Additionally there is a lack of information in regards to how Kangaroo Care helps fathers develop
paternal feelings, a sense of paternal competence and ability to handle the unexpected situation.
Therefore, this study will focus on fathers' and the benefits that Kangaroo Care provides for them.
The research questions this study hopes to answer are as follows: (a) does kangaroo care help fathers
develop paternal feelings, (b) will a fathers opportunity for being close to their infant and facilitating
attainment benefit their paternal role in the neonatal intensive care unit, (c) will educational support
and parenting techniques allow fathers to feel more connected with their infant? Ideally the
interviews and focus groups will give neonatal intensive care unit staff a better understanding of the
fathers experience with kangaroo care and what changes can be made to provide educational
support, emotional support, and how to assist them in their paternal role. It should also be

Miracle Baby Research Paper
For my extra credit project, I decided to shine a little light on the situation of my baby brothers birth.
He was an answer to prayers and ultimately a blessing on our family. He didn't arrive as scheduled

He was born as a five–pound baby and we believed that god saved my mom because she was
considered to be an "older mom", which means she falls in the category of being a high–risk mom,
making it harder to give birth. Even though she was considered high risk, this five–pound baby was
destined to be born. He was born at Moanalua Hospital and his initial due date to be born was
February 6th, 2017, but instead, he came 2 months early. Since he was born that early it put him in

The Treatment Of The Surgical Setting
The surgical setting is very different from the generalized floors that I have become accustomed to.
The exposure I had today included observing the operating room setting, the PACU, and the
exchange of patient care to a generalized or outpatient floor. In preparation for my clinical day I had
finished the module readings relating to peri–operative care which I found useful because I had a
base knowledge of information to build upon. I selected one patient to gather information from
which included his surgery, patient information, and care he received in the different stages of the
surgical process.
Patient Data
Patient JC is a 63–year–old male, he is Caucasian and is not Hispanic. He presented to the
emergency room on Monday April 4th after noticing pain, swelling, and a "blue tint" to his left fifth
digit. He has a history of hyperlipidemia, GERD, cardiac disorder, osteoarthritis, hematuria, prostate
cancer (resolved), and hammer syndrome. He has dentures and partial vision loss in both eyes. JC is
a construction worker. The patient's current problem with his left fifth digit is believed to come from
his history of hypothenar hammer syndrome. Hypothenar hammer syndrome is caused by repeated
use of the palm of the hand, such as when you use a hammer to push or grind objects. This can result
in damage of certain blood vessels in hand, especially the ulnar artery. This artery goes through the
hypothenar area of the palm and supplies blood to the fingers, damage to the ulnar

Health Care Reflection
The patient I cared for today, was admitted into the NICU on October 17th. He was experiencing
respiratory distress, has breathed (aspirated) meconium into the lungs around the time of birth. The
neonate had stabilized, so the nurse and I were preparing for the family to discharge from the
hospital. The immediate needs that were verbalized by the patient were hunger cries and soiled
diaper/ angry cries. Immediate needs from the family were instruction on how much formula to feed
the baby and circumcision care once discharged home. During my clinical experience, the patient
experienced pain from the circumcision process. The health care team members alleviated the
physical suffering by administrating lidocaine pain medication and giving the baby sugar water. I
think the only improvement that I would make to these interventions would be to administer the
lidocaine earlier so that it could be more effective in the patient's pain management. Long term
needs for the patient were speech therapy in order to improve the baby's feeding technique, infection
prevention, and teaching measures for the parents. The needs could be met through referral to a
speech therapist upon discharge from the hospital, teaching family members proper handwashing,
and educating parents on subject matters.

In the article ¨Moral Distress And Avoidance Behavior In Nurses Working In Critical Care And
Noncritical Care Units," by Mary Jo De Villers, she states ¨it has been predicted that there will be a
shortage of 500,000 nurses in the United States by 2025.¨ This counts for the United States alone.
Many articles state that more and more research is being done on moral distress relating to nurses all
around. This leaves researchers wondering, could this be the main reason for the shortage of nurses?
Moral distress comes into play dramatically with specific nurses such as pediatric intensive care,
neonatal intensive care, and intensive care unit. Many of the nurses have to deal with life–or–death
situations on a daily basis working in those units. Dealing with that dilemma daily makes it harder
for this medical staff to go back to the hospital. All nurses deal with some kind of moral distress, but
critical care nurses suffer from a more intense level of distress. There are many factors that play into
the shortage of nurses. In Anthony Tuckett's study, he discovers reasons why nurses are leaving the
profession. In attempt to understand, he interviewed nurses and asked for their opinion on the lack
of physicians assistant. Overall, Tuckett concluded that the primary reasons for this are no manager
support, co–worker support, and the fact that business policies are put before caring for the patient

