The dates that are to be covered
The name and details of the doctor or pharmacist who provides the
medical certificate (including provider number)
The signature of the doctor or pharmacist
10. CAN MEDICAL CERTIFICATES BE BACKDATED?
No, although the employer should consider the circumstances in each case.
If an employee informs a doctor that they were unable to attend work on a
particular day, the medical certificate must be dated according to the date of
examination, not the date the worker conveyed their inability to attend work.
The medical certificate can only state that on the day(s) the absenteeism
occurred, the patient stated they were suffering from the described condition.
The doctor is free to state if signs or symptoms still persist at the time of
examination.
