Neighborhood Family Practice is a federally qualified health center that is one of six in Cleveland and serves as the only provider on the city's west side. It provides primary care, behavioral health, women's health, dental, and pharmacy services to over 21,000 patients annually through seven locations. Due to the Covid-19 pandemic, the practice rapidly converted 75% of visits to telemedicine in March 2020 using Doxy.me instead of its normal electronic health record, in order to continue serving its largely low-income patient population remotely.

- Neighborhood Family Practice is a network of community health centers that delivers high quality, affordable, primary and preventive health care to people in Cleveland’s diverse west side neighborhoods. Our care teams provide comprehensive, personalized care and the support and education to foster better quality of living for patients, their families and communities. Founded in 1980, NFP is one of 6 FQHCs in Cleveland and 49 in Ohio. We have five locations, separated by mere miles, and one which is located right in your neighborhood. We provided integrated Primary Care and BH including two sites with the Centers. NFP is recognized by NCQA as a PCMH and accredited by the Joint Commission which speaks to the quality of care that we deliver at our health centers. A Federally Qualified Health Center (FQHC) is more commonly known as a Community Health Center (CHC) and is a primary care center that is community-based and patient-directed. By mission and design, CHCs exist to serve those who have limited access to health care although all are welcome. A Federally Qualified Health Center (FQHC) is a reimbursement designation from the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services. Medicare and Medicaid statutes define an FQHC as a health center that receives federal funding under Section 330 of the Public Health Service Act to provide comprehensive primary care services to uninsured and underinsured populations.
