The prevalence of sarcopenia increases with age from 5-13% in 60-70 year olds to 11-50% in those over 80. Causes include changes in hormones, immobility, age-related muscle changes, nutrition, and neurodegenerative changes. Diagnosis involves measuring low muscle mass, slow walking speed, weak grip strength, and difficulty with daily activities. Management focuses on exercise, medication, nutrition, and supplements to help aging muscles respond better to anabolic stimuli and prevent further muscle loss.
Case report Duchenne Muscular Dystrophy
This case report describes the treatment of a 7-year-old boy diagnosed with Duchenne muscular dystrophy (DMD) using stem cell therapy. DMD is a genetic disorder causing progressive muscle degeneration. The boy showed symptoms of muscle weakness and difficulty walking from age 2. After receiving no treatment elsewhere, stem cell therapy was started in October 2012. Progress notes since then show improvements in arm flexion, reduced skin infection, and weight gain. The conclusion is that stem cell treatment for DMD shows clinically improved results, and better outcomes require early and ongoing multi-disciplinary care.

A study of 40 recreational athletes aged 40-81 found that mid-thigh muscle area and lean mass did not decrease with age despite an increase in total body fat percentage. Muscle area, quadriceps area, and strength did not decline with age. Chronic exercise may prevent the loss of lean muscle mass and strength seen in sedentary aging adults. Blood samples obtained before and up to 48 hours after exercise showed no significant changes in circulating levels of inflammatory markers. Chronic exercise preserves lean muscle mass in masters athletes. A study of 40 recreational athletes aged 40-81 found that mid-thigh muscle area and lean mass did not decrease with age despite an increase in total body fat percentage. Muscle area, quadriceps area, and strength did not decline with age. Chronic exercise may prevent the loss of lean muscle mass and strength seen in sedentary aging adults. This maintenance of muscle mass and strength can decrease falls, functional decline, and loss of independence in aging.

Telomerase is an enzyme that helps rebuild telomeres, the end caps of chromosomes, and longer telomeres are associated with younger cells. A study found that making improvements in diet, supplements and lifestyle for just 3 months increased telomerase activity levels in men with prostate cancer by 29.84%. Lifestyle factors like managing stress, regular exercise, meditation and following a whole foods diet can help reduce disease risk and extend one's lifespan. A study examined the effects of an acute bout of lower-body resistance exercise on inflammatory gene expression in skeletal muscle in 24 post-menopausal women. Muscle biopsies obtained before and 3 hours after exercise showed significant increases in mRNA content for several inflammatory genes including TNF, IL1, IL6, IL8, and others. Blood samples obtained before and up to 48 hours after exercise showed no significant changes in circulating levels of inflammatory markers. The results indicate that resistance exercise can up-regulate transcription of inflammatory mediators locally within skeletal muscle.

Kelompok lansia dipandang sebagai kelompok masyarakat yang berisiko (population at risk) mengalami gangguan kesehatan. Oleh karenanya, kelompok lansia merupakan kelompok risiko tinggi yang menjadi perhatian utama dalam cabang ilmu keperawatan komunitas. Masalah keperawatan yang menonjol pada kelompok tersebut adalah meningkatnya disabilitas fungsional fisik sebagai dampak dari respon lansia terhadap proses penuaan, penyakit kronis, atau status psikososialnya. Disabilitas fungsional lansia sebagai efek dari perubahan fisiologis (umur depresi dan demensia) memungkinkan untuk dijelaskan melalui Model Sistem Neuman (MSN). Mengingat MSN memiliki banyak interrelasi konsep sehingga derivasi teori konseptual tersebut lebih bersifat kontekstual. Oleh karenanya, peneliti bermaksud agar penelitian ini dapat digunakan sebagai studi pendahuluan terhadap penelitian-penelitian mengenai disabilitas fungsional yang lebih kompleks. Gigliotti (2003) berpendapat bahwa kredibilitas MSN hanya dapat dikembangkan melalui proses derivasi dan pengujian teori antara (middle-range theory) sebagai derivat dari MSN.

This case series examines the use of medical shockwave therapy for 10 patients with chronic low back pain. Shockwave therapy involves using focused acoustic pulses to target deep tissue. After 3 sessions of 1000 pulses each over 3 weeks, 8 of the 9 patients showed excellent improvement in pain levels, functional disability, and reduced need for pain medication that was maintained at the 12 week follow up. This positive preliminary outcome supports further investigation of shockwave therapy as a potential disease-modifying treatment for chronic low back pain.

Abstract
Introduction: Sarcopenia is a progressive and generalized loss of skeletal muscle mass and strength associated with ageing. Regular exercise has been indicated to be the only strategy which consistently improves sarcopenia and physical function in elderly. The aims of this study are to review the mechanisms of sarcopenia and to evaluate the role of exercise in the elderly with sarcopenia and the current recommendations about the exercise interventions.
Discussion: Exercise significantly improve mobility, functionality, muscle mass, bone mineral density, balance and proprioception; thus, decreasing the risk of falls, increasing quality of life and reducing morbidity and mortality. Different types of exercise may be of interest in patients with sarcopenia: aerobic, progressive resistance, flexibility and balance training. Supervised or group exercise programs may be preferable for elders, with regard to both safety and compliance.
Conclusion: Sarcopenia is a strong predictor of disability, morbidity, and mortality in elderly. Anaerobic exercise seems to be the most important tool to prevent and treat sarcopenia.

