The number of coronavirus patients at a primary care clinic increased due to referrals from the main hospital. The main hospital stopped accepting coronavirus patients due to an inability to deal with large numbers of chronic patients, as there was no implementation of a disaster plan. Through a root cause analysis using the 5 whys technique, it was determined that poor management led to the lack of a disaster plan, which caused the issues at both the primary care clinic and main hospital in dealing with coronavirus patients.