Feeding difficulties often persist past NICU discharge with an incidence ranging from 19 to 80
percent (Shaker, 2013; Sundseth, 2013). The average daily cost for NICU care often exceeds $3,500
per infant and a delay in establishing full oral feedings further increases the cost of care (Murakas,
2008). Delayed discharge also places infants at risk for hospital related morbidity, leads to
prolonged maternal child separation and increases parental stress (Gennattasio, Perri, Baranek, &
Rohan, 2015).
Preterm infants face a number of challenges in attaining full oral feedings including physiologic
instability, neurologic immaturity and behavioral state disorganization (Gennattasio, Perri, Baranek,
& Rohan, 2015). The time in which preterm infants are learning to feed is characterized by rapid
motor and neurodevelopment in which every feeding experience plays a role (Shaker, 2013).
Physiologic stress during feeding may encourage sensory–motor pathways in the brain, leading to
maladaptive feeding behaviors, learned feeding refusals and long–term feeding aversions (Shaker,
2013). Traditional, volume–driven feeding practices fail to account for an infant's hunger cues, the
physiologic stress caused by the challenge of oral feeding and the quality of an individual feeding

The inability to sleep is a source of anxiety and stress for patients in the intensive care unit (ICU);
this inability can lead to cardiorespiratory disturbances, immune system dysfunction, impaired
wound healing, hormonal and metabolic imbalances, cognitive changes, and delirium. Through the
use of quiet times and nursing actions that encourage sleep, or, sleep hygiene bundles, nurses can
assist in improving sleep quality in the ICU. These bundles include noise and light reduction,
scheduling routine procedures outside of reserved sleep times, the use of earplugs and eye masks,
and the grouping of nursing care activities to minimize sleep interruptions. The aim of this proposal,
therefore, will be to implement a nurse–driven, sleep hygiene protocol, in order to improve patient
care by promoting quality sleep in ICU patients. Barriers to implementation include staff resistance

During this time, progression of the delivery halted for several hours because the cervix would not
dilate more than 3cm (10cm is the magic number for delivery). It was discovered that the Foley
catheter was the culprit and had to be adjusted. The other problem was the anesthesiologist who was
assigned to insert the epidural got it to go, but it slipped out and became ineffective. He tried to re–
insert it for almost two hours to no avail, all while my wife was experiencing labor pains with no
pain killer. There was one other anesthesiologist in the hospital, but he was detained in a surgery and

Scope of Quality and Safety in NICU practice Patient's in the NICU are considered to be very tiny
and fragile and immunocompromised due to their immature organ systems which can lead to many

Observing A Right Hip Fracture Repair
Operating Room Observation Experience This past week, I was able to observe a right hip fracture
repair. The patient had broken his hip and was undergoing surgery to place screws and plates into his
hip. I was able to see the whole process through the preoperative, intraoperative and postoperative
stages. The preoperative stage is when the patient comes to the OR and is being prepped for the
surgery. The patient is verified by the nurse, who needs to check patient identification, patient
records and make sure is calm for the surgery. Before this particular surgery, the anesthesiologist
came down before the surgery to administer a block to the patient. The block is to dull the nerve
ending so when the patients comes out of surgery he will be in less pain. The consent form is signed
by two people, the patient and the doctor performing the surgery. The consent form is the
responsibility of the doctor, the nurse just verifies that both the doctor and the patient signed the
form. It is very common for a patient to be anxious right before a major surgery. My patient didn't
seem anxious but just wanted it to be over and done with. One of the most important part of a
nurse's job is to keep the patient calm and relaxed. That can be accomplished just by talking to them

Intraoperative Room
During this weeks clinical rotation I was given the opportunity to observe how nurses provide care
in the post anesthesia care unit also known as the PACU. The PACU is a place where patients go
after receiving surgery and anesthesia. The PACU provides patients with intense observation and
care until the patient is stable enough to be discharged home or to another unit in the hospital. Last

when we had down time Liz went over the ECG strips with us. She also planned out a scavenger
hunt to locate commonly used IV solutions as well has other commonly used equipment. My
classmates and I mixed up some of the solutions so Liz explained to us the difference and why it
was important to know when and when not to use specific solutions. Liz also created scenarios that
nurses frequently come across during patient care in the PACU. We were able to critically think of
interventions we would use if we were the nurses caring for this patient. Liz then let us know if we
was right or wrong and she then provided us with the rationale as to why we would perform that

