This document discusses opportunities and barriers for mobile health (mHealth) in Asia. It finds that mHealth is poised for accelerated growth due to factors like increasing smartphone adoption and expanding middle classes. Key opportunities include remote consulting, disease monitoring, prevention/wellness, and personal health records. Barriers include the untapped rural market, limitations of care via mobile, and lack of standards/interoperability. Stakeholders like consumers, providers, insurers, and life sciences companies all stand to benefit. The document argues that telecom companies are well positioned to take a leading role in mHealth due to existing infrastructure, reach, and consumer trust. Critical success factors include ensuring community trust, financial sustainability, appropriate technology, government
Neighborhood Family Practice is a federally qualified health center that is one of six in Cleveland and serves as the only provider on the city's west side. It provides primary care, behavioral health, women's health, dental, and pharmacy services to over 21,000 patients annually through seven locations. Due to the Covid-19 pandemic, the practice rapidly converted 75% of visits to telemedicine in March 2020 using Doxy.me instead of its normal electronic health record, in order to continue serving its largely low-income patient population remotely.
The document summarizes Virginia's Medicaid managed care delivery system. Value-based Healthcare today and tomorrow by Deloitte.DayOne
VBHC today and tomorrow
There is an increasing need for a value-based healthcare model based on holistic patient and health system outcomes, which can only be achieved through genuine collaboration between stakeholders. Value-based contracts require defining appropriate outcome measures, collecting and analyzing real-world data, and addressing challenges such as misaligned incentives among payers, providers, and pharmaceutical companies. The COVID-19 pandemic is likely to accelerate trends in healthcare such as greater emphasis on value-based care, data analytics, technology innovation, and patient engagement. Post-COVID, there are opportunities for life sciences companies to strengthen evidence of value, support health system capacity and digital adoption, and engage differently with regulators and payers.
This document provides information about Paul Grundy, the director of healthcare transformation at IBM and president of the Patient Centered Primary Care Collaborative. It discusses his background and accomplishments in leading the patient-centered medical home model. It also summarizes evidence that implementing medical home interventions can reduce costs and improve outcomes by decreasing hospital days, ER visits, and costs while increasing medication adherence. Specific examples from studies in Pennsylvania, Michigan, and New York are highlighted that show reductions in costs and utilization from medical home programs.

The document describes ROC, a digital health platform that provides remote and self-management support for patients with chronic conditions like IBD, cancer, and mental health issues. It allows patients to track symptoms and receive personalized advice, while also keeping clinicians updated. The platform aims to enable needs-based care rather than clinic-based care. It uses artificial intelligence to analyze patient-reported data and recommend self-management resources, integrate with NHS systems, and provide feedback to improve patient outcomes and experiences.
This document discusses 5 elements of a successful patient engagement strategy:
1. Define your organization's vision for patient engagement.
2. Create a culture of engagement within the practice.
3. Employ the right technology and services like patient portals.
4. Empower patients to become collaborators in their care.
5. Continuously evaluate progress and be ready to adapt the strategy.
True patient engagement involves patients managing their own health, a practice culture that prioritizes engagement, and collaboration between patients and providers.

Summary -- Patient Centered Medical Home the Necessary Foundation for Accountable Care and Population Management.
In the next 10 years, we will be living in 1) mobile world 2) in the middle of an aging and chronic disease epidemic and 3) data. But , we will also have the ability to analyze data in a cognitive way this will do for doctors’ minds what X-ray and medical imaging have done for their vision. How? By turning data into actionable information. Take, for instance, IBM’s intelligent supercomputer, Watson. Watson can analyze the meaning and con-text of human language and quickly process vast amounts of information. With this in-formation, it can suggest options targeted to a patient’s specific circumstances.
We need the basic foundation to support this transformation a system integrator where data at the level of a patients flows and is held accountable and that model is the Patient Centered Medical Home. (PCMH) starts to happen when clinicians/ healers step up to comprehensive relationship based care empowered by tools to manage the data and communicate effectively. This move to PCMH level care requires the discipline of leading a team that delivers population health management, patent centered prevention, care that is coordination, comprehensive accessible 24/7 and integrated across a deliver system and all of that is power by data made into meaningful information.
But at its core it is a move toward integration of a healing relationship in primary care and population management all at the point of care with the tools to do just that.
The Patient-Centered Medical Home (PCMH) lies at the center of the effort to get at population health, integrated and coordinated care. PCMH is where the Primary care healer leads an organization that delivers clinician-led primary care, with comprehensive, accessible, holistic, coordinated, evidence-based coordination and management. In the USA this is now the standard in the US Veterans Administration and the US Military and under the ACA.
The document provides information about applying for and using the CHIP (Children's Health Insurance Program) in Utah. It explains that the Department of Workforce Services processes applications and determines eligibility. It outlines the three CHIP plans (A, B or C) based on income level and how each plan determines premiums, deductibles, and copays. It provides details on choosing health and dental plans, contacting a Health Program Representative for assistance, changing plans annually, and additional CHIP resources online.
This document summarizes a webinar about increasing revenue from Chronic Care Management (CCM) programs. It discusses how the CCM program works, how practices can participate, and how the Discharge IQ application can help practices manage patients and maximize their CCM revenue. Discharge IQ is a mobile application that monitors chronic care patients daily through simple diagnostic questions. It alerts doctors to issues and helps manage large panels of chronic patients more efficiently to qualify for the $42 per month CCM reimbursement from Medicare.

Effective communication is one of the most important aspects of team dynamics and can lead a team to be extremely efficient and successful or inefficient and a failure. There are four crucial elements for effective communication: willingness to have open and authentic discussions, active listening, willingness to confront conflicts, and mutual understanding. When creating an environment for open discussions, a team must determine how each member communicates, understand each member's strengths and weaknesses in expressing views, ensure more expressive members don't dominate introverts, encourage objective conversations to build trust, and present information factually. With effective communication, teams can meet deadlines, accomplish difficult tasks, and maintain morale. Successful relationships and increased productivity also emerge when team members communicate ideas and tactics to help one

Making Seven Day Services a reality, pop up uni, 2 pm, 3 september 2015NHS England
Following discussions on reducing weekend mortality rates, four clinical standards were identified as having the most impact: timely consultant review, access to diagnostics, access to interventions, and ongoing review. Each NHS trust was asked to complete a self-assessment tool to establish a baseline for meeting these standards by September 2015. The results will be used to track national progress in implementing seven-day services. Key lessons from early adopter sites included the importance of workforce, shared vision, increased partnerships, measurement, leadership, and patient experience.
