Candida septic arthritis is a debilitating condition affecting joint function. Candida tropicalis, an organism found in normal human flora is noted to be the third most common pathogenic yeast in the elderly and immunosuppressed population. Infections are rare in the US, typically limited to the neonate
and elderly populations. Most infections occur in the south America and southeast Asian regions. In the last 2 decades, Tropialis infection rates have risen impart due to antifungal resistance. We present a case of Candida tropicalis infection in the knee of a 13-year-old female with a past history of relapsed Acute Myeloid Leukemia following bone marrow transplant, pancytopenia and graft vs. host disease.

A major challenge faced by diabetic patients is infected diabetic ulcers usually associated with substantial morbidity and mortality. Worse issues arise from antibiotic resistant microorganisms. This study was conducted to determine the antibiogram of bacteria isolated from wounds of diabetic patients on hospital admission. Nine wound swab samples were collected from nine diabetic in-patients with ulcers. These were processed using standard protocols. Multi antibiotic sensitivity discs (Gram negative and Gram positive) containing ten antibiotics respectively were used. Total of 91 bacterial isolates were obtained belonging to five species. Staphylococcus aureus was most predominant (34.07%) and Proteus mirabilis was the least isolated (7.69%). Pseudomonas aeruginosa showed highest (100%) resistance to the antibiotics used, followed by Proteus mirabilis (90%), Staphylococcus aureus (80%) and Escherichia coli (30%). Klebsiella pneumoniae was 100% susceptible. Streptomycin was the most efficacious antibiotic while Ciprofloxacin and Augmentin were the least. The level of resistance exhibited by these clinical isolates is worrisome and likely to impede treatment outcomes. Streptomycin showed broad spectrum activity and may be the best drug of choice for treating wounds in diabetic patients however, there is need for antibiotic susceptibility testing and consideration of patient’s physiologic disposition before introducing antibiotic regimen.

The increasing incidence of Candidiasis affecting the genitourinary tracts as well as the introduction of new antifungal drugs has recently encouraged the need for performing fungal susceptibility tests. The study was aimed at evaluating the antifungal susceptibility profile of Candida species isolated from female patients attending Aminu Kano Teaching Hospital (AKTH), with suspected Candidal infections between August, 2012 to June, 2013. Five hundred and twenty one (521) clinical samples comprising 342 urine and 179 high vaginal swabs were cultured on Sabouraud dextrose agar. The Candida species isolated were identified to species level using Chromogenic agar and API 20 C AUX test kit. Antifungal susceptibility tests were performed using commercially prepared single antifungal disc (Bioanalyse Turkey). Out of these 521 samples analyzed only 59 yielded Candida species giving the overall prevalence of 11.3% with Candida albicans 22 (37.3%) as the common species isolated followed by C. glabrata 19 (32.2%), C. tropicalis 5(8.5%), C. krusei 3 (5.1%), C. magnoliae 3 (5.1%), C. lusitaniae 2 (3.4%), C. parapsilosis 2 (3.4%), C. famata 2 (3.4%) and C. guilliermondii 1 (1.7%). The antifungal susceptibility test shows that 81.4% of the isolates were susceptible to ketoconazole and only 3.4% to nystatin. However, 33.9% were susceptible, 13.6% intermediate susceptible and 52.5% resistant to fluconazole. Similarly 28.8% were susceptible, 5.1% intermediate susceptible and 66.1% resistant to voriconazole. Additionally 25.4% were susceptible, 11.9% intermediate susceptible and 62.7% resistant to flucytosine. All the C. krusei isolates were completely resistant to azole drugs while C. famata were resistant to all the drugs tested. A two staged surgical intervention yields excellent results. A case of infected laparoscopic mesh repair treated by a two staged operation is presented along with a brief review of literature to highlight the safety and efficacy of this approach.

