An appraisal form must directly correspond to an individual's job outline, describing the what, how, measurements, performance evidence, and timelines of their specific job functions. To be effective, a performance appraisal needs comprehensive measurement criteria and evidence to back it up. Simply rewriting an ineffective form is not enough - the entire performance management system, including the individual's job outline and measurements, needs to be reviewed.
