The Benefits of Kangaroo Care for Fathers with an Infant in the Neonatal Intensive Care Unit
(NICU): Educational, Emotional and Paternal Role Support.
Introduction
Take a second to imagine you are a father, who has an infant admitted to the neonatal intensive care
unit (NICU) for the first time. How are you feeling? Afraid? Confused, even? If your baby is
admitted to the NICU, your first question probably will be: What is this place? With equipment
intended for infants and medical staff specially qualified in newborn care, the NICU is an intensive
care unit created for sick newborns who require specialized treatment. A common example for
parent's ... Show more content on Helpwriting.net ...
Additionally there is a lack of information in regards to how Kangaroo Care helps fathers develop
paternal feelings, a sense of paternal competence and ability to handle the unexpected situation.
Therefore, this study will focus on fathers' and the benefits that Kangaroo Care provides for them.
The research questions this study hopes to answer are as follows: (a) does kangaroo care help fathers
develop paternal feelings, (b) will a fathers opportunity for being close to their infant and facilitating
attainment benefit their paternal role in the neonatal intensive care unit, (c) will educational support
and parenting techniques allow fathers to feel more connected with their infant? Ideally the
interviews and focus groups will give neonatal intensive care unit staff a better understanding of the
fathers experience with kangaroo care and what changes can be made to provide educational
support, emotional support, and how to assist them in their paternal role. It should also be
informative for all medical staff involved in working with infants and their caregivers.
The purpose of this case study to is to examine the research problem of father's involvement in the
neonatal intensive care process and how providing kangaroo care will benefit them. At this stage of
research, kangaroo care will be generally defined as prolonged and continuous skin–to–skin contact
between the

Miracle Baby Research Paper
For my extra credit project, I decided to shine a little light on the situation of my baby brothers birth.
He was an answer to prayers and ultimately a blessing on our family. He didn't arrive as scheduled
or when we thought he was. In addition, nothing went as planned but all I knew is that everything
was eventually going to fall into place. Moreover, there was nothing really normal about my
brothers birth or his situation, but in my eyes, that is what makes him a miracle baby to me.
Before my baby brother was born, I was the youngest in my family. My sister and I were the only
kids, we are five years apart, she was born in 1994 and I was born in 1999. We were inseparable
growing up because it was just the two of us and we only had each other. ... Show more content on

He was born as a five–pound baby and we believed that god saved my mom because she was
considered to be an "older mom", which means she falls in the category of being a high–risk mom,
making it harder to give birth. Even though she was considered high risk, this five–pound baby was
destined to be born. He was born at Moanalua Hospital and his initial due date to be born was
February 6th, 2017, but instead, he came 2 months early. Since he was born that early it put him in
the category of a preemie, which means when a baby is born before the optimal birth weeks to be
born. When my mom then gave birth the Neonatal Intensive Care Unit (NICU) nurses came to take
him. Since he was so early she couldn't hold him, he had to be taken and hooked up to machines. In
order to save his life he needed to be rushed there, "it was the hardest thing to do" my mom said
because all she wanted to do was hold him but she knew that his life and his breathing was more
important. She knew that if she let him go now she would have a lifetime to be able to hold him and
love him unconditionally. My mom then got housed in The "Mother Baby" unit where she and I
resided until she healed enough to be able to be sent

The Treatment Of The Surgical Setting
The surgical setting is very different from the generalized floors that I have become accustomed to.
The exposure I had today included observing the operating room setting, the PACU, and the
exchange of patient care to a generalized or outpatient floor. In preparation for my clinical day I had
finished the module readings relating to peri–operative care which I found useful because I had a
base knowledge of information to build upon. I selected one patient to gather information from
which included his surgery, patient information, and care he received in the different stages of the
surgical process.
Patient Data
Patient JC is a 63–year–old male, he is Caucasian and is not Hispanic. He presented to the
emergency room on Monday April 4th after noticing pain, swelling, and a "blue tint" to his left fifth
digit. He has a history of hyperlipidemia, GERD, cardiac disorder, osteoarthritis, hematuria, prostate
cancer (resolved), and hammer syndrome. He has dentures and partial vision loss in both eyes. JC is
a construction worker. The patient's current problem with his left fifth digit is believed to come from
his history of hypothenar hammer syndrome. Hypothenar hammer syndrome is caused by repeated
use of the palm of the hand, such as when you use a hammer to push or grind objects. This can result
in damage of certain blood vessels in hand, especially the ulnar artery. This artery goes through the
hypothenar area of the palm and supplies blood to the fingers, damage to the ulnar

Health Care Reflection
The patient I cared for today, was admitted into the NICU on October 17th. He was experiencing
respiratory distress, has breathed (aspirated) meconium into the lungs around the time of birth. The
neonate had stabilized, so the nurse and I were preparing for the family to discharge from the
hospital. The immediate needs that were verbalized by the patient were hunger cries and soiled
diaper/ angry cries. Immediate needs from the family were instruction on how much formula to feed
the baby and circumcision care once discharged home. During my clinical experience, the patient
experienced pain from the circumcision process. The health care team members alleviated the
physical suffering by administrating lidocaine pain medication and giving the baby sugar water. I
think the only improvement that I would make to these interventions would be to administer the
lidocaine earlier so that it could be more effective in the patient's pain management. Long term
needs for the patient were speech therapy in order to improve the baby's feeding technique, infection
prevention, and teaching measures for the parents. The needs could be met through referral to a
speech therapist upon discharge from the hospital, teaching family members proper handwashing,
and educating parents on subject matters.
The healthcare team interacted with the patient and family in an excellent fashion. They utilized the
teach back method when educating parents about infant care, used compassionate care, and used

Moral Distress And Avoidance Behavior
In the article ¨Moral Distress And Avoidance Behavior In Nurses Working In Critical Care And
Noncritical Care Units," by Mary Jo De Villers, she states ¨it has been predicted that there will be a
shortage of 500,000 nurses in the United States by 2025.¨ This counts for the United States alone.
Many articles state that more and more research is being done on moral distress relating to nurses all
around. This leaves researchers wondering, could this be the main reason for the shortage of nurses?
Moral distress comes into play dramatically with specific nurses such as pediatric intensive care,
neonatal intensive care, and intensive care unit. Many of the nurses have to deal with life–or–death
situations on a daily basis working in those units. Dealing with that dilemma daily makes it harder
for this medical staff to go back to the hospital. All nurses deal with some kind of moral distress, but
critical care nurses suffer from a more intense level of distress. There are many factors that play into
the shortage of nurses. In Anthony Tuckett's study, he discovers reasons why nurses are leaving the
profession. In attempt to understand, he interviewed nurses and asked for their opinion on the lack
of physicians assistant. Overall, Tuckett concluded that the primary reasons for this are no manager
support, co–worker support, and the fact that business policies are put before caring for the patient
(364). New nurses feel belittled when entering the profession and old nurses feel as

Improving Feeding Outcomes Of An Infant Driven Feeding...
Improving Feeding Outcomes in the NICU:
Establishing an Infant–Driven Feeding Pathway
Laura Pabich BSN, RN
The University of Iowa College of Nursing Improving Feeding Outcomes in the NICU:
Establishing an Infant–Driven Feeding Pathway
The most common reason preterm infants face prolonged hospitalization in a Neonatal Intensive
Care Unit (NICU) is a delay in acquiring full oral feedings (Shaker, 2013). The purpose of this
project is to improve feeding outcomes for preterm infants at Blank Children's Hospital's (BCH)
NICU in Des Moines, Iowa. Traditionally, feeding practices in the NICU have followed a
physician–led, volume–driven model based on an infant's weight and gestational age. This model
defines a preterm infant's ... Show more content on Helpwriting.net ...
Feeding difficulties often persist past NICU discharge with an incidence ranging from 19 to 80
percent (Shaker, 2013; Sundseth, 2013). The average daily cost for NICU care often exceeds $3,500
per infant and a delay in establishing full oral feedings further increases the cost of care (Murakas,
2008). Delayed discharge also places infants at risk for hospital related morbidity, leads to
prolonged maternal child separation and increases parental stress (Gennattasio, Perri, Baranek, &
Rohan, 2015).
Preterm infants face a number of challenges in attaining full oral feedings including physiologic
instability, neurologic immaturity and behavioral state disorganization (Gennattasio, Perri, Baranek,
& Rohan, 2015). The time in which preterm infants are learning to feed is characterized by rapid
motor and neurodevelopment in which every feeding experience plays a role (Shaker, 2013).
Physiologic stress during feeding may encourage sensory–motor pathways in the brain, leading to
maladaptive feeding behaviors, learned feeding refusals and long–term feeding aversions (Shaker,
2013). Traditional, volume–driven feeding practices fail to account for an infant's hunger cues, the
physiologic stress caused by the challenge of oral feeding and the quality of an individual feeding
(Newland, Weems L'Huillier, & Petrey, 2013). According to Sundseth Ross and Browne (2013),
feeding practices must move from

Creative Writing: The Birth Of My Daughter
The Birth of my Daughter In the early morning of January 5th 2016, I awoke to my wife of six
months, Scarlett, complaining of abdominal pain. I thought it was more pregnancy pains that she
had experienced for the previous five months. Then, she told me that she believed they 89were
contractions and she had been timing their intervals and we needed to get to the hospital. My initial
thoughts were frantic; cs it's much too soon, it's two months early. After my initial worries were
addressed in my mind, I began to run the logistics of getting to the hospital. We were in Straelen, a
small village in northwest Germany with no reliable transportation, and Scarlett certaintly could not
make the bike ride to the next town with a hospital. I went outside and began asking strangers to
take me and my wife over to Geldern. No one would oblige my ... Show more content on

