The US healthcare system has had quite a number of flaws that have for a very long time been ignored due to the assumption that the two main methods of catering for healthcare in the US, Medicaid and Medicare, are sufficient and efficient. However, late developments have been made to cub the issues brought about by their inadequacies. These include the famous affordable care act famously referred to as Obamacare (Patient protection and affordable care act) (Pipes, 2013).
A closer look at there issues with all over the country shows that the two most common methods of helping individuals cope with healthcare needs mostly ignore children. Medicare is for individuals that are beyond 65 years while Medicaid is not a guarantee not unless there is clear evidence of low income levels which unfortunately, is also not a guarantee. Children Healthcare Program at least is available for people that are considered to have an income too high to qualify for Medicaid. All these however aren’t supported by the data at the North Memorial Hospital of Minnesota. Children continue suffering compared to adults at least in comparison and the benchmark on government help through health funding.
There is a great need to look at this close and find out whether these facts are so or it is an assumption and if the rest of the state of Minnesota is still in the same state.
Availability of Medicaid reduces chances of children receiving health care from the medical canter. This research design is going to rely on Medicaid as the independent variable. Health care on children received through CHIP (Children Health Insurance Program) is going to be the dependent variable. Therefore, the problem under research here is: Availability of Medicare to (some) families undermine the probability of children to get access to CHIP and thus resulting to less affordability and availability of healthcare to children through the medical center.
PURPOSE STATEMENT
This research method document aims at establishing the relationships between various Healthcare products and platforms that are provided by the Federal government in association with the state of Minnesota and the way they interact to affect accessibility and affordability of healthcare with the case study of the North Memorial Hospital in Minnesota.
It will identify the role of, and the statistics around, Medicare, Medicaid and Children Health Insurance Program and their interplay to understand if they have any effect on the healthcare affordability and availability to children. Over the years, the dynamics of uncompensated care have been changing, which has, in
turn, necessitated the formulation of better ways of managing it. Market aspects that have been
changing are affecting the type and level of coverage in the economy. For instance, as the
demand for health services increases with the growth in the general population, more people
from the low and middle social classes are still accessing health services even if it is at the lowest
costs. In these days, uncompensated care often comes from people that don't have insurance and
cannot afford to pay the cost of care that will be offered to them in a hospital. These patients are
managed in different ways, including through the use of charity care for the needy ones, which
leads to a chance to provide support to the mission of healthcare institutions. Additionally,
uninsured patients could be managed by categorizing their uncollectible funds as bad debts.
While health institutions do their best to collect the dues held by their patients, collection efforts
are faced with challenges such as that some of the patients may be uninsured or that they might
not be able to pay for the services offered to them. From this point of view, uncompensated care
makes part of the payments that have to be met by the hospitals. In my opinion, to adequately
address the issues related to uncompensated care, it is crucial to apply strategies such as revenue
cycle automation to track debts and avoid bad debts, as well as encouraging more charity care.
Revenue cycle automation will help to prevent bad debts and thus, extra costs to
hospitals, which helps in tracking the uncompensated care costs (HFMA, 2008). At some point

in time, most hospitals play a charitable purpose, which means that they prefer charity care to
bad debts because they show that they are giving back to the community. A good example of this
is when hospitals, whose supply is higher than the demand of patients, donate medicines,
especially in low-income areas. Revenue cycle automation helps in streamlining and speeding
the data collection processes during the process of giving patients services (Marcinko and Hetico
72). Such data might include information on patient’s history, income, and other economic
considerations. It is through the analysis of this data that an institution can easily clarify if
patients’ health fees should be followed up and collected or considered as bad debts. From this
point of view, hospitals will be able to cut off some of the bad debts that could have been
incurred. Additionally, the automation will help institutions to document patient’s information
more efficiently and at the same time ensure that all patients are treated without discrimination

