Normally during fetal development the gut rotates to position the duodenojejunal flexure on the left, but in this patient malrotation left the bowel susceptible to twisting around the superior mesenteric artery, known as midgut volvulus. The barium study and ultrasound were able to diagnose malrotation and volvulus without other tests.
ACHALASIA CARDIA: ENDOSCOPIC THERAPY (POEM)
POEM is a highly effective treatment for achalasia, providing long-term symptom relief in over 90% of patients. Studies have shown POEM to have similar efficacy to laparoscopic Heller myotomy with benefits including shorter procedure time, less pain, and shorter hospital stay. Subsequently inferior mesenteric vein was dissected and clipped and divided. Distal dissection proceeded just behind the superior rectal artery and after identification and preservation of the hypogastric nerves, upper rectum was mobilised. Division of bowel was done at upper rectum after giving adequate distal margin and end to end anastomosis was done using circular stapler.
Case report fatal pneumothorax after ERCP

Laparoscopic Anterior resection. After insertion of ports with patient in steep trendelenburg position Inferior mesenteric artery was identified and high ligation done with division of left colic artery and then medial to lateral dissection was done. Subsequently inferior mesenteric vein was dissected and clipped and divided. Distal dissection proceeded just behind the superior rectal artery and after identification and preservation of the hypogastric nerves, upper rectum was mobilised. Division of bowel was done at upper rectum after giving adequate distal margin and end to end anastomosis was done using circular stapler.

Minimally Invasive Esophagectomy
1) The authors reviewed outcomes of 104 consecutive minimally invasive esophagectomies (MIEs) performed between 1998-2007.
2) Surgical approaches included thoracoscopic/laparoscopic esophagectomy with cervical anastomosis (n=47), minimally invasive Ivor Lewis esophagectomy (n=51), and others.
3) Complications included anastomotic leak in 9.6% of patients and stricture in 26%. Mortality was 1.9% at 30 days and 2.9% in-hospital. Mean lymph nodes retrieved was 13.8.
Use of laparoscopy in the management of abdominal trauma a center experience
This study evaluated the use of laparoscopy in managing 65 patients with abdominal trauma at Zagazig University from 2011-2014. For patients who were hemodynamically stable, laparoscopy was considered for diagnosis and treatment. The results showed that laparoscopy avoided laparotomy in 81.5% of cases. Therapeutic laparoscopy was effective in repairing injuries like stomach penetrations, liver lacerations, diaphragmatic injuries, and splenic lacerations. The study demonstrates that laparoscopy can be performed safely and effectively in stable patients with abdominal trauma to reduce unnecessary laparotomies.

Ercp
ERCP is an endoscopic procedure that combines endoscopy and fluoroscopy to diagnose and treat issues in the duodenum, bile ducts, pancreatic duct, and gallbladder. During ERCP, an endoscope is inserted and a catheter is used to inject radiocontrast dye to identify any blockages. If needed, a wire can enlarge the opening to the bile duct to remove gallstones or perform other procedures like stent placement. The case demonstrates the successful treatment of necrotizing pancreatitis in a pediatric patient with minimally invasive endoscopic drainage.

Laparoscopic gastrectomies for cancer
This study compared short-term outcomes of laparoscopic versus open gastric resections for adenocarcinoma. The laparoscopic group had decreased blood loss and length of stay but increased operative time. There was no difference in oncologic outcomes like margin status or lymph node retrieval between groups. Laparoscopic and robotic D2 lymphadenectomies were performed. The study concluded laparoscopic gastrectomy is an oncologically adequate alternative to open surgery with potential short-term benefits.

PGx Analysis in VarSeq: A User’s Perspective
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data. Luckily, both approaches are supported with the VarSeq software platform. The genotyping method being used will also dictate what the scope of the tertiary analysis will be. For example, are your PGx reports a standalone pipeline or would your lab’s goal be to handle a dual-purpose workflow and report on PGx + Diagnostic findings.