We spoke with her attending doctor and found that she had developed sepsis while in the womb, her
lungs were underdeveloped, and she had an extra thumb, all of which were similar to my birth. We
wound up staying at the hospital for thirty–three days tending to Svanja, learning her cries while she
got stronger and the sepsis faded away. We prepared to go back home, we had our apartment as
prepared as possible. The hospital called a free taxi for us and we put Svanja in a carseat for the first
time. She felt so fragile I was hesitant to tighten the straps. After we got back to our apartment we
slept in shifts to take care of her until it was time to go back to America. All in all, it was one of the
most stressful times of my life, but also one of the happiest. Holding my infant daughter and
realizing how fragile she is, and how it is my responsibility for her well being and protection had a
profound impact on me and the way I think. It is what has inspired me to go to college so I can

Essay On Beneficence In Nursing
Introduction
Ethical decisions and issues in neonatal nursing deem conflicting amongst families and the
healthcare team since it is unknown if an infant born severely premature, between 22–24 weeks in
gestation at 450g, if survival is imminent. Many advancements in treatment, technology, and
healthcare perceive feasible, but when a premature infant's life is compromised, due to severe
prematurity, finding the best possible solutions and treatment options may put the healthcare team,
caring for the infant, in a conflicting situation. Survival rates amongst premature newborns and
severely ill infants have skyrocketed due to the increased advancement and development in nursing,
medicine, and treatment plans. Finding the best possible solutions ... Show more content on

Nurse Driven, Sleep Hygiene Protocol, A Source Of Anxiety...
The inability to sleep is a source of anxiety and stress for patients in the intensive care unit (ICU);
this inability can lead to cardiorespiratory disturbances, immune system dysfunction, impaired
wound healing, hormonal and metabolic imbalances, cognitive changes, and delirium. Through the
use of quiet times and nursing actions that encourage sleep, or, sleep hygiene bundles, nurses can
assist in improving sleep quality in the ICU. These bundles include noise and light reduction,
scheduling routine procedures outside of reserved sleep times, the use of earplugs and eye masks,
and the grouping of nursing care activities to minimize sleep interruptions. The aim of this proposal,
therefore, will be to implement a nurse–driven, sleep hygiene protocol, in order to improve patient
care by promoting quality sleep in ICU patients. Barriers to implementation include staff resistance
to the concept, concerns regarding patient care during sleep times and difficulties in coordinating
care with other units. Studies used to gather information for this proposal were published between
the years of 2011 and 2016 and were found in several databases: Academic Search Complete,
Medline, and OVID Journals.

It Is Delivery Day?
It is delivery day, even though having a child was not originally in the cards for us. Our family was
relegated to my wife and me, and our families who raised us. For me, however, my family had all
spread out across the country, and I have some, including a brother, in Europe so to me, family was
just the two of us. After trying to get pregnant for more than a year, with the aid of a fertility
specialist and fertility medication, we had all but given up, thinking, "This is it, this is what a family
is for us: just us two." Then on day we saw it, it was actually there – a plus sign! We tried not to get
too excited about it, which is impossible, because we know false positives can happen. We
scheduled the appointment, and it was confirmed by ... Show more content on Helpwriting.net ...
During this time, progression of the delivery halted for several hours because the cervix would not
dilate more than 3cm (10cm is the magic number for delivery). It was discovered that the Foley
catheter was the culprit and had to be adjusted. The other problem was the anesthesiologist who was
assigned to insert the epidural got it to go, but it slipped out and became ineffective. He tried to re–
insert it for almost two hours to no avail, all while my wife was experiencing labor pains with no
pain killer. There was one other anesthesiologist in the hospital, but he was detained in a surgery and
was not able to help us for a while. To add to that, the uninterested nurse told us she was not able to
administer any other pain killer while the anesthesiologist was trying to get the epidural to go in.
When he finally did arrive, he asked the nurse why she had not given any pain killer to my wife. She
responded with, "I didn't think I was allowed to." Then he got the epidural in on his first try!
Frustrating relief!
Finally, after all the complications, were over (or so we thought), here comes my first baby! With
family and friends surrounding my wife and the doctor (seven of us total, as I recall), our baby's
head came out. But wait, the cord is around her neck. I asked the doctor, "How dangerous is that?
Why are you pushing her back in?" The doctor had to push her back in a little to relieve tension and
provide enough slack to cut the cord. "Oh well," I thought to

Risk And Implement Falls Prevention Measures
More Staff is Not the Solution to Decrease Patient Falls Falls in an acute care setting lead the list of
injury related deaths and deaths in the elderly. "A fall is defined as any event which patients are
found on the floor (observed or unobserved) or an unplanned lowering of the patient to the floor by
staff or visitors" (Kalisch, Tschannen, and Lee, 2012, p. 6). Medicare and Medicaid changes in 2008
list falls as one of the 10 hospital acquired conditions for which hospitals will no longer be
reimbursed because falls are considered preventable conditions. Joint Commission accredited
hospitals are required to assess for falls risk and implement falls prevention measures. Central to
nursing ethics is the principle of nonmaleficence ... Show more content on Helpwriting.net ...
Patients being admitted to hospitals today have multiple co–morbidities and are on a number of
medications making them a high risk for falls. Having an awareness of a patients diagnoses,
pertinent history and current medications is key in establishing an appropriate plan of care. Staffing
shortages can lead to omission of tasks such as a complete admission history and falls risk
assessment. As with any assignment, regardless of the amount of patients one is caring for,
prioritization needs to be utilized when completing tasks and making care decisions. "Further work
must be done to assist nurses in completing necessary tasks...which may or may not mean additional
staff members" (Kalisch, Tschannen & Lee, 2012, p. 11). Developing strategies such as
computerized reminders and checklists are necessary to ensure complete and appropriate nursing
care is delivered. (Kalisch, Tschannen & Lee, 2012, p. 11) The lack of proper education on
identification of falls risk and falls prevention measures impact patient falls. Staff turnover and the
use of temporary staff contribute to miscommunication or lack of communication on processes such
as fall prevention measures. Experience levels vary on any given unit. Educational needs
assessments should be routine and include temporary and new staff. Authors Manojlovich, Sidani,
Covell, and Antonakos (2011) define nurse dose as "the level of nurses required to provide patient

Future Neonatal Nurses
Love Future Neonatal Nurses
Goals today keep the tiny humans; big journeys begin with small steps. However, the important
thing is to be able at any moment to sacrifice and serve the ones that need it the most. It is not how
much we do, but how much love we put in the doing. These individuals are the lucky stars that these
cutting – edge facilities exist, having a premature baby is getting one of God's miracles in the
middle of their creation, one of the wonderful moments in our lives are allowing our tiny patients to
leave their footprint on our heart. Neonatal have involve light–years since they first open their

("Rasmussen College"). Most neonatal nurses work in a typical hospital environment – usually in
the neonatal intensive care unit (NICU) or maternity ward. ("Rasmussen College"). However, not all
neonatal nurses work in such intense settings. Some care for healthy babies in mother–baby
nurseries, and others work in clinics or home healthcare to provide follow–up care for infants.
("Rasmussen College"). Neonatal nurse will have a variety of job duties throughout the day. They
may do anything from resuscitating infants and administering medication to helping a new mom get
started breastfeeding. ("Rasmussen College"). Caring for sick babies and offering support to their
families is rewarding in itself. However, a neonatal nursing career offers benefits beyond the
fulfilling job of patient care. One reason why you should consider a career in neonatal nursing is that
it will give you an opportunity to help newborn babies. Premature or underweight babies need
monitoring and special care all the time, having this field of profession is rewarding if you love
babies. Nonetheless, you have to undergo training since babies often face health risks that require
experienced care. ("Rasmussen College"). If you want to pursue a career in neonatal nursing, you
need to put in mind that your responsibilities are far beyond taking care of the child. Rather, you
have an obligation to provide information; guidance and support to parents of the infants under your
care take more than love of infants to succeed in this

Quality And Safety Of Neonatal Care
Quality and Safety in Neonatal Care in Nursing Practice Infants within the neonatal intensive care
unit (NICU) are one of the most vulnerable patient population in the hospital. The quality and safety
of these patients are at top priority because of their immune systems have not fully developed yet
causing them to be at the highest risk for infections. According to the Institute of Medicine (IOM,
2001), quality is defined as "the degree to which health services for individuals and populations
increase the likelihood of desired health outcomes and are consistent with current professional
knowledge" (Masters, 2005). The Quality and Safety Education for Nurses (QSEN) defines safety
as, "the minimization of risk of harm to patients and ... Show more content on Helpwriting.net ...
Scope of Quality and Safety in NICU practice Patient's in the NICU are considered to be very tiny
and fragile and immunocompromised due to their immature organ systems which can lead to many
dangerous medical problems. Patricia W. Stone states "maintaining a safe environment reflects a
level of compassion and vigilance for patient welfare that is as important as any other aspect of
competent health care" (Stone, 2008). The patient's safety should always be a top concerned for a
nurse because in a health care facility the purpose is to heal the patient and get them on the road to
recovery. Nurses must learn from the errors of the past and use their knowledge to improve the
quality of nursing to the patients to ensure if errors do happen again that the use of evidence–based
practices are put into place to improve their outcomes. According to Higher Quality of Care and
Patient Safety, "Registered Nurses (RNs) are instrumental in achieving multiple care goals,
including promoting infant health and clinical stability, maintaining the integrity and cleanliness of
central catheters, and preparing families for their role in infant care and successful transition it
home" (Lake, 2016). Nurses are reasonable for the education of the families of the patients to ensure
that the best quality of care for the patient is maintained outside the hospital to ensure the best
medical outcome for the infants.