In the abiding endeavor to enhance human service programs, service providers, policy makers and funders are more and more recognizing the significance of thorough program evaluations. They want to recognize which programs function for which groups, and they need endings based on proof, as opposed to impassioned pleas and testimonials. Pyramids usually tend to be easy to understand and work well to capture tiered concepts, so pyramids have been used to depict the tiered nature of primary healthcare, secondary healthcare, and tertiary healthcare services, which is the inverse relationship of effort needed and health impact of different interventions and nutrition recommendations (Issel et al., 2022). The public health pyramid is divided into four categories: direct healthcare services, enabling services, population-based services and infrastructure services.
The direct services level of the public health pyramid focuses on health programs for individuals. Due to the fact that this service focuses on individuals, the sampling may be a challenge because of who would be participating and non-participating in an intervention or non-interventions. Instead of randomly assigning individuals to either the program or a control group, it might be feasible for participants to become their own type of control (e.g., pretest and posttest) or perhaps future participants can serve as controls while they remain on a waiting list. Additionally, the costs can vary greatly depending on the type of data, and the frequency of data collection as well.
The enabling services, as mentioned above, focuses on groups of individuals and are provided in a wide range of contexts. The sampling issue in this case would be because of the several outcomes. It may be a challenge to recognize and recruit a comparison group with the same characteristics as the program’s participants considering the broad range of contexts. Thus, health services programs may not be appropriate for experimental designs, but can be suitable for quasi-experimental designs instead. Perhaps initiating random assignments can be a possibility depending on the group or at a community level as well.
The population-based level focuses on providing to the entire population. A sampling issue may be that, because this level focuses on a wider range of people, there are limitations of evaluation options to design that can be appropriately implemented among populations. In this case, time-series designs can be useful for evaluating population-level programs. Thus, existing data on populations are likely to be less costly to conduct as well.
The infrastructure services level focuses on all said levels, and interventions touches on changing healthcare operations in an organization or the overall public health system. With that said, the outcome evaluation is likely to determine whether the evaluation show actual changes in the infrastructure or changes to the health status of patients (or clients). Over the years, the dynamics of uncompensated care have been changing, which has, in
turn, necessitated the formulation of better ways of managing it. Market aspects that have been
changing are affecting the type and level of coverage in the economy. For instance, as the
demand for health services increases with the growth in the general population, more people
from the low and middle social classes are still accessing health services even if it is at the lowest
costs. In these days, uncompensated care often comes from people that don't have insurance and
cannot afford to pay the cost of care that will be offered to them in a hospital. These patients are
managed in different ways, including through the use of charity care for the needy ones, which
leads to a chance to provide support to the mission of healthcare institutions. Additionally,
uninsured patients could be managed by categorizing their uncollectible funds as bad debts.
While health institutions do their best to collect the dues held by their patients, collection efforts
are faced with challenges such as that some of the patients may be uninsured or that they might
not be able to pay for the services offered to them. From this point of view, uncompensated care
makes part of the payments that have to be met by the hospitals. In my opinion, to adequately
address the issues related to uncompensated care, it is crucial to apply strategies such as revenue
cycle automation to track debts and avoid bad debts, as well as encouraging more charity care.
Revenue cycle automation will help to prevent bad debts and thus, extra costs to
hospitals, which helps in tracking the uncompensated care costs (HFMA, 2008). At some point

in time, most hospitals play a charitable purpose, which means that they prefer charity care to
bad debts because they show that they are giving back to the community. A good example of this
is when hospitals, whose supply is higher than the demand of patients, donate medicines,
especially in low-income areas. Revenue cycle automation helps in streamlining and speeding
the data collection processes during the process of giving patients services (Marcinko and Hetico
72). Such data might include information on patient’s history, income, and other economic
considerations. It is through the analysis of this data that an institution can easily clarify if
patients’ health fees should be followed up and collected or considered as bad debts. From this
point of view, hospitals will be able to cut off some of the bad debts that could have been
incurred. Additionally, the automation will help institutions to document patient’s information
more efficiently and at the same time ensure that all patients are treated without discrimination