Understanding Sarcopenia
Muscle mass and strength decrease with age, known as sarcopenia. By age 75, sedentary adults can lose up to 30% of muscle mass. This contributes to chronic diseases and loss of physical function and independence. Resistance training and adequate protein intake can help prevent or reverse age-related muscle loss. While older adults can regain strength after training, they may have smaller gains in muscle size compared to younger adults due to diminished muscle regeneration ability with aging.

This study investigated variability in musculoskeletal traits in laboratory rats. The weights of three hindlimb muscles (tibialis anterior, extensor digitorum longus, and soleus) and tibia bone length were measured in around 500 outbred rats divided into male and female groups at two ages. Significant variability was found in muscle weights, ranging from around 2-fold differences between individuals of the same sex and age. Strong positive correlations were observed between the weights of tibialis anterior and extensor digitorum longus muscles, as well as between muscle weights and tibia bone length. Weaker correlations were found between muscles of different fiber types. This data will be used in future genome-wide association studies to identify specific genes

Background and Purpose. The purpose of this study was to determine
the level of muscle force associated with ability to walk in the
community without assistance, in the community with assistance, or at
home only in individuals with amyotrophic lateral sclerosis (ALS).
Subjects and Methods. Percentage of predicted maximal muscle force
(%PMF) of lower-extremity muscles was determined, and walking
ability was categorized in 118 patients with ALS during periodic visits to
the Neuromuscular Research Unit. Data were derived from consecu-
tive visits in which subjects demonstrated declines in walking ability.
Means for %PMF of each muscle group and a limb average were
calculated at each consecutive visit. Results. The mean lower-extremity
average %PMF was: (1) 54.01% (SD512.76%) for subjects who walked
independently in the community and 50.19% (SD514.38%) during
the next visit when these same subjects required assistance in the
community (difference53.82%, 95% confidence interval [CI]5
2.4525.19);(2) 37.52% (SD515.17%) during the last visit that subjects
walked with assistance in the community and 32.18% (SD513.83%)
during the next visit when they walked only at home (differ-
ence55.33%, 95% CI53.61–7.06); and (3) 19.12% (SD59.08%) dur-
ing the visit when subjects were last able to ambulate at home versus
13.70% (SD57.36%) when they became unable to walk (differ-
ence55.42%, 95% CI52.9727.96). Conclusion and Discussion. The
findings suggest there are required levels of lower-extremity muscle
force for various categories of walking ability. Variations in forces
within and between categories of walking ability, however, indicate the

Muscle Force to Walking Ability in
Patients With Amyotrophic
Lateral Sclerosis
Background and Purpose. The purpose of this study was to determine
the level of muscle force associated with ability to walk in the
community without assistance, in the community with assistance, or at
home only in individuals with amyotrophic lateral sclerosis (ALS).
Subjects and Methods. Percentage of predicted maximal muscle force
(%PMF) of lower-extremity muscles was determined, and walking
ability was categorized in 118 patients with ALS during periodic visits to
the Neuromuscular Research Unit. Data were derived from consecu-
tive visits in which subjects demonstrated declines in walking ability.
Means for %PMF of each muscle group and a limb average were
calculated at each consecutive visit. Results. The mean lower-extremity
average %PMF was: (1) 54.01% (SD512.76%) for subjects who walked
independently in the community and 50.19% (SD514.38%) during
the next visit when these same subjects required assistance in the
community (difference53.82%, 95% confidence interval [CI]5
2.4525.19);(2) 37.52% (SD515.17%) during the last visit that subjects
walked with assistance in the community and 32.18% (SD513.83%)
during the next visit when they walked only at home (differ-
ence55.33%, 95% CI53.61–7.06); and (3) 19.12% (SD59.08%) dur-
ing the visit when subjects were last able to ambulate at home versus
13.70% (SD57.36%) when they became unable to walk (differ-
ence55.42%, 95% CI52.9727.96). Conclusion and Discussion. The
findings suggest there are required levels of lower-extremity muscle
force for various categories of walking ability. Variations in forces
within and between categories of walking ability, however, indicate the

Geriatrics: rheumatoid arthritis
This document provides information on a case of a 67-year-old woman with rheumatoid arthritis. It outlines her medical history, including a 2-year history of rheumatoid arthritis affecting multiple joints. On examination, she had decreased range of motion in her shoulders, elbows, wrists, knees and ankles, along with swelling and tenderness. Blood tests showed increased inflammatory markers. X-rays found soft tissue swelling and bone erosion in her hands and knees. She was started on treatment with aspirin, NSAIDs and disease-modifying antirheumatic drugs to manage her elderly-onset rheumatoid arthritis.
