• MSRA, Tool Box Talk, JSA are conducted and a Permit to Work has been obtained.
• The safe working load is not exceeded, and canopy is fitted.
• Tools and equipment are secured appropriately. Tool lanyards must be utilized.
• Whilst the cradle is in use no other work must be carried out directly above and under the cradle including any worker that manages the cradle wires.
• All person/s operating in Cradles must be trained and competent and must always be supervised.
• If malfunctions occur, work is stopped immediately and the malfunctions reported.
• An appropriate rescue from heights plan must be in place.
• When weather conditions deteriorate, wind, rain etc. worked is stopped immediately.
• Areas below where Cradles are working must be properly barricaded and warning sign posted to prevent entry of unauthorized personnel (Exclusion Zone).
• The barricades/ticker tape and warning signs should be stored in the cradle when not in use and put in place by cradle operator before use.
• An independent life line rope/wire and rope grabbing device must be installed.
• All person/s in the Cradle must wear a harness, with the lanyard attached to the independent safety line at all times.
• All cradles are to be fitted with a remote control extension cord (which is attached to the main control box).
• Electrical Distribution Panels must be wired correctly, with Residual Current Device (RCD)/ Earth Leakage Circuit Breakers (ELCB) installed and must have lockable cover.
• Cradles can only be entered/exited from ground floor or purpose built landing platforms.

- 4. Specific Precautions Approval must be obtained from the contract controller by the “Responsible Person” prior to starting any excavation. Working areas shall wherever practical, be fenced or barricaded off and appropriate warning notices erected. This applies particularly to jobs being done above and below ground level or on operating work sites. Such guards, fencing notices, lights etc. shall be provided and maintained by the Contractor or his Sub-Contractors. 4
- 5. Specific Precautions All such precautions shall meet the requirements of the relevant Statutory Authority and the Contract controller. Such OHS precautions must be checked by the Contractor’s “Responsible Person” or his Deputy before leaving the site each day. 5

Specific Precautions • Sufficient fire extinguishers appropriate to fire hazards that apply to particular jobs shall be supplied and maintained by the Contractor or his Sub-Contractors. • Bulldozer blades, front-end loader baskets, forklifts and pile driving hammers shall be lowered to the ground when the equipment is left unattended. • The lighting of fires in drums etc. for heating purposes is not permitted. 6
- 7. Specific Precautions Welding screens are to be provided by the Contractor or their Sub Contractors and the “Responsible Person’ shall ensure that screens are used during arc welding operations. Suitable eye protection must be worn by the operator and persons within the screened area. 7
- 8. Specific Precautions • All fixed grinders shall be fitted with appropriate guards, tool rests and vision shields. All portable grinders shall be fitted with appropriate guards. • During sandblasting and/or working in designated areas all operators shall wear appropriate approved breathing apparatus and protective equipment. 8
- 9. Specific Precautions • When painting in a confined space, forced or ducted ventilation shall be provided and operators are to wear appropriate breathing equipment. If inflammable vapors are present, the entry to a confined space shall be roped off and “No Smoking”, “No Naked Light” notices erected. • An entry permit must be obtained from the Contract Controller by the “Responsible Person” prior to working in any confined space. 9
- 10. Specific Precautions Contractors and Sub-Contractors using any fixed or mobile equipment requiring licenses to operate must be currently certificated by the relevant authority. 10
