1) The study characterized and compared cases of type 2 diabetes (T2DM) and type 1 diabetes (T1DM) in males aged 18-40. 2) Patients with T1DM were significantly younger (30.2 years) than those with T2DM (36.3 years). 3) Metabolic syndrome was present in 67.8% of T2DM patients compared to 10.5% of T1DM patients. Cardiovascular risk factors were common in both diabetes types in this age group.

Clinical Profile of Acute Coronary Syndrome among Young Adults
Acute Coronary Syndrome accounts for 30% of hospital admissions with cardiovascular diseases. The risk of this syndrome is increasing among the younger adults, and a deep insight into the clinical profile among these patients will help in devising a preventive strategy, in order to alleviate the morbidity and mortality due to the syndrome. A cross sectional study was done among 125 subjects admitted to our tertiary care hospital with Acute Coronary Syndrome. Their risk factors were assessed and a 12 Lead electrocardiogram and 2D Echocardiogram were taken. Cardio III panel which consists of Troponin I, CK MB, BNP by COBAS meter machine was also measured. STEMI was present in 73.6% of the patients, while unstable angina was present in 16%. About 90% of STEMI patients were males and 62% of them were hypertensives. LV Ejection Fraction <30% was found in 9% of STEMI patients. This study elucidates the need for a preventive strategy for primordial prevention of cardiovascular events among young adults. The study envisaged the male, urban preponderance towards these events.

This study analyzed data from hyperuricemia screening programs in India to determine the prevalence of high uric acid levels. The results showed that 25.8% of over 29,000 subjects had hyperuricemia. Males and older individuals over 50 years of age had higher rates of elevated uric acid. Over 30% of subjects with type 2 diabetes, hypertension, or both conditions also exhibited hyperuricemia. The proportion with high uric acid increased with longer duration of diabetes and hypertension. The study concludes screening for uric acid levels may help identify risks of comorbidities and complications.

Several significant risk factors were identified for the patients compared to controls. These included lower income, more sedentary work, higher rates of smoking tobacco and consuming non-smoke tobacco, lower fruit consumption, higher rates of diabetes, known dyslipidemia, higher waist-hip ratio, and history of heart attack. Multivariate analysis identified the most significant independent risk factors as previous smoking, high waist-hip ratio, history of hypertension, and lower income.Recommended

- The study investigated the life expectancy and costs of treating acute myeloid leukemia (AML) in Norway over a 5-year period using a probabilistic decision model.
- The results showed the overall mean cost was $199,828 and mean life expectancy was 31.87 months, ranging significantly depending on patient age, treatment response, and regimen. Younger patients who responded to treatment had higher costs ($180,702) and longer life expectancy (33.53 months) than elderly patients.

Abstract— Non Alcoholic Fatty Liver Disease is also becoming public health impotance nowadays. So this study was aimed to determine the association of Non Alcoholic Fatty Liver Disease with metabolic syndrome and Cardio-Vascular disease along with assessment of degree of severity of NAFLD with respect to number of components of metabolic syndrome. This study includes a total of 222 subjects were enrolled as per the inclusion/exclusion criteria, out of which 110 cases who had NAFLD with hepatic steatosis on ultrasonography and 112 subjects who did not have NAFLD were considered control. These cases and controls were interrogated and investigated further. Observations were recorded and association of Non Alcoholic Fatty Liver Disease with metabolic syndrome and Cardio-Vascular disease along with assessment of degree of severity of NAFLD with respect to number of components of metabolic syndrome. Statistical methods used were unpaired student’s t-test for continuous variables, Fischer’s and chi-sq test for categorical variables using bivariate analysis by Graph Pad Instat Version 3.10. Risk was assessed in terms of Odd's Ratio. The patients with MS and NAFLD had a higher proportion of CVD compared with those who did not have NAFLD (29.1 vs 18.1 %). This study concludes that NAFLD is significantly associated with MS; most significant with WC, followed by TG and FBS and thus can be considered as hepatic component of MS. This needs more research with large multi-centric prospective studies to evaluate NAFLD as an independent risk factor for CVD. Post transplant erythrocytosis
This study examined risk factors for post-transplant erythrocytosis (PTE) in 235 kidney transplant recipients. The results showed that being male significantly increased the risk of developing PTE. Patients with polycystic kidney disease or glomerulonephritis also had a higher risk. While smoking was associated with PTE, further analysis revealed this was likely due to the relationship between smoking and male sex rather than a direct effect. The study did not find significant relationships between PTE and other factors like hypertension, diabetes, transplant organ source, or immunosuppressant regimen.