The direct services level of the public health pyramid focuses on health programs for individuals. Due to the fact that this service focuses on individuals, the sampling may be a challenge because of who would be participating and non-participating in an intervention or non-interventions. Instead of randomly assigning individuals to either the program or a control group, it might be feasible for participants to become their own type of control (e.g., pretest and posttest) or perhaps future participants can serve as controls while they remain on a waiting list. Additionally, the costs can vary greatly depending on the type of data, and the frequency of data collection as well.
The enabling services, as mentioned above, focuses on groups of individuals and are provided in a wide range of contexts. The sampling issue in this case would be because of the several outcomes. It may be a challenge to recognize and recruit a comparison group with the same characteristics as the program’s participants considering the broad range of contexts. Thus, health services programs may not be appropriate for experimental designs, but can be suitable for quasi-experimental designs instead. Perhaps initiating random assignments can be a possibility depending on the group or at a community level as well.
The population-based level focuses on providing to the entire population. A sampling issue may be that, because this level focuses on a wider range of people, there are limitations of evaluation options to design that can be appropriately implemented among populations. In this case, time-series designs can be useful for evaluating population-level programs. Thus, existing data on populations are likely to be less costly to conduct as well.
The infrastructure services level focuses on all said levels, and interventions touches on changing healthcare operations in an organization or the overall public health system. One of the first cases that inspired Freud in the development of what would eventually become the Psychoanalytic Theory was the case of Anna O. Anna O. was actually a patient of one of Freud’s colleagues Josef Breuer. Using Breuer’s case notes, Freud was able to analyze the key facts of Anna O’s case.
Anna O. first developed her symptoms while she was taking care of her very ill father with whom she was extremely close. Some of her initial symptoms were loss of appetite to the extent of not eating, weakness, anemia, and development a severe nervous cough. Eventually she developed a severe optic headache and lost the ability to move her head, which then progressed into paralysis of both arms. Her symptoms were not solely physical as she would vacillate between a normal, mental state and a manic-type state in which she would become extremely agitated. There was even a notation of a time for which she hallucinated that the ribbons in her hair were snakes.
Toward the end of her father’s life she stopped speaking her native language of German and instead only spoke in English. A little over a year after she began taking care of her father he passed away. After his passing her symptoms grew to affect her vision, a loss of ability to focus her attention, more extreme hallucinations, and a number of suicidal attempts (Hurst, 1982).
Both Freud and Jung would acknowledge that unconscious processes are at work in this woman's problems. However, they would come to different conclusions about the origin of these problems and the method by which she should be treated.

Point Mar Case Study
Walden University
Point Mar is a small, low-income population that is
struggling to combat life-threatening illnesses and diseases such
as Chlamydia. Due to the lack of funding Point Mar is
challenged to operate in hospitals and health departments with a
limited amount of staff. Though the Point Mar health
department has hired a new health educator assisting with
combatting chlamydia in teens has been a constant battle.
Chlamydia trachomatis is a sexually transmitted disease that is
prominent in teens and young adults that reside in low-income
areas (Hitti, 2005). This sexually transmitted disease is caused
by bacteria and can be contracted through unprotected sex with
an infected person (Centers for Disease Control and
Prevention, 2014). As a health educator, it is imperative to
educate teens on sexually transmitted diseases, ways to prevent
sexually transmitted diseases, and the available resources. The
reason is that it will decrease the number of teens contracting
the disease as well as preventing sterilization and other
complications that occur when chlamydia is left untreated.
In an attempt to decrease the number of infected teens, it is
vital to create an effective health education program. As a
health educator, it would be beneficial to collaborate with local
schools to set up programs such as the STD Community
Intervention Program (SCIP) (City of Santa Cruz, 2020). The
reason is that the program will educate teens on sexually
transmitted diseases such as chlamydia to identify the issue,
where the breakdown in information or resources occur and
reduce the number of teens suffering from the disease. Though
the SCIP program is currently only implemented in California,
17. it is ideal to begin a program such as this in other states to raise
awareness and prevent the spread of sexually transmitted
diseases.
When addressing the Point Mar Case study on chlamydia, it is
imperative to gather information from peer-reviewed literature.

Conclusion
Eliminating chlamydia in teens will be a team effort between
health educators, medical professionals, schools, and persons
within the community. Though it will take much work

