Recomendações da OMS sobre cuidados maternos e neonatais para uma experiência pós-natal positiva.
Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.

4. Advocacy for health equity: a synthesis review
Aim: To bring together, for the first time, all evidence on
practices that can aid advocacy efforts

search of academic and grey literature.

5. Experts workshop
Aim: To analyse initial findings and fill gaps with the help of

7. Conclusions and recommendations
Evidence is unlikely to be the only conclusive factor in policy
decisions, and policy makers value a variety of different kinds
of evidence that do not necessarily accord with the ‘hierarchy
of evidence’
Evidence should be translated for different audiences; it
needs to be easy to understand, contain key facts, avoid
jargon and present a balance of different kinds of evidence.
Value of different kinds of advocacy message(s):
Importance of tailoring, context. Health/health equity or the
SDH? Health as a value, social justice, economic and human
rights arguments, sustainability and self-interest
8. Conclusions and recommendations
Science and advocacy: Scientists can have a clear role to
play in advocacy, but some issues of compatibility exist.
Advocacy organisations can bridge evidence and policy
Need for capacity : Individual and organisational capacities
including inclination, the valuable contribution of people
experiencing exclusion to advocacy
Barriers: a pervasive zeitgeist that blames people for their
ill health, biomedical approaches and political short-termism
Enablers: increased public understanding of SDH, greater
contact with disadvantage in syllabuses and training in
advocacy, enabling value of overlooked allies
9. Research to support advocacy efforts
Evaluations of policies particularly their differential effects
across the social gradient, for discrete social groups.
ROI/cost-benefit
Applied research to support policy strategies, particularly
examples of cross-sectoral collaboration, proportionate
universalism, the life-course approach, etc.

Editor's Notes
Systematic keyword search of four journal databases (PubMed, Web
of Science, PsycINFO (EBSCO) and SocInfo), and a fixed-length keyword
search of Google.
• Inclusion criteria based on relevance (e.g. advocacy for policy change…)
• 21,673 pieces of literature were reviewed; 196 included in analysis.
• Thematic analysis including: 1) familiarisation, 2) reading entire texts,
3) distilling meaning and developing themes, 4) refining themes and
developing sub-themes, discussion, 5) iteratively revisiting the literature
to ensure balance.
Three case studies (Finland, Greece and the Netherlands) built advocacy into their research.
Health 2015: Improving inter-sectoral co-operation to improve health equity (The National Institute for Health and Welfare, Finland)
Methods: Interviews with contact points in four ministries, development of materials for group discussion, recorded group discussions with staff at non-health ministries, analysis and coding.
Some findings: Demonstrate cost/benefit, find common goals, do not use health language outside health sector, understanding SDH a prerequisite to intersectoral co-operation, advocacy should be rewarding & needs permanent structures.
Child health in disadvantaged families in the Netherlands: Advocacy components of a child health intervention for disadvantaged families (Institute for Healthcare Improvement, the Netherlands)
Methods: Document analysis, interviews with stakeholders, questionnaires/phone calls to policy makers in regions where there is no intervention on poverty and child health, analysis.
Some findings: Explain scale of problem and basic mechanisms, need local information, identify the important people to convince, national networks on the issue of poverty and child health would facilitate knowledge transfer and policy action.
Think Family: Understanding the processes involved in promoting a programme to tackle social exclusion at local and national level (Blackburn with Darwen Borough Council, England)
Methods: Document analysis, semi-structured interviews with key informants, thematic coding and analysis.
Some findings: Scientific evidence important at national level, testimonials and qualitative more important at local level, need for cost-savings data, identify champions, tailor message to audience, visual presentations and explanations, argument ‘health as an enabler of participation/work’ (+ savings to budgets), national policy can present window of opportunity.
Food Aid For Deprived Children: Developing advocacy arguments to promote a programme which provides nutritious free school meals to children in deprived areas of Greece (Institute of Preventive Medicine, Environmental & Occupational Health, Greece)
Methods: Semi-structured interviews, data analysis, preparation of materials, public-private information event.
Some findings: Context is important (crisis), need to demonstrate expertise and capacity, show evaluated evidence of intervention/cost-effectiveness, qualitative data and stories can be powerful, EU-level action as an enabler, importance of private sector.
Mental Health First Aid: Understanding implementation of Mental Health First Aid (Public Health Wales)
Methods: Document analysis, semi-structured interviews with stakeholders.
Some findings: Important to fit with the 'policy frame', make the economic case, lower standards of evidence acceptable if an intervention is 'off the shelf', CSR awards are important to employers.
Evidence is unlikely to be the final word in policy decisions, and policy
makers value a variety of different kinds of evidence that do not necessarily
accord with the ‘hierarchy of evidence’.
• Increased contacts between the producers and users of research would
improve uptake of evidence in policy processes.
• Advocacy organisations can play an important role in bridging the cultural
worlds of policy, research and practice, transferring and translating research findings, and responding to windows of opportunity to exert
timely pressure on decision-making processes.
• The health sector can lead initiatives, but needs to adapt language to other
sectors and find ‘win win’ objectives to work towards; it can provide expertise
and input to help other sectors achieve their aims – which simultaneously
help work towards health equity.
• There is a need for more research on advocacy, particularly regarding how
applicable findings are to different European contexts