The model considers the progression of patients' cholesterol levels using Markov chains calibrated by clinical data. Results show the guidelines imply penalty factors for treatment disutility ranging from 0.04-0.29% per life-year, with some guidelines favoring nearly 30 years of treatment. While guidelines aim to reduce cardiovascular risk, treatment durations could be shortened by up to 4 years without increasing overall risk. The approach quantifies adverse effects of treatment to help evaluate guidelines and facilitate better clinical decision making.
Tokyo guidelines for cholangitis and cholecystitis

Study of Congenital Heart Diseases in Pediatric Patient
Introduction: Congenital heart disease is important cause of morbidity and mortality in pediatric practice. Objectives: - 1) To establish the diagnosis and find out incidence of CHD in various age group. 2) To study various mode of presentation of various CHDs. 3) To study morbidity and mortality in various CHDs in children. Methodology:- Total 60 cases of congenital heart diseases coming to K.T. Children hospital, Rajkot from July 2016 to November 2016 were studied. Informed consent and detailed history was taken followed by thorough clinical examination as per Performa attached. Echocardiography of all patients was done. School health cards were prepared for the patients having abnormal echocardiography and referred to higher centre for cardiologist opinion. Conclusion:- Correct diagnosis can be made with help of thorough history and clinical examination especially in acyanotic congenital heart diseases. Early diagnosis and timely referral of patient can greatly improve outcome of patients with CHDs.

This study examined dyslipidemia among type 2 diabetes patients in Somalia. The study found:
- 22.5% of patients had dyslipidemia, slightly lower than other African studies.
- Risk factors for dyslipidemia included female sex, insufficient physical activity, and high BMI.
- Many patients had multiple cardiovascular risk factors like hypertension and a family history of heart disease, despite a short time since diabetes diagnosis.
- Management challenges for diabetes in Somalia include lack of funding, unreliable access to medications, and inequity in healthcare access between public and private sectors. Effective prevention and treatment strategies are needed that address Somalia's unique healthcare context.

