DEFINITION
โ€ข Inability to place back
(cover) the retracted
prepucial skin over the
glans is called as
paraphimosis.
โ€ข It causes ring like
constriction proximal to the
corona and prepuceal skin.
CAUSES
โ€ข after sexual intercourse
โ€ข iatrogenically after
urethral catheterisation.
CLINICALFEATURES
โ€ข As a result the glans will be
swollen
โ€ข Oedematous
โ€ข severe painand tenderness.
โ€ข Oftenglans will undergo
necrosis or becomes
gangrenous.
TREATMENT
โ€ข Manual reductionof
prepuceal skin is to be
tried.
โ€ข If not possible, initial dorsal
slit is made to relieve the
oedema and compression.
โ€ข Antibiotics and analgesics
โ€ข Circumcision is done after
3 weeks.
โ€ข Sedation and
hyaluronidase injection
in 10 ml saline into the
constriction ring
โ€ข multiple needle punctures
over the oedematous part,
reduces the oedema and
makes the paraphimosis to
get reduced. Later
circumcisionshould be
done.