Observing A Right Hip Fracture Repair
Operating Room Observation Experience This past week, I was able to observe a right hip fracture
repair. The patient had broken his hip and was undergoing surgery to place screws and plates into his
hip. I was able to see the whole process through the preoperative, intraoperative and postoperative
stages. The preoperative stage is when the patient comes to the OR and is being prepped for the
surgery. The patient is verified by the nurse, who needs to check patient identification, patient
records and make sure is calm for the surgery. Before this particular surgery, the anesthesiologist
came down before the surgery to administer a block to the patient. The block is to dull the nerve
ending so when the patients comes out of surgery he will be in less pain. The consent form is signed
by two people, the patient and the doctor performing the surgery. The consent form is the
responsibility of the doctor, the nurse just verifies that both the doctor and the patient signed the
form. It is very common for a patient to be anxious right before a major surgery. My patient didn't
seem anxious but just wanted it to be over and done with. One of the most important part of a
nurse's job is to keep the patient calm and relaxed. That can be accomplished just by talking to them
and reassuring them. Keeping the patient company will also ease their nerves. The nurse makes sure
the patient is calm and relaxed right before surgery. It is also important for the holding area nurse to
make sure

Budget Analysis
Budget Management Analysis for the Post Anesthesia Care Unit The budget management process is
not an easy task. It is time consuming and difficult. There are some ways to overcome the challenge
of managing budgets within forecasts. One way is to budget and report beyond the ledger. Data
should be looked at beyond the company's financial system. This will allow the company to create
more accurate forecasts. Next the budget software should be user–friendly. If it is easy to use, the
happier the employees are to use and understand it. Thirdly, the budget should have the options to
incorporate a flexible financial model. Managers should be able to input information on how their

budget of $1,800. This is a shared budget for pre–op and PACU. The actual amount spent in the
current month was $2,102. The explanation given for the $302 unfavorable variance was the need
for additional thermometers. There were multiple broken and non–working thermometers in both
areas so this was an uncontrollable expense. The prior year's budget $1,500. Equipment
Management For the current month, the budget for equipment management was for $600. The actual
cast for equipment management was $330. This had a favorable variance of $270. In the PACU,
there is little equipment to manage. Each bay has a monitor that displays cardiac rhythm, heart rate,
blood pressure, pulse oximetry, and respirations. The majority of equipment comes from other
departments. The monitoring or maintenance done on the monitors or IV pumps are done remotely.
The previous year's budget was $500. Printing and Copying There are one full–size copier and one
copier for approximately 12 computers on wheels and two desktops. There are more desktops in the
surgery department in which can print to the copier or printer in the PACU. The budget is $350 for

During this weeks clinical rotation I was given the opportunity to observe how nurses provide care
in the post anesthesia care unit also known as the PACU. The PACU is a place where patients go

was important to know when and when not to use specific solutions. Liz also created scenarios that
nurses frequently come across during patient care in the PACU. We were able to critically think of
interventions we would use if we were the nurses caring for this patient. Liz then let us know if we

Disadvantages Of Nicu Nursing
The nursing profession has come a long way since Florence Nightingale's pioneering of the first
structured nursing education programs in the hospitals of 19th century Britain. Furthermore, the
profession has grown with on–going research and education; nurses continue to receive
professional, standardized education and provide holistic care for each patient that they encounter.
Additionally, a specialized branch of nurses, the Neonatal Intensive Care Unit (NICU) nurses are
committed to the care of the smallest and most vulnerable of patients: newborn and premature
infants. These highly–trained NICU nurses care for babies suffering from critical health conditions,
and they function as each baby's comforter, primary caregiver, and advocate. Moreover, Neonatal

Firstly, NICU nurses spend the most time administering direct care to their patients, and they
consistently provide 24 hours a day, 7 days a week care for newborn and premature babies.
Moreover, NICU nurses, in addition to providing physical care to the infants such as feeding and
cleaning, help to comfort these babies and provide emotional support in the absence of their parents.
NICU nurses are also able to provide continuity of care for a child that can last from the time the
child is born up until he or she turns 2 years old. Furthermore, NICU nurses care for babies in the
hospital's neonatal wards that include Levels II to IV NICU wards, and which are based on the
severity of an infant's condition. Levels II and III NICU wards deal with slightly less critical to more
advanced health issues while the Level IV ward is reserved for extremely critical conditions.
Secondly, NICU nurses are able to perform certain, advanced procedures related to the care of their
patients. Additionally, NICU nurses can attend births that are potentially life–threatening to the
unborn child,

Strengths And Weaknesses Of Parent Staff Communication
The main weakness of this study is the fact that the study is exploratory. There was no control group
to compare scores of the PROMIS or PSS–NICU. This study just shows how scores went up with
length of NICU stay. However, this does provide another instrument that can be used to assess
parent health. Parent–Staff Communication Wigert, Delienmark, & Bry (2013) explored the
strengths and weaknesses of staff communication in the NICU. They gave 270 parents a 16–
question survey with answers using a Likert scale. A common statement from most parents was that
they had received information from nurses and doctors and when they did the information was easy
to understand. Some parents felt the staff did not understand their situation. There was a significant
difference at the .05 level between parent communication with nurses versus doctors (Wigert, Blom,
& Bry, 2013). There was also some poor information giving between nurses, showing a lack of
communication between nurses. Doctors and nurses were continuously seen as professional. This
study had a decent size sample and a consistent survey for all parents to take. There was a look at

A. Sentinel Event
This sentinel event involves child abduction from the surgical unit of Nightingale Community
Hospital on Thursday, September 14, 2014 at approximately 1230hrs. The patient, a three–year old
female, arrived accompanied by her mother, for an outpatient surgical procedure at 0800hrs and
proceeded to registration where all currently required documentation was completed and signed by
the mother; this included the authorization forms for the surgery. After registration, the patient and

Specifically, the pre–op nurse who acquired the parental contact information, or upon being
informed that the mother would be leaving the facility did not document it in the patient chart or
pass it along to the O.R. nurse. 3) Surgeon: Was directly involved in the events leading up to the
sentinel event. The surgeon was responsible for all activities taking place in the surgical suite and
directly related to the surgery of the pediatric patient. The surgery was completed safely and
successfully; however, the surgeon had relevant information in the patient chart at his office yet did
not share this information with the hospital. He also did not supply an appropriate or accurate
H&P that would have included custodial status for the pediatric patient to the hospital. The
surgeon is greatly concerned in the events that lead to the sentinel event and wants to ensure that his
patients will be cared for and safe at Nightingale Community Hospital. 4) O.R. Nurse: Was directly
involved in the events leading up to the sentinel event. The O.R. nurse is responsible for assisting
the surgeon in the surgical suite and providing continuity of care throughout the surgical procedure
from pre–op to post–op. The surgery was completed safely and successfully and the patient was
handed over to PACU for recovery appropriately; however, the O.R. nurse did not verify that all

Research Paper On Professional Nursing
Professional Nursing: Emerging and Growing Through Research
Nursing as a profession involves multidisciplinary and holistic approaches to integrate the essence
of scientific practices into compassionate and humanistic approaches in rendering care to patients. It
is an art that reflects an expression of science, emotions, power and creativity to deliver the so–
called "tailored" delivery of patient care. From the time of Florence Nightingale up to the present,
countless developments and improvements in the field of nursing evolved. This is the outcome of
thorough research activities that influenced the modernization of this noble profession. Nursing
research is critical to the practice of professional nursing, it is a continuous process that entails

The information is disseminated to nurses to be applied and incorporated into practice to be a better
advocate for patients and to provide optimal nursing care (Tingen, Burnett, Murchison, & Zhu,
2013).
Defining Fundamental Pattern of Nursing Knowledge
As a nursing professional, several aspects of knowledge are involved in the application of care. The
fundamental pattern of nursing knowledge starts with science or empirical evidences governed by
factual and explicit information and acceptable knowledge, such as the procedures and guidelines of
nursing practice. It is derived from scientific methodology that supports the intellectual explanation
of health and illness phenomena and the processes involve in the cycle of human life. (Carper,
1978). The second pattern represents the esthetic part of nursing knowledge which is also known as
the art of nursing. The concept that is represented in this pattern is the expressive nature of delivery
of care rather than descriptive and objective in nature.