This study analyzed data from over 2,400 pediatric hospital admissions for systolic heart failure without congenital heart disease. It found that anemia was present in 26% of admissions and was associated with younger patient age, more systemic comorbidities like kidney and liver failure, and greater resource utilization including longer hospital stays and higher costs. While anemia was linked to worse outcomes like cardiac death on initial analysis, this association did not remain significant after accounting for other clinical factors. Tuberculosis affected mankind for over 5000 yrs and continue to be a major public health problem. So to generate hypothesis about tuberculosis this study was conducted to study the profile of tuberculosis cases attending at DTC Jaipur. A record based cross-sectional study of tuberculosis patients attended at District Tuberculosis Centre, Jaipur were taken. Chi-square test was used to find out association. It was found in this study that Cure Rate significantly varied with type of case, age and sex of case. It was observed highest in cat.III and below 20 yrs of age. Defaulters were max. in Relapse retreated cases and in elderly (>60 yrs.) cases whereas it was not affected significantly with age . Failure Rate was highest in category I cases and significantly varied with age and sex. About 3/4th of total deaths were observed in Failure and Relapse retreated cases. Case Fatality Rate (CFR) showed not significant variation with sex. So it can be concluded with this study that maximally affected age group with tuberculosis is either children or 40-60 years with male predominance. Category III cases are more easier to cured and Category II cases are most difficult to cured. Cure rate was lesser in 40-60 years of age group and males. Defaulters and CFR were more in category II, 40-60 year of age group and males. Optimal treatment strategy for acute cholecystitis based on predictive factors
This article summarizes the results of a large, international multicenter retrospective study examining optimal treatment strategies for acute cholecystitis. The study included over 5,000 patients from Japan and Taiwan who were divided into four treatment groups: primary cholecystectomy, cholecystectomy after gallbladder drainage, gallbladder drainage alone, or medical treatment alone. The study found significant differences in mortality rates between patients with low versus high Charlson comorbidity index scores. For less severe cases, factors like low BMI and higher CCI predicted higher mortality. For severe cases, jaundice, neurological or respiratory dysfunction predicted higher mortality. Abstract— Non Alcoholic Fatty Liver Disease is also becoming public health impotance nowadays. So this study was aimed to determine the association of Non Alcoholic Fatty Liver Disease with metabolic syndrome and Cardio-Vascular disease along with assessment of degree of severity of NAFLD with respect to number of components of metabolic syndrome. This study includes a total of 222 subjects were enrolled as per the inclusion/exclusion criteria, out of which 110 cases who had NAFLD with hepatic steatosis on ultrasonography and 112 subjects who did not have NAFLD were considered control. These cases and controls were interrogated and investigated further. Observations were recorded and association of Non Alcoholic Fatty Liver Disease with metabolic syndrome and Cardio-Vascular disease along with assessment of degree of severity of NAFLD with respect to number of components of metabolic syndrome. Statistical methods used were unpaired student’s t-test for continuous variables, Fischer’s and chi-sq test for categorical variables using bivariate analysis by Graph Pad Instat Version 3.10. Risk was assessed in terms of Odd's Ratio. The patients with MS and NAFLD had a higher proportion of CVD compared with those who did not have NAFLD (29.1 vs 18.1 %). This study concludes that NAFLD is significantly associated with MS; most significant with WC, followed by TG and FBS and thus can be considered as hepatic component of MS. This needs more research with large multi-centric prospective studies to evaluate NAFLD as an independent risk factor for CVD. Post transplant erythrocytosis
This study examined risk factors for post-transplant erythrocytosis (PTE) in 235 kidney transplant recipients. The results showed that being male significantly increased the risk of developing PTE. Patients with polycystic kidney disease or glomerulonephritis also had a higher risk. While smoking was associated with PTE, further analysis revealed this was likely due to the relationship between smoking and male sex rather than a direct effect. Informed consent and detailed history was taken followed by thorough clinical examination as per Performa attached. Echocardiography of all patients was done. School health cards were prepared for the patients having abnormal echocardiography and referred to higher centre for cardiologist opinion. Conclusion:- Correct diagnosis can be made with help of thorough history and clinical examination especially in acyanotic congenital heart diseases. Early diagnosis and timely referral of patient can greatly improve outcome of patients with CHDs.

Tuberculosis affected mankind for over 5000 yrs and continue to be a major public health problem. So to generate hypothesis about tuberculosis this study was conducted to study the profile of tuberculosis cases attending at DTC Jaipur. A record based cross-sectional study of tuberculosis patients attended at District Tuberculosis Centre, Jaipur were taken. Chi-square test was used to find out association. It was found in this study that Cure Rate significantly varied with type of case, age and sex of case. It was observed highest in cat.III and below 20 yrs of age. Defaulters were max. in Relapse retreated cases and in elderly (>60 yrs.) cases whereas it was not affected significantly with age . Failure Rate was highest in category I cases and significantly varied with age and sex. About 3/4th of total deaths were observed in Failure and Relapse retreated cases. Case Fatality Rate (CFR) showed not significant variation with sex. So it can be concluded with this study that maximally affected age group with tuberculosis is either children or 40-60 years with male predominance. Category III cases are more easier to cured and Category II cases are most difficult to cured. Cure rate was lesser in 40-60 years of age group and males. Defaulters and CFR were more in category II, 40-60 year of age group and males. Optimal treatment strategy for acute cholecystitis based on predictive factors
This article summarizes the results of a large, international multicenter retrospective study examining optimal treatment strategies for acute cholecystitis. The study included over 5,000 patients from Japan and Taiwan who were divided into four treatment groups: primary cholecystectomy, cholecystectomy after gallbladder drainage, gallbladder drainage alone, or medical treatment alone. The study found significant differences in mortality rates between patients with low versus high Charlson comorbidity index scores. Background: Diabetes mellitus is among the most common chronic non-communicable diseases. The development of microalbuminuria in type 1 diabetes increases the risk for renal and cardiovascular disease.
Methods: A cross sectional study was conducted at the Primary Health Care Clinics at King Fahad Armed Forces Hospital, Jeddah, Saudi Arabia. A total of 334 Saudi with type 1 diabetes were randomly selected.
Results: Total of 334 patients with T2DM included in this study; 102 (30.5%) male and 232 (69.5%) female with mean age 25.8±3.4. MA was present in 99 (29.6%). MA was not significantly more prevalent in female (69.4%) with female predominance (sex ratio male: female) 1:2.3. HTN with MA was significantly more prevalent in 51(51.5%) of MA group with odd ratio 1.7 (1.2-2.4), p=0.001 with no siginificant difference between both gender. Patients with MA have significant higher HbA1c than patients with normal buminuria and there was a significant difference between gender (p< 0.0001) and when compared to HbA1c groups (p=0.002).
Conclusion: The frequency of microalbuminuria in patients with type 1 diabetes in this study is high. It is mandatory to have adequate diagnostic, therapeutic and educational resources in addition to competent physicians who can manage microalbuminuria in diabetic patients by using a continuing, comprehensive and coordinated approach.Project of Biostatistics

