Teacher Portfolio Submission Rubric

(4) Distinguished – (3) Proficient – (2) Basic – (1) Unsatisfactory
I. Domain I: Planning and Preparation
Rating Area of Focus
LessonPlans: Teachers must submit 2 sets from bi-weekly submissions of what they believe were their most
effective and efficient set of lesson plans during the quarter and submit a short paragraph for each set explaining
how the lesson plans were effective as it pertains to student mastery of a particular subject or content area. -
Student Assessment: Teachers are to provide 1 copy of a summative, formative and informal assessment used to
measure student mastery of a content area or a particular Common Core Standard throughout the quarter. Please
label each submitted assessment.-
MAPS Scores: If applicable at the time of submission, please submit all student MAPS scores showing data of
progress of growth or regression in the content area. Please print those scores out. -
Interventions: Please submit a list of interventions (may be bulleted) that you have used to develop student
understanding and mastery of a standard or unit in which students struggled to master/How did you differentiate
the instruction -

Parent Communication: Teachers must submit evidence of any parent communication including but not limited
to phone logs, parent letters, newsletters, etc.
Student Attendance: Teachers must report students who have been excessively tardy or have five or more

unexcused absences. (See student handbook for clarity)
Classroom Management: Teachers must submit a copy of all discipline referrals or written documentation
regarding discipline incidents that have occurred within the classroom or on any part of the campus throughout the
quarter.
Professional Development: Provide data from any Professional Development that you have received throughout
the quarter. This includes conferences, workshops and internal meetings. Examples include but are not limited to
the following: Agendas, Programs, Power Point Presentations, etc.
Staff Meetings: Provide agendas from all staff meetings you have attended throughout the quarter at your building
Leadership: Teachers should provide a written overview of leadership in which you have demonstrated at your
building or in the district during the quarter. ***Note: This could be a bulleted list.

  • 1. McIntosh School District, South Dakota Teacher Portfolio Submission Rubric 2017 - 2018 (4) Distinguished – (3) Proficient – (2) Basic – (1) Unsatisfactory I. Domain I: Planning and Preparation Rating Area of Focus LessonPlans: Teachers must submit 2 sets from bi-weekly submissions of what they believe were their most effective and efficient set of lesson plans during the quarter and submit a short paragraph for each set explaining how the lesson plans were effective as it pertains to student mastery of a particular subject or content area. - Student Assessment: Teachers are to provide 1 copy of a summative, formative and informal assessment used to measure student mastery of a content area or a particular Common Core Standard throughout the quarter. Please label each submitted assessment.- MAPS Scores: If applicable at the time of submission, please submit all student MAPS scores showing data of progress of growth or regression in the content area. Please print those scores out. - Interventions: Please submit a list of interventions (may be bulleted) that you have used to develop student understanding and mastery of a standard or unit in which students struggled to master/How did you differentiate the instruction - Summary Scorefor Rating in Domain I Overall Rating for Domain I II. Domain IV: Professional Responsibilities Rating Area of Focus Parent Communication: Teachers must submit evidence of any parent communication including but not limited to phone logs, parent letters, newsletters, etc. Student Attendance: Teachers must report students who have been excessively tardy or have five or more
  • 2. McIntosh School District, South Dakota Teacher Portfolio Submission Rubric 2017 - 2018 unexcused absences. (See student handbook for clarity) Classroom Management: Teachers must submit a copy of all discipline referrals or written documentation regarding discipline incidents that have occurred within the classroom or on any part of the campus throughout the quarter. Professional Development: Provide data from any Professional Development that you have received throughout the quarter. This includes conferences, workshops and internal meetings. Examples include but are not limited to the following: Agendas, Programs, Power Point Presentations, etc. Staff Meetings: Provide agendas from all staff meetings you have attended throughout the quarter at your building Leadership: Teachers should provide a written overview of leadership in which you have demonstrated at your building or in the district during the quarter. ***Note: This could be a bulleted list.