positive attitude towards condom usage in hopes of preventing

Summary:
After completing this activity, the importance of health
educators has become more evident. Chlamydia is a sexually
transmitted disease that does not receive the proper amount of
attention, which is the reason that more teens are contracting
the disease and encountering more severe health problems
(Adderly-Kelly, 2005). After starting various programs within
the community and analyzing the results, health educators
realize that there is still more work that needs to be done to
convince teens how important it is to practice safe sex. One of
the analyses from the program is that teens who are not
adequately educated or provided with the proper resources are
still unaware of the importance of condoms. On the other hand,
81% of teens who were educated on how to prevent the
transmission and spread of chlamydia and other sexually
transmitted diseases stated that they would use condoms as a
preventative measure (Asare, 2010). Though health educators
are making a positive impact on teens to help prevent teen
pregnancy as well as the spread of sexually transmitted
diseases, there are still teens who are suffering in silence.
Theoretical applications are critical as it applies to reduce or to
eliminate sexually transmitted diseases because it allows health
educators to analyze and exercise the various stages. The
contemplation stage is acknowledging that teens are contracting
sexually transmitted diseases, which causes health educators to
gather a plan that will eliminate the spread of sexually
transmitted diseases. In the preparation stage, health educators
need to find a program or method that will educate as many
teens as possible to reduce the percentage of teens who are
21. affected. The action stage is crucial because it gives teens and
health educators that opportunity to see how well the plan or
program is working so that they can address any issues that may
arise and make changes as necessary. Finally, the maintenance
stage gives health educators and students the statistics from
before and after the program started to give them an idea of how
successful the program is as it applies to keep teens free from
having children at a young age or contracting sexually
transmitted diseases. The ultimate goal is to ensure that health
educators are passionate about their job and ability to help
reduce the spread of sexually transmitted diseases as well as
ensuring teens can lead a healthy life.

Chlamydia
Sexually transmitted infections are significant health
concerns in the United States, causing numerous health issues.
A large percentage of diseases reported by the Centers for
Disease Control and Prevention are sexually transmitted
diseases such as chlamydia and gonorrhea. Chlamydia infections
have been reported as the most common infections affecting
teens residing in the United States. The Centers for Diseases
Control and Prevention indicates that the country records about
4 million acute infections each year (Adderley-Kelly &
Stephens, 2005). Sexually transmitted diseases are common,
particularly among young individuals, with the United States
reporting nearly 20 million new infections each year. About 50
percent of sexually transmitted infections are in persons aged
between 15 years and 24 years (CDC, 2014). In light of this,
school-based behavioral intervention programs are suitable for
educating teenagers about the causes, implications, and
prevention of Chlamydia.
Reasons for Using School-Based Health Education
There is no doubt that several approaches have been used to
reduce teenagers' from contracting diseases from sexual
intercourse. They include services offered in youth centers such
as condom distribution, the establishment of teenage friendly
health centers that promote prevention methods, school-based
healthcare services, and preventive education in learning
institutions. School-based health education is the appropriate
approach for addressing the increasing rate of Chlamydia among
teenagers in Vista County because it is not only the most
inclusive but also comprehensive approach compared to the
other interventions. Sani, Abraham, Denford, and Ball (2016)
indicate that school-based health education programs are the
most effective amongst teens who are adequately educated and
provided with the appropriate resources. Mavedzenge, Luecke,
and Ross (2014) found substantial evidence for the
54. effectiveness of school-based health education programs
concerning various sexually transmitted infection-related
outcomes from assessments across the world and recommended
for full implementations of such interventions. Picot et al.
(2012) found that school-based health education programs for
the prevention of sexually transmitted infections in adolescents
and young adults can result in increased self-efficacy as well as
knowledge improvements. The decision has been arrived at
based on the fact Vista County has three high schools and four
middle schools that serve at least 33 percent of its school-aged
population. These institutions provide suitable avenues for
promoting Chlamydia awareness and prevention programs.
Role of School-Based Sexual Health Educators
School-based sexual health educators should work toward
ensuring that every high, middle, as well as elementary school
in Vista County, develops and maintains comprehensive and
well-coordinated sexual health programs. The Institute of
Medicine defines a comprehensive school health program as an
integrated set of planned, sequential, school-affiliated
strategies, activities, and services (Marya, 2011, p. 241) to
promote optimal physical, emotional, and educational, as well
as social development of learners. The local community needs
to determine the program, resources, requirements, plus
standards, and not only incorporate but also support family
requirements. Coordinated by a multi-disciplinary team, the
program must be accountable to the local community for quality
as well as effectiveness (2011).
School-based sexual health educators should, therefore, ensure
that schools develop health programs that focus on priority
behaviors affecting health and learning, act as a basis of support
for every teenager, and appraise, protect as well as promote
health. The program should also provide multiple interventions
for preventing Chlamydia, value coordinated approach and
oversight activities, and offer systematic program planning.
Moreover, a health educator should ensure that a school-based
program promotes ongoing staff development and active student