Strengths And Weaknesses Of Parent Staff Communication
The main weakness of this study is the fact that the study is exploratory. There was no control group
to compare scores of the PROMIS or PSS–NICU. This study just shows how scores went up with
length of NICU stay. However, this does provide another instrument that can be used to assess
parent health. Parent–Staff Communication Wigert, Delienmark, & Bry (2013) explored the
strengths and weaknesses of staff communication in the NICU. They gave 270 parents a 16–
question survey with answers using a Likert scale. A common statement from most parents was that
they had received information from nurses and doctors and when they did the information was easy
to understand. Some parents felt the staff did not understand their situation. There was a significant
difference at the .05 level between parent communication with nurses versus doctors (Wigert, Blom,
& Bry, 2013). There was also some poor information giving between nurses, showing a lack of

six past decades, surgical procedures have become more extensive and complicated and thus require

2013). The PACU of today is an intensive care specialty that provides care to wide range surgical
patients. Many of these patients have more than one chronic condition, such as chronic obstructive

Getting Out Of A Sound Of Squealing Brakes
The familiar sound of squealing brakes signaled the beginning of the best part of my night. Getting
out of the house, without a sound, was the tricky part. If I timed it just right, I could use the car's
engine to my advantage and cover the creak from the door. Once I was outside, I was in the clear. I
scampered over to the XL white Suburban and gingerly climbed into the passenger seat. My mom
sat inside, listening to soft jazz, decompressing from a long day's work as a nurse in the ICU. As
with most nights, she feigned annoyance before excitedly walking me through the events at the
hospital. Some nights were filled with triumphant saves, others drowned in an untimely loss. The
stories varied, yet a central idea remained: take care of the person instead of the patient. In my
youth, her stories registered at face value. All too soon, I would see the truth in her words firsthand.
It started with a dull ache through my left ear immediately following diving practice. A number of
weeks passed, allowing the ache to progress to a stabbing pain. My family grew concerned with my
discomfort, culminating in a visit to an otolaryngologist. It took Dr. C only a moment to confirm

Ultimately, I noticed that the parents were having a hard time by not being able to hold their baby
and bond. NICU care can interfere with the process of bonding between the family and by not
allowing them to touch or hold there baby creates a rift in the interaction process (Tessier et al.,
2009). As time progressed while I was at clinical the baby was becoming more self sufficient and
the suggestion of kangaroo care came about when mom was trying to breastfeed as well as when
they had to supplement with bottle feedings. When the subject of kangaroo care was brought up
neither mom nor dad had heard of it before. During my time with the family I felt that they were at
risk for impaired parenting related to prolong separation from the parent as evidence by long
amounts of time with no physical contact with the baby. It was clear that both mom and dad were
emotionally stressed by not being able to provide basic needs for their newborn, and physical
contact was restricted to small amounts of touching through the isolette. It was also clear that mom
and dad had still not developed any comfort with independently holding or feeding the baby. I
observed that they wanted to become more comfortable and have more physical interaction with
their child. Overall they wanted to become more integrated into the physical care of their child.

Nursing Reflection
Throughout my time on the Mother Baby Care Unit, I have faced many instances in which I have
been able to reflect on both my patients and the care that I was providing them. One situation in
particular that I found myself critically reflecting upon involved a new mother who was feeling
slightly stressed about being discharged as her newborn son would not be going home with her. For
confidentiality reasons, this patient will be given the pseudonym of Kayla for the remainder of this
reflection. Kayla and I met on the morning of her discharge while she was finishing packing up her
belongings. At this time, I completed her final set of vital signs and began to talk with her about
going home. As I spoke about this topic, I noticed that Kayla was becoming slightly stressed as her
voice sounded a little shaky and she was increasingly restless. I then proceeded to interpret why she
could be feeling this way to which I realized that as Kayla has yet to be more than one hospital zone
away from her son and the Neonatal Intensive Care Unit (NICU), she was likely feeling quite
uneasy about going home. Like any mother with a sick child, she did not want to leave her son's side
during this tough time and by being discharged, Kayla may have felt that a part of her would no

Working in a neonatal intensive care unit has been a learning experience filled with a roller coaster
of emotions. So many emotions have passed over me through different situations that I have
encountered in my three years of working in this type of unit. When I first began my journey in
becoming a NICU nurse I was exposed to many things that a registered nurse may not encounter
unless they are in an intensive care unit setting. For example, I needed to learn to work with
ventilators, medication drips, everyday use of an Ambu–bag and all this with premature babies
nonetheless. I have learned that bio–reactions are an automatic signal in our brains which

three personal experiences of my personal bio–reactions. I want to recall a particular incident that

was no longer working with my preceptor and I was on my own caring for these tiny humans. I have

