An explosion occurred at an olefins plant in Amuay, Venezuela on August 25, 2012 around 1:10 AM. Olefins were being produced on site and stored in spherical tanks. There was a leak from an olefins pump that was not properly addressed for days. Atmospheric conditions caused the wind to slow and change direction, preventing dispersion of the leaking vapors. This led to a build up of an ignitable vapor cloud. It is believed a vehicle ignited the cloud, causing an explosion that killed 51 people and injured many others. The explosion was the result of an unconfined vapor cloud explosion (UVCE) where the rapid combustion of the cloud generated a powerful pressure wave without confinement.
This document analyzes workers' compensation data from 2005-2007 on falls among roofers and carpenters in 38 states. It finds that falls from elevations for roofers cost on average $54 million annually, with average claims costing $106,000 each. For carpenters, falls from elevations cost approximately $93 million annually, with average claims costing over $97,000. An explosion occurred during routine maintenance on a water pipe at a Colombian power station, killing eight workers. The CSB has identified over 60 fatalities since 1990 due to explosions and fires from hot work activities like welding and cutting. Proper hot work safety programs include evaluating the work, requiring permits, monitoring the atmosphere for hazards, clearing the area of combustibles, providing fire watches, and training workers. An accident occurred when welders were not informed of explosive materials in a slurry tank and the tank atmosphere was not monitored prior to welding.
A $1.6 million settlement was reached in a boiler explosion case where a man suffered severe injuries. Boiler explosions and fires have injured many people in the last ten years due to lack of safety training. A 1999 explosion at a power plant killed 6 people and injured 30. Proper boiler safety requires management leadership, hazard prevention, education and training to prevent accidents. The top causes of boiler accidents are issues with maintenance, lockout procedures during startup, falls, carbon monoxide exposure, and fly ash hazards.

Upon passing a series of tests on one's head, including immersion in water, withstanding impacts from an eight pound steel ball dropped from 15 feet, exposure to acids and solvents, resistance to a propane torch, and sustaining 2,200 volts of electricity, one would receive a certificate stating their head meets ANSI safety standards and does not require wearing a hard hat on the jobsite. The tests subject the head to a range of temperatures from -20 to 140 degrees Fahrenheit. For those unsure if their head can pass the tests, wearing a hard hat is recommended for protection.

2) A safety culture focuses on eliminating the root causes of incidents rather than just reactions. It views safety as an integral part of business operations and empowers employees to take responsibility for safety.
3) Key elements of an effective safety culture include management commitment, well-defined safety policies and goals, employee training, hazard analysis, and recognizing employees for safe behaviors. With the right systems and commitment from all levels, any organization can achieve an incident-free work environment.
PSM Chemical Plant NEP UPDATE March 2011Bryan Haywood
This document summarizes inspection data from the OSHA Chemical PSM National Emphasis Program. It shows that 151 inspections were opened, with 96 resulting in citations. On average, inspections found 9 citations per inspection, with average penalties of $25,129 per inspection. The most common citations were for mechanical integrity, process safety information, and process hazard analysis requirements. Common violations involved procedures, documentation, hazard analysis and safety reviews. The data provides insights into emphasis areas and compliance issues under the OSHA PSM standard.
Over 1000 deaths have occurred since 2000 relating to ladder use. Falls are the largest risk factor. 91 of the 101 deaths in 2010 were due to falls, with distances ranging from under 6 feet to over 40 feet. Training employees on proper ladder setup and fall protection can prevent many accidents. Alternatives to ladders such as aerial lifts should be considered when practical to eliminate fall risks.

Note: A CDX account is associated with a specific person, not
a facility.
If a person at your facility has an account and they will be involved in
RMP submissions, they can use that account.
But, if the person with the existing CDX account will not be involved in
RMP submissions, then the person who will certify and/or prepare the
RMP will need a CDX account.

A: No. They have to designate an actual person (not a company) and a signature is
required.

A: The Certifying Official can be the owner, plant manager, or a senior manager at
the facility – they should be someone who can sign official documents.
Q: Where can I find the definition of “Certifying Official?”
A: Page 1 of the RMP*eSubmit Users’ Manual.

Q: How long does it take to get the Authorization Code once the ESA has
been submitted?
A: Once received, processing could take a few days to a week before you receive the
AuthCode.
Don’t wait to the last minute to send your ESA.
Q: We sent an ESA with incorrect information. Should we send another ESA?
A: No! Contact the Reporting Center (703-227-7650) for instructions.

A Correction should be used to report administrative or
other changes at your facility (e.g. changes in emergency
contact information, facility address, or changes in accident
history.) This does not require an update of your entire RMP.
Submitting a corrected RMP does not change your five-year
anniversary date. If your facility has not submitted an RMP
Resubmission by its anniversary date, you will not be able to
make an RMP Correction – you must submit an RMP
Resubmission in this instance. The Correction button enables
you to enter data for your facility and update the RMP as a
Correction.
A Resubmission is an update of all nine sections of your
RMP. Revising your previous RMP appropriately and
submitting the updated RMP Resubmission will re-set your
five-year anniversary date. The Resubmission enables you to
enter the data for your facility and update the RMP as a
Resubmission.
