainability, by mobilizing all societal resources and advance multi-sectoral actions. In
this perspective, the UHC is linked firmly to the Right to Health and converts an aspirational
goal into an entitled provision.
The HLEG also recognizes that, for such a vision of the UHC to be realized, a tax based system of
health financing is essential. This is also the global experience, wherein countries which have
introduced UHC have mostly depended on general revenues rather than on unsteady streams
of contributory health insurance which offerincomplete coverage and restricted services. For
UHC to succeed in India, political and financial commitments are required from the central as
well as state governments. We hope this report will catalyze those commitments and channelize
their concerted actions for the early adoption and effective implementation of UHC.
The HLEG’s report provides a framework for designing the UHC system. Even as that framework
is discussed and debated in the public domain, delivery of UHC requires many implementation
pathways to be identified and several operational processes to be detailed. Much work lies
ahead but we hope this report provides a useful beginning.