This study examined the effects of metabolic syndrome and atherosclerotic risk factors on blood sugar control in 200 diabetic patients in Thailand. The results showed that patients with a higher number of metabolic syndrome components, such as hypertension, obesity, high triglycerides, and low HDL, had greater difficulty controlling their blood sugar. Younger patients and those who did not regularly attend the diabetic clinic were also more likely to have uncontrolled blood sugar. While atherosclerotic risk factors like smoking and family history were associated with worse control, the relationship was inconclusive due to the study's small sample size. In conclusion, metabolic syndrome components and younger age were linked to challenges with blood sugar management in diabetic patients.

biochemical characteristics of geriatric patients with type 2
diabetes mellitus (DM) attending tertiary care teaching hospital,
SVIMS, Tirupati. OBJECTIVES: To document clinical
features, biochemical parameters and anti-diabetic medications
received as per records. METHODOLOGY: This
retrospective study was performed in Department of
Endocrinology in SVIMS, Tirupati, over a period of 6 months.
Demograghic details, laboratory parameters and
pharmacotherapy details were collected in the pre-designed
annexure form. RESULTS: A total of 100 diabetic elderly
patients were included in the study. Out of which 57 were men,
43 were women. Most of them were under the age group of
60-64 years followed by other age groups. 28 patients were
having the diabetes duration of 11-15 years. Some of them were
having diabetic complications (such as diabetic retinopathy,
diabetic neuropathy, diabetic nephropathy). Among 100
patients, 32 patients were normal weight, 22 patients were
overweight, 28 patients were obese-I, 18 patients were obese-II.
About 85% of patients were having high lipid levels. Some
patients were on Oral Hypoglycemic Agents (OHAS), some
patients were using both Insulin & OHAS.CONCLUSION:
The present study comprised of 100 elderly type 2 diabetic
patients in which males were higher in number when compared
to females, most of the patients were in the age of group of
60-64 years. Hypertension was the most common co-morbidity
associated with DM followed by Coronary Artery Disease
(CAD). Diabetic neuropathy was the most prevalent
complication followed by Diabetic retinopathy and diabetic
nephropathy. In this study 46% of the patients were obese. Most
of the patients were on OHAS alone, some were on OHAS and
Insulin combination therapy while very few were on insulin
therapy alone. Dyslipidemia was present in 85% of the patients
and the most common form of dyslipidemia was low HDL and
high LD

Abstract— Diabetes Mellitus (DM) is a chronic and progressive condition with a hereditary predisposition which is further induced by unhealthy lifestyle. It is a silent killer with cardiovascular complications being most common cause of morbidity and mortality in patients with T2DM.
Objective: To find out association of socio demographic and clinical parameters of diabetes type 2 with hypertension and dyslipidemia among Diabetes Mellitus type 2 cases aged 18 to 70 years. Methodology: A cross sectional hospital based study was conducted on 272 type 2 DM patients attending Department of Medicine in a secondary care referral hospital after taking consent.
Results: Hypertension was present in 192 (70.59%) and dyslipidemia was present in 93 (34.19%) of type 2 diabetes patients. Age, family income, presence of family history, duration of illness, type of treatment, consumption of alcohol , BMI, Hba1c level were found to be associated significantly with both hypertension and dyslipidemia in type 2 DM patients.