Hospital Observation
The hospital lobby was filled with the smell of hand sanitizer. I remember feeling the deep
anticipation of seeing my little sister for the first time. The night before I remember overhearing my
aunt on the phone, saying it was happening. Instantly, I figured out that it meant my mom was
having the baby. Full of excitement, I woke up early the next day, rushed down the stairs, and woke
my dad up. My first question was what is her name, but he said mom wanted to tell us all together
when we went to the hospital in an hour. In response to this, I whined for ten minutes till he gave up
and told me that it had to be our little secret. Her name was Narayani. We quickly drove to the
hospital, where we spent three minutes getting visitor badges.
We entered the elevator next. The elevator was as long and narrow as a prison confinement. Slowly
and steadily, it glided up to the 5th floor. The inside of the elevator smelled like fresh hand sanitizer.
My sisters and I had a million questions we wanted to ask but at that time all of us were silent in
awe. Noticing our reaction, dad looked at us and smiled as comfort. The doors opened and I leaped

wondered if something was wrong with my baby sister. So when we arrived to the NICU check in
center, I asked a nurse if my sister was okay. She had warm brown eyes, a gentle smile on her face
and auburn hair that fell past her shoulders. She chuckled and told me that my beautiful baby sister

Notes On Time Management Through Delegation
Time Management Through Delegation
A personal goal for this project is time management through delegation. The Plan–Do–Check–Act
cycle (PDCA) is a tool that will be used to develop a plan for quality improvement and achieve the
goals that would be beneficial for the nurse and patient care outcomes. The personal goals are to
discharge all patients with finished documentation according to the certified post anesthesia nurse
(CPAN) rules by using the process of delegation to obtain ancillary help. DELEGATION Delegation
is the cornerstone to being a nurse leader, it facilitates the work effort of the staff to accomplish the
mission and goals of the healthcare organization. The optimum administration of a unit is contingent
on how well the staff functions as a team. One study stated the most intricate skills a nurse leader
can have is delegation; proper judgment and knowledge is required to effectively transfer the
authority, responsibility, and accountability to a subordinate. Understanding the concept of
delegation is essential in conveying an assignment, this is the final accountability for the care of a
patient. When delegation is not effective, valuable resources are substandard (Weydt, 2010).
TIME MANAGEMENT As a critical care nurse working in a post anesthesia care unit the most
valued resources is time management. Resources are the means for a healthcare system to deliver
services; time is a valuable resource

NICU Nurse Registered Nurses have a lot of responsibilities and other jobs, the career I chose is a
NICU Nurse. They work in the rooms with babies to monitor any complications, but mostly
premature babies. I chose this career because I love babies and I would love to be the reason why a
baby got better. This career will be a great fit for my life goals because I've always wanted to be a
nurse and I love working with babies so why not put both the things I love into one. NICU Nurses

The Recovery Room
From the birth of the recovery room in the 1940s to the postanesthesia care unit (PACU) of the 21th
century, the look and function of this unit have been in a constant state of evolution. Throughout the
six past decades, surgical procedures have become more extensive and complicated and thus require
more specially prepared nursing staff and equipment for the care of the patient (Odom–Forren,
2013). The PACU of today is an intensive care specialty that provides care to wide range surgical
patients. Many of these patients have more than one chronic condition, such as chronic obstructive
pulmonary disease, diabetes mellitus, chronic pain, and chronic heart problems. In order to provide
safe patient care, the PACU nurse needs to develop the ability to blend expert clinical knowledge
that is based on experience, education, and collegial sharing with caring practices that comes from
within and from being a nurse (Odom–Forren, 2013).
For many years, critical care experience was a must have requirement prior to working in the PACU.
The PACU was staffed with experienced intensive care unit (ICU) nurses with different backgrounds
and a solid nursing knowledge base. Nowadays, many of these nurses are retiring or taking different
paths in their careers forcing the PACU managers to revise hiring requirements and open the PACU
doors to non–ICU and new graduate nurses. In order to assist the new graduate nurses transition to
their professional roles in the critical care setting, the Christ

Patient Care Technicians ( Pct )
Patient care technicians (PCT's), formally known as nursing assistants, are the backbone to any
nursing department. They create rapport with the patients and family members, as well as the
nursing and medical staff. Some of the tasks PCT's are responsible for include: obtaining and
recording vital signs, collecting and labeling specimens, blood glucose specimen, and obtaining
electrocardiograms (ECG). All these tasks are important and critical in an emergency. PCT's
designated to work in medical surgical floors may not remember the steps for obtaining a good ECG
reading. Like the saying goes: if you don't use it, you lose it. The most common reason ECG's are

misinterpretation of the ECG. If the electrodes are placed incorrectly, the ECG may read as ST
changes, electrical axis, location of bundle branch blocks and location of infarcts (Riddle, 2008,
para. 5). During the ECG, a total of 10 electrodes are applied to the patient. Six electrodes (V1–V6)
are placed on the anterior chest in the proper anatomical landmarks, and these leads must be placed
precisely for an accurate ECG interpretation. Electrodes V1 and V2 are placed on the fourth
intercostal space with V1 on the right and V2 on the left (Riddle, 2008). Electrodes V3–V6 are
placed on the left chest wall on the fifth intercostal space in the following order: V3 to the right of
V4, V4 at the midclavicular line, V5 to the right of V6, V6 to the midaxillary line (Riddle, 2008).
The remaining four electrodes are placed on both upper and lower extremities without touching the
chest.

Abstract
As a clinical requirement for my Adult 1: Medical–Surgical course, I had the opportunity to observe
a patient in the Operating Room and in the Post Anesthesia Unit of Advocate Good Samaritan
Hospital. The procedure that I observed was a left total knee replacement. The patient needed this
surgery because she was experiencing osteoarthritis, and this surgery could alleviate her pain and
discomfort. I was with the patient from the end of her stay in the pre–operative holding area to the
Operating Room, and then to the Post Anesthesia Care Unit. This paper will include background

X–rays can be used for the diagnosis of osteoarthritis; the images would reveal damage and other
changes related to osteoarthritis. With osteoarthritis of the knees, the patient would experience
progressively increasing pain, stiffness, and they would have a decrease in daily functions. Patients
experiencing these symptoms can be eligible for a total knee replacement. The surgical procedure. A
total knee replacement is a surgical procedure where the diseased knee joint is completely replaced
by artificial materials that resemble the original knee joint. The orthopedic surgeon removed the end
of the femur and the end of the tibia by using metal pieces and sawing the bone, to ensure that he
removes the right amount of bone. The end of the femur bone is replaced with metal and the end of
the tibia bone is replaced with plastic and metal. A plastic piece was added under the patella because
the surface under the patella was damaged as well. These artificial materials, called prosthesis, have
smooth surfaces so when they rub against each other, it does not cause damage and is pain–free. The
purpose of this surgery is to remove the diseased portions of the joint and replace it with artificial
materials to prevent further deterioration and eliminate pain, stiffness, and decreases in function that
were caused by the osteoarthritis.

i have a passion for helping sick people and i don 't want little babies to die. I love seeing healthy
newborn babies but it is heartbreaking having so many premi's babies and they need to get better.
Not many people could handle this job not only the emotional work but the environment that these
babies are put in. Not every baby that comes through the NICU would come out to be healthy and

body and how to prevent things like that. The People in this career would be small but thats my
opinion now that i researched how many actually work in this field i was shocked about the strong
hearts these people have to not get emotionally connected to their job. Mostly maybe around 1000
NICU nurse in Arkansas. The job is very demanding and intense for the right people to work in this
field. It requires a love to help sick babies and a special person to separate themselves from the work
field. The employment rate by 2020 would be up to 26% growth. The NICU is going to be
expanding across the U.S to meet the needs of premature babies. The job outlook for this career is
very good because they always need people to help in this career. Like i 've said it takes a certain
person to have that job. The jobs that are found in this career is basically in every hospital in the
U.S. It depends on what the care needed for the newborn that chooses the quality the hospital has.
Most places have more advanced technology than others. Like at Arkansas Children 's Hospital they
have a great Neonatal Intensive Unit where i hope to be working someday. But every hospital has a
labor and delivery and would hope they would be prepared for a premature baby also. May not have

Getting Out Of A Sound Of Squealing Brakes
The familiar sound of squealing brakes signaled the beginning of the best part of my night. Getting
out of the house, without a sound, was the tricky part. If I timed it just right, I could use the car's
engine to my advantage and cover the creak from the door. Once I was outside, I was in the clear. I
scampered over to the XL white Suburban and gingerly climbed into the passenger seat. My mom
sat inside, listening to soft jazz, decompressing from a long day's work as a nurse in the ICU. As
with most nights, she feigned annoyance before excitedly walking me through the events at the
hospital. Some nights were filled with triumphant saves, others drowned in an untimely loss. The
stories varied, yet a central idea remained: take care of the person instead of the patient. In my
youth, her stories registered at face value. All too soon, I would see the truth in her words firsthand.
It started with a dull ache through my left ear immediately following diving practice. A number of
weeks passed, allowing the ache to progress to a stabbing pain. My family grew concerned with my
discomfort, culminating in a visit to an otolaryngologist. It took Dr. C only a moment to confirm
what my pediatrician had suspected. I had a tumor. Any other physician would have referred to it as
a cholesteatoma. Dr. C never used that word with me. There was no use for medical jargon; he knew
it was not what I needed to hear. Instead, Dr. C understood how scary the word tumor sounded and
recognized I

My Rotation For Obstetrics
My rotation for obstetrics was in Brighton, CO at Platte Valley Medical Center. I was fortunate
enough to experience multiple avenues of this field including labor and delivery and postpartum, but
ultimately enjoyed spending most of my time in there Neonatal Intensive Care Unit (NICU). While
providing care to one specific child I was encouraged by my instructor to help provide instruction to
the parents in several different areas, but as I worked with mom and dad to help take care of the
baby I noticed some deficients in the knowledge of care for their child. The most being about
bonding with their child since the child was to remain in an isolette. The child was born at 33 weeks

Ultimately, I noticed that the parents were having a hard time by not being able to hold their baby
and bond. NICU care can interfere with the process of bonding between the family and by not
allowing them to touch or hold there baby creates a rift in the interaction process (Tessier et al.,
2009). As time progressed while I was at clinical the baby was becoming more self sufficient and
the suggestion of kangaroo care came about when mom was trying to breastfeed as well as when
they had to supplement with bottle feedings. When the subject of kangaroo care was brought up
neither mom nor dad had heard of it before. During my time with the family I felt that they were at
risk for impaired parenting related to prolong separation from the parent as evidence by long
amounts of time with no physical contact with the baby. It was clear that both mom and dad were
emotionally stressed by not being able to provide basic needs for their newborn, and physical
contact was restricted to small amounts of touching through the isolette. It was also clear that mom
and dad had still not developed any comfort with independently holding or feeding the baby. I
observed that they wanted to become more comfortable and have more physical interaction with
their child. Overall they wanted to become more integrated into the physical care of their child.

