1) This document contains a 14 question Mediterranean diet score questionnaire that asks about consumption of foods like olive oil, vegetables, fruits, red meats, dairy, wine, legumes, fish, nuts and use of sauces made with tomato and olive oil. 2) Participants are asked to select boxes indicating their consumption frequency of these foods to calculate a total Mediterranean diet score. 3) A higher score indicates greater adherence to the traditional Mediterranean diet which is characterized by high intake of plant-based foods, olive oil as the primary fat source and moderate alcohol consumption, specifically wine.