This study examined the relationship between gastrin hormone levels and microalbumin urea in 67 patients with type 2 diabetes mellitus and 25 controls. The patients were divided into three groups based on diabetes duration: less than 10 years, 10-20 years, and over 20 years. Both gastrin hormone levels and microalbumin urea levels were measured. There were significant differences found between gastrin levels and microalbumin urea levels for each diabetes duration group compared to controls. Specifically, gastrin levels were higher in patients with microalbuminuria compared to those without, and this difference increased with longer diabetes duration.

Metformin and vascular health in youth with type 1 diabetes T1D
Metformin may improve cardiovascular outcomes in youth with type 1 diabetes based on results from two recent randomized controlled trials:
1) The first study found that metformin improved vascular smooth muscle function over 12 months compared to placebo, with greater effects at 3 months when A1C also improved. Metformin also reduced daily insulin needs.
2) The second study found that metformin improved insulin sensitivity, reduced weight and BMI, and improved aortic stiffness and wall shear stress measures over 3 months compared to placebo. Metformin improved vascular health even in normal weight youth.

BACKGROUND • A global diabetes epidemic is currently ongoing, with the number of cases increasing worldwide in all age groups and all socio-economic classes. • Young adults with early onset T2DM are more likely to develop chronic Microvascular and Macrovascular complications than older adults with type 2 diabetes.
- 4. AIMS • The present study aims to characterize clinically, biologically and therapeutically cases with T2DM and to and compare them with type 1 DM (T1DM) patients in the same age group . • To establish the prevalence of cardiovascular risk factors in both diabetes types.
- 5. PATIENTS AND METHODS • STUDY GROUPS CONSISTED OF 283 T2DM Males ( age : 18 - 40 years) and same period 48 T1DM Males ( age : 18 - 40 years) were included in the study. • Patients data was recorded From the follow-up charts . It consisted of Gender Age Diabetes type Diagnosis circumstances Height Weight at diagnosis maximal weight Blood Glucose lipid panel Presence of hypertension diabetes treatment Presence of chronic diabetes complications,

PATIENTS AND METHODS • For Each Group we Calculated Gender distribution Mean age at onset Distribution depending on the type of onset Average BMI Distribution based on weight status Mean TC, TG and HDLc Prevalence of cardiovascular risk factors Metabolic syndrome (MetS) Chronic complications. • Data was statistically processed with a threshold for significance of 0.05.

RESULTS • Patients with T1DM were significantly younger compared to those With T2 DM (30.2±17.0 years vs. 36.3±3.6 years, p<0.0001). • Most patients from the T2DM Group were diagnosed by Fasting Plasma Glucose OGTT (Oral Glucose Tolerance Test) • T1DM Patients was most commonly diagnosed in the Presence of ketosis/ketoacidosis Hypertension Chronic complications were rarely encountered. MetS was present in 67.8% of T2DM patients vs. 10.5% in T1DM Subgroup (p<0.001).

RESULTS Patients’ characteristics are presented in Table 1. • No differences were found regarding gender distribution and background between T2DM and T1DM. • Patients with T1DM were significantly younger compared to those with T2 DM (30.2±17.0 years vs. 36.3±3.6 years, p<0.0001). • In T2DM group, 17 (6%) were between 18 and 30 years, while in T1DM 41.7% were aged between 18 and 30 years.
- 9. RESULTS • Almost half of T2DM patients have at least one relative with T2DM, compared to only 30% in T1DM.
- 12. RESULTS • Most patients from the T2DM group were diagnosed by routine fasting plasma glucose measurement (65.0%) AND Oral Glucose Tolerance Test (OGTT) (14.1%).

DISCUSSIONS • Subjects with T2DM are at increased risk for cardiovascular disorders and often exhibit various cardiovascular risk factors such as abnormal lipid levels, hypertension and obesity. • T2DM is a common disease in the 18-40 years age group. It affects mostly obese and overweight individuals and is very frequently associated with other cardiovascular risk factors, even from the diagnosis. • Cardiovascular risk factors are also present in T1DM