Abstract—In Italy the hydatid disease is more prevalent and new cases are highlighted more frequently in Sicily, Sardinia, (Italy). Aim of this study is to put the indication in search of iaditea nature in both spleen swelling and muscle tendon.
Material and Method Patients observed during the period 2007-2009 at the Surgical Clinic III and Digestive Surgery, Policlinico G Rodolico were explored for Hydatid cyste at various sites. Diagnosis of cysts ecchinococcus occurred primarily for various four reasons either for compression of bodies involved or for eosinophilia or for instrumental investigation or for anaphylactic reaction to rupture of cysts. Biological diagnosis is based on serology rather than isolation of the parasite (indirect diagnosis);
Results Patients attended during the period 2007-2009 Hydatid cyst was found in 0.5% of all cases in liver along with 4 in the lung, 3 in splenic, 2 in the mammary and 2 in the chest wall No 2. The Surgical treatment with the complete removal of the cyst with a satisfactory postoperative course in the absence of cases of relapse of the disease and by following the therapeutic act, the assumption of mebendazole 50mg / kg / day for 3 weeks at a dose of 400mg for 4 months
Conclusions There is a need to define diagnostic methods with high specificity and sensitivity, which can provide a valid diagnostic aid for the cases clinically difficult to diagnose. And the final diagnosis must then also be based on the development of immunological methods that allow the determination of specific antibodies in the serum and their titration and / or the circulating antigen determination.

Abstract— Joint replacement operations which are applied to reduce the pain and increase the movement capacity are among the surgical procedures that are used mostly nowadays. Even though a dramatic recovery is seen in the life of the patient after total knee prosthesis, possible prosthesis infection increases cost and causes high morbidity. This study was conducted with the aim to determine rates of surgical site infection after performing primer total knee prothesis operation in our clinic. Furthermore, it has been intended to understand risk factors which may cause infection and then take precautions. This study was conducted from January 2008 to January 2013, 252 knees underwent primary total knee arthroplasty operations. Among these patients infection rates, relationship to risk factors and infection treatments were analysed. It was observed that iIn 252 knees, 10 (4%) superficial infections were found, debridement and antibiotics were applied to 3 knees out of 10 and only antibiotic treatment was applied to the rest 7 knees. Deep infection was detected in 4 knees (1,6%) out of 252. Acute deep infection in 1 knee and recovery was provided with debridement and intravenous antibiotics treatment. Late deep infection was not detected in any of patients. Delayed deep infection was detected in 3 (1,1%) of these knees though. Among all risk factors only increased body mass index showed increased superficial wound infection rate. It can be concluded that among the factors like rheumatoid arthritis, diabetes, age, gender, body mass index, just body mass index has an impact on superficial infection rate. Our infection rates were comparable to rates mentioned in universal literature for primary total knee replacement operations.
Our case describes an unusual presentation of dermatomyositis
in a patient with ovarian carcinoma. Th e eruption appeared as a venous stasis like dermatitis. Th e temporal sequence of onset after chemotherapy administration suggested a possible drug-induced process. However, in the context of underlying ovarian carcinoma, a paraneoplastic process offered an alternative explanation for
dermatomyositis.
A 10 year old boy presented with fever, local swelling around the ankle with tenderness and antalgic gait, without any antecedent trauma or discharging sinus. He had no evidence of pulmonary tuberculosis. Laboratory tests revealed signs of inflammation with normal Mantoux test and chest X-ray. An irregular lytic lesion of the talus was seen on the x-ray of the affected part. Ziehl Nelson staining of the aspirated fluid revealed acid-fast bacilli. Material obtained after curettage and bone grafting confirmed the diagnosis of tuberculosis. He was treated conservatively with splintage and 4 drug anti-tubercular chemotherapy for 18 months, without any residual tenderness or foot deformity at the final follow-up. This case has been reported because of its rarity and atypical clinical presentation.
Coccidioidomycosis Meningitis in an Immunocompetent Worker: A Case Reportkomalicarol
While pulmonary coccidiomycosis infections in the endemic places are seen very often, especially among immunocompromised
people, CNS coccidiomycosis infection from environmental exposure in immunocompetent individual is not that common, but
when it happens it is fatal. Risk is very high in certain occupations in endemic regions and the diagnosis may be challenging
in absence of high clinical suspicions. Here, we report a case of
coccidiomycosis meningitis in an immunocompetent individual,
who went to Arizona for a week for his painting job. The purpose of
this paper is to prevent the occupation-related dreaded incidence of
fungal brain infection in endemic places with robust public health
measures.
Recently published papers have defined the clinical characteristics and overall outcomes of COVID-19 patients with the influence on the healthcare system. Especially, general surgeons are uniquely affected due to the broad range of procedures they perform, many of which are conducted routinely in the outpatient setting. This report aims to represent the clinical presentation and outcomes of elective surgical patients during the COVID-19 epidemic.
Extramedullary Hematopoiesis (EMH) is a compensatory mechanism in response to ineffective hematopoiesis or stress erythropoiesis, in sites other than the bone marrow. The EMH often presents as pseudotumors and may be confused with other benign or malignant tumors. The cause of EMH often
is an underlying hematopoietic disorder, and it may be adequately treated by treating the underlying cause. This will avoid unnecessary surgical interventions. Hence, knowledge of this entity is of utmost importance, and careful history, clinical examination and necessary investigations must be carried out
before deciding the course of therapy.
Dunbar syndrome or celiac artery compression syndrome was described for the first time by Harjola in 1963. It’s an infrequent clinical condition, with few criteria for diagnosis and with obscure pathophysiology. It is usually associated with an extrinsic compression upon the celiac axis near its takeoff from the aorta by fibrous diaphragmatic bands or sympathetic
neural fibers. This syndrome is as uncommon cause for upper abdominal angina. The classic sympthoms include vomiting associated to pos prandial pain, weight loss and soufle
in the epigastric region. Diagnosis is made by abdominal angiotomography, arteriography and magnetic resonance imaging. Surgical ligament release is indicated in case of severe
compression of the celiac trunk or in patient’s refractory to clinical treatment [1].

