Aromatherapy uses essential oils from plants to alter mood and health when inhaled or applied topically. Massage involves manipulating muscles and tissues through various techniques including pressure. Physiotherapy aims to improve mobility and function through examination, evaluation, and physical intervention. Phytotherapy studies herbal medicines and plant extracts. Reflexology applies pressure to feet, hands, or ears corresponding to areas of the body. Acupuncture treats patients by inserting thin needles into specific acupuncture points.
ALTERNATIVE AND COMPLEMENTARY THERAPIES DURING INTRANATAL PERIODKanchan Mehra
Alternative and complementary therapies are increasingly popular during pregnancy and childbirth to help cope with symptoms and relieve pain. These include acupuncture, acupressure, herbal medicine, homeopathy, hypnotherapy, massage, aromatherapy, yoga, reflexology, therapeutic touch, osteopathy, and hydrotherapy. While many of these therapies can provide benefits, they must be used accurately and some carry risks, so it is important to consult medical professionals before trying any alternative or complementary therapies during pregnancy or labor.

Traditional Chinese Medicine originated in China 4000 years ago and is based on the concept of qi (vital energy) and the balance of yin and yang in the body. Acupuncture and acupressure involve stimulating points along meridian pathways to restore balance. Herbal medicine, massage, tai chi, and meditation are also components of Traditional Chinese Medicine.
Rheumatoid arthritis is a chronic autoimmune disorder that causes inflammation of the joints. Common causes include stress, genetics, and diet. Treatment involves reducing stress, gentle exercises, hot/cold therapies, massage, fasting and a yoga/meditation practice.
The document discusses alternative medicines such as acupuncture and aromatherapy. It summarizes various studies that have shown acupuncture can effectively reduce pain from osteoarthritis, migraines, and chronic back, neck, and shoulder pain, though its effectiveness for acute low back pain is unconfirmed. While aromatherapy has been shown to improve sleep and reduce stress, scientific evidence of its physical effects is still lacking. The conclusion is that although some diseases are exceptions, alternative medicines like acupuncture and aromatherapy can be as effective as conventional medicine for certain applications.

Naturopathy focuses on protection and non-toxic natural therapies with a holistic approach and avoids surgery and drugs. Herbal medicine involves using plants for medicinal purposes and studying such uses. Acupuncture stimulates specific points along the skin using thin needles, heat, pressure or lasers. Ventosa or cupping therapy creates local suction on the skin believed to mobilize blood flow and promote healing. Reflexology applies pressure to feet, hands or ears using specific techniques without oil or lotion. Acupressure also stimulates points but uses pressure instead of needles.
Alternative medicine is any practice that aims to achieve the healing effects of medicine, but which lacks biological plausibility and is untested, untestable or proven ineffective. Complementary medicine (CM), complementary and alternative medicine (CAM), integrated medicine or integrative medicine (IM), and holistic medicine are among many rebrandings of the same phenomenon. Alternative therapies share in common that they reside outside medical science, and rely on pseudoscience. Traditional practices become "alternative" when used outside their original settings without proper scientific explanation and evidence. Frequently used derogatory terms for the alternative are new-age or pseudo, with little distinction from quackery.

Alternative medicine exists in all cultures to some degree and terms such as traditional medicine, indigenous medicine or folk medicine etc. are used to describe such practices. These medicines date back hundred or even thousands of years depending on the country and culture concerned. There are more than 100 systems of alternative medicines still in practice all over the world. Every country, region or area has its own traditional system of health and medical cares such as for the Chinese it is acupuncture, for the French, magnetic healing; for the Germans, Heilpraxis; for the English, Herbalism; for India, Ayurveda with Siddha being widely practice in the southern part of the country; for Japan, Shiatsu etc. The most popular forms of alternative medicine are ayurveda, homeopathy, naturopathy, yoga, acupuncture, acupressure, magneto therapy, shiatsu, herbalism, meditation, aromatherapy, Bach Flower Remedies, chromo therapy, diet therapy, hydropathy and reiki. Alternative medicine can help during pregnancy and labour where western medicine might pose a risk to the unborn child (such as certain medications in conditions such as headache). It’s also much more gentle and non-invasive, and addresses many of the emotional issues western medicine generally ignores.
This document provides an overview of alternative systems of medicine. It discusses definitions and introduces some major alternative medicine systems practiced in India such as Ayurveda, Siddha Medicine, Unani, Homeopathy, Naturopathy and Yoga. For each system, it describes concepts of health and disease causation, principles, diagnostic methods, treatment approaches, commonly treated conditions, and important concepts like humors, doshas, materia medica etc. The document classification different alternative medicine categories and provides details on popular alternative treatment methods.