An Evaluation Of A Laparoscopic Hysterectomy Essay
During my rotation in the operating room at Community medical center, I observed the
preoperative, intraoperative, and postoperative care for a patient who underwent a laparoscopic
hysterectomy. I believe that an appropriate preoperative plan of care for this patient would have
included a full physical exam and an interview for patient history, a pelvic exam to look over and
understand the nature of the patient's complications, blood testing including a CBC and WBC to
note any signs of infection or contraindications for the procedure, and a urine test to rule out any
urinary tract infections or pregnancy. It would be important to interview the patient and ask
questions to determine how the patient is feeling about their procedure and to better assist with any
anxiety or pain they may be dealing with preoperatively. It is important to consult with the patient
well before the procedure to ensure that she knows to refrain from smoking for at least 8 weeks
before the procedure because this reduces the risks of complications such as infections, issues with
blood pressure, heart rate, blood flow, and respirations when under anesthesia, and promoting
overall health and risks associated with smoking after the procedure. (ASAHQ) It is also important
to educate the patient to consume no food or drinks after midnight the night before the scheduled

Session Three Goal– Work through possible comparison of babies, progression and regression while
in the NICU. Group members are struggling with the many facets of having a NICU baby. This
session will deal with some of the very common issues. Parents of NICU babies are often worried
about how slowly their child may be progressing in comparison to other babies. This session the
group will work through some of those progressions and regressions and help the parents to cope
with this. Parents are sitting in the NICU for hours, watching other babies do things that their baby
is not doing. However, this session will help show the parents that they are all doing this. Every
parent in the group has likely been the parent on either end of that frustration/joy. Regression can be
hard for parents and does happen often during a NICU stay. Group members may show some
reluctant to talk about their child's set–backs. They may be discouraged by them. Parents have a
strong desire to know when their child will be discharged from the hospital. This can cause them to
become upset when that does not happen (Pepler et. Al, 2012). The counselor should take note of
what group members seem more down than usual or less likely to share during this time. These
group members could have less positive news to share with the group. The counselor should remind

completed many IB and AP courses throughout my high school career. I have taken AP Biology, AP

and will complete IB HL 2 (similar to Calculus 3), AP Psychology, AP Chemistry, and AP Statistics.

Researching a medical topic and then presenting it is very exciting to me. The Honors Day was also
informative about the seminar style classes and the thought provoking discussions that the classes
are going to provide. A CSU student once said to the group tour I was on that, "You can make a big
school feel small but you can't make a small school feel big." I am trying to accomplish that with
CSU. I am in the Biomedical Sciences program, which is a vigorous major, but I also want my other
courses to be challenging as well. The Biomedical Science program is a small program and the
Honors Program is also a small program. Seminars sound like a way to delve into the curriculum

The Post Anesthesia Care Unit At Miami Children's Hospital
A Harmonious Exemplification
On the post anesthesia care unit at Miami Children's Hospital, students and nurses alike witnessed a
harmonious exemplification of the six competencies of Quality and Safety Education for Nurses,
more commonly known as QSEN. The post anesthesia care unit, PACU, was off to a slow start just
before teamwork between the preoperative unit, also called PREOP, the operating room, and PACU
were put to the test. A 4 year–old, already extubated, female patient had come to the unit from the
operating room, more commonly referred to as the OR. The OR nurse proceeded to inform the
receiving PACU nurse that the patient just had an arterial line placed in the femoral artery in order to
later undergo apheresis for cancer treatment. Upon performing an assessment, the nurse noticed that
the patient's pressure dressing was saturated with blood due to a hemorrhage. A team consisting of
PACU nurses, the anesthesiologist, the OR nurse and students rushed to the bedside to assist in
restraining the squirming patient in order to keep her flat and still. Ultimately, the dressing was
replaced a total of four times before the hemorrhage ceased, the patient received one unit of packed
red blood cells, and had a new intravenous line, or IV, started because the previous site had
developed a reaction to the Demorol administered in the OR. To prevent further distress for the
patient, the anesthesiologist, surgeon, and primary care physician collectively decided to

Career Overview In the early stages of my life I wanted to become everything when I was older. It
started at wanted to be a teacher, police officer and then a firefighter I was young and didn't know
what I wanted to be. Because I liked all these occupations I concluded that I just wanted a job that
involved me helping others. As I grew older and based on my experiences I know that I want to be
in the nursing field. I come to believe that I want to become a Nurse Practitioner and work in the
Neonatal Intensive Care Unit. My love for babies is unexplainable, and knowing the fact that I can
have the chance to help babies become healthy touches my heart. Babies are being born at earlier
than 24 weeks' gestation and are living and growing naturally. Working as a doctor or nurse in the
neonatal intensive care unit(NICU) can be very stressful and challenging. After researching this
occupation, I feel that this is the perfect job for me because I have a passion for babies and helping
others. My desire to become a nurse became stronger when I got pregnant with my first child. The
things a mother must go through to make sure her child is safe and healthy are what inspired me. I
had so many complications during my pregnancy, which eventually caused be have my daughter
