This document describes the Rapid Access to Consultative Expertise (RACE) program, a joint partnership aimed at improving care for patients with complex chronic conditions. RACE provides a telephone advice line allowing family practitioners rapid access to specialist expertise in areas such as cardiology, nephrology, and psychiatry. The program aims to simplify patient care, improve outcomes, reduce costs, and strengthen relationships between acute and primary care providers. An evaluation found the program reduced unnecessary referrals and emergency department visits while improving provider satisfaction. The document concludes that expanding the program province-wide could enhance the patient and provider experience while controlling health care costs.