support. Such a perceived difference can be attributed to macroecological dissimilarities like the general well-being of the population and a more extent, its demographic structure.
The demographic characteristics, most notably the age structure and population density, has important implications in the spread of infectious diseases [2,3]. Like most of the infectious
diseases we have known so far, COVID-19 is seen more severely in groups like the elderly and those with comorbidities [4,5]. Robustness of the health system and abundance of resources might have helped the western countries to report proportionately a greater number of cases. However huge proportion of elderly population in these countries might have contributed to higher reporting of severe illness and deaths in these countries [6].

:Extraocular foreign bodies (EOFBs) are a common presentation to the emergency
department (ED). Given that inadequate management can result in severe complications including visual
impairment, ED clinicians may be overly cautious and often schedule patient reviews in the ED even
where it is unnecessary, placing a burden on hospital resources.
Occurrence of a mural thrombus in a diseased descending thoracic aorta (atherosclerotic or aneurysmal) is a well-known and commonly encountered vascular entity. However, thrombus formation in a normal appearing descending thoracic aorta (NADTA) is rarely reported in literature so far. We present an
unusual case report with a brief literature review of an aortic mural thrombus (AMT) in descending thoracic aorta in a young male. He presented at our center in the emergency department with acute onset abdominal pain and underwent a contrast enhanced CT scan that confirmed mural thrombus in
NADTA.
Two case reports: the first, 50-year-old male patient, A.M.B., who suffered from hemorrhoids (2003 to 2005). Moisturizers, medications and cauterization were not successful. The patient started treatment with auricular acupuncture and apex ear bloodletting. He also received measurement of the

Inflammatory fibroid polyp (IFP) is a rare benign lesion, originating from the submucosa in the gastrointestinal tract. It generally appears as an isolated benign lesion, rarely located at the level of the ileum. Its origin is controversial. Clinical presentation varies depending on its location; invagination and
obstruction are the most common indicative symptoms when the polyp is located at the level of the small intestine. We report the case of a 60-year old patient with abdominal pain, nausea and vomiting and a personal history of intermittent constipation. Radiological imaging objectified ileo-ileal invagination
completely obstructing the ileum light. Segmental resection of the obstructed ileal segment and terminalterminal anastomosis were performed. The final diagnosis of IFP was established using histological examination.