Medicines work in the body in different ways such as killing pathogens, preventing their reproduction, reducing inflammation and pain. People must be careful when taking medicines due to risks of side effects, addiction, resistance, and interactions.
The document summarizes some of the fundamental principles of naturopathy. It discusses 10 principles: (1) the body heals itself, (2) the main cause of disease is enervation or fatigue of the nervous system, (3) the deposit of metabolic waste products in the body is what causes disease, (4) acute disease is a remedial process by which the body heals itself, (5) food is a building material but does not increase vitality, (6) fasting provides an opportunity for the body to heal itself, (7) germs are found in diseased conditions but do not cause disease, (8) exercise maintains balance between nutrition and drainage, (9) external treatments only provide
This document discusses a study on the effectiveness of slow back massage on sleep quality among intensive care unit (ICU) patients. The study used a non-equivalent pre-test post-test control group design with 60 patients divided into a control group and experimental group. The experimental group received 10-12 minutes of slow back massage nightly for 3 consecutive nights while in the ICU, while the control group received normal night nursing care without massage. Sleep quality was assessed before and after using a standardized scale. The results showed that the massage therapy helped induce sleep and improve sleep quality among the ICU patients compared to the control group.
Naturopathy and yoga therapy are holistic systems that aim to treat illnesses and improve health. Naturopathy utilizes natural therapies like hydrotherapy, massage, mud therapy, and fasting to aid the body's natural healing abilities. Yoga therapy combines postures, breathing, meditation, and imagery to integrate the mind, body and spirit. Both approaches recognize the body's innate ability to heal itself and focus on preventing illness through healthy lifestyle practices.

Fire ant stings: pathophysiology and natural remedyKevin KF Ng
Fire ant stings cause local inflammation and pain through the action of venom components. In rare cases, they can cause systemic allergic reactions. Current topical treatments are limited because they target only a few inflammatory mediators. The document proposes that phytochemicals extracted from plants may provide a more complete treatment by inhibiting multiple mediators of inflammation. Experimental evidence shows that a phytochemical preparation reduced inflammation from fire ant stings better than untreated controls. The conclusion is that phytochemicals offer a safe, effective, and inexpensive way to update the treatment of fire ant stings.

Many patients experience stomach irritation or other unpleasant side effects from taking pain medication. Some have difficulty taking the medication in its commercially available form. Pharmacy compounding can provide alternate methods of delivery to make the process easier. Instead of a capsule or tablet, pain medications often can be compounded as dosage forms such as:
A topical gel, cream or spray form that can be applied directly to the site of the pain and absorbed through the skin.
A custom-flavored troche that dissolves under the tongue, a nasal spray, or a suppository.
Such dosage forms may bypass the gastrointestinal tract, providing optimal results with less GI irritation, and help patients who have difficulty swallowing pills, removing yet another source of aggravation.

Topical medications are drugs applied directly to the skin or mucous membranes and are intended to have a local effect. Some topical medications can also be absorbed into the bloodstream and cause systemic effects, especially if the skin is thin or damaged or exposure is prolonged. Common topical medications include ointments, creams, lotions, patches, and drops administered through various mucous membranes. Proper application techniques aim to deliver the medication safely and effectively.
This document discusses transdermal drug delivery systems (TDDS). It provides an introduction to TDDS, outlines potential advantages and disadvantages, and describes factors that influence drug permeation through the skin. These include physicochemical properties of drugs, biological factors, and formulation components. The document also discusses various strategies to enhance drug permeability, including chemical permeation enhancers, electrically assisted methods like iontophoresis and electroporation, and the basic components and types of TDDS. Evaluation methods for TDDS are also summarized.
This document discusses the history and generations of transdermal drug delivery systems (TDDS). It describes 1st generation TDDS which use liquid reservoirs or adhesive matrices to deliver small, lipophilic drugs. 2nd generation systems use techniques like chemical enhancers, heat, or iontophoresis to increase skin permeability for small molecules. Recent 3rd generation techniques disrupt the skin barrier more extensively through iontophoresis, thermal ablation, ultrasound, or microneedle arrays to deliver larger or hydrophilic drugs like peptides. Transdermal delivery offers benefits over oral or injection routes but continuing innovation is needed to expand the types of drugs that can be delivered through the skin.
The document provides information on palliative care, including:
- Palliative care aims to improve quality of life and relieve suffering for patients with life-limiting illnesses and their families.
- It focuses on pain management and other symptom relief without hastening or postponing death.
- Palliative care is appropriate at any stage of illness and can be provided alongside curative treatment.
This document provides an overview of transdermal drug delivery systems (TDDS). It defines TDDS as self-contained dosage forms that deliver drugs through the skin at controlled rates. It describes the layers of the skin and three routes of drug absorption. Factors affecting permeability are discussed like solubility, partition coefficient, and pH. It also describes permeation enhancers and the four main types of TDDS. The advantages of avoidance of presystemic metabolism and maintaining therapeutic drug levels are highlighted, along with limitations like only suitable for potent drugs.
Growing great schools: what has the most impact?Ofsted
This document summarizes findings from Ofsted about what has the most impact on growing great schools. According to Ofsted, strong leadership and culture have the greatest impact. Effective school leaders establish a culture of high aspirations where staff and students' voices shape development. Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

19. The story of Phytochemicals
Sheng Nong (about 4,000 years ago): tasted and classified 365 Chinese herbs.
Hippocrates (460 bc – 375 bc): used bark of white willow tree for fever and pain.
Sir John R. Vane (1927-2004): discovered how aspirin reduced fever and pain in 1971.
The word “phytochemical” was invented on April 25, 1994
20. Rationale of Use of Topical Phytochemicals for Palliative Care
Many patients cannot tolerate oral medications.
Most phytochemicals are metabolized in the gastro-intestinal tract and liver.
Most phytochemicals are soluble in oil.
Massage oil contains fatty acids (oleic acid, linoleic acid, linolenic acid) which are easily
absorbed through the skin..
Therefore phytochemicals in massage oil may be absorbed through skin bypassing the portal
circulation to enhance their beneficial effects.
Phytochemicals act on multiple targets and they act in synergy.
