This document outlines important nursing diagnoses for several common conditions: 1. HYPERTENSION
• 1. Risk for decreased cardiac output related to increased afterload,
vasoconstriction, myocardial ischemia, ventricular hypertrophy.
• 2. Acute pain: headache related to increased cerebral vascular
pressure.
• 3. Ineffective Tissue Perfusion: cerebral, renal, cardiac related to
impaired circulation.
• 4. Knowledge deficit related to lack of information about the disease
process and self-care.
• 5.Anxiety related to disease process/treatment/complications.

5. Important Nursing diagnosis for APPENDECTOMY
1.Acute Pain related to Distension of intestinal tissues by
inflammation/Presence of surgical incision
2.Risk for Deficient Fluid Volume related to Preoperative vomiting,
postoperative restrictions Hyper metabolic state (e.g., fever, healing process)
3.Risk for Infection related to Inadequate primary defenses;
perforation/rupture of the appendix; peritonitis; abscess formation/Invasive
procedures, surgical incision.
4.Deficient Knowledge related to Lack of exposure/recall; information
misinterpretation/Unfamiliarity with information resources
5.Anxiety related to disease process/treatment/complications.
