Program Requirements and Guidance
A formal document, or series of documents, exists that defines goals and requirements and provides
guidance for implementation of the procedures program, including development, review, verification &
validation, control & updating, and detailed guidance on writing procedures (a writers guide).
The procedures program and program documentation addresses all types of procedures in the facility –
operations, maintenance, surveillance, administrative control, technical, design & engineering, etc., - with
specific requirements for each type.
All procedure writers have appropriate job/technical knowledge for the procedures they are writing; all
writers have training in procedure writing practice and specific training in use of the facility-approved
writers guide.
A requirement exists and a systematic process is in place to establish and maintain a documented basis
for each procedure; procedure basis includes all necessary technical and operational information
necessary to support the technical and operational accuracy of the procedure.
A systematic process is used to determine the appropriate level of use (e.g., in-hand or reference) based
on risk and economic consequence of inappropriate or incorrect performance.
Prior to writing or rewriting a procedure, the procedure writers conduct a comprehensive assessment of
pertinent information.
Before use, all procedures undergo a formal technical verification review by appropriate
engineering/technical staff.
Before use, all procedures undergo an independent operational validation review by appropriate
operations staff; the validation review includes both a table-top checklist review and a walk-down (with
actual or simulated performance of the tasks) by current qualified operators and someone other than the
writer.
A systematic process is in place to identify, report, and control procedure revisions, and all affected
personnel have been trained in using the process.
A revision history is prepared for each procedure, and all revisions are promptly entered and permanently
tracked; history includes identifying information and reason for change, as well as the specific change.
Scoring:
22 – 30…………Procedures program is well designed and implemented. Continue to monitor and enhance the
process.
13 – 21…………Procedure development and validation process requires improvement to ensure accuracy and
usability.
Less than 13……Significant weakness exists in the development and validation process. Procedures may be
out of date, vague, inaccurate, and contain excessive extraneous information.