Nursing Reflection
Throughout my time on the Mother Baby Care Unit, I have faced many instances in which I have
been able to reflect on both my patients and the care that I was providing them. One situation in
particular that I found myself critically reflecting upon involved a new mother who was feeling
slightly stressed about being discharged as her newborn son would not be going home with her. For
confidentiality reasons, this patient will be given the pseudonym of Kayla for the remainder of this
reflection. Kayla and I met on the morning of her discharge while she was finishing packing up her
belongings. At this time, I completed her final set of vital signs and began to talk with her about
going home. As I spoke about this topic, I noticed that Kayla was becoming slightly stressed as her
voice sounded a little shaky and she was increasingly restless. I then proceeded to interpret why she
could be feeling this way to which I realized that as Kayla has yet to be more than one hospital zone
away from her son and the Neonatal Intensive Care Unit (NICU), she was likely feeling quite
uneasy about going home. Like any mother with a sick child, she did not want to leave her son's side
during this tough time and by being discharged, Kayla may have felt that a part of her would no
longer be with him. Subsequent to forming this interpretation, I moved on and responded to Kayla's
stress by encouraging her to engage in self–care during this difficult time. As the actions taken
during self–care can

NICU Nursing Reflection
Working in a neonatal intensive care unit has been a learning experience filled with a roller coaster
of emotions. So many emotions have passed over me through different situations that I have
encountered in my three years of working in this type of unit. When I first began my journey in
becoming a NICU nurse I was exposed to many things that a registered nurse may not encounter
unless they are in an intensive care unit setting. For example, I needed to learn to work with
ventilators, medication drips, everyday use of an Ambu–bag and all this with premature babies
nonetheless. I have learned that bio–reactions are an automatic signal in our brains which
determines how our body will react in certain situations. In the following paragraphs, I will discuss
three personal experiences of my personal bio–reactions. I want to recall a particular incident that
occurred very early in my journey to becoming a NICU nurse. It was not long after orientation, I
was no longer working with my preceptor and I was on my own caring for these tiny humans. I have
the responsibility of total patient care, including their airway, especially if a respiratory therapist is
unavailable. In our unit, one can feel quite overwhelmed as it is large and there are always monitors
beeping for different reasons. We have multiple staff around including fellows, residents, respiratory
therapist and of course there are always parents and relatives on the unit since there are no restricted
hours. That day

Bloom Research and Response Paper
Bloom Research and Response Paper
Benjamin Bloom developed Bloom's Taxonomy in 1956. It identifies three domains: cognitive,
affective, and psychomotor, used to evaluate knowledge assimilated by the learner. Each domain has
hierarchical categories that progressively measure the level of understanding achieved. This paper
reviews each domain and list the categories found within, discuss how Bloom's taxonomy apply to
the case study presented by Larkin and Burton's article 'Evaluating a Case Study Using Blooms
Taxonomy of Education', and highlight the benefit of Bloom's taxonomy as it relates to developing
individualized nursing instructions.

The record does not document any nurse–initiated interventions or call to the doctor requesting a
chest x–ray or recommending a respiratory therapy consult for breathing treatment and incentive
spirometer. On post–op day two Ms. C's respiratory status declined requiring a non–rebreather
mask, rapid response team consult, and a transfer to the intensive care unit for a diagnosis of
respiratory distress (p. 392).
There were multiply factors that contributed to the above scenario; Larkin and Burton writes that
"after this near–miss, failure to rescue incident" (p. 394) a task force consisting of management,
clinical nurse specialist (CNS) and unit educator convened to discuss the event. The task force
concluded that the nursing staff members were ineffectual in critically evaluating the patient's signs
and symptoms. The CNS chose a framework that utilized "Bloom's Taxonomy of Educational
Objectives", that provided measurable outcomes to the educational activity and enabled the nursing
team to optimize their critical skill levels. A workshop to assist staff to navigate through the case
study in a realistic manner was implemented (Larkin & Burton, 2008, p.395).

Core Competencies For Healthcare Professionals
My Leadership SMART Goal
The Institute of Medicine has defined five core competencies for healthcare professionals. These
competencies are to "provide patient–centered care, work in inter–professional teams, employ
evidence–based practice, apply quality improvement, and utilize informatics."(Finkelman, 2012, p.
214). Nurse leaders recognize that the need to put the patient first is at the heart of patient–centered
care and is significant in all aspects of nursing care. Patient–centered care incorporates collaboration
with the patient and family and takes into consideration cultural diversity or issues that will affect
the patient's ability to manage and maintain their health. Keeping the lines of communication open
with our patients involves assuring that they are kept abreast of their health management and
provided with up to date education to help them utilize measures to maintain their health. As nurses
we are accountable to our patients and need to stay current with our own education and commit to
lifelong learning (Fights, 2012) to be sure we are passing on up to date and relevant information.
Acquiring certification is an important way of validating our practice and proving our commitment
to providing quality patient–centered care. Fights challenges us to ask ourselves if we are committed
to lifelong learning or furthering our education. Signing up at Chamberlain to obtain my BSN was
an important step for me but I will not let this be the end of learning.

An Evaluation Of A Laparoscopic Hysterectomy Essay
During my rotation in the operating room at Community medical center, I observed the
preoperative, intraoperative, and postoperative care for a patient who underwent a laparoscopic
hysterectomy. I believe that an appropriate preoperative plan of care for this patient would have
included a full physical exam and an interview for patient history, a pelvic exam to look over and
understand the nature of the patient's complications, blood testing including a CBC and WBC to
note any signs of infection or contraindications for the procedure, and a urine test to rule out any
urinary tract infections or pregnancy. It would be important to interview the patient and ask
questions to determine how the patient is feeling about their procedure and to better assist with any
anxiety or pain they may be dealing with preoperatively. It is important to consult with the patient
well before the procedure to ensure that she knows to refrain from smoking for at least 8 weeks
before the procedure because this reduces the risks of complications such as infections, issues with
blood pressure, heart rate, blood flow, and respirations when under anesthesia, and promoting
overall health and risks associated with smoking after the procedure. (ASAHQ) It is also important
to educate the patient to consume no food or drinks after midnight the night before the scheduled
procedure. (Health Communities) During my rotation I observed that the patient did indeed have
labs drawn and a urine test run. Her lab

Work Through Possible Comparison Of Babies, Progression...
Session Three Goal– Work through possible comparison of babies, progression and regression while
in the NICU. Group members are struggling with the many facets of having a NICU baby. This
session will deal with some of the very common issues. Parents of NICU babies are often worried
about how slowly their child may be progressing in comparison to other babies. This session the
group will work through some of those progressions and regressions and help the parents to cope
with this. Parents are sitting in the NICU for hours, watching other babies do things that their baby
is not doing. However, this session will help show the parents that they are all doing this. Every
parent in the group has likely been the parent on either end of that frustration/joy. Regression can be
hard for parents and does happen often during a NICU stay. Group members may show some
reluctant to talk about their child's set–backs. They may be discouraged by them. Parents have a
strong desire to know when their child will be discharged from the hospital. This can cause them to
become upset when that does not happen (Pepler et. Al, 2012). The counselor should take note of
what group members seem more down than usual or less likely to share during this time. These
group members could have less positive news to share with the group. The counselor should remind
parents to focus on the here and now both in group and with their child. Take the time to notice what
their child is succeeding in. Helping the