Capnography is a simple non-invasive monitoring technique that provides prompt and valuable information about patient’s ventilation, perfusion and metabolism. We are presenting a case of a 10 year-old boy
with a history of repaired Tetralogy of Fallot (TOF) undergoing a cardiac catheterization for recurrent left pulmonary artery (LPA) stenosis. A variable plateau capnogram waverform was detected and attributed
to ventilation-perfusion abnormalities from differential lung perfusion secondary to critical LPA stenosis. Capnogram can prove vital monitoring tool in detecting critical perfusion abnormalities.
The objective of this letter is to report recent advances on the diagnosis and therapy of the loin pain hematuria syndrome (LPHS). The patho-aetiological link of LPHS with symptomatic
nephroptosis (SN) is a novel discovery of the 21st century that was first preliminary reported in 2002 [1]. The link was confirmed in 2016 in an article that précised the exact pathoaetiology of LPHS; ischemic nephritis and sympathetic neuropathy of SN. It also advanced a 100% curative therapy for LPHS namely; the renal sympathetic denervation and nephropexy (RSD&N) surgery [2].

While the incorporation of pertuzumab to a chemotherapy and trastuzumab backbone (dual HER2
blockade) yielded a robust improvement in the outcomes of HER2-positive metastatic patients in the
CLEOPATRA study, in the adjuvant setting the same magnitude of benefit was not reproduced with the
addition of pertuzumab in the overall population of the APHINITY study, being the reasons for this discrepancy unknown so far. In the present manuscript, we discuss biological and clinical differences between metastatic and early-stage HER2-positive breast cancer that may potentially explain the different magnitudes of benefit observed with pertuzumab in the different disease settings.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

Introduction
Septic arthritis due to Candida species is a relatively rare infection typically seen in
patients with immunocompromising conditions. While C. albicans is the most commonly
encountered fungal species affecting humans, C. tropicalis has demonstrated increasing
resistance to anti-fungal medications leading to persistent and difficult to control infections
[1-15]. C. tropicalis is part of the normal human microbiota and is typically found on the skin
and mucosal surfaces of the body. It is the third most common Non-Candida albicans Candida
species (NCAC) isolated in clinical practice and primarily found in tropical regions, particularly
Asia and South America [1,8]. Few reports of Tropicalis septic arthritis in North America have
been published. We present a case report of a confirmed monoarticular C. tropicalis septic
arthritis in a pediatric patient with chemotherapy induced granulocytopenia the setting of
relapsed acute myeloid leukemia that lead to systemic candidiasis and patient mortality.
Case Report
Our patient is a 13-year-old Caucasian female who presented to our institution with
relapsed AML following bone marrow transplant with clinical signs and symptoms concerning
for graft vs. host disease. She had undergone previous chemotherapy treatment a little over a
month prior to her admission and had an indwelling catheter. The patient was scheduled for
intrathecal chemotherapy administration and was undergoing preoperative evaluation when
itwasnoticedshehadadiffuserashandendorsingfebrileepisodesaswell asdiffusejointpain.
Pre-operative labs showed of white count of 0.1 cells/mm3
as well as elevated inflammatory
markers. She was admitted to our Children’s hospital for further work up and evaluation. 2

Abstract
Background:Candidasepticarthritisisadebilitatingconditionaffectingjointfunction.Candidatropicalis,
an organism found in normal human flora is noted to be the third most common pathogenic yeast in the
elderly and immunosuppressed population. Infections are rare in the US, typically limited to the neonate
and elderly populations. Most infections occur in the south America and southeast Asian regions. In the
last 2 decades, Tropialis infection rates have risen impart due to antifungal resistance. We present a case
of Candida tropicalis infection in the knee of a 13-year-old female with a past history of relapsed Acute
Myeloid Leukemia following bone marrow transplant, pancytopenia and graft vs. host disease.
Methods: Patient case is thoroughly discussed, and a review of the current literature performed.
Result: Patient underwent multiple open irrigation and sharp debridement as well as arthroscopic
complete synovectomy in addition to medical management. Patient remained pancytopenic throughout
her course and eventually underwent granulocyte transfusion. During her course, she developed a relapse
of her AML and subsequently expired. Limited case reports exist in the literature, with no recent reports
of Candida arthritis in an adolescent in the United States.
Discussion: Candida septic arthritis remains a rare but potentially devastating condition, particularly in
the immunocompromised patient population. Candida arthritis should be high on the treating physician’s
suspicion in immunocompromised patients presenting with new onset joint pain. Aggressive medical
and surgical management is recommended, and current literature has shown a high cure rate with early
treatment.
