What is RCA?
General Principles of RCA
Tracing a problem to its origin
Root cause Analysis process
RCA Template
RCA Tool (Tree Diagram)
RCA : RCA is a method of problem solving and tries
to identify the root causes of the problem or event
Different type of approaches of RCA:
Safety based RCA
Process based RCA
Production based RCA
Failure based RCA
System based RCA






RCA must be performed Systematically.
Problem will be having more than one root cause.
Purpose of identifying all solutions to the problem.
Analysis should be in a sequence of events.
Root causes identification is depends on the way in
which the problem is defined.
Fixing symptoms of the problem : Problem will occur
again and again
We need to follow RCA
 Determine what happened?
 Determine why it happened?
 What to do to reduce the likelihood that it will
happen again?
Basic types of causes
 Physical causes
 Human causes
 Organizational causes
The process will be in 5 steps
Step 1 : Define the problem
 What do you see happening?
 What are the specific symptoms?
Step 2: Collect Data
 What proof do you have that the problem exists?
 How long has the problem existed?
 What is the impact of the problem?
Step 3: Identify possible causal factors
 What sequence of events leads to the problem?
 What conditions allow the problem to occur?
 What other problems surround the occurrence of
the central problem?
Step 4: Identify the root causes
 Why the causal factor does exist?
 What is the real reason the problem occurred?
Step 5: Recommended and Implement Solutions
 What can you do to prevent the problem from
happening again?
 How will the solution be implemented?
 Who will be responsible for it?
 What are the risks of implementing the solution?
In this session, you gained insight on