The Post Anesthesia Care Unit At Miami Children's Hospital
A Harmonious Exemplification
On the post anesthesia care unit at Miami Children's Hospital, students and nurses alike witnessed a
harmonious exemplification of the six competencies of Quality and Safety Education for Nurses,
more commonly known as QSEN. The post anesthesia care unit, PACU, was off to a slow start just
before teamwork between the preoperative unit, also called PREOP, the operating room, and PACU
were put to the test. A 4 year–old, already extubated, female patient had come to the unit from the
operating room, more commonly referred to as the OR. The OR nurse proceeded to inform the
receiving PACU nurse that the patient just had an arterial line placed in the femoral artery in order to
later undergo apheresis for cancer treatment. Upon performing an assessment, the nurse noticed that
the patient's pressure dressing was saturated with blood due to a hemorrhage. A team consisting of
PACU nurses, the anesthesiologist, the OR nurse and students rushed to the bedside to assist in
restraining the squirming patient in order to keep her flat and still. Ultimately, the dressing was
replaced a total of four times before the hemorrhage ceased, the patient received one unit of packed
red blood cells, and had a new intravenous line, or IV, started because the previous site had
developed a reaction to the Demorol administered in the OR. To prevent further distress for the
patient, the anesthesiologist, surgeon, and primary care physician collectively decided to

what I wanted to be. Because I liked all these occupations I concluded that I just wanted a job that

than 24 weeks' gestation and are living and growing naturally. Working as a doctor or nurse in the
neonatal intensive care unit(NICU) can be very stressful and challenging. After researching this
occupation, I feel that this is the perfect job for me because I have a passion for babies and helping
others. My desire to become a nurse became stronger when I got pregnant with my first child. The
things a mother must go through to make sure her child is safe and healthy are what inspired me. I
had so many complications during my pregnancy, which eventually caused be have my daughter

she was okay and informing me on

Improving Feeding Outcomes in the NICU Through Infant-Driven Care

  • 1. Kangaroo Care Essay Jessica Plimpton FCS 497: Intro to Research Methods Fall 2015 The Benefits of Kangaroo Care for Fathers with an Infant in the Neonatal Intensive Care Unit (NICU): Educational, Emotional and Paternal Role Support. Introduction Take a second to imagine you are a father, who has an infant admitted to the neonatal intensive care unit (NICU) for the first time. How are you feeling? Afraid? Confused, even? If your baby is admitted to the NICU, your first question probably will be: What is this place? With equipment intended for infants and medical staff specially qualified in newborn care, the NICU is an intensive care unit created for sick newborns who require specialized treatment. A common example for parent's ... Show more content on Helpwriting.net ... Additionally there is a lack of information in regards to how Kangaroo Care helps fathers develop paternal feelings, a sense of paternal competence and ability to handle the unexpected situation. Therefore, this study will focus on fathers' and the benefits that Kangaroo Care provides for them. The research questions this study hopes to answer are as follows: (a) does kangaroo care help fathers develop paternal feelings, (b) will a fathers opportunity for being close to their infant and facilitating attainment benefit their paternal role in the neonatal intensive care unit, (c) will educational support and parenting techniques allow fathers to feel more connected with their infant? Ideally the interviews and focus groups will give neonatal intensive care unit staff a better understanding of the fathers experience with kangaroo care and what changes can be made to provide educational support, emotional support, and how to assist them in their paternal role. It should also be informative for all medical staff involved in working with infants and their caregivers. The purpose of this case study to is to examine the research problem of father's involvement in the neonatal intensive care process and how providing kangaroo care will benefit them. At this stage of research, kangaroo care will be generally defined as prolonged and continuous skin–to–skin contact between the ... Get more on HelpWriting.net ...

  • 3. Miracle Baby Research Paper For my extra credit project, I decided to shine a little light on the situation of my baby brothers birth. He was an answer to prayers and ultimately a blessing on our family. He didn't arrive as scheduled or when we thought he was. In addition, nothing went as planned but all I knew is that everything was eventually going to fall into place. Moreover, there was nothing really normal about my brothers birth or his situation, but in my eyes, that is what makes him a miracle baby to me. Before my baby brother was born, I was the youngest in my family. My sister and I were the only kids, we are five years apart, she was born in 1994 and I was born in 1999. We were inseparable growing up because it was just the two of us and we only had each other. ... Show more content on Helpwriting.net ... He was born as a five–pound baby and we believed that god saved my mom because she was considered to be an "older mom", which means she falls in the category of being a high–risk mom, making it harder to give birth. Even though she was considered high risk, this five–pound baby was destined to be born. He was born at Moanalua Hospital and his initial due date to be born was February 6th, 2017, but instead, he came 2 months early. Since he was born that early it put him in the category of a preemie, which means when a baby is born before the optimal birth weeks to be born. When my mom then gave birth the Neonatal Intensive Care Unit (NICU) nurses came to take him. Since he was so early she couldn't hold him, he had to be taken and hooked up to machines. In order to save his life he needed to be rushed there, "it was the hardest thing to do" my mom said because all she wanted to do was hold him but she knew that his life and his breathing was more important. She knew that if she let him go now she would have a lifetime to be able to hold him and love him unconditionally. My mom then got housed in The "Mother Baby" unit where she and I resided until she healed enough to be able to be sent ... Get more on HelpWriting.net ...

  • 5. The Treatment Of The Surgical Setting The surgical setting is very different from the generalized floors that I have become accustomed to. The exposure I had today included observing the operating room setting, the PACU, and the exchange of patient care to a generalized or outpatient floor. In preparation for my clinical day I had finished the module readings relating to peri–operative care which I found useful because I had a base knowledge of information to build upon. I selected one patient to gather information from which included his surgery, patient information, and care he received in the different stages of the surgical process. Patient Data Patient JC is a 63–year–old male, he is Caucasian and is not Hispanic. He presented to the emergency room on Monday April 4th after noticing pain, swelling, and a "blue tint" to his left fifth digit. He has a history of hyperlipidemia, GERD, cardiac disorder, osteoarthritis, hematuria, prostate cancer (resolved), and hammer syndrome. He has dentures and partial vision loss in both eyes. JC is a construction worker. The patient's current problem with his left fifth digit is believed to come from his history of hypothenar hammer syndrome. Hypothenar hammer syndrome is caused by repeated use of the palm of the hand, such as when you use a hammer to push or grind objects. This can result in damage of certain blood vessels in hand, especially the ulnar artery. This artery goes through the hypothenar area of the palm and supplies blood to the fingers, damage to the ulnar ... Get more on HelpWriting.net ...

  • 7. Health Care Reflection The patient I cared for today, was admitted into the NICU on October 17th. He was experiencing respiratory distress, has breathed (aspirated) meconium into the lungs around the time of birth. The neonate had stabilized, so the nurse and I were preparing for the family to discharge from the hospital. The immediate needs that were verbalized by the patient were hunger cries and soiled diaper/ angry cries. Immediate needs from the family were instruction on how much formula to feed the baby and circumcision care once discharged home. During my clinical experience, the patient experienced pain from the circumcision process. The health care team members alleviated the physical suffering by administrating lidocaine pain medication and giving the baby sugar water. I think the only improvement that I would make to these interventions would be to administer the lidocaine earlier so that it could be more effective in the patient's pain management. Long term needs for the patient were speech therapy in order to improve the baby's feeding technique, infection prevention, and teaching measures for the parents. The needs could be met through referral to a speech therapist upon discharge from the hospital, teaching family members proper handwashing, and educating parents on subject matters. The healthcare team interacted with the patient and family in an excellent fashion. They utilized the teach back method when educating parents about infant care, used compassionate care, and used open ... Get more on HelpWriting.net ...

  • 9. Moral Distress And Avoidance Behavior In the article ¨Moral Distress And Avoidance Behavior In Nurses Working In Critical Care And Noncritical Care Units," by Mary Jo De Villers, she states ¨it has been predicted that there will be a shortage of 500,000 nurses in the United States by 2025.¨ This counts for the United States alone. Many articles state that more and more research is being done on moral distress relating to nurses all around. This leaves researchers wondering, could this be the main reason for the shortage of nurses? Moral distress comes into play dramatically with specific nurses such as pediatric intensive care, neonatal intensive care, and intensive care unit. Many of the nurses have to deal with life–or–death situations on a daily basis working in those units. Dealing with that dilemma daily makes it harder for this medical staff to go back to the hospital. All nurses deal with some kind of moral distress, but critical care nurses suffer from a more intense level of distress. There are many factors that play into the shortage of nurses. In Anthony Tuckett's study, he discovers reasons why nurses are leaving the profession. In attempt to understand, he interviewed nurses and asked for their opinion on the lack of physicians assistant. Overall, Tuckett concluded that the primary reasons for this are no manager support, co–worker support, and the fact that business policies are put before caring for the patient (364). New nurses feel belittled when entering the profession and old nurses feel as ... Get more on HelpWriting.net ...

  • 11. Improving Feeding Outcomes Of An Infant Driven Feeding... Improving Feeding Outcomes in the NICU: Establishing an Infant–Driven Feeding Pathway Laura Pabich BSN, RN The University of Iowa College of Nursing Improving Feeding Outcomes in the NICU: Establishing an Infant–Driven Feeding Pathway The most common reason preterm infants face prolonged hospitalization in a Neonatal Intensive Care Unit (NICU) is a delay in acquiring full oral feedings (Shaker, 2013). The purpose of this project is to improve feeding outcomes for preterm infants at Blank Children's Hospital's (BCH) NICU in Des Moines, Iowa. Traditionally, feeding practices in the NICU have followed a physician–led, volume–driven model based on an infant's weight and gestational age. This model defines a preterm infant's ... Show more content on Helpwriting.net ... Feeding difficulties often persist past NICU discharge with an incidence ranging from 19 to 80 percent (Shaker, 2013; Sundseth, 2013). The average daily cost for NICU care often exceeds $3,500 per infant and a delay in establishing full oral feedings further increases the cost of care (Murakas, 2008). Delayed discharge also places infants at risk for hospital related morbidity, leads to prolonged maternal child separation and increases parental stress (Gennattasio, Perri, Baranek, & Rohan, 2015). Preterm infants face a number of challenges in attaining full oral feedings including physiologic instability, neurologic immaturity and behavioral state disorganization (Gennattasio, Perri, Baranek, & Rohan, 2015). The time in which preterm infants are learning to feed is characterized by rapid motor and neurodevelopment in which every feeding experience plays a role (Shaker, 2013). Physiologic stress during feeding may encourage sensory–motor pathways in the brain, leading to maladaptive feeding behaviors, learned feeding refusals and long–term feeding aversions (Shaker, 2013). Traditional, volume–driven feeding practices fail to account for an infant's hunger cues, the physiologic stress caused by the challenge of oral feeding and the quality of an individual feeding (Newland, Weems L'Huillier, & Petrey, 2013). According to Sundseth Ross and Browne (2013), feeding practices must move from ... Get more on HelpWriting.net ...

I thought it was more pregnancy pains that she had experienced for the previous five months. Then, she told me that she believed they 89were contractions and she had been timing their intervals and we needed to get to the hospital. My initial thoughts were frantic; cs it's much too soon, it's two months early. After my initial worries were addressed in my mind, I began to run the logistics of getting to the hospital. We prepared to go back home, we had our apartment as prepared as possible. The hospital called a free taxi for us and we put Svanja in a carseat for the first time. She felt so fragile I was hesitant to tighten the straps. After we got back to our apartment we slept in shifts to take care of her until it was time to go back to America. All in all, it was one of the most stressful times of my life, but also one of the happiest. Many advancements in treatment, technology, and healthcare perceive feasible, but when a premature infant's life is compromised, due to severe prematurity, finding the best possible solutions and treatment options may put the healthcare team, caring for the infant, in a conflicting situation. • 17. Nurse Driven, Sleep Hygiene Protocol, A Source Of Anxiety... The inability to sleep is a source of anxiety and stress for patients in the intensive care unit (ICU); this inability can lead to cardiorespiratory disturbances, immune system dysfunction, impaired wound healing, hormonal and metabolic imbalances, cognitive changes, and delirium. Through the use of quiet times and nursing actions that encourage sleep, or, sleep hygiene bundles, nurses can assist in improving sleep quality in the ICU. These bundles include noise and light reduction, scheduling routine procedures outside of reserved sleep times, the use of earplugs and eye masks, and the grouping of nursing care activities to minimize sleep interruptions. The aim of this proposal, therefore, will be to implement a nurse–driven, sleep hygiene protocol, in order to improve patient care by promoting quality sleep in ICU patients. Barriers to implementation include staff resistance to the concept, concerns regarding patient care during sleep times and difficulties in coordinating care with other units. Studies used to gather information for this proposal were published between the years of 2011 and 2016 and were found in several databases: Academic Search Complete, Medline, and OVID Journals. Keywords: ICU, intensive care unit, critical care, quiet time, noise, and nursing. Quiet Time in the ICU One of the top three sources of anxiety and stress for patients during an intensive care unit (ICU) stay is the inability to sleep (Kamdar, Needham, & Collop, 2012). He tried to re– insert it for almost two hours to no avail, all while my wife was experiencing labor pains with no pain killer. There was one other anesthesiologist in the hospital, but he was detained in a surgery and was not able to help us for a while. To add to that, the uninterested nurse told us she was not able to administer any other pain killer while the anesthesiologist was trying to get the epidural to go in. When he finally did arrive, he asked the nurse why she had not given any pain killer to my wife. She responded with, "I didn't think I was allowed to." Then he got the epidural in on his first try! Frustrating relief! Finally, after all the complications, were over (or so we thought), here comes my first baby! With family and friends surrounding my wife and the doctor (seven of us total, as I recall), our baby's head came out. But wait, the cord is around her neck. Risk And Implement Falls Prevention Measures More Staff is Not the Solution to Decrease Patient Falls Falls in an acute care setting lead the list of injury related deaths and deaths in the elderly. "A fall is defined as any event which patients are found on the floor (observed or unobserved) or an unplanned lowering of the patient to the floor by staff or visitors" (Kalisch, Tschannen, and Lee, 2012, p. 6). Medicare and Medicaid changes in 2008 list falls as one of the 10 hospital acquired conditions for which hospitals will no longer be reimbursed because falls are considered preventable conditions. Show more content on Helpwriting.net ... Patients being admitted to hospitals today have multiple co–morbidities and are on a number of medications making them a high risk for falls. Having an awareness of a patients diagnoses, pertinent history and current medications is key in establishing an appropriate plan of care. Staffing shortages can lead to omission of tasks such as a complete admission history and falls risk assessment. As with any assignment, regardless of the amount of patients one is caring for, prioritization needs to be utilized when completing tasks and making care decisions. "Further work must be done to assist nurses in completing necessary tasks...which may or may not mean additional staff members" (Kalisch, Tschannen & Lee, 2012, p. 11). Developing strategies such as computerized reminders and checklists are necessary to ensure complete and appropriate nursing care is delivered. (Kalisch, Tschannen & Lee, 2012, p. 11) The lack of proper education on identification of falls risk and falls prevention measures impact patient falls. Staff turnover and the use of temporary staff contribute to miscommunication or lack of communication on processes such as fall prevention measures. Experience levels vary on any given unit. Most neonatal nurses work in a typical hospital environment – usually in the neonatal intensive care unit (NICU) or maternity ward. ("Rasmussen College"). One reason why you should consider a career in neonatal nursing is that it will give you an opportunity to help newborn babies. Premature or underweight babies need monitoring and special care all the time, having this field of profession is rewarding if you love babies. Quality And Safety Of Neonatal Care Quality and Safety in Neonatal Care in Nursing Practice Infants within the neonatal intensive care unit (NICU) are one of the most vulnerable patient population in the hospital. The quality and safety of these patients are at top priority because of their immune systems have not fully developed yet causing them to be at the highest risk for infections. According to the Institute of Medicine (IOM, 2001), quality is defined as "the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge" (Masters, 2005). The patient had broken his hip and was undergoing surgery to place screws and plates into his hip. I was able to see the whole process through the preoperative, intraoperative and postoperative stages. The preoperative stage is when the patient comes to the OR and is being prepped for the surgery. The patient is verified by the nurse, who needs to check patient identification, patient records and make sure is calm for the surgery. Before this particular surgery, the anesthesiologist came down before the surgery to administer a block to the patient. The block is to dull the nerve ending so when the patients comes out of surgery he will be in less pain. The consent form is signed by two people, the patient and the doctor performing the surgery. The consent form is the responsibility of the doctor, the nurse just verifies that both the doctor and the patient signed the form. It is very common for a patient to be anxious right before a major surgery. My patient didn't seem anxious but just wanted it to be over and done with. One of the most important part of a nurse's job is to keep the patient calm and relaxed. That can be accomplished just by talking to them and reassuring them. Parent–Staff Communication Wigert, Delienmark, & Bry (2013) explored the strengths and weaknesses of staff communication in the NICU. They gave 270 parents a 16– question survey with answers using a Likert scale. A common statement from most parents was that they had received information from nurses and doctors and when they did the information was easy to understand. Some parents felt the staff did not understand their situation. There was a significant difference at the .05 level between parent communication with nurses versus doctors (Wigert, Blom, & Bry, 2013). The Plan–Do–Check–Act cycle (PDCA) is a tool that will be used to develop a plan for quality improvement and achieve the goals that would be beneficial for the nurse and patient care outcomes. The personal goals are to discharge all patients with finished documentation according to the certified post anesthesia nurse (CPAN) rules by using the process of delegation to obtain ancillary help. DELEGATION Delegation is the cornerstone to being a nurse leader, it facilitates the work effort of the staff to accomplish the mission and goals of the healthcare organization. Getting out of the house, without a sound, was the tricky part. If I timed it just right, I could use the car's engine to my advantage and cover the creak from the door. Once I was outside, I was in the clear. I scampered over to the XL white Suburban and gingerly climbed into the passenger seat. My mom sat inside, listening to soft jazz, decompressing from a long day's work as a nurse in the ICU. As with most nights, she feigned annoyance before excitedly walking me through the events at the hospital. Some nights were filled with triumphant saves, others drowned in an untimely loss. The stories varied, yet a central idea remained: take care of the person instead of the patient. In my youth, her stories registered at face value. All too soon, I would see the truth in her words firsthand. It started with a dull ache through my left ear immediately following diving practice. A number of weeks passed, allowing the ache to progress to a stabbing pain. My family grew concerned with my discomfort, culminating in a visit to an otolaryngologist. It took Dr. C only a moment to confirm what my pediatrician had suspected. I had a tumor. Any other physician would have referred to it as a cholesteatoma. Dr. C never used that word with me. There was no use for medical jargon; he knew it was not what I needed to hear. Instead, Dr. C understood how scary the word tumor sounded and recognized I ... Get more on HelpWriting.net ...

  • 59. My Rotation For Obstetrics My rotation for obstetrics was in Brighton, CO at Platte Valley Medical Center. I was fortunate enough to experience multiple avenues of this field including labor and delivery and postpartum, but ultimately enjoyed spending most of my time in there Neonatal Intensive Care Unit (NICU). While providing care to one specific child I was encouraged by my instructor to help provide instruction to the parents in several different areas, but as I worked with mom and dad to help take care of the baby I noticed some deficients in the knowledge of care for their child. The most being about bonding with their child since the child was to remain in an isolette. The child was born at 33 weeks and 6 days due to mom having HELLP syndrome and the ... Show more content on Helpwriting.net ... Ultimately, I noticed that the parents were having a hard time by not being able to hold their baby and bond. NICU care can interfere with the process of bonding between the family and by not allowing them to touch or hold there baby creates a rift in the interaction process (Tessier et al., 2009). As time progressed while I was at clinical the baby was becoming more self sufficient and the suggestion of kangaroo care came about when mom was trying to breastfeed as well as when they had to supplement with bottle feedings. When the subject of kangaroo care was brought up neither mom nor dad had heard of it before. During my time with the family I felt that they were at risk for impaired parenting related to prolong separation from the parent as evidence by long amounts of time with no physical contact with the baby. It was clear that both mom and dad were emotionally stressed by not being able to provide basic needs for their newborn, and physical contact was restricted to small amounts of touching through the isolette. It was also clear that mom and dad had still not developed any comfort with independently holding or feeding the baby. I observed that they wanted to become more comfortable and have more physical interaction with their child. Overall they wanted to become more integrated into the physical care of their child. ... • 61. Nursing Reflection Throughout my time on the Mother Baby Care Unit, I have faced many instances in which I have been able to reflect on both my patients and the care that I was providing them. One situation in particular that I found myself critically reflecting upon involved a new mother who was feeling slightly stressed about being discharged as her newborn son would not be going home with her. For confidentiality reasons, this patient will be given the pseudonym of Kayla for the remainder of this reflection. Kayla and I met on the morning of her discharge while she was finishing packing up her belongings. At this time, I completed her final set of vital signs and began to talk with her about going home. As I spoke about this topic, I noticed that Kayla was becoming slightly stressed as her voice sounded a little shaky and she was increasingly restless. I then proceeded to interpret why she could be feeling this way to which I realized that as Kayla has yet to be more than one hospital zone away from her son and the Neonatal Intensive Care Unit (NICU), she was likely feeling quite uneasy about going home. Like any mother with a sick child, she did not want to leave her son's side during this tough time and by being discharged, Kayla may have felt that a part of her would no longer be with him. Subsequent to forming this interpretation, I moved on and responded to Kayla's stress by encouraging her to engage in self–care during this difficult time. As the actions taken during self–care can ... Get more on HelpWriting.net ...

  • 63. NICU Nursing Reflection Working in a neonatal intensive care unit has been a learning experience filled with a roller coaster of emotions. So many emotions have passed over me through different situations that I have encountered in my three years of working in this type of unit. When I first began my journey in becoming a NICU nurse I was exposed to many things that a registered nurse may not encounter unless they are in an intensive care unit setting. For example, I needed to learn to work with ventilators, medication drips, everyday use of an Ambu–bag and all this with premature babies nonetheless. I have learned that bio–reactions are an automatic signal in our brains which determines how our body will react in certain situations. In the following paragraphs, I will discuss three personal experiences of my personal bio–reactions. I want to recall a particular incident that occurred very early in my journey to becoming a NICU nurse. It was not long after orientation, I was no longer working with my preceptor and I was on my own caring for these tiny humans. I have the responsibility of total patient care, including their airway, especially if a respiratory therapist is unavailable. In our unit, one can feel quite overwhelmed as it is large and there are always monitors beeping for different reasons. We have multiple staff around including fellows, residents, respiratory therapist and of course there are always parents and relatives on the unit since there are no restricted hours. That day ... • 65. Bloom Research and Response Paper Bloom Research and Response Paper Benjamin Bloom developed Bloom's Taxonomy in 1956. It identifies three domains: cognitive, affective, and psychomotor, used to evaluate knowledge assimilated by the learner. Each domain has hierarchical categories that progressively measure the level of understanding achieved. This paper reviews each domain and list the categories found within, discuss how Bloom's taxonomy apply to the case study presented by Larkin and Burton's article 'Evaluating a Case Study Using Blooms Taxonomy of Education', and highlight the benefit of Bloom's taxonomy as it relates to developing individualized nursing instructions. The record does not document any nurse–initiated interventions or call to the doctor requesting a chest x–ray or recommending a respiratory therapy consult for breathing treatment and incentive spirometer. On post–op day two Ms. C's respiratory status declined requiring a non–rebreather mask, rapid response team consult, and a transfer to the intensive care unit for a diagnosis of respiratory distress (p. 392). There were multiply factors that contributed to the above scenario; Larkin and Burton writes that "after this near–miss, failure to rescue incident" (p. 394) a task force consisting of management, clinical nurse specialist (CNS) and unit educator convened to discuss the event. The task force concluded that the nursing staff members were ineffectual in critically evaluating the patient's signs and symptoms. The CNS chose a framework that utilized "Bloom's Taxonomy of Educational Objectives", that provided measurable outcomes to the educational activity and enabled the nursing team to optimize their critical skill levels. A workshop to assist staff to navigate through the case study in a realistic manner was implemented (Larkin & Burton, 2008, p.395). The cognitive domain contains six intellectual skills that measure: knowledge, comprehension, application, analysis, synthesis, and evaluation of information ... Get more on HelpWriting.net ...

  • 67. Core Competencies For Healthcare Professionals My Leadership SMART Goal The Institute of Medicine has defined five core competencies for healthcare professionals. These competencies are to "provide patient–centered care, work in inter–professional teams, employ evidence–based practice, apply quality improvement, and utilize informatics."(Finkelman, 2012, p. 214). Nurse leaders recognize that the need to put the patient first is at the heart of patient–centered care and is significant in all aspects of nursing care. Patient–centered care incorporates collaboration with the patient and family and takes into consideration cultural diversity or issues that will affect the patient's ability to manage and maintain their health. Keeping the lines of communication open with our patients involves assuring that they are kept abreast of their health management and provided with up to date education to help them utilize measures to maintain their health. As nurses we are accountable to our patients and need to stay current with our own education and commit to lifelong learning (Fights, 2012) to be sure we are passing on up to date and relevant information. Acquiring certification is an important way of validating our practice and proving our commitment to providing quality patient–centered care. Fights challenges us to ask ourselves if we are committed to lifelong learning or furthering our education. Signing up at Chamberlain to obtain my BSN was an important step for me but I will not let this be the end of learning. Achieving ... Get more on HelpWriting.net ...

  • 69. An Evaluation Of A Laparoscopic Hysterectomy Essay During my rotation in the operating room at Community medical center, I observed the preoperative, intraoperative, and postoperative care for a patient who underwent a laparoscopic hysterectomy. I believe that an appropriate preoperative plan of care for this patient would have included a full physical exam and an interview for patient history, a pelvic exam to look over and understand the nature of the patient's complications, blood testing including a CBC and WBC to note any signs of infection or contraindications for the procedure, and a urine test to rule out any urinary tract infections or pregnancy. It would be important to interview the patient and ask questions to determine how the patient is feeling about their procedure and to better assist with any anxiety or pain they may be dealing with preoperatively. It is important to consult with the patient well before the procedure to ensure that she knows to refrain from smoking for at least 8 weeks before the procedure because this reduces the risks of complications such as infections, issues with blood pressure, heart rate, blood flow, and respirations when under anesthesia, and promoting overall health and risks associated with smoking after the procedure. (ASAHQ) It is also important to educate the patient to consume no food or drinks after midnight the night before the scheduled procedure. (Health Communities) During my rotation I observed that the patient did indeed have labs drawn and a urine test run. Her lab ... Get more on HelpWriting.net ...

  • 71. Work Through Possible Comparison Of Babies, Progression... Session Three Goal– Work through possible comparison of babies, progression and regression while in the NICU. Group members are struggling with the many facets of having a NICU baby. This session will deal with some of the very common issues. Parents of NICU babies are often worried about how slowly their child may be progressing in comparison to other babies. This session the group will work through some of those progressions and regressions and help the parents to cope with this. Parents are sitting in the NICU for hours, watching other babies do things that their baby is not doing. However, this session will help show the parents that they are all doing this. Every parent in the group has likely been the parent on either end of that frustration/joy. Regression can be hard for parents and does happen often during a NICU stay. Group members may show some reluctant to talk about their child's set–backs. They may be discouraged by them. Parents have a strong desire to know when their child will be discharged from the hospital. This can cause them to become upset when that does not happen (Pepler et. Al, 2012). The counselor should take note of what group members seem more down than usual or less likely to share during this time. These group members could have less positive news to share with the group. The counselor should remind parents to focus on the here and now both in group and with their child. Take the time to notice what their child is succeeding in. Helping the ... • 75. The Post Anesthesia Care Unit At Miami Children's Hospital A Harmonious Exemplification On the post anesthesia care unit at Miami Children's Hospital, students and nurses alike witnessed a harmonious exemplification of the six competencies of Quality and Safety Education for Nurses, more commonly known as QSEN. The post anesthesia care unit, PACU, was off to a slow start just before teamwork between the preoperative unit, also called PREOP, the operating room, and PACU were put to the test. A 4 year–old, already extubated, female patient had come to the unit from the operating room, more commonly referred to as the OR. The OR nurse proceeded to inform the receiving PACU nurse that the patient just had an arterial line placed in the femoral artery in order to later undergo apheresis for cancer treatment. Upon performing an assessment, the nurse noticed that the patient's pressure dressing was saturated with blood due to a hemorrhage. A team consisting of PACU nurses, the anesthesiologist, the OR nurse and students rushed to the bedside to assist in restraining the squirming patient in order to keep her flat and still. Ultimately, the dressing was replaced a total of four times before the hemorrhage ceased, the patient received one unit of packed red blood cells, and had a new intravenous line, or IV, started because the previous site had developed a reaction to the Demorol administered in the OR. To prevent further distress for the patient, the anesthesiologist, surgeon, and primary care physician collectively decided to ... Get more on HelpWriting.net ...
